1.The safety factor analysis on ventilator application based on quality management tools

Objective:Along with general hospital assessment opportunity, we explore to optimize ventilator management processes to reduce security risks by using the advanced quality management tools. Methods:Based on the ventilator maintenance statistical data in year 2012, we applied Pareto chart, Pareto curves and fishbone diagrams etc. quality management tools, and conducted PDCA cycle analysis on safety factors of ventilator clinical application. At the end, we summarized the results and provide improvements recommendations. Results:The cumulative proportions fell from 23.6%to 80%range, the human operator, compressor inlet;pipeline leaks and consumables management are a major are major factors. Through fishbone diagram analysis, we categorized them into 4 aspects:personnel, operations, equipment and supplies management. We optimized existing systems and processes from three areas:the device management, supplies management and training. By implementing these improvements plans, the security risk caused by supplies management was basically solved, the effect is very obvious. Conclusion:By using quality management tools, the safety problems during clinical application of ventilator can be reflected directly and clearly, and protect ventilator safe and stability to next level, so that it helps medical equipment management be more scientific, effective and normative.
2.Analysis of risk factors for deep venous thrombosis of lower limbs after laparoscopic surgery

Objective To investigate the risk factors for deep vein thrombosis (DVT) of lower limbs after laparoscopic surgery.Methods A total of 230 patients underwent laparoscopic surgery were selected in Huanggang Central Hospital from January 2014 to August 2016.The patients were divided into DVT group and non-DVT group according to DVT of the lower limbs,and the related risk factors for DVT of lower limbs after laparoscopic surgery were analyzed.Results In the 230 patients,there were 10 patients with DVT of lower limbs and 220 patients without DVT of lower limbs,the incidence of DVT of lower limbs was 4.35% (10/230).Logistic regression analysis showed that age ≥40 years old,operation time ≥120 min,postoperative ambulation time ≥ 30 h,the intraoperative position of high-head and low-foot,plasma D-Dimer level ≥0.5 mg · L-1 were the independent risk factors for DVT of lower limbs after laparoscopic surgery (P < 0.05).Conclusion There is a certain incidence of DVT of the lower limbs after laparoscopic surgery.The effective prevention measures should be taken to reduce the incidence of DVT after laparoscopic surgery according to the related factors of DVT.
3.Investigation on the nutritional status and quality of life in 95 scattered elderly in Shanghai

Objective: To investigate the quality of life (QOL) and its influence factors in the scattered elderly in Shanghai, and to evaluate the effect of nutrition status on the QOL. Methods: Ninty-five randomly-selected elderly were investigated with QOL questionnaire, food frequency questionnaire and 24 h dietary recalls, their nutrition status were awarded by nutrient intake and medical examinations. Results: The percentage of poor, moderate and good QOL were 1.05%, 61.05% and 37.89%. The significant influence factors of QOL were health status, nutrition status, livelihood satisfaction, mentality health, family harmony, medical examination and income. The main nutrition intake of the 95 cases were sufficient, such as energy, protein, but the intake of diet fat and salt were overtaken. The rate of overweight and obesity were 26.3% and 40.0%. Conclusion: Nutrition status is the main influence factor of QOL in the elderly. To improve the QOL of the elderly, health education and balance nutrition guidance should be strengthened.
4.The clinical value of MSCT in diagnosis of internal abdominal hernia

Objective To investigate the clinical value of multi-slice spiral CT (MSCT)in diagnosis of internal abdominal hernia (IAH).Methods The MSCT findings in 1 5 patients with IAH confirmed by surgery were retrospectively analyzed.Results MSCT showed intestinal obstruction in 14 patients with strangulating intestinal obstruction in 8.The typical features of MSCT were as follows:translocated position of intestines (occupying effect),closed loop intestinal obstruction (gathered intestinal loops with different forms),assembled,expanded and translocated blood vessels of mesenteries,and thickened intestinal walls with ischemic edema and abnormal density.Surgical results showed paraduodenal hernia in 2,transomental hernia in 2,transmesenteric hernia in 5,ankylenteron strap hernias in 4,pericecal hernia in 1 and hernia through the Douglas fossa in 1,Intestinal obstruction in 14 and strangulating intestinal obstruction in 8 were also confirmed by surgery.The CT findings of IAH were correlated with the surgical results,and the accurate rate of grading of intestinal obstruction with CT was also consistent with the surgical results (Kappa=0.758,t=3.462,P=0.001).Conclusion IAH can be diagnosed accurately by MSCT in most patients,MSCT is helpful for guiding the surgical programs.
6.TEAS for prevention and treatment of orthodontic toothache and oral dysfunction: a randomized controlled trial.

