1.?-lactamase production in multidrug-resistant Pseudomonas aeruginosa

Objective To study the production of ?-lactamase in multidrug-resistant Pseudomonas aeruginosa and to guide the proper use of antibiotics in clinical practice. Methods The modified three-dimensional extract test was employed to detect ?-lactamase in 30 multidrug-resistant Pseudomonas aeruginosa strains screened from antimicrobial susceptibility test in our hospital,and isoelectric focusing electrophoresis was performed on the enzyme-producing strains. Results No metalloenzyme was detected in all the 30 strains.Twenty-six strains produced ?-lactamase,among which 25 continuously yielded large amount of AmpC enzyme and the other one both AmpC enzyme and extended-spectrum ?-lactamases(ESBLs).86.7% of multidrug-resistant Pseudomonas aeruginosa produced enzyme. ConclusionThe majority of the multidrug-resistant Pseudomonas aeruginosa in our hospital yielded large amount of AmpC enzyme in a continuous way.The modified three-dimensional extract test can eliminate some interference such as the decrease of outer membrane permeability and overexpression of efflux pump,facilitating the effective and accurate detection of ESBLs in Pseudomonas aeruginosa.

3.Quality Control of Yanyan Syrup

Objective To establish the quality standard for Yanyan syrup. Methods Thin layer chromatography ( TLC) was used for the qualitative identification of Puerariae Lobatae Radix and Scrophulariae Radix. High performance liquid chromatography (HPLC) was used to determine the content of puerarin on Diamonsil C18(200 mm×4.6 mm,5μm) column with mobile phase consisting of methanol-0.5% acetic acid (25:75) at a flow rate of 1.0 mL?min-1.The detection wavelength was set at 250 nm. Results TLC spots were clear and well-separated without negative interference.The linear range of puerarin was 3-120μg?mL-1( r=0.999 7) with an average recovery of 97.44% ( RSD=2.07%,n=6) . Conclusion The method for quality and quantity of Yanyan syrup is simple, specific, accurate and reliable.It can be used for the quality control of Yanyan syrup.
4.Study on Quality Standard for Shangtongning Capsules

Objective: To establish the quality standard for Shangtongning capsules. Methods: Microscopic identification was used for the qualitative identification of Bombyx batryticatus, Scorpio, Eupolyphaga steleophada, Pheretima, Notoginseng radix et rhizo-ma and Gastrodiae rhizoma. A TLC method was used for the qualitative identification of Chuanxiong Rhizoma, Angelicae Sinensis Ra-dix, Notoginseng Radix ET Rhizoma, Dipsaci Radix and Glycyrrhizae Radix ET Rhizoma. HPLC was used to determine the content of liquiriti. The determination was performed on an Agilent TC C18 (250 mm × 4. 6 mm,5μm) column with the mobile phase consisting of acetonitrile-water(15:85)at the flow rate of 1. 0 ml·min-1. The detection wavelength was set at 276 nm. Results: The features of the microscopic identification were significantly visible. The TLC spots were clear and well-separated without any negative interference. The linear range of liquiriti was 0. 341-1. 193 μg (r=0. 999 9)with the average recovery of 98. 97%(RSD=0. 77%, n=6). Con-clusion:The method is simple, accurate and reproducible, which is effective in controlling the quality of Shangtongning capsules and provides the basis for improving the quality standard for Shangtongning capsules.
5.Clinical Observation of Budesonide,Azithromycin Combined Terbutaline in the Treatment of Infantile Acute Bronchitis

