1.EXPERIMENTAL STUDY ON THE EFFECT OF REMIFENTANIL ON THE EXPRESSION OF ADHENSIVE FACTORS IN LUNG INJURY IN RATS

To determine the effects of Remifentanil on the expression of adhesive factors during acute lung injury, we used immunohistochemistry and reverse transcription polymerase chain reaction (RT PCR) technics to measure the expression levels of ICAM 1, and P Selectin in the lung in a rat model of hemorrhagic shock. We duplicated modified Wigger hemorrhagic shock model in healthy SD rats. The rats were divided into three groups: control group, resuscitation group, resuscitation and drug using group. The positive ratios of ICAM 1 and P Selectin in the lungs at different time points, namely preshock, shock, resuscitation 2 hours, and resuscitation 4 hours were determined. There were 6 rats in each group and each time point. The degree of pulmonary injury in the drug using group was less than that in other two groups. Resuscitation with lactated Ringer's solution led to most serious injury in the resuscitation group. Objective To determine the modulating effects of remifentanil on the expression of apoptosis factors during acute lung injury as a result of hemorrhagic shock, we observed the expression of C-Jun, Bax, Bcl-2 in lung with immunohistochemistry in rats with hemorrhagic shock. Methods Hemorrhagic shock model was replicated with modified Wigger's method in healthy SD rats. Seventy-two rats were randomized into three groups: sham resuscitation, resuscitation with lactated Ringer′s solution, and fluid resuscitation with remifentanil. Positive rates of C-Jun, Bax, Bcl-2 were assayed in the lungs at different time points: preshock, shock, 2 hours after resuscitation, and 4 hours after resuscitation. 4.Design and Application of Oral Prescription Drug Labels in Hospital Pharmacy

OBJECTIVE:To further standardize oral prescription drug labels in hospital pharmacy. METHODS: The prescription drug labels were specially made based on drug package inserts. RESULTS & CONCLUSIONS: The new oral prescription drug labels contain more information in which drug usage has been clearly identified thus helpful for pharmacists to reduce dispensing errors, the related information of medicines are available. Carrying out rational drug use and pharmaceutical care services plays a key role for the safe, effective and rational drug use.
5.Research on congestive refractory heart failure with renal failure treated by slow continuous ultrafiltration and relationship between negative fluid balance and predicts prognosis

Objective:To investigate the curative effect and safety of slow continuous ultrafiltration(SCUF) treatment on congestive refractory heart failure with renal failure, and to investigate the correlation between fluid balance and the postoperative complications.Methods:Thirty patients with congestive refractory heart failure with renal failure were chosen to receive slow continuous ultrafiltration treatment after deep vein catheterization, blood flow volume being 100-150 ml/min and therapy time being 24h. PiCCO was used to monitor the hemodynamic parameters, along with the change of blood pressure, blood oxygen saturation, heart beat and electrolyte.Results: After the slow continuous ultrafiltration treatment, the hemodynamic parameters of all patients showed obvious improvements, with better results on cardiac function and electrolyte balance. All the patients patients developed cardiopulmonary complications postoperatively have a larger total fluid balance of the first two days postoperatively than those without complications.Conclusion:SCUF have therapeutic effect and safety for treating congestive refractory heart failure with renal failure, and the status of the fluid balance can be a predictor of the postoperative morbidity and mortality.
6.Preparation and in vitro Drug Release of Sorafenib-Eudragit RS Nanoparticles

Objective:To prepare and optimize sorafenib-Eudragit RS nanoparticles( S-E üPs)and investigate the physicochemi-cal properties. Methods:S-E üPs were prepared by a solvent-nonsolvent method. Single factor experiments were used to research the effect of solvent,stabilizer type,carrier ratio and the proportions of water phase and organic phase on the physicochemical properties of S-E üPs. S-E üPs were evaluated by the particle size,zeta potential and morphology,and the in vitro drug release of S-E üPs was studied using dialysis technology. Results:The mean size was(86. 72 ± 3. 71)nm,the PDI and zeta potential was(0. 20 ± 0. 032)and (36. 6 ± 0. 3)mV,respectively,S-E üPs showed spherical shape with uniform distribution. The drug release in vitro was accorded with a Weibull equation. Conclusion:The solvent-nonsolvent method is appropriate for the preparation of S-E üPs. The nanoparticles have small particle size,uniform distribution,regular morphology and significant sustained-release property.

8.RP-HPLC in determination of dextromethorphan and dextrophan in human urine:phenotype analysis of CYP2D6

Objective To establish a RP-HPLC method for determination of the concentrations of dextromethorphan and its metabolites dextrorphan in human urine.Methods Phenacetine was used as internal standard,and the urine sample was hydrolyzed by enzyme,alkalified and extracted with hexane-butanol(91).The separation was carried out on DiamonsilTM C18 column(250 mm?4.6 mm,5 ?m) with mobile phase of acetonitrile-1% triethylamine buffer solution(pH adjusted to 2.2 with H3PO4).Gradient elution was done for 0-15 min(20%-35% A).The flow rate was 1.0 ml/min.The detection wavelength was set at 280 nm and the column temperature was 40℃.Results The linear ranges of dextromethorphan and dextrorphan were 0.05-2.0 ?g/ml(r=0.999 9,n=5) and 0.5-20.0 ?g/ml(r=0.999 9,n=5),respectively,and their lowest detecting concentrations were 0.04 ?g/ml and 0.4 ?g/ml,respectively.The intra-day and inter-day precision were both less than 10%.The low,middle and high extraction recoveries were between 94%-108%.Conclusion Our method is accurate and sensitive,and is suitable for the CYP2D6 phenotype analysis and pharmacokinetic studies of dextromethorphan and its metabolites in human.
9.Research in application value of outpatient mobile infusion system in the department of infectious outpatient

Objective To observe the effect of the outpatient mobile infusion system in the department of infectious outpatient night infusion.Methods From July to December in 2012,520 patients in department of infectious outpatient infusion in our hospital were selected and were divided into the observation group (using outpatient mobile infusion system)and the control group (adopting conventional infusion) with 260 cases in each group.The infusion effect was compared between two groups.Results The average occupancy per patient care time in the observation group was less than the control group,the difference was statistically significant.The satisfaction degree in the observation group was higher than that of the control group,the difference was statistically significant.There was no nursing error occurred in both of the observation group or the control group.Conclusion The use of outpatient mobile infusion system can significantly improve the nurses work efficiency,ensure nursing safety,improve quality of care and patients' satisfaction degree.
