1.Value of MiR-155 in Prognostic Evaluation of Elderly Patients with Primary Immune Thrombocytopenia.

OBJECTIVE:
To explore the value of has-microRNA-155 (miR-155) in peripheral blood mononuclear cells (PBMNC) in prognostic evaluation of elderly patients with primary immune thrombocytopenia (PITP).
METHODS:
One hundred and thirty elderly PITP patients and 60 healthy volunteers in our hospital were selected. The relative expression level of miR-155 in PBMNC was detected by RT-PCR. Unconditional logistic multivariate regression analysis was used to analyze the relationship between miR-155 expression and prognosis of PITP patients, and Kaplan-Meier was further used to analyze the relationship between miR-155 and PITP recurrence.
RESULTS:
The relative expression level of miR-155 in PBMNC of elderly PITP patients was significantly higher than that in healthy volunteers, and increased significantly with the severity of the disease (P<0.05). The overall effective rate of elderly PITP patients with miR-155 low-expression was significantly higher than that in the patients with miR-155 high-expression (96.92% vs 72.31%) by after treatment with glucocorticoid. Multivariate analysis showed that miR-155 was an independent risk factor for PITP patients. Elderly patients with high expression of miR-155 showed a higher risk of recurrence.
CONCLUSION
miR-155 in PBMNC has a high accuracy for PITP diagnosis, and the elderly patients with high level of miR-155 show a poor prognosis and a higher risk of recurrence.

2.Content determination of 6 components in Jinlian qingre granules by QAMS method based on a variety of internal reference substances

OBJECTIVE To establish a quantitative analysis of multi-components by single marker (QAMS) method based on a variety of internal reference substances for the content determination of 6 components in Jinlian qingre granules, such as mangiferin, 2″-O-β-L-galactopyranosylorientin, orientin, veratric acid, vitexin, harpagoside. METHODS The determination was performed on Agilent Eclipse Plus C18 column with mobile phase consisted of acetonitrile-0.1% phosphoric acid solution (gradient elution) at the flow rate of 1 mL/min. The column temperature was 30 ℃, and the detection wavelength was set at 270 nm. Taking orientin, vitexin and 2″-O-β-L-galactopyranosylorientin as internal references, the relative correction factors (RCF) of the other 5 components to be determined and internal substances were determined by QAMS. The contents of 6 components in 21 batches of Jinlian qingre granules were calculated and then compared with the results of the external standard method. RESULTS The contents of mangiferin, 2″-O-β-L-galactopyranosylorientin, orientin, veratric acid, vitexin and harpagoside in 21 batches of samples were determined by QAMS in the range of 0.234-0.516, 1.804-2.270, 2.143-2.606, 0.190-0.223, 0.594-0.782, 0.080-0.152 mg/g; the contents of them determined by external standard method were 0.235-0.523, 1.798-2.265, 2.137-2.599, 0.190-0.224, 0.597-0.786, 0.077-0.151 mg/g, respectively. The percentage difference between the results measured by the two methods should not exceed 4.00%. CONCLUSIONS QAMS has been constructed for the simultaneous determination of 6 components in Jinlian qingre granules based on a variety of internal reference substances. The results obtained by this method are not significantly different from those obtained by the external standard method, and can be used for the quality control of Jinlian qingre granules.
3.Feasibility Analysis of Constructing the Advance Medical Directive System for the Elderly in China

Advance medical directive has been implemented in many countries and regions, has also been widely concerned by medical and ethical circles in China, and has gradually entered the public consciousness. The phenomenon of loneliness, disability and dementia of the elderly is gradually increasing, which reflects the importance of advance medical directive in this large group. Therefore, this paper focused on the main body of medical decision-making patients, patients’ families and medical staff to describe the development status of medical decision-making in China, and analyze the reasons for obstacles in the promotion of advance medical directive in China from cultural, social and economic factors. By comparing the system, culture and medical insurance payment methods of the advance medical directive system in China and western countries, this paper explored the feasibility of constructing this system in China, and put forward that the construction of the advance medical directive system in China should be prepared in the cultural, economic and other aspects, so as to better improve the life quality of the elderly at the end of life in the future, and also provide some useful reference for its promotion in China’s medical practice.
4.Cognition and countermeasures of medical science and technology ethics management among medical staff in five tertiary hospitals in Ningxia Hui Autonomous Region

