1.Cognition and countermeasures of medical science and technology ethics management among medical staff in five tertiary hospitals in Ningxia Hui Autonomous Region

ObjectiveTo understand the current situation and influencing factors of cognition level, as well as existing problems in the construction of medical science and technology ethics management among medical staff in five tertiary hospitals in Ningxia Hui Autonomous Region, to provide references for enhancing their awareness of medical science and technology ethics management and improving the quality of clinical research. MethodsUsing a self-designed and validated “Medical Science and Technology Ethics Management Cognitive Questionnaire for Medical Staff,” medical staff from five tertiary hospitals in five cities in Ningxia were selected as the research subjects for investigation. ResultsFinally, 436 valid questionnaires were collected, among which the total average score for medical staff’s cognition of medical science and technology ethics management was (61.58±12.96) points. The scores of ethics system cognition, ethics committee cognition, ethics education and training cognition, and ethics management informatization cognition were (14.18±2.84) points, (24.19±5.47) points, (15.07±4.00) points, and (8.14±4.42) points, respectively. Multiple linear regression analysis showed that age, work city, education level, professional title, whether they had assessed the implementation of the hospital ethics system, whether they had participated in the hospital’s ethics review work, and whether they had organized or participated the hospital’s ethics training, were all influencing factors on the cognitive score of medical science and technology ethics management among medical staff, which can explain 51.00% of the variation (P<0.05). Medical staff considered that inadequate promotion of ethical systems, opaque ethical review procedures and processes, conflicts between ethical training time and work tasks, and the lack of a dedicated ethical review information platform were the main problems in constructing medical science and technology ethics. ConclusionThe cognition level of medical science and technology ethics management among medical staff in tertiary hospitals in Ningxia was generally not high and was influenced by various factors. Hospital managers should strengthen the publicity and training of ethical systems, as well as improve their implementation and supervision mechanisms; standardize ethical review procedures and processes, as well as build an ethical review information platform; increase investment in ethics education and arrange training time reasonably, to enhance the awareness of medical science and technology ethics management among medical staff and improve the quality of clinical research.
2.Observation on the therapeutic effect of a modified Devine procedure with subcutaneous sliding fixation method for concealed penis.

To evaluate the therapeutic effect of a modified Devine procedure with a subcutaneous sliding fixation method for the treatment of congenital concealed penis, we retrospectively selected 45 patients with congenital concealed penises who were admitted to General Hospital of Ningxia Medical University (Yinchuan, China) between September 2020 and November 2023. In all cases, the penis was observed to be short, and retracting the skin at the base revealed a normal penile body, which immediately returned to its original position upon release. All patients underwent the modified Devine procedure with subcutaneous sliding fixation and completed a 12-week postoperative follow-up. A statistically significant increase in penile length was observed postoperatively, with the median length increasing from 4.0 (interquartile range [IQR]: 3.5-4.8; 95% confidence interval [CI]: 3.9-4.4) cm to 8.0 (IQR: 7.8-8.0; 95% CI: 7.7-7.9) cm, with P < 0.001. The parents were satisfied with the outcomes, including increased penile length, improved hygiene, and enhanced esthetics. Except for mild foreskin edema in all cases, no complications (such as infections, skin necrosis, or penile retraction) were observed. The edema was resolved within 4 weeks after the operation. This study demonstrates that the modified Devine procedure utilizing the subcutaneous sliding fixation method yields excellent outcomes with minimal postoperative complications, reduced penile retraction, and high satisfaction rates among patients and their families.

3.Self-degradable "gemini-like" ionizable lipid-mediated delivery of siRNA for subcellular-specific gene therapy of hepatic diseases.

Tailored lipid nanoparticles (LNPs)-mediated small interfering RNA (siRNA) nanomedicines show promise in treating liver disease, such as acute liver injury (ALI) and non-alcoholic steatohepatitis (NASH). However, constructing LNPs that address biosafety concerns, ensure efficient delivery, and target specific hepatic subcellular fractions has been challenging. To evade above obstacles, we develop three novel self-degradable "gemini-like" ionizable lipids (SS-MA, SS-DC, SS-MH) by incorporating disulfide bonds and modifying the length of ester bond and tertiary amino head. Our findings reveal that the disulfide-bond-bridged LNPs exhibit reduction-responsive drug release, improving both biosafety and siRNA delivery efficiency. Furthermore, the distance of ester bond and tertiary amino head significantly influences the LNPs' pK a, thereby affecting endosomal escape, hemolytic efficiency, absorption capacity of ApoE, uptake efficiency of hepatocytes and liver accumulation. We also develop the modified-mannose LNPs (M-LNP) to target liver macrophages specifically. The optimized M-MH_LNP@TNFα exhibits potential in preventing ALI by decreasing tumor necrosis factor α (TNFα) levels in the macrophages, while MH_LNP@DGAT2 could treat NASH by selectively degrading diacylglycerol O-acyltransferase 2 (DGAT2) in the hepatocytes. Our findings provide new insights into developing novel highly effective and low-toxic "gemini-like" ionizable lipids for constructing LNPs, potentially achieving more effective treatment for hepatic diseases.
4.The splicing factor HNRNPH1 regulates Circ-MYOCD back-splicing to modulate the course of cardiac hypertrophy.

