1.Clinical value of scanning the initial segment of right subclavian artery routinely in carotid artery ultrasonography

Objective To explore the clinical value of scanning the initial segment of right subclavian artery routinely in carotid artery ultrasonography, thus to increase the detectable rate of arteriosclerosis.Methods 2 896 subjects who had undergone carotid artery ultrasonography were analyzed retrospectively from November 2010 to June 2015 in Shuishang Park Hospital,Tianjin.The initial segment of right subclavian artery had been examined rou-tinely in carotid artery ultrasonography from February 2013.The detectable rate of arteriosclerosis at the carotid artery was compared in the two groups.The detectable rate of arteriosclerosis at the bifurcation of common carotid artery and the initial segment of right subclavian artery was analyzed and compared in different age -groups from February 2013 to June 2015 ( group B ) .Results The detectable rate of arteriosclerosis at the carotid artery in group B (74.21%) was significantly higher than 57.34%of group A(χ2 =91.82,P=0.000).The detectable rate of arterio-sclerosis at the carotid artery in group B was increased by age ( the initial segment of right subclavian artery χ2 =181.829,P=0.000;the bifurcation of common carotid arteryχ2 =635.298,P=0.000).The detectable rate of arteri-osclerosis at the initial segment of right subclavian artery was higher than that of the bifurcation of common carotid artery in group of age 36-45 years and >45-55 years (the group of age 36-45 years χ2 =8.988,P=0.003;the group of age >45-55 yearsχ2 =19.507,P=0.000).Conclusion It is necessary that examining the initial seg-ment of right subclavian artery routinely in carotid artery ultrasonography to increase the detectable rate of arterioscle-rosis,and the initial segment of right subclavian artery is the important part to find the arteriosclerosis of carotid artery early,it is of guiding significance to prevent cardiovascular and cerebrovascular disease.
2.Study of the Effect and Functional Mechanism of Pine Pollen on Reducing the Blood-fat Level of the Experimental Mice

Objective To study the effect of lecithin pollen on hyperlipidemia model experimental animals.Methods Mice were feed with high-fat feeds in order to establish the hyperlipidemia models.The contents of TC,TG and HDL-C in the serum of the mice were tested before the experiment and the mice were divided into 5 groups according to their TC level,induding the control group,the model group and 1.25,2.50and 3.75g/kg lecithin pollen groups.Lecithin pollen was given preventively for 30 days,and then the TC,TG and HDL-C level of the mice were retested.Results Compared with the model group, the TC level of the middle and high dose group decreased. The TG level of the high dose group also decreased and the HDL-C level of the high dose group increased. The difference was significant. Conclusion The lecithin pollen is effective in preventing the formation of hyperlipidemia in the experimental mice.
3.The Assay of Total Flavonoids Lecithin Pollen Pellet.

Objective To establish the method of assaying total flavonoids in Lecithin Pollen Pellet.Methods Suo-shi pheresis was wsed to detect wave length of 500nm with Eldrin being as control article and sodium nitrite-aluminum nitrate-sodium hydroxide as developer.Results There was good linear correlation between Absorbability A and Eldrin mass concentration C rangmg from 0 to 48?g/ml;Average recovery rate of spotting was 98.7%(RSD=1.41%).Conclusion With the character of handling simply,accurate result and good reproduction quality,the method was a scientific method for quality control of assaying effective composition Lecithin Pollen Pellet.
4.Analgesic Effect of Dezocine after Laparoscopic Cholecystectomy

Objective:To analyze the analgesic effectiveness of dezocine after laparoscopic cholecystectomy. Methods: Totally 80 patients with laparoscopic cholecystectomy were randomly divided into the observation group and the control group. The control group was given tramadol 0. 2 mg·kg-1 for anesthetize, the observation group was given dezocine 0. 1 mg·kg-1 analgesia. Conducted according to a simgle drug additional pain status of the patient. After the surgery, the time of surgery, VAS and BCS pain condition score of the pa-tients were analyzed and compared, and the incidence of adverse reactions of the two groups were recorded. Results:After the surgery, the operation time, opening eyes, awaking time and extubation time showed no significant difference in the two groups (P>0. 05). VAS scores of the observation group in 30min, 1h, 2h and 6h after the surgery were significantly lower than those of the control group, and BCS score was significantly higher than that of the control group (P<0. 05). The incidence of adverse reactions in the observation group was significantly lower than that in the control group (χ2 =4. 020 5,P=0. 045 0). Conclusion:The analgesic effect of dezocine after lap-aroscopic cholecystectomy is promising, which can significantly alleviate the pain after the operation and is worthy of wider application.
5.A Comparison between Qualitative Research Method and Quantitative Research Method in Education Research