ObjectiveTo understand the current situation and influencing factors of cognition level, as well as existing problems in the construction of medical science and technology ethics management among medical staff in five tertiary hospitals in Ningxia Hui Autonomous Region, to provide references for enhancing their awareness of medical science and technology ethics management and improving the quality of clinical research. MethodsUsing a self-designed and validated “Medical Science and Technology Ethics Management Cognitive Questionnaire for Medical Staff,” medical staff from five tertiary hospitals in five cities in Ningxia were selected as the research subjects for investigation. ResultsFinally, 436 valid questionnaires were collected, among which the total average score for medical staff’s cognition of medical science and technology ethics management was (61.58±12.96) points. The scores of ethics system cognition, ethics committee cognition, ethics education and training cognition, and ethics management informatization cognition were (14.18±2.84) points, (24.19±5.47) points, (15.07±4.00) points, and (8.14±4.42) points, respectively. Multiple linear regression analysis showed that age, work city, education level, professional title, whether they had assessed the implementation of the hospital ethics system, whether they had participated in the hospital’s ethics review work, and whether they had organized or participated the hospital’s ethics training, were all influencing factors on the cognitive score of medical science and technology ethics management among medical staff, which can explain 51.00% of the variation (P<0.05). Medical staff considered that inadequate promotion of ethical systems, opaque ethical review procedures and processes, conflicts between ethical training time and work tasks, and the lack of a dedicated ethical review information platform were the main problems in constructing medical science and technology ethics. ConclusionThe cognition level of medical science and technology ethics management among medical staff in tertiary hospitals in Ningxia was generally not high and was influenced by various factors. Hospital managers should strengthen the publicity and training of ethical systems, as well as improve their implementation and supervision mechanisms; standardize ethical review procedures and processes, as well as build an ethical review information platform; increase investment in ethics education and arrange training time reasonably, to enhance the awareness of medical science and technology ethics management among medical staff and improve the quality of clinical research.
5.Comparison of disinfection effects of different methods for nucleic acid pollution in 2019-nCoV

【Objective】 To evaluate the ability of different disinfection methods to remove nucleic acid pollution in 2019-nCoV so as to obtain the best removal scheme. 【Methods】 2019-nCoV positive quality control nucleic acid of 50 μL was applied to plastic, metal and glass with medical cotton swabs, respectively. After drying, we dropped 50 μL of 750 mL/L alcohol (ethanol), chlorine-containing disinfectant (2 000 mg/L and 5 500 mg/L), and PCR Cleaner, respectively. After 1 min, the contaminated area was wiped with medical cotton swabs and soaked in 300 μL of pure water. After shaking and mixing, 5 μL was taken as a template. The Ct values of ORF1ab and N genes and IC genes of internal standard fragment in the amplified target area of 2019-nCoV after wiping with different disinfection methods were compared to evaluate the effect of eliminating nucleic acid pollution, and each experiment was repeated for three times. Similarly, the effects of ultraviolet irradiation for 0, 1, 2, 3, 4, and 5 hours on the removal of nucleic acid pollution were compared. 【Results】 After 2 000 mg/L and 5 500 mg/L chlorine-containing disinfectant wiped the contaminated area, the Ct values of ORF1ab and N genes and IC genes of internal standard fragment in the amplified target area in 2019-nCoV were all 0, and the Ct values of all genes in the contaminated area in groups 3, 4 and 5 h after UV irradiation were all 0, which completely cleared the pollution and had a strong effect. The effect of PCR Cleaner was second, and 750 mL/L ethanol was the worst. 【Conclusion】 2 000 mg/L and 5 500 mg/L chlorine-containing disinfectant and ultraviolet irradiation for 3 hours have the best effect of eliminating nucleic acid pollution, which is worth popularizing under appropriate conditions.
6.Distribution characteristics and results of allergens in patients with allergic rhinitis in Ningxia area.