OBJECTIVETo observe the clinical efficacy of transcutanclus electrical acupoint stimulation (TEAS) on prevention and treatment of orthodontic toothache and oral dysfunction.
METHODSA total of 85 patients of malocclusions in the preliminary diagnosis were randomly divided into a control group (20 cases), a psychological intervention group (22 cases), a medication group (20 cases) and a TEAS group (23 cases). Orthodontics treatment was given in all the groups. Patients in the control group received no further treatment; patients in the psychological intervention group received comprehensive psychological intervention, including cognitive education and music therapy; patients in the medication group received oral administration of ibuprofen; patients in the TEAS group received TEAS at Juliao (ST 3), Jiachengjiang (Extra) and auricular point Ya (LO1). The treatment was given twice a day, one in morning and one at night, for 7 days. The pain scores of orthodontic toothache and changes of oral dysfunction were observed in all groups.
RESULTS(1) At 5 time points from the 12th hour to the 4th day, the scores of spontaneous pain in TEAS group were lower than those in the control group (all P < 0.01); during the time points, the scores in TEAS group were lower than those in the psychological intervention group (P < 0.05, P < 0.01), which were similar to those in the medication group (all P > 0.05). (2) During the peak cycle of spontaneous toothache, the scores of irritation pain in TEAS group were significantly lower than those in the control group (all P < O.01), regardless of time-point statistics or general statistics; the scores of irritation pain in the TEAS group were also significantly lower than those in the psychological intervention group (all P < 0.01), which were similar to those in the medication group (all P > 0. 05). (3) Compared with control group, the grading of talking disorder in the remaining groups did not change significantly (P > 0.05). (4) Compared with control group, the grading of moderate-severe eating disorder in TEAS group was significantly reduced (P < O.05), which was not different from that in the medication group (P > 0.05). The differences of the grading of moderate-severe eating disorder were not significantly different between the psychological intervention group and control group (P > 0.05). (5) There were 3 cases of digestive system adverse reactions in the medication group.
CONCLUSIONTEAS can efficiently prevent orthodontic toothache and oral dysfunction, which is superior to psychological intervention and similar to medication. In addition, it can avoid possible side-effect of medication.

7.A rapid PCR method to determine the types of cyp2d6~*10 alleles

Aim To establish a new simple, easy and economical PCR method for determining cyp2d6~*10 allele and studying its distributive frequencies in Chinese population. Methods The new PCR are established and compared with the classical PCR-RFLP. The 224 samples have been determined with the new PCR.Results the results of the two methods go on the way. The allele distribution frequencies of cyp2d6~*10 resembles the reports. Conclusion the new method is proved to be accurate and convenient. It provides a genetics basis for the drug therapy with individuals in clinics.
8.Changes of Cardiac Function in CHF rats after Blocking the Renin Angiotensin-Aldosterone System at Different Sites

PurposeTo observe the effects of blocking the renin-Angiotensin-Aldosterone system at different sites in experimental congestive heart failure (CHF)rats. Methods Six weeks after coronary ligation, rats( n = 66)were randomized to benalapril, losartan, spironolactone, placebo, and treated for 6 weeks. Sham-operated rats( n = 18) served as controls. Six weeks later left ventricular hemodynamics were measured. Results In the hemodynamic variables, HR was not significantly different between groups. Compared four treatment groups with sham-operated group, LVEDP increased, LVPSP and ± dp/dtmax decreased (P<0.05). Compared with placebo group,all data in spironolactone group were not significantly different, LVEDP, mortality in benalapril group and in losartan group decreased, LVPSP and ± dp/dtmax in benalapril group and in losartan group increased(P < 0.05). ConclusionsAdministration of ACEI and Ag Ⅱ-AT1 antagonist are effective in preventing from cardiac function deterioration, while spironolactone treatments are not. Spironolactone has tendency in decreasing LVEDP.
9.Studies on HPLC fingerprint of Herba Rhodobryi Rosei

Objective To establish an HPLC fingerprint of Herba Rhodobryi Rosei.Methods The fingerprint chromatography has been determined by RP-HPLC.The analysis was carried out with Dikma ODS C18 column(250 mm?4.6 mm,5 ?m).The mobile phase were:0.5% HAcCN(A),0.5% HAcH2O(B);Elution method:0-50 min,A was 20%-55%;50-60 min,A was 55%-95%;60-85 min,A was 95%-100%;keeping 5 min.Flow-rate was 1.0 mL/min.Wavelength was 360 nm and temperature was 30 ℃.It was analysized with the Estimating System of Similarity of 2004A Version(the Country's Pharmacopeia Committee)on the Chinese Medicine Fingerprint Chromatography.Results The fingerprint chromatography of Herba Rhodobryi Rosei was estabilished.Conclusion The method can be used in quality control of Herba Rhodobryi Rosei with accuracy and better repeatability.
10.Determination of the Plasma Protein Binding Rate of Plasma Brucine in Rats by HPLC

Objective To study the protein binding rate of brucine in plasma of rats.Methods The plasma balance dialysis was used.HPLC was performed for the quantitative determination of brucine,and then the protein binding rate of brucine in rats plasma was calculated.Results The protein binding rate was 59.679 %,59.935 %,and 56.387 %when the concentration of plasma brucine was 2.5,1,and 0.4 ?g?mL-1 respectivley.Conclusion The HPLC method used to determine brucine is simple,rapid,and sensitive,and with good specificity,precision and accuracy.Brucine has medium capacity in binding to plasma protein.