OBJECTIVE:To observe the efficiency and safety of budesonide,azithromycin combined with terbutaline in the treatment of infantile acute bronchitis. METHODS:150 children with acute bronchitis were randomly divided into control group(75 cases) and observation group(75 cases). All children were given electrolyte supplement,antiasthmatic,antispasmodic and other conventional treatment;based on it,control group was treated with 10 mg/kg Azithromycin injection,adding into 250 ml 5% Glu-cose injection,by intravenous infusion with time of more than 60 min,once everyday+2 mg Terbutaline sulfate spray solution,add-ing into 2 ml 0.9%sodium chloride injection,aerosal inhalation,twice a day,15 min for every times. Observation group was addi-tionally given 1.0 mg Budesonide aerosol;3 times a day,10 min for every times. The treatment course for both groups was 10 d. FEV1,MEF50,PEF before and after treatment,total effective rate and disappearance time of clinical symptoms,hospitalization time and incidence of adverse reactions were recorded. RESULTS:The total effective rate in observation group were significantly higher than control group,disappearance time of clinical symptoms and hospitalization time were significantly shorter than control group,the differences were statistically significant(P<0.05). After treatment,FEV1,MEF50 and PEF in 2 groups were significant-ly higher than before,and observation group was higher than control group,the differences were statistically significant(P<0.05);there was no significant difference in the incidence of adverse reactions between 2 groups(P>0.05). CONCLUSIONS:Based on conventional treament,budesonide,azithromycin combined with terbutaline is effective in the treatment of infantile acute bronchi-tis,and it can significantly improve lung function,with good safety.
6.Therapeutic Observation of Ginger-partitioned Moxibustion in Treating Myelosuppression Induced by Chemotherapy for Multiple Myeloma

Objective To observe the clinical efficacy of ginger-partitioned moxibustion in treating myelo- suppression induced by chemotherapy for multiple myeloma (MM), and to discuss the effect of ginger-partitioned moxibustion in preventing and treating the adverse reactions of chemotherapy.Method Forty-four MM patients with myelosuppression induced by chemotherapy were randomized into a treatment group and a control group, 22 cases each. The treatment group was intervened by ginger-partitioned moxibustion, while the control group was intervened by oral administration of Leukogen tablets, both for 14 d. The white blood cell (WBC) count was observed before the treatment, on treatment day 7 and 14, and 1 week after the intervention; the incidence of nausea and vomit and clinical efficacies were compared between the two groups.Result The total effective rate was 81.8% in the treatment group versus 63.6% in the control group, and the difference was statistically significant (P<0.05). The WBC count on the 7th treatment day was significantly different from that before the treatment in both groups (P<0.05); the WBC count in the treatment group was significantly different from that in the control group on the 7th treatment day (P<0.05). The WBC count on the 14th treatment day was significantly different from that on the 7th day in both groups(P<0.05); the WBC count in the treatment group was significantly different from that in the control group on the 14th treatment day (P<0.05). The WBC count 1 week after the treatment was insignificantly different from that on the 14th treatment day in both groups (P>0.05); there was no significant difference in comparing the WBC count 1 week after the treatment between the two groups (P>0.05). The incidence of nausea and vomit in the treatment group was significantly lower than that in the control group after the treatment (P<0.05).Conclusion Ginger-partitioned moxi- bustion can effectively treat myelosuppression due to chemotherapy for MM, and it can significantly increase the WBC count.
7.Analysis on the diagn osis results of thyroid gland on school-aged children in excessive iodine region of Shandong Province

Objective To find out prevelence of thyroid goiter on school-aged children in excessive iodine region of Shandong Province.Methods By employing B-ultrasonography and palpation method,the thyroid volume were investigated and the thyroid goiter rate were calculated among 6415 children aged 8~10 years old in execssire iodine region.Results By employing B-ultrasonography method.The volumes of thyroid in goiter children aged 8,9,10 years were(5.67±1.49),(6.07±1.24),(7.30±2.01)ml,respectively.The goiter rates were 28.84%(683/2368),20.89%(448/2144),11.82%(225/1903).The volumes of thyroid in non-goiter children aged 8,9,10 years were(3.36±0.67),(3.64±0.77),(4.02±0.94)ml.When the normal value of thyroid volume in 8 and 9 year-groups lowered 0.5 ml,the goiter rate would be changed to 18.20%(430/2368)and 12.92%(277/2144) respectively.The thyroid volumes of boys and girls were(4.20±1.38),(4.18±1.73)ml,respectively.The goiter rates of boys and girls were 21.92%(732/3340)and 20.29%(624/3075)respectively.The rates of goiter detected by palpation in 8,9 and 10 year-groups were 10.05%(238/2368),10.31%(221/2144)and 14.45%(275/1903) respectively.Conclusions Along with the increasing age,the volume of thyroid in children aged 8~10 is increasing in excessive iodine region of Shandong Province.But the goiter rate is decreasing ahmg the increasing age.This phenomena may be caused by improper normal value of thyroid volume.Therefore,the normal value of thyroid volume should be revised urgently.

