1.The effect of combination of Beyond cold light whitening and fluoride-removing material bleaching system for dental fluorosis

Objective:To evaluate the clinical efficacy of combination of Beyond cold light whitening and fluoride-removing material bleaching system for treatment of dental fluorosis. Methods: Seventy-five patients with light, medium and heavy dental fluorosis were randomly divided into three groups. Twenty-five patients with 150 dental fluorosis from the experiment group were treated with Beyond cold light bleaching combined with fluoride-removing material;Twenty-five patients with 148 dental fluorosis from the control group one were treated with Beyond cold light bleaching. Twenty-five patients with 148 dental fluorosis from the control group two were treated with fluoride-removing material. The changes of teeth color were evaluated by Vita shade guide matching after treatment. Results: All of the light dental fluorosis in three groups had good lightening efficacy and 100% bleaching efficacy. The Shade value improvement and overall efficacy of observer group were better than the control groups for the medium and heavy fluorosis(P<0.05). Conclusion: Beyond cold light whitening combined with fluoride-removing material in bleaching is a good method for dental fluorosis bleaching.
2.Histone deacetylase inhibitor LBH589 induces apoptosis of multiple myeloma cell line U266 in vitro and its mechanism

Objective To study the effect of new generation histone deacetylase inhibitor LBH589 single-drug or combined with the mouse serum which contains the radix echinopsis on multiple myeloma (MM) cell line U266 and their mechanism.Methods The acetylation level of α-tublin which was a substrate of HDAC6 was detected by Western blot.The chemical force between Heat shock protein 90 (HSP90) and its client proteins was detected by immune precipitation (IP).Results The different concentrations of LBH589 single drug (0,20,50 nmol/L),and 50 nmol/L combined with the mouse serum which contained the radix echinopsis (1 g/ml) were able to inhibit the proliferation of U266 cell.With the increase of drug concentration and the extension of time,the acetylation levels of α-tublin and HSP90 increased gradually (at 24 or 48 hours) in a dose dependent (P < 0.05).The inhibition of LBH589 combined with the mouse serum was stronger than that of LBH589 single drug (P < 0.05).Conclusion LBH589 could inhibit the growth of MM cells and their cell cycles,and induce the apoptosis of MM cell line U266.

5.Models and methods of failure analysis and risk assessment during medical treatment

BACKGROUND:Medical risks are al unsafe events or that damage to the patient during medical services. Present medical risk management is mainly qualitative experience, and lacks of regular failure analysis and risk assessment for established medical treatment.
OBJECTIVE:To construct models of medical process failure analysis and risk assessment, find possible risks during medical treatment, and propose effective measures to eliminate or decrease above risks.
METHODS:Failure mode and effects analysis during reliability engineering were used in production. That was, risk assessment was conducted in the possible technical failure modes, causes and al impacts on the product during each process. Improvement measures were made for weak link during the process. The risk could reach an acceptable level. In accordance with failure mode and effects analysis during production, the procedure of medical process failure analysis and risk assessment could be made to analyze the potential failures during medical treatment. Moreover, the improvement measures were proposed for weak link with high risks so as to prevent the occurrence of risk of significant adverse effects on patients.
RESULTS AND CONCLUSION:The methods and basic procedures of medical process failure analysis and risk assessment were established by using the experience of failure mode and effects analysis. Taking the rescue process of myocardial infarction in the emergency of a hospital as an example, the analysis of failures, reasons and impacts was performed taking“chewing 300 mg aspirin”in the rescue steps as a key. The improvement measures and suggestions were proposed for unacceptable failures and reasons. Seen from the analysis results, proposed improvement measures and suggestions can obviously decrease the risks of failures caused by this step to patients. Therefore, the application of failure mode and effects analysis in medical treatment has a strong practical value.
6.Effect of Electroacupuncture on Experimental Epilepsy: Roles of Different Acupoints and Stimulation Parameters

