2.Experimental study and clinical application of transurethral electrochemical therapy on benign prostatic hyperplasia

Objective To study the safety and efficiency of transurethral electrochemical therapy (ECT) on benign prostatic hyperplasia (BPH). Methods An experimental electrechomical therapy on prostate has been carred out in 12 dogs.The prostate tissue and the nearly organs were studied for any morphological changes.On the basis of the experiment,ECT was then carried out for 11 patients suffered from BPH.The relevant parameters were assessed. Results In the experimental group,good therapeutic effect was achieved if the electrode was 10 mm shorter than the prostatic urethra.On the other hand if the electrode was 10 mm longer than the prostatic urethera,urethro-rectal fistula and urine incontinence might result.In the clinical group,the effective rate was 82% with no complication observed.Before the treatment RI was 0.70?0.04 and 2 weeks after treatment 0.73?0.03;IPSS 26.7?6.2 and 19.5?4.7;QOL 4.9?0.5 and 3.5?0.6;Qmax (7.8?2.0) and (12.4?3.1)ml/s;PVR (129.1?40.8) and (64.1?27.4)ml;V (48.0?13.1) and ( 47.0?13.5)ml.There was significant difference of IPSS,QOL,Qmax,PVR,RI before and after treatment (P0.05). Conclusions With proper maniputation and adequate length of the electrode used,ECT is a safe,efficient and mini-invasive means for treating BPH in high risk patients nontolerable to open surgery.
4.Clinical study of intelligent phacoemulsification for hard nucleus cataract extraction

AIM: To compare the efficiency and safety of torsional phacoemulsification with or without intelligent phacoemulsification ( IP ) software in hard nucleus cataract extraction.
METHODS: Ninety two eyes with Ⅳ - Ⅴ grades cataracts were enrolled in this randomized prospective study. Operated eyes were divided into two groups -those operated without IP software ( non- IP group, n =43) and those operated using IP software (IP group, n =49 ). The two groups were compared in terms of ultrasound time (UST) and cumulative dissipated energy (CDE). Post- operative outcome measures included the corneal edema and best-corrected visual acuity (BCVA) at 1,7d and 3mo postoperatively, corneal endothelial cell density and percentage of hexagonal cell at 7d and 3mo postoperatively.
RESULTS: UST was measured as 52. 51±9. 64s in non-IP Group and 48. 79±7. 13s in IP Group (P = 0. 030). CDE was 15. 78±3. 73% in non-IP Group and 14. 29±2. 77% in IP Group ( P = 0. 026). At the first postoperative day, the rate of BCVA>0. 1 in non-IP Group was 56%, and the rate in IP Group was 79% (P= 0. 066). Corneal edema in non-IP Group was 2. 98±0. 77 scores, and in IP Group it was 2. 61±0. 64 scores (P = 0. 021). At the postoperative 7 and 30d, the BCVA and corneal edema were no differences between two groups. At the postoperative 7d, corneal endothelial cell density in non- IP Group were 2497. 95 ±211. 48 / mm2 , less than 2586. 26±154. 71 / mm 2 in IP Group (P= 0. 029);percentage of hexagonal cell in IP group was 48. 33±8. 69%,higher than 44. 19±9. 48% of non-IP group(P= 0. 030).CONCLUSION: In hard nucleus cataract extraction, the IP software can combine the advantages of the two kinds of ultrasonic modes, which is more effective with lower ultrasound energy and less injury for the corneal endothclium, and is helpful for the recovery of vision at early stage after surgeries.

