2.Analysis of the efficacy of cinepazide maleate injection on acute cerebral ischemic stroke

Objective To systematically review the efficacy of cinepazide maleate injection in the treatment of the acute Cerebral Ischemic Stroke.Methods We searched the PubMed,EmBase,Cochrane Central Register of Controlled Trials,Clinical Trials,CNKI,Wanfang Database,VIP and Chinese Bio-Medicine Database from established to Sept.2016.We used RevMan 5.3 software to perform Meta-analysis of National Institute of Health Stroke Scale (NIHSS) score improvement and response rate.The intervention measures of the treatment group were cinepazide maleate injection with basic treatment while which of the control group are basic treatment with placebo,with western medicine injections or with traditional Chinese medicine injections.Results Twenty randomized controlled trials involving 2237 patients were included.The results of meta-analyses:The NIHSS score of treatment group decreased 2.73 more than the control group.The response rate of the treatment group was 1.22 times higher than that of control group.The differences were statistically significant (all P <0.01).Conclusion Cinepazide maleate injection has good efficacy which could decrease the NIHSS score and response rate.
3.Experimental study on therapeutic effect of Dabuyin Wan on true precocious puberty in female rats.

OBJECTIVETo study the therapeutic effect of Dabuyin Wan on true precocious puberty of female rats and its possible mechanism.
METHODTwenty-two-day-old female SD rats were subcutaneously injected with 40 mg x kg(-1) N-methyl-DL-aspartic acid (NMA) at 14:00 and 16:00 every day; meanwhile, the rats were given Dabuyin Wan for intervention. Visual inspection was conducted for the time of vaginal opening. The first estrus was observed by yaginal smear test. Their ovaries and uterus were weighed to calculate organ coefficients. Conventional pathological slices were made to observe morphological changes in ovaries and uterus and calculate the thickness of uterine walls and the number of corpus luteums. The level of E2 in serum was detected to assess the therapeutic effect of Dabuyin Wan on NMA precocious puberty in rats. expressions of GnRH, GPR54 and Kiss-1 mRNA in hypothalamus were measured by semi-quantitative RT-PCR to investigate the possible mechanism of Dabuyin Wan.
RESULTDabuyin Wan at 3.24 g x kg(-1) and 1.62 g x kg(-1) significantly decreased the organ coefficients in rats with precocious puberty (P < 0.05), decrease the number of vaginal openings in rats (P < 0.01) and the thickness of uterine walls and the number of corpus luteums (P < 0.05), and notably down-regulated expressions of GnRH, GPR54 and Kiss-1 mRNA in hypothalamus (P < 0.05), without significant impact on E2 in serum.
CONCLUSIONDabuyin Wan may inhibit GnRH synthesis and release as well as startup of hypothalamic-pituitary-gonadal axis by down-regulating Kiss-1/GPR54 mRNA expression in hypothalamus, in order to realize the therapeutic effect on true precocious puberty.

4.Influence of shared medical appointments on health-related quality of life and quality of sleep in patients after liver transplantation

Objective:To observe the influence of shared medical appointments on health-related quality of life and quality of sleep in patients after liver transplantation.Methods:By randomized controlled study, a total of 124 patients after liver transplantation were included from our hospital from January 2018 to January 2019, and according to the lottery method, all subjects were divided into the routine management group ( n=64) who received routine outpatient intervention and the shared medical management group ( n=60) who received shared medical appointments management. The health-related quality of life and quality of sleep were investigated and compared by post-liver transplant quality of life questionnaire (pLTQ) and Pittsburgh sleep quality index (PSQI) before intervention (the day of discharge) and after intervention (the end of the last shared outpatient service). Results:After intervention, the dimension scores of worry, economics, body function, emotional function, health service, complication and total score of pLTQ were improved in tow groups than before intervention [the routine management group: (41.90±7.61), (18.13±4.22), (22.22±5.10), (14.92±3.28), (20.39±4.87), (14.63±3.99), and (132.19±37.09) vs (32.25±5.55), (12.77±3.47), (17.58±4.72), (9.23±1.38), (15.17±4.81), (10.89±1.51) and (98.00±29.03) score, t=8.20, 7.85, 3.58, 12.79, 6.10, 7.01, 5.81, all P<0.001; shared medical management group: (46.12±7.92), (24.16±5.34), (25.55±5.42), (17.90±3.60), (24.81±5.12), (17.93±3.60) and (155.47±41.00) vs (32.57±5.69), (12.81±3.82), (17.00±4.70), (9.60±1.39), (15.39±4.84), (11.00±3.52) and (98.37±28.96) score, t=10.76, 13.39, 9.23, 16.66, 10.36, 10.66, 8.81, all P<0.001], and those in the shared medical management group were higher than those in routine management group ( t=3.03, 6.95, 3.53, 4.82, 4.93, 4.83, 3.32, all P<0.05). After intervention, the total score of PSQI scale were lower than before intervention in the routine management group [(10.48±2.14) vs (11.89±2.45) score, t=3.47, P=0.001], and the dimensions score of sleep quality, full-sleep time, sleep time, sleep efficiency, sleep disorders, daytime function, hypnotic and total score of PSQI were lower than before intervention in the shared medical management group [(1.41±0.32), (0.54±0.13), (1.17±0.26), (1.11±0.35), (1.21±0.27), (1.30±0.33), (1.08±0.21) and (8.05±1.75) vs (1.88±0.53), (0.86±0.37), (1.84±0.41), (2.05±0.56), (1.39±0.33), (1.47±0.43), (1.22±0.32) and (11.71±2.43) score, t=-5.88, -6.32, -10.69, -11.03, -3.27, -2.43, -3.65, -9.47, all P<0.05], and those in the shared medical management group were lower than those in routine management group ( t=-6.68, -6.39, -10.43, -10.97, -2.62, -2.12, -3.54, -6.90, all P<0.05). Conclusion:Shared medical appointments model can improve the health-related quality of life and quality of sleep in patients after liver transplantation, and improve the effectiveness of outpatient intervention.
5.Correlation analysis on self-care ability and self-efficacy of cirrhosis patients