Objective: The strategies against epilepsy are currently very limited. The author performed this work to determine the effect of electroacupuncture (EA) on epileptic seizures and define the optimal EA conditions (acupoints and stimulation parameters). Methods: Experimental epilepsy was induced by injecting kainic acid into the lateral cerebral ventricle of adult male SD rats. EA with a current of 1 mA/100 Hz or 1 mA/10 Hz was applied to the model starting at 0.5 h after the injection for 30 min. Four pairs of acupoints were tested, including Renzhong (GV 26) + Dazhui (GV 14), Jinsuo (GV 8) + Yaoqi (Ex-B 9), Neiguan (PC 6) + Quchi (LI 11) and Fenglong (ST 40) + Yongquan (KI 1). Results: Behavioral scores showed that (1) either high (100 Hz) or low (10 Hz) frequency EA at different acupoints attenuated epileptic seizures (P<0.05 or 0.01 vs. the control), except EA at Neiguan (PC 6) + Quchi (LI 11) with 1 mA/10 Hz current; (2) EA with low frequency stimulation (10 Hz) induced a better effect at Fenglong (ST 40) + Yongquan (KI 1) than other acupoints (P<0.05 or 0.01); (3) there was no significant difference in EA effects induced by various acupoints when exposed to high frequency EA (100 Hz); and (4) high frequency EA (100 Hz) elicited a better effect than low frequency EA (10 Hz) in all groups (P<0.05 or 0.01) except for that of Jinsuo (GV 8) + Yaoqi (Ex-B 9). EEG showed the similar tendency, i.e., EA generally reduced the amplitude and frequency of epileptic discharges. Conclusion: Electroacupuncture attenuates epileptic seizures, which is relatively specific to EA parameters and acupoints.
7.CD40 expression in septic neutropenic rats with acute lung injury

Objective To study the activation of macrophage and upregulation of CD40 in neutropenic and nonneutropenic lipopolysaccharide (LPS)-induced acute lung injury (ALI).Methods Forty mice were randomly divided into four groups (neutropenic septic group,neutropenic control group,non neutropenic septic group and normal control group).The pathologic changes of mice lungs were observed after hematoxylin and eosin staining.CD40 expression was determined by flow cytometry.Cytokine tumor necrosis factor-α levels from bronchoalveolar lavage fluid (BALF) and serum were measured by ELISA.Results The average weight and peripheral blood white cell counts were increased significantly in the neutropenic group as compared to the non neutropenic group before the rats were sacrificed [(181.13 ± 16.94) g vs (168.56 ±5.03) g,(9.54 ± 1.25) × 109/L vs (2.56 ± 1.12) × 109/L,P <0.05,respectively].The cell counts in BALF were higher in neutropenic septic and non neutropenic septic groups compared to the normal control group [(3.33 ±0.26) × 106/L,(3.23 ± 0.06) × 106/L,(1.68 ± 0.23) × 106/L,P < 0.05].The concentration of tumor necrosis factor-α from serum were increased significantly in neutropenic septic and non neutropenic septic groups as compared to the normal control group [(2 337.08 ± 1 148.85) pg/ml,(2 274.01 ± 1 569.37) pg/ml,(96.20 ± 93.69) pg/ml,P < 0.05].The CD40 molecular expressions were higher in neutropenic septic and non neutropenic septic groups compared to the normal control group [(42.06 ± 11.90) %,(46.38 ±7.42) %,(21.62 ± 4.86) %,P < 0.01].Conclusion Upregulation of CD40 molecule induces the inflammatory cytokines releasing,then leads to ALI.Therefore both the inflammatory cytokines and CD40 play vital role in the pathogenesis of ALI.

Objective To investigate the effect of dexmedetomidine on the renal function and the expression of tight junction protein ZO-1 and occluding in the kidney of rats with septicopyemia induced by lipopolysaccharide.Methods Thirty adult male SD rats were randomly divided into 3 groups ( n=10 each):control group( group C) , LPS group ( group M) , and dexmedetomidine group ( group Dex ) .In the groups Dex and LPS, the rats were infused with saline and lipopolysaccharide (5 mg/kg, i.v.) respectively.In the Dex group, after intravenous injection of lipopolysaccharide (5 mg/kg) , the rats were firstly infused the loaded dose of dexmedetomidine (7μg/kg) for 15 minutes, and then changed to 5 μg/kg· min for 30 minutes.Blood samples were collected at 24 h later for measuring TNF-α, IL-1β, Scr and BUN.The kidney tissues were examined by histopathology.The expression of ZO-1 and occludin was detected by Western blot. Results Compared with the group C, the kidney tissue of group M was extensively damaged with tubular dilatation and inflammation, while reduced in the group Dex.Compared with the group C, the expressions of Scr, BUN, IL-1βand TNF-αwere all enhanced in the groups M and Dex ( P <0.05 ) , while the inflammatory factors in the group Dex were significantly lower than those of the group M ( P <0.05 ) .The Results The Western blot analysis showed that the expressions of protein ZO-1 and occludin in the group Dex were significantly higher than those of the group M (P<0.05). Conclusions Dexmedetomidine can improve the renal function of rats with septicopyemia, inhibit the acute renal injury and inflammation, increase the expression of protein ZO-1, and exert certain protective effect on the kidneys.
