5.Research on expression of miRNA-21 in the peripheral blood of coronary heart disease and its clinical significance.

OBJECTIVETo study the expression difference in MicroRNA-21 (miRNA-21) levels of the plasma between the patients with coronary heart disease and the subjects without coronary artery lesions, and its clinical significance.
METHODSPlasma was obtained from the patients with coronary heart disease(trial group,56 cases) and the subjects without coronary artery lesions(control group, 10 cases), patients with coronary disease were divided into angina(AP, 39 cases) and acute myocardial infarction(AMI, 17 cases)subgroup, the contents of miRNA-21 were detected using qRT-PCR method, and the differential expression of miRNA-21 in each group was analyzed. The levels of creatine kinase isoenzyme (CK-MB), high sensitive troponin I(cTnI), B type natriuretic peptide urea (BNP), Gensini, left ventricular ejection fraction (LVEF), integral value of coronary left ventricular end diastole diameter (LV) and homocysteine (HCY) were determined and the correlation between miRNA-21 and these clinical indexes was analyzed.
RESULTSCompared with control group, there was a significant difference in expression of miRNA-21 in patients with angina and AMI (P < 0.05), and miRNA-21 expression in AMI group was much higher than that in AP group. There was statistical significance in CK, CK-MB, cTnI, Genisis integral comparison between AMI group and control group (P < 0.05). The correlation analysis showed that there was a positive correlation between expression of CK, CK-MB, cTnI and the level of circulating miRNA-21 in patients with acute myocardial infarction. And there was a negative correlation between BNP, Gensini integral, LVEF value of coronary artery, LV and circulating miRNA-21.
CONCLUSIONmiRNA-21 was significantly elevated in acute myocardial infarction subgroup than the control group. The level of miRNA-21 associates with the degree of coronary artery stenosis, and might be a potential marker for the diagnosis of acute myocardial infarction. miRNA-21 may play an important role in protecting myocardium from ischemia/reperfusion injury.

6.Correlation analysis of the severity of EEG and secondary epilepsy in elderly patients with cerebral infarction

Objective To investigate the correlation between EEG severity and secondary epilepsy in elderly patients with cerebral infarction.Methods 128 cases with cerebral infarction were selected as the research subjects. According to whether the patients happened secondary epilepsy,they were divided into the observation group (65 cases) and the control group (63 cases).The difference of EEG severity between the two groups was compared,and its correlation with secondary epilepsy was analyzed.Results There were 18 cases (27.69%)in the observation group graded as moderate differences,which was significantly higher than that in the control group of 12 cases (19.05%), the difference was statistically significant (χ2 =4.43,P =0.04).There were 23 cases (35.38%)in the observation group graded as severe differences,which was significantly higher than that in the control group of 9 cases (13.85%),the difference was statistically significant (χ2 =6.51,P =0.01 ).There were 21 cases(32.31%)of EEG changes of observation group being only diffuse abnormalities forms,which was significantly higher than that of the control group of 11 cases(17.46%),the difference was statistically significant (χ2 =4.18,P =0.04).There were 28 cases(43.08%)of EEG changes of the observation group being both focal abnormalities and diffuse abnor-malities forms,which was significantly higher than that of the control group of 14 cases(22.22%),the difference was statistically significant (χ2 =5.40,P =0.02).Compared open -closed test,hyperventilation provocation test results of the observation group being abnormal or not fit with the control group,the difference was not statistically significant (P >0.05).Conclusion According to the classification of EEG,EEG changes and EEG related tests to determine the severity of EEG.The more severe EEG,the greater the possibility of secondary epilepsy happen in patients with cerebral infarction.
7.Optimization of the Soxhlet Extraction Technology of Atractylodes macrocephala Formula Granules by Or-thogonal Test

OBJECTIVE:To optimize the soxhlet extraction technology of Atractylodis macrocephalae,and to provide evi-dence for research and preparation of its formula granules. METHODS:Using the contents of atractylenolide Ⅰ,Ⅱ,Ⅲ as index, based on single factor test,the Soxhlet extraction technology of A. macrocephalae formula granules was optimized and verified by L9(34)orthogonal test with extraction time,solid-liquid ratio,extraction times as factors,and then compared with other technolo-gies (normal temperature extraction method,ultrasonic extraction method,reflux extraction method). RESULTS:The optimal ex-traction technology was as follows as 6-fold ethanol,extracting for 3 times,lasting for 8 h. Results of validation test showed that the extraction amounts of atractylenolide were 0.769,0.752,0.781 mg/g (RSD=1.99%,n=3) for 3 times,which were higher than the extraction amounts of other 3 methods(0.683,0.489,0.693 mg/g). CONCLUSIONS:The optimized extraction technolo-gy possesses high extraction rates of atractylenolide Ⅰ,Ⅱ,Ⅲ,and can be used for the extraction of internal ether from A. macro-cephalae formula granules.
8.Maxillary and/or mandible fractures: Retrospective clinical analysis of 39 cases

Objective: Maxillary and mandible fractures are severe intermaxillary injuries,frequently involving other vital organs,and usually difficult to be treated.This study discusses the treatment of maxillary and mandible fractures.Methods: We retrospectively analyzed the clinical data of 39 cases of maxillary fractures and/or mandible fractures.Results: Of the 39 cases,34 males,5 females and 84.64% aged from 20 to 50,87.17% were caused by vehicle accidents,and 94.87% were accompanied by injuries of other parts of the body.After surgical treatment,78.95% gained normal or basically normal ocular esthetic results and 68% achieved good or improved facial esthetic results.Conclusion: The results of treatment are closely related with the type and severity of fracture and the time and plan of treatment.Preoperative CT scanning and facial profile three-dimensional reconstruction are necessitated for a thorough evaluation of the fractures and an analysis of the causes and extent of facial and ocular changes.The best treatment plan for maxillary and/or mandible fractures would include anatomic reduction by surgery,intermaxillary elastic traction and rigid internal fixation with the Ti plate.
10.Systemic rehabilitation for stiffness of finger joints after hand injury

Objective To investigate the clinical effect of systemic rehabilitation on stiffness of finger joints after hand injury. Methods A total of 213 stiff fingers were observed in patients with hand injury. All the patients were administered with therapeutic exercise, physical modalities and splinting. The improvement of the patients in terms of active range of motion (AROM) and passive range of motion (PROM) of their fingers was graded as excellent, good, adequate or poor according to the standard the authors set. Results With regard to their AROM and PROM, 82.2% and 94.8% of the patients got excellent improvement, 97.7% and 100% of the patients got excellent to adequate improvement, respectively. The finger function was increased from 17% to 56%. Conclusion Systemic rehabilitation can increase the AROM and PROM of stiff finger effectively and improve the patients' hand function.