Objective To investigate the correlation between self-efficacy and self-care ability of cirrhosis patients .Methods 497 eligible cirrhosis patients , treated in Hepatology Department in the First Affiliated Hospital of Zhengzhou University from January 2014 to April 2015 , were surveyed about their self-efficacy and self-care ability , by Chronic disease self-efficacy scale ( CDSES ) and Exercise of self-care agency Scale (ESCA).Survey instruments included general information survey , Chronic disease self-efficacy scale, and Exercise of self-care agency scale .The patients′self-efficacy and self-care ability were evaluated and analyzed in correlation .Results Self-care ability of most cirrhosis patients was in a medium or lower level .The scores of self-efficacy and self-care ability of cirrhosis patients were (45.42 ±3.33) and (115.49 ±18.82), with the two of high-positive correlation (r=0.851, P<0.01).Conclusions The patient′s self-care ability can be directly enhanced by improving patient′s self-efficacy, and thus the patients′quality of life can be guaranteed .
6.Investigation of willingness in individualized medication of voriconazole in fungal infectious patients

Objective To investigate the perception and willingness of patients on individualized medication of voriconazole,to discuss the factors influencing patients' decision-making,and to support the development of the practice guideline for individualized medication of voriconazole.Methods A cross-sectional study involving various types of patients in 9 hospitals was conducted by questionnaire to evaluate the perception and willingness on therapeutic drug monitoring (TDM) of voriconazole and CYP2C19 genotype testing.Results A total of 119 patients were involved,including 24 (20.17%) children.Ninety five (79.83 %) patients thought voriconazole TDM was necessary during the treatment of fungal infections.There was a significant difference on preference towards voriconazole TDM between patients who had received voriconazole TDM,before and patients who had never taken voriconazole TDM (100% vs 75.76%,P < 0.05).Fifty four patients (45.38%) agreed on CYP2C19 gene test.For this question,there was a significant difference among patients with different types of medicare (P < 0.05).The ranking of factors that contributed to the 2 decision-making was consistent (P <0.05).The clinical efficacy was the most important factor (mean rank 3.46 and 3.87,respectively),while cost (mean rank 1.89) and potential benefit (mean rank 2) was the least important factor for the 2 decision-making,respectively.Conclusion There was a significant variance among different types of patients on preferences towards voriconazole TDM and CYP2C19 genotype testing,which should be considered for clinicians to make clinical decisions.The perception and willingness of patients should also be considered during the development of clinical practice guidelines and clinical practice.
7.Preliminary study of the effect of HLA-Cw on haploidentical hematopoietic stem cell transplantation.

OBJECTIVETo investigate the effect of HLA-Cw on haploidentical hematopoietic stem cell transplantation (HHSCT) without T-cell depletion.
METHODSHLA-Cw were detected with PCR-SSP, the clinical data of 21 cases of haploidentical hematopoietic stem cell transplantation, including 8 standard risk and 13 high risk cases from July 2002 to March 2006 were summarized, and the effect of HLA-Cw in HHSCT was analyzed.
RESULTSTwenty patients achieved sustained, full-donor-type engraftment. The HLA-Cw matched and mismatched groups attained neutrophil recovery at a median of 12 days and 13 days, and platelet recovery to more than 20 x 10(9)/L at a median of 20 days and 23 days respectively (P > 0.05). The cumulative incidences of grades II-IV acute GVHD were 76.9% in HLA-Cw matched group and 14.3% in the mismatched group(P < 0.05). The incidences of chronic GVHD were 85.7% in HLA-Cw matched group and 57.1% in the mismatched group(P > 0.05). The 28 months disease-free survival probabilities were 49.0% in HLA-Cw matched group, and 85.7% in the mismatched group (P > 0.05). The Karnofsky score of survival patients was over 90%.
CONCLUSIONHLA-Cw mismatched in donor and recipient of HHSCT is beneficial for reducing II-IV aGVHD, and being in favor of long term survival.