OBJECTIVES:
To explore the mechanism of Circ-MYOCD back-splicing and its regulatory role in myocardial hypertrophy.
METHODS:
Sanger sequencing and RNase R assays were performed to verify the circularity and stability of Circ-MYOCD, whose subcellular distribution was determined by nuclear-cytoplasmic fractionation. Bioinformatics analysis and mass spectrometry from pull-down assays were conducted to predict the RNA-binding proteins (RBPs) interacting with Circ-MYOCD. In rat cardiomyocytes H9C2 cells, the effects of HNRNPH1 and HNRNPL knockdown and overexpression on Circ-MYOCD back-splicing were evaluated. In a H9C2 cell model of angiotensin II (Ang II)-induced myocardial hypertrophy, the expression of HNRNPH1 was detected, the effects of HNRNPH1 knockdown and overexpression on progression of myocardial hypertrophy were assessed, and the regulatory effect of HNRNPH1 on Circ-MYOCD back-splicing was analyzed.
RESULTS:
Sanger sequencing confirmed that the junction primers could amplify the correct Circ-MYOCD sequence. RNase R and nuclear-cytoplasmic fractionation assays showed that Circ-MYOCD was stable and predominantly localized in the cytoplasm. Bioinformatics analysis and mass spectrometry from the Circ-MYOCD pull-down assay identified HNRNPH1 and HNRNPL as the RBPs interacting with Circ-MYOCD. In H9C2 cells, HNRNPH1 knockdown significantly enhanced while its overexpression inhibited Circ-MYOCD back-splicing; HNRNPH1 overexpression obviously increased the expressions of myocardial hypertrophy markers ANP and BNP, while its knockdown produced the opposite effect. In Ang II-induced H9C2 cells, which exhibited a significant increase of HNRNPH1 expression and increased expressions of ANP and BNP, HNRNPH1 knockdown obviously increased Circ-MYOCD expression, decreased MYOCD expression and lowered both ANP and BNP expressions.
CONCLUSIONS
HNRNPH1 regulates Circ-MYOCD back-splicing to influence the progression of myocardial hypertrophy.

5.Nigella sativa L. seed extract alleviates oxidative stress-induced cellular senescence and dysfunction in melanocytes.

Nigella sativa L. seeds have been traditionally utilized in Chinese folk medicine for centuries to treat vitiligo. This study revealed that the ethanolic extract of Nigella sativa L. (HZC) enhances melanogenesis and mitigates oxidative stress-induced cellular senescence and dysfunction in melanocytes. In accordance with established protocols, the ethanol fraction from Nigella sativa L. seeds was extracted, concentrated, and lyophilized to evaluate its herbal effects via 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assays, tyrosinase activity evaluation, measurement of cellular melanin contents, scratch assays, senescence-associated β-galactosidase (SA-β-gal) staining, enzyme-linked immunosorbent assay (ELISA), and Western blot analysis for expression profiling of experimentally relevant proteins. The results indicated that HZC significantly enhanced tyrosinase activity and melanin content while notably increasing the protein expression levels of Tyr, Mitf, and gp100 in B16F10 cells. Furthermore, HZC effectively mitigated oxidative stress-induced cellular senescence, improved melanocyte condition, and rectified various functional impairments associated with melanocyte dysfunction. These findings suggest that HZC increases melanin synthesis in melanocytes through the activation of the MAPK, PKA, and Wnt signaling pathways. In addition, HZC attenuates oxidative damage induced by H2O2 therapy by activating the nuclear factor E2-related factor 2-antioxidant response element (Nrf2-ARE) pathway and enhancing the activity of downstream antioxidant enzymes, thus preventing premature senescence and dysfunction in melanocytes.

