BACKGROUND:It is controversial whether bone marrow mobilization can retain in cardiac injured position in congestive cardiomyopathy or differentiate into cardiomyocytes and vascular endothelial cells.
OBJECTIVE:To study the effects of granulocyte colony stimulating factor (G-GSF) on myocardium and angiogenesis in rats with congestive cardiomyopathy.
METHODS:Fifty Wistar rats with heart failure caused by adriamycin-induced cardiomyopathy were divided into heart failure group (n=20) treated with normal saline and bone marrow mobilization (n=30) treated with subcutaneous injection of recombinant human G-GSF. Ten rats from the bone marrow mobilization were kil ed at day 6 of mobilization, and myocardial tissue was taken for CD43 immunofluorescent staining. Blood samples were taken from the rat tail in each group before and 5 days after treatment to count total number of white blood cells and percentage of mononuclear cells. Meanwhile, mononuclear cells extracted from the peripheral blood were used for flow cytometry detection. At day 5 after treatment, bromodeoxyuridine (BrdU, 50 mg/kg) was successively given to al rats for 4 weeks before they were sacrificed. Myocardial tissues were taken to determine the homing of mononuclear cells and evaluate differentiation of mononuclear cells into cardiomyocytes and vascular endothelial cells using BrdU staining, BrdU/myosin heavy chain double staining, and BrdU/actin double staining. Hematoxylin-eosin staining was used for determination of blood vessel density.
RESULTS AND CONCLUSION:G-CSF mobilization increased the number of mononuclear cells that was significantly higher than before treatment (P<0.05). Flow cytometry showed that the number of CD34-positive mononuclear cells in the peripheral blood was higher in the bone marrow mobilization than in the heart failure group (P<0.05). Myocardial CD34 immunofluorescence showed that the heart failure group was negative and the bone marrow mobilization group was positive. In the bone marrow mobilization group, the myocardial tissue was positive for BrdU staining, BrdU/myosin heavy chain double staining and BrdU/actin double staining, while vascular endothelial cells in the region of myocardial injury was positive for BrdU;conversely, the heart failure group was negative. The density of blood vessels in the bone marrow mobilization group was significantly higher than that in the heart failure group (P<0.001). These findings indicate that bone marrow mobilization increases the number of mononuclear cells, and these cells are homing to myocardial injury, thereby playing a repair role in the myocardium and vascular tissue of heart failure rats with congestive cardiomyopathy.

BacKground:Helicobacter pylori ( Hp ) infection is an important etioIogicaI factor of gastric uIcer. AIthough eradicating Hp can heIp gastric uIcer heaIing,there is stiII a high recurrence rate of gastric uIcer. Therefore,other factors pIaying a roIe in the pathogenesis of gastric uIcer shouId be in concern. Aims:To investigate the non-reinfectious recurrence factors of gastric uIcer after Hp eradication. Methods:A totaI of 164 newIy diagnosed gastric uIcer patients with Hp infection from January 2011 to June 2012 at Ren Ji HospitaI,SchooI of Medicine,Shanghai Jiao Tong University were enroIIed. AII patients were given Hp eradication therapy and treatment of gastric uIcer. Patients with successfuI Hp eradication and heaIing of gastric uIcer were foIIowed up at 3,6,12 months after uIcer heaIing. Suspected risk factors of gastric uIcer recurrence were coIIected,and their reIationships with recurrence were anaIyzed. Results:A totaI of 123 patients positive for Hp infection compIeted the 12 months foIIow-up,gastric uIcer recurred in 18(14. 6%)of them. Chi-square test reveaIed that age,gender,smoking/drinking,non-steroidaI anti-infIammatory drugs( NSAIDs ),negative emotions and unheaIthy eating habits were significantIy associated with the recurrence of gastric uIcer(P<0. 05). Of the unheaIthy eating habits, irreguIar eating, and preference of indigestibIe food and fast food/carbonated drinks were significantIy associated with recurrence of gastric uIcer( P <0. 05 ). Conclusions:Patients with gastric uIcer shouId change their unheaIthy Iiving habits,foIIow rationaI use of drugs,and keep heaIthy mentaI status and eating habits for avoiding the recurrence of gastric uIcer.