Aim To investigate the effects of aralosdie C (SMC)isolated from aralosides on contractile func-tion and calcium transients in ischemia / reperfusion(I/R)-induced rat myocardial injury and the role of CaMK II. Methods The cardiomyocytes were divided into control group,I/ R group,I/ R + SMC group,KN-93+ I/ R + SMC group. The effect of SMC on the con-tractile function and calcium transient of I/ R cells was measured by cell shrinkage and ion concentration sim-ultaneous detection system. Results SMC(8 μmol· L - 1 )improved the contractile function of I/ R cardio-myocytes and the calcium transients,and SMC in-creased the rate of calcium transient and [Ca2 +]i up /down regulation significantly. While CaMK II was blocked by KN-93,the effect of SMC on contractile function and calcium transients was weakened. Con-clusions SMC can significantly improve the systolic function and calcium transients of cardiomyocytes in I/R rats,and the protective effect of SMC on cardiomyo-cytes may be related to CaMK II.
9.Simulated microgravity conditions and carbon ion irradiation induce spermatogenic cell apoptosis and sperm DNA damage.

OBJECTIVETo investigate the effect of simulated microgravity and carbon ion irradiation (CIR) on spermatogenic cell apoptosis and sperm DNA damage to the testis of male Swiss Webster mice, and assess the risk associated with space environment.
METHODSSperm DNA damage indicated by DNA fragmentation index (DFI) and high DNA stainability (HDS) was measured by sperm chromatin structure assay (SCSA). Apoptosis of spermatogenic cells was detected by annexin V-propidium iodide assay. Bax (the expression levels of p53) and proliferating cell nuclear antigen (PCNA) were measured by immunoblotting; p53 and PCNA were located by immunohistology.
RESULTSHDS, DFI, apoptosis index, and the expression levels of p53 and Bax were detected to be significantly higher in the experimental groups (P<0.05) compared with those in the control group; however, the PCNA expression varied to a certain degree. p53- and PCNA- positive expression were detected in each group, mainly in relation to the spermatogonic cells and spermatocytes.
CONCLUSIONThe findings of the present study demonstrated that simulated microgravity and CIR can induce spermatogenic cell apoptosis and sperm DNA damage. Sperm DNA damage may be one of the underlying mechanisms behind male fertility decline under space environment. These findings may provide a scientific basis for protecting astronauts and space traveler's health and safety.

10.Investigation and validation of magnetic resonance white matter atlas for 0 to 2 years old infants.

OBJECTIVE:
To construct and verify a standard template of white matter based on Chinese normal 0 to 2 years old infants by using nonlinear high registration accuracy of non-rigid diffeomorphism paradigm (large deformation diffeomorphic metric mapping, LDDMM).
METHODS:
Full-term spontaneous labor children without maternal pregnancy disease (hypertension, diabetes, etc.), intrauterine hypoxia and ischemia, head trauma, intracranial infection, intracranial surgery history, family history of mental disorders were selected. Diffusion tensor imaging (DTI) data from the 120 normal Chinese infants under 2 years old were acquired after excluding the existence of neurological diseases revealed by neurologists, radiologists and Gesell Developmental Scale. All the data were divided into six groups including group A: 1 day to 1.5 months, group B: 1.5 to 4.5 months, group C: 4.5 to 9.0 months, group D: 9 to 15 months, group E: 15 to 21 months, and group F: 21 to 24 months. Data pre-processing, normalizing, tensor fitting and calculation of all the images were performed by using MRlcron, DtiStudio, DiffeoMap and SPM software package combined with LDDMM image registration method based on the selected single template of each group. And the average templates of each group were constructed by MATLAB software platform. The set of templates included fractional anisotropy figure (FA), color map, T1 weighted image, b0 image and the mean of all DWfs figures.
RESULTS:
The templates of FA, T1, b0, DWfs and color map for the normal brain magnetic resonance white matter development of the Chinese infants aged 0 to 2 years were successfully established with the subjective scores exceeding 2 points. The objective evaluation root mean squared error was controlled below 0.19, and the cubic chart of brain alternation trend for the children aged 0 to 2 years was consistent with previous literature.
CONCLUSION
Constructing a standard template of white matter based on Chinese normal infants, by using nonlinear high registration accuracy of non-rigid diffeomorphism paradigm provides not only a foundation of further research on brain development, mechanism and treatment of pediatric diseases associated with brain, but also objective and fair imaging information for medical education and research.