Aim To observe the effect of dopamine (DA) and its antagonist on the electrical activities of pain excitation neurons (PEN) in the caudate nucleus (Cd) of morphine dependent rats. Methods The right sciatic nerve was stimulated by train of electrical impulses as noxious stimuli to morphine dependent rats.The electrical discharges of PEN in Cd were recorded with glass microelectrode to observe influences of intracerebroventricular (icv) injection of DA and droperidol on the electrical activities of PEN. Results DA could decrease excitabilities of PEN in Cd of morphine dependent rats. Conclusion Results suggest that the depression of DA on PEN response in Cd of morphine dependent rats might be blocked by droperidol.
7.Association between Serum C-Reactive Protein and Benign Prostatic and Patients with Prostatic Cancer Hyperplasia

Objective To explore the association between C reactive protein (CRP)with prostatic cancer (PCa)and benign prostatic hyperplasia (BPH).Methods Retrospective analysis the 110 patients of the First Affiliated Hospital of Zhejiang University In January 2010 to August 2012 whose TPSA>4 ng/ml,postoperative pathology confirmed the diagnosis of 54 cases of prostate cancer,5 6 cases of benign prostatic hyperplasia.Detected serum CRP levels by using transmission turbidim-etry and TPSA levels by using chemiluminescence immunoassay of 54 PCa and 56 BPH patients.According to the Gleason score,PCa patients were divided into high-risk and low-risk PCa two groups,the differences among high-risk PCa,low-risk PCa and BPH groups were analyzed by nonparametric statistics analysis.Results The CRP level of high-risk PCa was 4.20~2.12 mg/L,the CRP level of low-risk PCa was 1.90~0.91 mg/L and the CRP levels in BNP patients was 1.49±0.87 mg/L,the high-risk and low-risk PCa serum CRP level obviously higher than that of patients with BPH,the differences were statistically significant (P<0.05).Conclusion Serum CRP levels of PCa patients were increased significantly,espe-cially in high-risk PCa patients.
8.Effects of auricular point sticking on labor pain and anxiety

Objective:To observe the effects of auricular point sticking on pain and anxiety during the latent period of the first stage of labor in primiparas. Methods:Primiparas meeting eligibility criteria were recruited.The participants were randomized into an auricular point group,a placebo group,and a control group.The control group received daily care.The auricular point group received 120 min of auricular point sticking therapy.The placebo group received the same auricular plasters as the auricular point group but without pressing.Participants'pain,anxiety,and uterine contractions were measured at enrollment and 30,60,and 120 min of interventions. Results:Data from 78 participants were analyzed in this study.After uterine contraction was adjusted as a covariate,there was no significant difference among groups in the baseline anxiety,baseline pain,and anxiety at 30-min intervention(P>0.05),and no significant difference between the placebo group and the control group in each indicator at each time point(P>0.05).The anxiety scores of the auricular point group at 60 min and 120 min were lower than those of the placebo group and the control group(P<0.05).The pain in the auricular point group was less than that in the placebo group and the control group at 30,60,and 120 min of interventions(P<0.05). Conclusion:Auricular point sticking therapy can relieve anxiety and pain in women during the latent period of labor.Moreover,the effect is fast-acting.It can be used as a safe and effective complementary therapy.
9.In vitro activity of micafungin against Candida albicans biofilms

Objective To explore the susceptibility of Candida albicans biofilms to micafungin. Methods In vitro model of C. Albicans biofilm was established in 96-well microtiter plates with 30 C. Albicans isolates from the Department of Mycology, Changhai Hospital, Shanghai. The susceptibility of C. Albicans biofilms to fluconazole, amphotericine B and micafungin was evaluated by colorimetric XTT-reduction assay. Sessile MIC80 (SMIC80), defined as the lowest antifungal concentration at which an 80% reduction in fungal growth was achieved, was determined. Results Of the 30 C. Albicans isolates grown in sessile states, all were resistant to fluconazole (SMIC80≥64 μg/mL), 4 sensitive to amphotericine B (SMIC80≤1 μg/mL), 26 resistant to amphotericine B (SMIC80 > 1 μg/mL), 27 sensitive to micafungin (SMIC80 < 16 μg/mL), 3 resistant to micafungin (SMIC80 >16 μg/mL). Statistical analysis revealed a significant difference in the activity against C. Albicans biofilms between micafungin and fluconazole (χ2=736.36, P<0.01), micafungin and amphotericine B (χ2=529.95, P<0.01), but not between anphotericine B and fluconazole (χ2=2.29, P>0.05). Conclusion C. Albicans biofilms are resistant to routine antifungal agents such as fluconazole and amphotericine B, but relatively more sensitive to micafungin.
