Objective To investigate the clinical application of ultrasound biomicroscopy in the treatment of congenital corneal opacities.Methods Medical records of 20 eyes (15 patients) with congenital corneal opacity treated at our hospital from July 2004 to November 2011 were retrospectively reviewed.Best corrected visual acuity testing,intraocular pressure testing,slit-lamp anterior segment examination,fundus examination,slit-lamp microscopic photography,B scan examination,and ultrasound biomicroscopy were performed for analysis of complications of congenital corneal opacity and selection of surgical approaches.Results The ultrasound biomicroscopic examination showed that 5 eyes had no Descemet's membrane and corneal endothelium,20 eyes had anterior synechia,5 eyes had aniridia,3 eyes had loss of lens cortex,13 eyes had cataract,14 eyes had closed angle,and 3 eyes had pupillary membrane.14 of 20 eyes received surgical treatment,including penetrating keratoplasty combined with cataract extraction and trabeculectomy (5 eyes),penetrating keratoplasty combined with pupil angioplasty (3 eyes),penetrating keratoplasty combined with cataract extraction (3 eyes),penetrating keratoplasty combined with trabeculectomy (2 eyes),and lamellar keratoplasty (1 eye).Conclusions Ultrasound biomicroscopy is important to guide the diagnosis and treatment of congenital corneal opacity.

Objective To explore the effect of different position replacement orders on single side effect of blue light treatment of neonatal jaundice.Methods 100 cases newborns requiring phototherapy were randomly divided into the control group and the observation group.The control group adopted conventional phototherapy position change sequence (horizontal position-lateral position-prone position),the observation group used the new position replacement order (prone position-lateral position-horizontal position).The resistance reaction of babies and treatment effect of phototherapy were observed in two groups.Results Significant difference existed in resistance reaction (cry loudly,milk refusal)and phototherapy effects between the observation group and the control group.Conclusions The position change order such as prone position-lateral position-horizontal position by neonates during phototherapy,can reduce resistance reaction and improve treatment effect.

Objective To evaluate the value of tomographic ultrasound imaging(TUI)in the fetal heart screening.Methods One hundred and ten volume datasets acquired from normal fetal heart in the second trimester with spatio-temporal image correlation(STIC) were evaluated offline with TUI. The Bmode and CDFI static images as well as the dynamic loops of one cardiac eycle could be reviewed simultaneously. The visualizing rate and ultrasonic scores of the 4 chamber view,outflow tracts and 3 vessels (and trachea)view obtained from TUI and routine ultrasonography for fetal heart screening were respectively compared. Results Among all 110 cases,the visualizing rate of 4 chamber view,outflow tracts and 3 vessels(and trachea) view were respectively 100%(67),88.9%(45),80.0%(45),84.4%(32)in Bmode and 100%(33),86.2%(25),50.0%(14),100%(4)in CDFI under the same fetal Dositi.n.No significant differences of the visualization rate were found between TUI and routine screening either in Bmode or in CDFI(P＞0.05). The median score of 4 chamber view of TUI in B-mode was little higher than that in routine screening,while the right outflow tract of screening in CDFI significantly higher than that of TUI(P＜0.05). Conclusions The visualizing rates and qualities of TUI are sufficient for fetal heart screening offline as well as routine ultrasonography online in second trimester.

Objective To provide basis for routine fetal cardiac screening by comparing the ultrasound detection rate of fetal heart defects between the high-risk and low-risk population.Methods According to the questionnaire about maternal risk factors,all 7165 pregnant women were divided into two groups.The high-risk group included 2147 cases and the remained 5018 were considered as low-risk population.Routine fetal heart screening was performed by the four-chamber view,left and right outflow tract views.If some cardiac malformations were found,the detailed fetal echocardiography would be performed.Both the detection rate of fetal cardiac defects and distribution of the relevant risk factors were analyzed.Results Ninty-five(1.33%)cases of congenital heart disease were found in our study,32(1.49%)cases from highrisk group and 63(1.26%)cases from low-risk group.No significant was found between the two groups (X2=0.635,P＞0.05).Karyotyping were performed prenatally in 23 of 95 fetal congenital defects cases by cordocentesis and chorionbiopsy,and 11(47.83%)cases of concomitant chromosomal abnormality were found.Conclusions Routine fetal screening for congenital fetal diseases should be performed in non-selected population for the similar detection rate of fetal defect in high-risk and low-risk group.