Objective:To investigate the distribution of allergens in patients with allergic rhinitis (AR) in Ningxia, and provide theoretical data for the prevention and treatment of AR in this region. Methods:A total of 1664 patients diagnosed with AR in the Otorhinolaryngology Head and Neck Surgery Department of Yinchuan First People's Hospital Outpatient Clinic from January 2018 to December 2021 were retrospectively collected. Use the allergen sIgE antibody detection kit (immunoblotting method) to detect inhalation and ingestion allergens in patients.Results: ①Among all AR patients, 1 158 cases were detected positive, resulting in the detection rate was 69.59%; ②The detection rate of inhalation allergen was 65.87%, and the detection rate of ingestion allergen was 19.83%; ③Mugwort was the most sensitive allergen, and 76.32% of the patients having a positive grade ≥3; ④Out of the patients, 294 cases (25.39%) were allergic to only one allergen, 244 cases (21.07%) were allergic to two allergens, and 620 cases (53.54%) were allergic to three or more allergens; ⑤During different seasons, the highest number of positive allergens detected was in the summer, with 968 cases (83.59%). Mugwort was the main allergen during this season (69.01%). After the COVID-19 epidemic, the total positive rate of sIgE tests in AR patients decreased compared to before, and the difference was statistically significant (P<0.001); ⑥Mugwort, dog epithelium, mold combination, egg, peanut, soybean, Marine fish combination and fruit combination all showed statistically significant differences between different gender groups (P<0.05); ⑦Common ragweed, mugwort, dust mite combination, cockroach, egg, milk, Marine fish combination, shrimp, fruit combination and nut combination all showed statistically significant differences among different age groups (P<0.05); ⑧There were statistically significant differences in hay dust among different ethnic groups (P<0.05). Conclusion:Artemisia argyi is the main allergen in Ningxia, and the distribution characteristics of different allergens are influenced by treatment season, the COVID-19 epidemic, gender, age, ethnicity, and other factors, showing certain distribution patterns and rules.

7. Value and related factors of preoperative diagnosis of extramural vascular invasion of rectal cancer by 3.0T magnetic resonance imaging