9. Expression of TLR4 in splenocytes at early phase of severely burned rats and its implication

Objective:To investigate a potential role of Toll-like receptor 4(TLR4),a pathogen pattern recognition receptor, in the early phase of severely burned rats. Methods: Rats burn model(30% of total body surface area[TBSA], III grade) were established with vapor at 108°C for 8 seconds. Rats were sacrificed before and 2,5,12,24,48 and 72 h after burning, and the spleen specimens and peripheral blood samples were harvested. TLR4 mRNA and TNF-α mRNA expression in splenocyte was measured by reverse-transcription PCR (RT-PCR); the expression of TLR4 protein were measured by Western bloting; the endothelial toxicity concentration in plasma was detected by limulus lysate test. Results: It was found that the expression of TLR4 mRNA, TNF-α mRNA, TLR4 protein, and the level of ET were significantly increased in burned group compared with normal control group. The expression of TLR4 mRNA and protein peaked at 8 h after burning, the expession of TNF-α mRNA peaked at 12 h,and the level of ET peaked at 8 h after burning; the peak values of them were (3.66±0.51),(2.27±0.19), (1.65±0.23), and (11.68±2.63) Eu/ml,respectively,all significantly higher than those of the control group(P<0.01 or P< 0.05). The expression of TLR4 mRNA showed a positive correlation with that of ET and TNF-α mRNA(r=0.811,0.462, P< 0.01). Conclusion: Severe burn can cause endotoxemia and upregulate the expression of TLR4 and TNF-α in splenocytes. The up-regulation of TLR4 might participate in the pathological procedure of excessive inflammation.
10.Pathology and treatment of idiopathic membranous nephropathy in children

Objective To study features of clinical manifestation,pathology and treatment effects of idiopathic membranous nephropathy(IMN).Methods A retrospective study was performed in patients with membranous nephropathy through renal biopsy diagnosis in Beijing Children's Hospital Affiliated to Capital Medical University from November 2009 to February 2013.Patients were diagnosed according to renal biopsy apart from hepatitis B virus-associated glomerulonephritis,Henoch-Schonlein purpura nephritis,lupus nephritis,tumor and drug associated nephritis.The study summed the clinical and pathological features of IMN and evaluated the effect of treatment according the follow up.Results (1) There were 13 patients,with the ratio of male and female being 1.0 ∶ 1.6,among them 1 patient was younger than 6 years old,3 cases were between 6 and 12 years old,and 9 cases were elder than 12 years old.Nine patients were nephrotic syndrome,and 3 patients had renal insufficiency.(2)Ten patients had infection,among them 5 cases were upper respiratory infection with agent unfound,4 patients were mcoplasma infection,1 patient was influenza B virus infection,1 patient had positive result of Hepatitis B surface antibody(HBsAb) and Hepatitis B core antibody (HBcAb),and 2 patients had positive result of purified protein derivative (PPD).(3)Pathological feature of IMN was atypical membranous nephropathy.Atypical membranous nephropathy was characterized by an irregular,thickened glomerular basement membrane(GBM),mesangial cells,and a mesangial matrix with mild to moderate hyperplasia.Immune complexes were stained in the subepithelial region,GBM,and mesangial region under light microscopy.Electron microscopy showed various sizes,density,and shapes of electron-dense deposits in multiple regions.The cases of immunofluorescence were 13 for IgG,10 for C3,9 for IgM,9 for C1q,5 for IgA,2 for C4,and 0 for fibrinogen.Seven patients were strongly positive(3 + and above) for IgG and one patient was strongly positive for C3.(4)Seven patients had complete remission,three patients had partial remission,and three patients had non-remission.Conclusions (1) In this study,children with IMN were mainly girls,and the age distribution was beyond 12 years.Nephrotic syndrome was the most common clinical manifestation.(2)Atypical membranous nephropathy was the most common pathological feature in children with IMN,which should be distinguished from hepatitis B virus-associated glomerulonephritis by testing HBV antigen in renal biopsy.(3) Prednisone was effective for IMN children,but it was suggested to give enough and long course of application.Effect of prednisone combined with cyclophosphamide was still undefined.