Objective To investigate the post traumatic stress disorder scores of undergraduate and the correlation between emotional approach coping(EAC) in unaffected region after Wenchuan-earthquake.Methods PTSD Check list-Civilian Version (PCL-C) and emotional approach coping scale(EACS) were used to assessed 880 undergraduates in Nanjing. Results (1)The average score of PCL-C was 30.52±9.64, and the prevalence rates of PTSD was found 5.3%.(2)Gender difference were found in social function(male:1.66±0.80,female:1.52±0.63, t =2.664,P =0.005) and positive scores of PTSD(male:31.32±10.81,female:29.81±8.41,t =2.263,P =0.022).(3)No significant difference was found in different specialty between PTSD,but in emotional procession and emotional expression.(4)Emotional-approach coping and expose time in front of media came into the regression equation of PTSD symptoms.Conclusion The score of PCL-C of undergraduate in unaffected region are infected by how long he spend in front of media and how he use emotion-approach coping strategy to cope with this shock bringing by Wenchuan-Earthquake

Abstract: Objective To explore the relationship between peripheral blood and pleural effusion tuberculosis (TB) infection effector T cells, and to further evaluate the value of combined pleural effusion adenosine deaminase (ADA) for rapid diagnosis of tuberculous pleurisy. Methods The test data of 80 cases of tuberculous pleurisy and 70 cases of nontuberculous pleurisy treated in the Sixth People's Hospital of Nantong City from January 2017 to December 2020 were analyzed. The TBinfected effector T cells were also detected simultaneously in the peripheral blood and the pleural effusion by the T-SPOT technique, and the pleural effusion ADA was detected by the rate method. The subject operating characteristic curve (ROC) was applied to take the optimal pleural effusion ADA threshold to compare the sensitivity and specificity of different critical values. Person phase analysis was applied to analyze the correlation between peripheral blood and pleural effusion T-SPOT.TB. Data of peripheral blood, pleural effusion T-SPOT.TB and ADA were integrated. Results When pleural effusion ADA>45 U/L, the sensitivity and specificity for the diagnosis of tuberculous pleurisy were 50.0% and 94.3%, respectively; when ADA > 25.15 U/ L, the sensitivity and specificity were 80.0% and 72.9%. When ADA > 45 U / L, pleural/ blood T-SPOT.TB spot ratio (spot forming cells, SFCs) > 2 times, the specificity for the diagnosis of tuberculous pleurisy was 100% (highest); when 25.15 U/L< pleural effusion ADA ≤ 45 U/L, pleural/blood T-SPOT.TB spot ratio > 2 times, the specificity for the diagnosis of tuberculous pleurisy was 92.3% (second). When pleural effusion ADA ≤ 25.15 U/L, and the pleural effusion/blood T-SPOT.TB spot number ratio > 2 times, with 83.3% specificity (the lowest of the three groups). Conclusions The level of pleural effusion ADA is one of the most used methods for diagnosing tuberculous pleurisy. Further combination of pleural effusion and blood T-SPOT.TB, if the ratio of pleural effusion / blood T-SPOT. TB spots is greater than 2 times, it can further improve the diagnosis rate of tuberculous pleurisy.

Objective To use the real-time shear wave elastography(SWE)to detect the elasticmodulus values of different breast lesions,and to assess its application value in differential diagnosis of benign and malignant breast lesions.Methods SWE was per-formed in 71 breast lesions from 63 pathologically confirmed patients.The maximum,minimum and mean elasticmodulus values in breast lesions were detected and the ROC curves were used to determine the optimal diagnostic cut off value.Results Among 7 1 le-sions,48 cases were benign lesions and 23 cases were malignant lesions.The diagnostic accuracy and Youden index of the ordinary ultrasound were 77.5% and 0.55 respectively.The maximum,minimum and mean elasticity values for malignant lesions were (106.7±37.9),(23.7±6.4),(44.8±6.6)kPa respectively,whereas which for benign lesions were(42.7±14.6),(17.2±6.3), (29.4±8.0)kPa respectively.The maximum,minimum and mean elastic values had statistically significant differences between be-nign and malignant lesions(P<0.05).The areas under the curve(AUC)of the maximum,minimum and mean elastic values were 0.963,0.798 and 0.914 respectively.Taking 78.1 kPa as the threshold of the maximum elasticity value,the sensitivity was 82.6%and the specificity was 95.8%.Taking 37.6 kPa as the threshold of the mean elasticity value,the sensitivity was 87.0% and the specificity was 83.3%.Conclusion SWE provides a new way for the differential diagnosis of benign and malignant breast lesions, and the maximum and mean elastic value can be used as the diagnostic evidence for differentiating benign and malignant breast le-sions.