Objective:
To evaluate the value of preoperative diagnosis of extramural vascular invasion (EMVI) of rectal cancer with 3.0T high-resolution magnetic resonance imaging (MRI) and the MRI-related factors of EMVI in rectal cancer.
Methods:
The clinical and imaging data of 40 patients with rectal cancer were retrospectively analyzed. The postoperative pathological diagnosis was used as the gold standard to evaluate the diagnostic efficacy of preoperative diagnosis of EMVI of rectal cancer by high-resolution MRI, and to analyze the relationship between the EMVI and clinical and MRI features.
Results:
Of the 40 patients, 19 cases were diagnosed as positive EMVI and 21 were negative by MRI. Pathological diagnosis of EMVI was positive in 10 cases and negative in 30 cases. The sensitivity, specificity and accuracy of MRI in the diagnosis of EMVI were 100%, 70.0% and 77.5%, respectively. ObjectiveTo investigate the effect and mechanism of Nongsuo Dangguiwan in improving ovarian oxidative stress in rats with ovarian dysfunction. MethodThirty-six adult female SD rats were randomly divided into normal group, model group, positive drug group (Femoston, 0.3 mg·kg-1), and high-, medium-, and low-dose groups of concentrated Nongsuo Dangguiwan (2.08, 4.16, 8.32 g·kg-1), with six rats in each group. Rats, except for those in the normal group, were injected with 80 mg·kg-1 vinyl cyclohexene dioxide (VCD) per day for 14 consecutive days to induce ovarian dysfunction. From the 15th day, rats were treated with corresponding drugs by gavage, while those in the model group received 2 mL·kg-1 saline, once daily for 28 consecutive days. The ovarian index, levels of related hormones including estradiol (E2), follicle-stimulating hormone (FSH), luteinizing hormone (LH), and anti-mullerian hormone (AMH) in serum, as well as superoxide dismutase (SOD) activity and glutathione (GSH) content in serum were measured by enzyme-linked immunosorbent assay (ELISA). The malondialdehyde (MDA) content in serum was detected by the thiobarbituric acid (TAB) method. Ovarian morphology was observed by hematoxylin-eosin (HE) staining. The expression of nuclear factor E2-related factor 2 (Nrf2), heme oxygenase-1 (HO-1), SOD2, and SOD1 in ovarian tissues was detected by immunohistochemistry (IHC) and Western blot. ResultCompared with the normal group, the model group showed a significant reduction in growing follicles in the ovary, loose arrangement of granulosa cells in the follicle, decreased body weight, ovarian index, and serum AMH and E2 levels, increased LH and FSH levels (P<0.01), reduced levels of SOD and GSH in serum (P<0.01), and increased MDA level (P<0.01). Compared with the model group, the groups with drug intervention showed increased ovarian index (P<0.05, P<0.01), increased serum E2 level (P<0.05, P<0.01), decreased FSH, AMH, and LH levels (P<0.05, P<0.01), increased number of growing follicles in the ovary, potentiated SOD activity in serum, increased GSH content, decreased MDA content (P<0.05, P<0.01), and up-regulated expression levels of Nrf2, HO-1, SOD2, and SOD1 proteins in ovarian tissues (P<0.05, P<0.01). ConclusionNongsuo Dangguiwan can regulate serum hormone levels, increase the expression of Nrf2, HO-1, SOD2, and SOD1 in ovarian tissues, and improve ovarian antioxidant capacity to resist oxidative stress injury, thereby improving ovarian reserve function.
9.miR-153-3p regulates proliferation, invasion and migration of gastric cancer SGC7901 cells via targeting FZD3

[摘 要] 目的:探讨miR-153-3p对胃癌SGC7901细胞增殖、侵袭和迁移的作用及其机制。方法:收集2018年5月至2020年6月宁夏医科大学总医院收治的60例胃癌患者的癌和配对癌旁组织标本,以及人胃癌细胞系NCI-N87、AGS、SNU-5、SGC7901和胃上皮细胞GES-1,qPCR法检测miR-153-3p在胃癌组织与细胞中的表达水平。将miR-153-3p mimic及mimic对照序列转染至SGC7901细胞,用CCK-8、克隆形成、流式细胞术、TUNEL、Transwell和划痕愈合实验分别检测上调miR-153-3p对SGC7901细胞增殖、凋亡、侵袭和迁移的影响。构建裸鼠SGC7901细胞移植瘤模型,观察miR-153-3p对肿瘤生长的影响。通过生物信息学数据库和双荧光素酶报告基因实验预测并验证miR-153-3p与FZD3靶向关系,WB法检测miR-153-3p对FZD3蛋白及Wnt/β-catenin通路相关蛋白表达的影响。结果:miR-153-3p在胃癌组织和细胞中表达水平分别显著低于癌旁组织和GES-1细胞(均P<0.01),以SGC7901细胞中表达水平最低。上调miR-153-3p显著抑制SGC7901细胞的增殖、侵袭和迁移能力,并提高细胞凋亡率(均P<0.01),同时上调细胞中E-cadherin表达而下调N-cadherin、MMP2和MMP9表达(均P<0.01)。在体内实验表明,静脉注射miR-153-3p mimic显著降低移植瘤体积和瘤组织中Ki-67表达而上调P57表达(均P<0.01)。机制分析表明,miR-153-3p靶向结合FZD3基因的3′UTR区域,上调miR-153-3p会抑制FZD3表达并上调β-catenin、TCF-4和cyclin D1水平(均P<0.01)。结论:miR-153-3p靶向FZD3并通过Wnt/β-catenin信号通路调控胃癌SGC7901细胞的增殖、侵袭和迁移。