6.Effects of LncRNA PXN-AS1 on proliferation, apoptosis and invasion of gastric cancer cells

Objective :
To investigate the expression of LncRNA PXN-AS1(abbreviated as PXN-AS1) in gastric cancer tissues and its impact on the proliferation, apoptosis, and invasion of gastric cancer cells, and to explore its potential regulatory mechanisms.
Methods :
Gastric cancer tissues and adjacent tissues from 43 patients with gastric cancer were collected. The expression levels of PXN-AS1 and miR-125a-5p in gastric cancer tissues were detected using qRT-PCR, and the relationship between the expression level of PXN-AS1 and the clinical pathological parameters of patients with gastric cancer was analyzed. Pearson correlation analysis was conducted to evaluate the correlation between PXN-AS1 and miR-125a-5p in gastric cancer tissues. PXN-AS1 siRNA plasmid(si-PXN-AS1) and negative control plasmid(si-NC), as well as miR-125a-5p inhibitor(inhibitor) and negative control miRNA inhibitor(inhibitor NC), were transfected alone or in combination into HGC-27 cells. The cells were divided into si-NC group, si-PXN-AS1 group, si-PXN-AS1+inhibitor NC group, si-PXN-AS1+inhibitor group, and a blank control group was also established. qRT-PCR was utilized to assess the expression levels of PXN-AS1 and miR-125a-5p in each cell group. CCK-8 assay was employed to evaluate the proliferation levels of cells in each group. EdU experiment was conducted to assess the proliferation status of cells in each group. Transwell assay was performed to examine the migration and invasion capabilities of cells in each group; flow cytometry was employed to measure the apoptosis levels of cells in each group; the dual-luciferase reporter gene system was used to validate the sponge absorption of PXN-AS1 on miR-125a-5p and regulate its expression levels. Bioinformatics analysis was employed to screen downstream target genes of miR-125a-5p and perform GO and KEGG enrichment analysis.
Results :
Compared with adjacent cancerous tissues, the expression level of the PXN-AS1 in gastric cancer tissues significantly increased, while the expression level of miR-125a-5p markedly decreased(bothP<0.05). PXN-AS1 and miR-125a-5p exhibited a negative correlation(r=-0.452,P=0.002). Furthermore, gastric cancer patients with high expression of PXN-AS1 exhibited the proportions of lymph node metastasis positivity and low differentiation were significantly higher(allP<0.05). Compared to the blank group and si-NC group, the si-PXN-AS1 group of HGC-27 cells showed significantly reduced levels of PXN-AS1 expression, cell proliferation capacity, and the number of cell migration and invasion(allP<0.05), while the level of cell apoptosis significantly increased(P<0.05). The dual-luciferase reporter system results demonstrated specific adsorption of miR-125a-5p by PXN-AS1 in HGC-27 cells. Knockdown of miR-125a-5p could reverse the effect of PXN-AS1 silencing on the inhibition of proliferation, migration and invasion of gastric cancer cells, and promote apoptosis. The bioinformatics analysis results indicated that the downstream target genes of miR-125a-5p were involved in various biological processes, including DNA damage, T cell apoptosis and differentiation, RNA metabolism synthesis processes, and signal transduction. Additionally, miR-125a-5p was implicated in signaling pathways such as the HIF-1 pathway, JAK-STAT pathway, and cell apoptosis signaling pathway.
Conclusion
PXN-AS1 is upregulated in gastric cancer. Moreover, acting as a competitive sponge for miR-125a-5p, PXN-AS1 promotes the proliferation, migration, and invasion abilities of gastric cancer cells HGC-27 while inhibiting apoptosis.
7.Nrf2-mediated ferroptosis of spermatogenic cells involved in male reproductive toxicity induced by polystyrene nanoplastics in mice