Objective To establish the norm of occupational stress on the nurse in China. Methods Used stratified random cluster sampling,a total of 3091 nurses in 21 from China' s eastern,central and western parts of three regional hospitals, hospital were tested by Chinese nurse stressor scale (CNNS). Results ①The total norms for the CNNS and each factor are established. ②Descriptive statistics for CNSS for different regions samples, different working year' s samples,different marital status samples,different titles samples,different age's samples, different education background samples and different work nature samples were modulated. Conclusion The norm established can basically represent the occupational stress on nurses in China.

Objective To evaluate the effects of silicone pessary treatment on relieving the symptoms and improving the life quality of the pelvic organ prolapse patients.Methods From November 2005 to March 2009,33 pelvic organ prolapse (POP-Q stage Ⅲ～Ⅳ) patients received silicone pessary coneervative treatment were followed up and required to complete the Pelvic Floor Distress Inventory-20 (PFDI-20) before and after initiating pessary treatment.Results 31 of 33(93.94%) patients finished the follow-up completely.23 of 31(74.19%) patients continually used the pessary.The follow up time was 3 to 17 (10.04±2.57) months.27 patients completed the PFDI-20 questionnaires.Before silicone pessary treatment, PFDI20 total score,POPDI6 (Pelvic organ prolapse distress inventory),CARDI8 (colorectal anal distress inventory 8) and UDI6 (urinary distress inventory 6) scores were (54.16±36.12),(27.78±17.30),(0.61±1.01) and (25.77± 24.10),respectively.After the treatment of pessary,the scores decreased to (20.20±25.98), (4.48±5.88), (0.45±0.84) and (15.28± 21.53),respectively.Except for CARDI8,the PFDI scores decreased significantly after the pessary treatment (P＜0.05).Conclusion Silicone pessary is a viable noninvasive treatment for pelvic organ prolapse.It could relieve the symptoms and improve the life quality of protrusion and voiding dysfunction patients.

Objective To explore the association of dietary behavior of children with Tourette syndrome (TS) and tic symptoms.Methods 207 TS children and their 264 corresponding controls,who visited our hospital during the period of November 2008 to October 2010,were investigated with children' s dietary behavior questionnaire,under the guidance of professional staff,and the TS tic symptom severity was also evaluated according to The Yale Global Tic Severity Scale (YGTSS).Kruskal-Wallis H rank-sum test was applied for univariate analysis and multinominal logistic regression for further multivariate analysis,with values of odds ratio (OR) and population attributable risk (PAR) obtained to demonstrate the relation strength between dietary behavior and tic symptom severity.Results Results of univariate analysis showed that western fast meal,barbecues,cream food,cold food,and spicy food were related to TS tic symptom severity (P＜0.05).Results of multivariate analysis demonstrated that western fast meal,fruits and vegetables,cream food and spicy food were risk factors for mild TS compared with control group,with OR and 95％ CI of 3.282 (1.922,5.064),2.239 (1.298,3.861),2.341 (1.355,4.046),2.118 (1.327,3.380) and their corresponding PAR of 0.306,0.464,0.169,0.250 respectively.As to moderate and severe TS,the risk factors included western fast meal,fruits and vegetables,and spicy food,with their respective OR and 95％ CI of 2.581 (1.322,5.038),2.364 (1.166,4.795),1.822 (1.014,2.272) and PAR of 0.234,0.487,0.197.Conclusion Dietary behavior,especially western fast meal,fruits and vegetables,cream food and spicy food,are considered to be associated with TS tic symptom severity.Therefore it' s obligatory to rectify undesirable dietary behaviors for TS children.

Objective To evaluate the complications after trans-vaginal mesh-augmented pelvic floor reconstruction in treatment of pelvic organ prolapse (POP).MethodsFrom February 2007 to October 2009,vaginal mesh procedures were performed on 91 women with POP stage Ⅲ-Ⅳ in Pekiug University Third Hospital.The operative complications were studied.Results Ninety patients underwent successful surgery among 91 patients.Follow-up rate was 94％ (85/90) at a median follow-up of 28.4 ( 15 -44) months.One patient underwent intraoperative organ injuries,and 10 patients had postoperation mesh-related complications.The rate of mesh-related complications was 2％ ( 2/85 ),2％ ( 2/85 ),4％ ( 3/85 ),4％ ( 3/85 ) on 6,6 - 12,12 -24 and more than 24 months following up,respectively.Seven patients underwent conservative treatment and the symptoms were improved.Three patients underwent the second surgery,and the symptoms were cured or relieved.Conclusion The incidence of mesh-related complications was low,and interventions were effective in vaginal mesh procedure.
