1.Research progress on the relationship between Helicobacter pylori infection and the risk of coronary heart disease

Helicobacter pylori (Hp) can be colonized in human gastric mucosa for a long time and is related to various gastrointestinal diseases. At the same time, it may also participate in the occurrence of nervous system, blood system, endocrine and cardiovascular diseases. Coronary heart disease (CHD) is a common clinical cardiovascular system disease, which is characterized by the formation of coronary atherosclerotic plaques and myocardial ischemia damage. In serious cases, it may lead to myocardial infarction and heart failure, with a high risk of death and disability. Hp infection can play an important role in the occurrence and development of CHD through inflammation and immune response, endothelial dysfunction, hyperhomocysteinemia, dyslipidemia, metabolic syndrome, abnormal glucose tolerance, oxidative stress, and cytotoxic factors. In-depth research on the relationship between Hp infection and CHD is not only beneficial to improve clinical treatment regimen for Hp, but also helps to reduce the risk of Hp infection-related CHD, which provides reference for clinical treatment of the disease.
2.Correlation between severity of diabetic foot disease and serum UA, Hcy, HbAlc and LDL-c levels in Nanjing Area

Objective To analyze the correlation between the severity of diabetic foot (DF) and the levels of serum UA,Hcy, HbAlc and LDL-c in Nanjing area, and to provide a basis for evaluating the severity of DF. Methods A total of 628 T2DM patients in Nanjing area from June 2019 to June 2020 were selected as subjects. According to whether patients had DF, they were divided into control group (n=502, without DF) and observation group (n=126,with DF). DF patients were divided into 6 grades (0-5) according to Wagner grading method. There were mild (0-1 grade, n=68), moderate (2-3 grade, n=42) and severe (4-5 grade, n=18). Serum UA, Hcy, HbAlc and LDL-c levels of diabetic foot patients in different severity groups were compared. ROC curve was drawn to analyze the efficacy of Hcy, HbAlc and LDL-C in the differential diagnosis of diabetic foot. Spearman correlation was used to analyze the correlation between the severity of diabetic foot and serum UA, Hcy, HbAlc and LDL-C levels. Results The serum levels of UA, Hcy, HbAlc and LDL-c in observation group were significantly higher than those in control group (P<0.05). The serum levels of UA, Hcy, HbAlc and LDL-C in severe group were significantly higher than those in mild and moderate groups ( P<0.05). The serum levels of UA, Hcy, HbAlc and LDL-c in moderate group were significantly higher than those in mild group (P<0.05). Spearman correlation analysis showed that DF illness severity and UA, Hcy, HbAlc, and there was a positive correlation of LDL-c (r=0.457 , 0.468 , 0.511 , 0.492 , P<0.05 ) ; ROC curve showed that the sensitivity and specificity of the combined detection of serum UA, Hcy, HbAlc and LDL-c in predicting the severity of DF patients were 95.65% and 96.78% respectively. Conclusion: Serum UA, Hcy, HbAlc and LDL-C of DF patients in Nanjing were significantly correlated with the severity of the disease, which has important clinical value for monitoring the progress of DF patients.
3.Mental health status and influencing factors of permanent residents in Yichang, Hubei Province

Objective To investigate the characteristics and influencing factors of mental health status of permanent residents in Yichang, Hubei Province. Methods A total of 9 576 permanent residents aged 18 years and older from Yichang City were selected by a multistage random sampling method between June and October 2022. The PHQ-9 was used to assess the residents’ depressive symptoms, the GAD-7 was used to assess their anxiety symptoms, the ISI was used to assess their insomnia status, and the PCL-5 was used to assess their stress status. The influence factors of depression and anxiety were analyzed using χ2 test and logistic regression. Results A total of 9 122 valid questionnaires were completed. The detection rate of depression, anxiety, insomnia, and stress symptoms were 29.98%, 19.03%, 11.97% and 1.58%, respectively. Gender, education level, monthly family income, self-rated health status, mental health literacy level, total GAD-7 score, total ISI score, and total PCL-5 score were the main factors that caused residents' anxiety symptoms, while gender, education level, self-rated health status, total PHQ-9 score, total ISI score, and total PCL-5 score were the main factors that caused residents' anxiety symptoms. Conclusion The prevalence of depression and anxiety is high among the permanent residents in Yichang, while the situation of insomnia and stress is relatively good. Measures such as improving the level of mental health literacy can be taken to improve mental health level of residents in Yichang.
4.Cancer screening and its association with core knowledge of cancer in community residents

Objective To understand the situation of cancer screening of community residents and its association with the knowledge of cancer core knowledge, and to provide reference for the formulation of secondary cancer prevention measures. Methods A cross-sectional survey was conducted among 2 037 residents aged 18-79 who lived in Bao 'an District, Shenzhen. Multivariate Logistic regression analysis was used to investigate the relationship between core knowledge and cancer screening behavior after adjusting for age, gender, education, marriage, occupation and smoking. Results The cancer screening rate of community residents was 28.10%, and the screening rate of middle-aged and elderly people aged 40-79 was 33.50 %.The proportion of “not very well understanding”, “partial understanding” and “basic understanding” of the core knowledge of cancer were 43.91%, 42.33% and 13.76%, respectively, which was higher than that of the non-participants. Multivariate analysis showed that the women, the higher the level of education of college/university or above and the higher the level of knowledge of cancer prevention core knowledge, the higher the probability of participating in cancer screening was OR (95%CI ) : 2.40 (1.86-3.12), 1.65 (1.29-2.10), 1.38 (1.18 - 1.62), respectively. Conclusion The proportion of cancer screening in community residents needs to be improved, and the degree of core knowledge of cancer is closely related to cancer screening behavior. It is suggested to increase the publicity and education of the core knowledge of cancer, and at the same time, carry out the health education of secondary cancer prevention for the key population such as men and people with low education level, so as to improve the proportion of “early detection, early diagnosis and early treatment” of cancer.
5.Analysis and prospect of free medication for severe mental disorders in Beijing

Objective To analyze the situation and changing trend of free medication service in Beijing from 2013 to 2020, and to propose policies and service improvement measures. Methods A retrospective survey was conducted in Beijing to collect and analyze the service subjects, service process, drug catalog, fiscal investment and patient participation of free medication in each district in recent years. Results Among the 16 districts of Beijing, 81.25% of the districts included the patients listed in Item (2) of Paragraph 2 of Article 30 of the Mental Health Law into the scope of free medication, and 75% of the districts broke the restriction that the service targets were registered residents in Beijing. 56.25% of the districts provided a variety of free medication modes, and 81.25% of the districts provided a mode of nearby community service. The catalog of free medications in various areas was still based on the Beijing catalog, and only 18.75% of the districts supplemented the list. In 2020, the total investment of free medicines and subsidies was 88.3768 million RMB, and the per capita investment was 1 616.02 RMB. The total cost showed an increasing trend year by year. From 2013 to 2020, the difference of per capita annual financial investment in drugs and subsidies was statistically significant (F=9.248, P<0.001). The per capita cost basically showed an increasing trend year by year except for individual years. In 2020, 54,688 patients participated in the free medication service in Beijing, and the participation rate was 67.23%. From 2013 to 2020, the difference of annual participation rate of patients was statistically significant (χ2=37098.759, P<0.001), and the number and proportion of patients increased year by year. Conclusion The practice of the district level has proved feasible to expand the scope of free medication service and to optimize the service process, which should be paid more attention in the future. With the continuous advancement of policies and services, the level of patient participation in various areas will tend to be relatively balanced after rising year by year.