Microplastics(MPs)and nanoplastics(NPs)have become hazardous materials due to the massive amount of plastic waste and disposable masks,but their specific health effects remain uncertain.In this study,fluorescence-labeled polystyrene NPs(PS-NPs)were injected into the circulatory systems of mice to determine the distribution and potential toxic effects of NPs in vivo.Interestingly,whole-body imaging found that PS-NPs accumulated in the testes of mice.Therefore,the toxic effects of PS-NPs on the reproduction systems and the spermatocytes cell line of male mice,and their mechanisms,were investigated.After oral exposure to PS-NPs,their spermatogenesis was affected and the spermatogenic cells were damaged.The spermatocyte cell line GC-2 was exposed to PS-NPs and analyzed using RNA sequencing(RNA-seq)to determine the toxic mechanisms;a ferroptosis pathway was found after PS-NP exposure.The phenomena and indicators of ferroptosis were then determined and verified by ferroptosis inhibitor ferrostatin-1(Fer-1),and it was also found that nuclear factor erythroid 2-related factor 2(Nrf2)played an important role in spermatogenic cell ferroptosis induced by PS-NPs.Finally,it was confirmed in vivo that this mechanism of Nrf2 played a protective role in PS-NPs-induced male reproductive toxicity.This study demonstrated that PS-NPs induce male reproductive dysfunction in mice by causing spermatogenic cell ferroptosis dependent on Nrf2.
8.Relevance of Household Chemical Usage to Respiratory Diseases in Older Adults in China.

OBJECTIVE:
This study investigated the association between household chemical use and respiratory disease (RD) in older Chinese adults.
METHODS:
The data were from the 2018 China Longitudinal Health and Longevity Survey (CLHLS) database, which included 12,866 participants aged ≥ 65 years. The prevalence of RD was based on self-reported medical history, and patients were divided into diseased and non-diseased groups. The frequency of household chemical usage was divided into four categories, and a total score for eight household chemical usage categories was constructed. Binary logistic regression was used to determine the relationship between the frequency of household chemical use and RD, and a restricted cubic spline was used to determine the dose-response association.
RESULT:
After adjusting for all covariates, regular use of repellents [odds ratios ( OR) = 1.28, 95% CI 1.06-1.55] and oil removers ( OR = 1.28, 95% CI 1.03-1.58) were associated with RD. There was a dose-response association between the total score of household chemicals usage and RD risk ( P non-linearity > 0.05, P for trend < 0.01). Using patients with the total score below 9 as a reference, the OR for patients with the total score ranging from 25 to 32 is 2.33 (95% CI 1.25-4.09).
CONCLUSION
Regular use of repellents and oil removers increased the risk of RD, and the dose-dependent relationship was also observed.

9.Mechanism of Shenqi Yiliu Prescription Combined with Cisplatin on H22 Liver Cancer-bearing Mice Based on NLRP3/Caspase-1/GSDMD Pyroptosis Pathway

ObjectiveTo explore the anti-tumor effect and mechanism of Shenqi Yiliu prescription in the intervention of pyroptosis. MethodTen male BALB/c mice were randomly selected and assigned to the blank group. The remaining 40 mice underwent the induction of the liver cancer xenograft model. After 5 days of modeling, 40 surviving mice were randomly divided into model group, cisplatin group [2.5×10-3 g·kg-1·(3 d)-1], Shenqi Yiliu prescription group (27 g·kg-1·d-1), and a combination group (Shenqi Yiliu prescription group + cisplatin). The mice in the blank group and the model group were treated with an equal volume of normal saline for 10 days. The general conditions of mice in each group were observed. After the intervention, the tumor weight of the mice was weighed and the tumor inhibition rate was calculated. Hematoxylin-eosin (HE) staining was used to observe the pathological changes in tumor tissues. The levels of mouse liver function indicators, including alanine aminotransferase (ALT) and aspartate aminotransferase (AST) were detected. The TdT-mediated dUTP-biotin nick end labeling (TUNEL) assay was used to detect DNA damage in mouse tumor tissue cells. Immunohistochemistry (IHC), immunofluorescence (IF), and Western blot were used to detect the protein expression levels of NOD-like receptor protein 3 (NLRP3), cysteinyl aspartate-specific protease-1 (Caspase-1), and gasdermin D (GSDMD) in tumor tissues. The levels of interleukin-1β (IL-1β) and interleukin-18 (IL-18) in tumor tissues were detected by enzyme-linked immunosorbent assay (ELISA). ResultCompared with the mice in the blank group, those in the model group were in a poor mental state, sleepy, and lazy, and their fur color was dull, with increased levels of serum ALT and AST in liver function tests (P<0.01). Compared with the model group, the groups with drug intervention showed improved mental state, inhibited tumor growth to varying degrees, and decreased tumor weight, and the tumor inhibition rate in the combination group was the highest (P<0.01). HE staining showed that the pathological and morphological lesions of the tumor tissues in the model group were significant, while those in all groups with drug intervention were improved to a certain extent. The karyolysis and nuclear rupture in the Shenqi Yiliu prescription group and the combination group were more significant. In the liver function test, the serum ALT and AST levels of mice in the Shenqi Yiliu prescription group and the combination group decreased (P<0.01), and the inflammatory factors IL-1β and IL-18 in each group with drug intervention decreased (P<0.05, P<0.01). Among them, the declining trend of IL-1β and IL-18 in the Shenqi Yiliu prescription group was the most significant (P<0.01). TUNEL staining showed that the positive TUNEL staining in each group with drug intervention decreased after intervention (P<0.05, P<0.01), especially the cisplatin group and Shenqi Yiliu prescription group (P<0.01). Western blot, IHC, and IF found that the protein expression levels of NLRP3, Caspase-1, and GSDMD in each group with drug intervention decreased (P<0.05, P<0.01). Compared with the mice in the cisplatin group, those in the Shenqi Yiliu prescription group and the combination group had better mental state and regular tumor morphology, and the tumor weight of the mice in the combination group decreased (P<0.05). The levels of ALT and AST in the Shenqi Yiliu prescription group decreased (P<0.05), and the levels of IL-1β and IL-18 in the Shenqi Yiliu prescription group and the combination group decreased (P<0.05, P<0.01), especially in the combination group (P<0.01). The results of IHC showed that the expression of GSDMD protein in the tumor tissues of mice in the combination group was reduced (P<0.01). IF detection showed that the expression of NLRP3 in the tumor tissues of the Shenqi Yiliu prescription group was reduced (P<0.01). The results of Western blot showed that the expression level of NLRP3 protein in the Shenqi Yiliu prescription group and the combination group decreased (P<0.01), and the expression level of Caspase-1 protein in the combination group decreased (P<0.01). The decrease in GSDMD protein expression was not significant, and the difference was not statistically significant. ConclusionShenqi Yiliu prescription combined with cisplatin has an obvious anti-tumor effect, which may be achieved by down-regulating the NLRP3/Caspase-1/GSDMD inflammatory pyroptosis pathway to inhibit cell pyroptosis, and relieve the inflammatory response in mice with liver cancer.
10.Comparison of Effect of Different Microbial Agents on Quality of Lycii Fructus Based on Multiple Index Components