Objective To study the structural features and characteristics of a novel gene Schistosoma japonicum 79(Sj79), and observe its effect of RNA interference(RNAi),so as to provide the experimental basis for its further function study and mechanism study of anti reproductive development of schistosome. Methods The gene structure and characteristics of Sj79 were analyzed by bioinformatics methods. Then the expressions of Sj79 messenger RNA(mRNA)during the different develop?mental stages of schistosome were analyzed and the effects of RNAi silencing were observed by the soaking method. The tran?scriptional levels of Sj79 after RNAi were detected by real time PCR. Results The open reading frame of Sj79 contained 696 base pairs with an exon structure. The gene had obvious stage specificity,and its transcriptional level in mature female worms was the highest. After soaking for 3 d,the Sj79 mRNA level[(41.0 ± 12.3)%]in the siRNA?1 group with low dosage(20 nmol/L) was lower than that in the siRNA?NC group[(103.2 ± 14.4)%],the difference was statistically significant(t=3.28,P<0.05). When with high dosage(200 nmol/L ),both the Sj79 mRNA levels in the siRNA?1 group[(15.8 ± 10.9)%]and siRNA?2 group [(11.1 ± 8.8)%]were significantly lower than that in the siRNA?NC group[(100.1 ± 6.3)%](t=13.44,27.84,both P<0.01). ObjectiveTo find out if the immune system derived thyroid stimulating hormone(TSH) β splice variant(TSHβ-Ⅴ) would be regulated by circulating thyroid hormone levels to get a further understanding of the function and mechanism of this TSHβ-Ⅴ in thyroid homeostasis.MethodsA total of 20 weaning Balb/c mice (half male and half female) were selected and randomly divided into two groups according to their body mass and gender(n =10).Mice of control group were fed with common diet and deionized water.Mice of the low-iodine(LI) group were fed with low-iodine diet(containing iodine 20 - 40 μg/kg,iodine-intake about 0.25 μg/d) and deionized water.The experimental period was 3 months.At the end of the experiment,mice were executed and the blood was collected to observe the levels of TSH and thyroid hormone by chemiluminescence immunoassay (CIA) ; bone marrow (BM),peripheral blood(PBL),thyroid gland and pituitary were collected to assay the TSHβ-Ⅴ mRNA expression by real-time quantitative reverse transcriptase polymerase chain reaction (RT-PCR).ResultsThe serum free thyroxine(FT4) and total thyroxine(TT4) levels in LI group of mice[(0.47 ± 0.70)nmol/L,(2.41 ± 0.28)pmol/L] were significantly lower than that of the control group of mice [(55.2 ± 3.68) nmol/L, (32.72 ± 1.02) pmol/L,t =43.81,86.04 、all P ＜ 0.01 ] and the serum total triiodothyronine(TT3) and free triiodothyronine(FT3) reduction in LI group of mice[ (0.76 ± 0.08)nmol/L,(4.01 ± 0.40)pmol/L] were significantly lower than that of the control group of mice [ (1.10 ± 0.06)nmol/L,(5.40 ± 0.38)pmol/L,t =9.81,7.5 1,P ＜ 0.01 ].Iodine insufficiency strongly elevated the serum TSH in LI group of mice[ (35.67 ± 17.39)mU/L] than that in control group of mice[ (0.24 ± 0.10)mU/L,t =- 6.11,P ＜ 0.01 ].The mRNA levels of TSH β-Ⅴ in BM (9.62 ± 0.60) and in PBL( 9.25 ± 0.83 ) of LI group of mice were lower than those in control group of mice (7.69 ± 0.36,7.11 ± 0.41,t =6.77,5.64,P ＜ 0.01),while the mRNA level of TSH β-Ⅴ in pituitary of LI group of mice (1.99 ± 0.61) was increased compared with that in control group of mice (5.75 ± 0.98,t =- 8.02,P＜ 0.01).Compared with control group of mice(9.12 ± 0.62),the level of thyroid TSH β-Ⅴ mRNA in LI group of mice (9.32 ± 0.91 ) was not significantly changed (t =0.45,P ＞ 0.05).There was no detectable native TSHβ in BM,PBL and thyroid.The mRNA level of native TSHβ in pituitary in LI group of mice( - 7.17 ± 1.78) was dramatically elevated compared to that in control group of mice( - 1.43 ± 0.51,t =- 7.60,P ＜ 0.01 ).ConclusionsThe mRNA levels of TSHβ-Ⅴ are suppressed in BM and PBL in low iodinediet induced hypothyroidism mice,which suggest that immune system derived TSHβ-Ⅴ may be more important thannative TSHβ in immune-thyroid regulation.