Objective:To investigate the function of CD 59 in LAT induced T lymphocytes'proliferation and activation.Methods:Transfected LAT-GFP recombinant lentiviral vectors into Jurkat cells and established a fusion-protein stable express cell line ( Jurkat-GFP ).Junket-GFP cells were transfected with pSUPER-siCD59 plasmids by electroporetion or stimulated by anti-CD59 antibody.The cellular locations of CD 59 and LAT were observed under fluorescence microscope with the immunofluorescence cytochem -istry.The cells proliferation were measured by MTT assay.Furthermore,Western blot was used to detect the total and phosphorylation levels of several down-stream proteins after T cell activated .Results: Jurkat-GFP cells successfully transfected with pSUPER-siCD59 plasmids showed lower fluorescence staining.CD59 and LAT distributed uniformly on the cell surface before stimulated with anti-CD59 antibody and formed clusters once upon stimulation.Jurkat-GFP cells stimulated with anti-CD59 antibody showed a higher level of pro-liferation and protein phosphorylation ,compared with the others.Conclusion:CD59 contributed to LAT induced signaling transduction of T lymphocytes ,and stimulated CD59 molecule partly promoted T cell activation.
9.Study on anti-tumor and anti-metastasis mechanism of alcohol extracts from pharbitidis semen against Lewis lung cancer.

OBJECTIVETo observe the effect of alcohol extracts from Pharbitidis Semen on the proliferation and metastasis of Lewis lung cancer, and study its anti-tumor mechanism.
METHODIn vitro, MTT assay and scratch assay were adopted to detect the effect of alcohol extracts from Pharbitidis Semen on the proliferation and metastasis of Lewis lung cancer cells. The cell autophagy was detected by the acridine orange staining. The gap-junction intercellular communication (GJIC) was investigated by the fluorescent yellow transfer. The expression of aquaporin 1 (AQP1) was analyzed by the Western blotting. In vivo, the subcutaneous implant model and the experimental pulmonary metastasis model of Lewis lung cancer in mice were established to evaluate the anti-tumor and anti-metastasis effects of alcohol extract from Pharbitidis Semen. The serum carcinoembryonic antigen (CEA) and beta2 microglobulin (beta2-MG) of mice bearing Lewis lung cancer were detected by the electrochemiluminesence immunoassay. The expressions of lung AQP1 and Connexin 43 (Cx43) were examined by the immunohistochemical method.
RESULTIn vitro, alcohol extracts from Pharbitidis Semen inhibited the cell proliferation in a dose-dependent matter, significantly prevented the cell migration, down-regulated AQP1 proteins of cells, promoted GJIC, and decreased the serum-free autophagy of tumor cells. In vivo, compared with untreated model mice, alcohol extracts from Pharbitidis Semen inhibited the tumor growth in a dose-dependent matter, prevented the tumor metastasis and prolonged the life span of mice bearing Lewis lung cancer, while decreasing serum CEA and beta2-MG of mice bearing Lewis lung cancer, enhancing the immumohistochemical staining intensity of Cx43 and weakening aquaporins AQP1 positive intensity.
CONCLUSIONAlcohol extracts from Pharbitidis Semen could prevent the proliferation and metastasis in Lewis lung cancer cells. Its mechanism may be related to the promotion of GJIC and the down-regulation of AQP1.

10.Retrospective research on the effect of cementless prosthesis in total hip replacement of 178 middle aged patients during 7-year follow-up

BACKGROUND: Total hip replacement in middle-aged patients is challenging regarding restoration and survival,because these patients are more active than old patients.OBJECTIVE: To retrospectively investigate whether a cementless prosthesis could restore hip function,decrease osteolysis,wear,and enhance prosthesis survival in middle-aged patients.METHODS: Clinical and radiological evaluations of patients undergoing single-side total hip replacement with cement and cementless prosthesis were analyzed preoperatively as well as at 6 months,1,4 and 7 years postoperatively.The outcomes of total hip replacement were assessed using Harris hip scores and survival rate.End point was loosening or revision of the femoral component for any reason.RESULTS AND CONCLUSION: Cementless prosthesis had gained better Harris hip scores than that of cement prosthesis group at 6 months,1,4 and 7 years postoperatively(P < 0.05).The survival rate was greater in cementless prosthesis compared with cement prosthesis during 7-year follow-up(P < 0.05).Results have suggested that cementless prosthesis achieves high rate of functional restoration and a low rate of complications in middle-aged patients following total hip replacement.