ObjectiveTo reveal the effects of different microbial agents on quality of Lycii Fructus by comparing the differences in the contents of multiple types of chemical components in Lycii Fructus after the application of different microbial agents. MethodTaking Ningqi No. 7 as experimental material, four microbial agents, namely Peiyuan combined with Xinterui(TP group), Trichoderma harzianum combined with Bacillus subtilis(BW group), Genwuyou(MT group) and Junyiduo(JYD group), were applied, and no microbial agents was used as the blank group(CK group). Then the contents of total phenolics, total flavonoids, saccharides, amino acids, nucleosides and bases, betaine and other components in Lycii Fructus were determined by ultraviolet spectrophotometry(UV), high performance liquid chromatography(HPLC) and ultra-high performance liquid chromatography-mass spectrometry(UHPLC-MS/MS), and the methods such as multiple comparisons, principal component analysis(PCA) and partial least squares-discriminant analysis(PLS-DA) were used to analyze the effect of different microbial agents on the quality of Lycii Fructus. ResultMicrobial agents had different effects on chemical components of Lycii Fructus. The content of total phenolics was the highest in the TP group, and it varied significantly from the CK group(P<0.05). The total flavonoid content was the highest in the BW group, followed by the TP group. Both polysaccharide and alduronic acid contents were the highest in the JYD group. Betaine content in the TP and BW groups were significantly higher than that in the CK group(P<0.05). For the determined 23 kinds of amino acids, most of them were the lowest in the JYD group, and the highest in the MT group, while the nucleoside bases were higher in the MT and BW groups. It indicated that Lycii Fructus from different treatment groups could be distinguished clearly based on the determined 45 chemical components. The result of PLS-DA showed that the major differential components in each group were polysaccharides, glucose, fructose, betaine, alduronic acid, asparagine, sucrose, threonine, total flavonoids, alanine and total phenolics. The results of PCA composite scores based on the main differential components showed that composite scores of chemical components in each group were BW group>TP group>MT group>CK group>JYD group. ConclusionThe application of microbial agents of BW, TP and MT can promote the quality improvement of Lycii Fructus, and the application of JYD can promote the accumulation of polysaccharides and alduronic acid to a certain extent, but the overall effect on the quality of Lycii Fructus is not clear. This study lays the foundation for the green and healthy development of Lycii Fructus industry.
