Interleukin 24 ( IL-24) has a good prospect in tumor therapy because it can specifically inhibit proliferation in a variety of tumor cells in vitro and in vivo and induce apoptosis of tumor cells without affecting normal cells .Gene therapies which use recombinant adenovirus as a vector have some limitations that restrict the clinical application of IL-24. In comparison, protein drugs have tremendous advantages .In this paper, the progress in research on IL-24 gene engineering protein is elaborated .
2.Research on relative factors of abnormal glycometabolism in chronic hepatitis C patients

Objective Hepatitis C virus patients are often accompanied by insulin resistance and diabetes.To probe the relative factors of abnormal glycometabolism in chronic HCV infections.Methods A total of 1 039 treatment-naive patients that were confirmed chronic HCV infected were enrolled in the study.The demographics,biochemical index parameters and other data about liver function and HCV viral load were got from infectious disease department of Jurong Pepole's Hospital in China.Results A total of 140 (13.5%) patients were diagnosed with some forms of abnormal glycometabolism.The body mass index (BMI) (x2 =9.231,P =0.010),waist circumference (x2 =7.984,P =0.018),systolic blood pressure (x2 =16.366,P <0.001),diastolic blood pressure (x2 =13.970,P =0.001),alanine aminotransferase(ALT) (x2 =4.809,P =0.028),HCV-RNA viral load (t =-3.818,P <0.001) were significantly different between non-diabetic HCV patients and abnormal glycometabolism patients.Multivariate logistic regression analysis showed that ALT(OR =2.986,95% CI:1.171-7.615) and HCV-RNA viral load (OR =2.061,95% CI:1.165-3.644) were found as risk factors in multivariate regression analysis for patients with chronic hepatitis C who had abnormal glucose metabolism.Conclusions Chronic hepatitis C patients with higher ALT and HCV-RNA level were more probably to suffer from abnormal glycometabolism.In order to find potentially novel risk factors of HCV with abnormal glucose metabolisn,further studies about genetic and other clinical factors need to be processed.
3.Lipogranuloma of the lung: a case report.

Lipogranuloma is a rare benign disease accompanied by a reactive inflammatory process related to exogenous or endogenous lipids. We report a case of lipogranuloma of the lung mimicking pulmonary neoplastic disease in a 57-year-old woman, who presented with the symptom of coughing for a month. The diagnosis of lung cancer was considered for the CT finding of a mass in right lower lobe. The patient underwent subsequent operation, and pathological examination of the surgical specimen revealed a stromal lesion consisting of lipid vacuoles with foreign body type giant cells and scattered lymphocytes. The final diagnosis of lipogranuloma of the lung was established. Clinicians should be aware of lipogranuloma in the lung, which can be easily confused with lung tumors or other benign diseases. A careful differential diagnosis is necessary.

4.Peripheral blood immune cells in long-term survival patients after liver transplantation

Objective To explore the features of peripheral blood immune cells in long-term survival recipients after liver transplantation.Methods The expression of T subsets (Th1,Th2,Th17,Th22,Tregs),NK cells,NKt cells,Bregs,MDSC in long-term survival recipients (postoperative follow-up time ≥5 years,30 cases),short-term survival recipients(postoperative follow-up time ≤1 year,15 cases) and healthy control (15 cases) were determined by flowcytometry.Results Th17 cells were significantly higher in the long-term group compared with short-term group and healthy control group(P <0.01).Tregs in long-term group compared with short-term group were significantly higher (P < 0.01),but the difference was not statistically significant compared with healthy control group (P > 0.05).NK cells were significantly higher in long-term group compared with short-term group and healthy control group (P < 0.01).MDSC were significantly higher in long-term group compared with short-term group and healthy control group (P <0.01).Conclusions Th17,Tregs,NK cells and MDSC were significantly higher in long-term survival of liver recipients,which may be related to immune tolerance.
5.Relationships between serum lipids, CA153 level and breast cancer incidence and clinicopathological features of patients

Objective:To explore the relationships between serum lipids, CA153 level and breast cancer incidence and clinicopathological features of patients.Methods:A total of 198 patients with breast cancer diagnosed and treated at Nanjing Drum Tower Hospital, the Affiliated Hospital of Nanjing University Medical School were enrolled as the case group, and 198 healthy women were selected with 1∶1 age pairing as controls. Five milliliters of fasting venous blood was collected to measure serum lipids levels in all subjects and CA153 levels in breast cancer patients. The difference of serum lipids levels between the two groups was compared. Logistic regression model was used to analyze the risk factors of breast cancer. For 165 breast cancer patients who did not receive neoadjuvant chemotherapy, independent sample t-test was used to compare serum lipids and CA153 levels in breast cancer patients with different pathological features, and Pearson correlation analysis was used to calculate the correlation between variables and CA153. Results:The triglyceride (TG) levels in the case group and the control group were (1.22±0.73) mmol/L and (1.06±0.52) mmol/L respectively, and the difference was statistically significant ( t=2.559, P=0.011); the total cholesterol (TC) levels were (4.47±0.86) mmol/L and (4.99±0.80) mmol/L respectively, and the difference was statistically significant ( t=-6.228, P<0.001); the high-density lipoprotein cholesterol (HDL-C) levels were (1.32±0.34) mmol/L and (1.53±0.38) mmol/L respectively, and the difference was statistically significant ( t=-5.913, P<0.001). Higher TC and HDL-C levels were independent protective factors for breast cancer ( OR=0.350, P<0.001; OR=0.531, P=0.013). The TC levels in lymph node positive and lymph node negative patients were (4.36±0.73) mmol/L and (4.67±0.83) mmol/L respectively, and the difference was statistically significant ( t=-2.518, P=0.013); low-density lipoprotein cholesterol (LDL-C) levels were (2.53±0.58) mmol/L and (2.77±0.70) mmol/L respectively, and the difference was statistically significant ( t=-2.312, P=0.022). The TC levels in patients with stage Ⅰ and stage Ⅱ/Ⅲ were (4.90±0.89) mmol/L and (4.46±0.76) mmol/L respectively, and the difference was statistically significant ( t=2.855, P=0.005); LDL-C levels were (2.95±0.71) mmol/L and (2.60±0.63) mmol/L respectively, and the difference was statistically significant ( t=2.705, P=0.008). The level of CA153 in triple-negative breast cancer patients [(14.94±7.45) U/ml] was significantly higher than that in non-triple-negative breast cancer patients [(11.96±5.96) U/ml], and the difference was statistically significant ( t=2.359, P=0.020). The level of CA153 was positively correlated with the level of TG ( r=0.167, P=0.032). Conclusion:Dyslipidemia is associated with an increased risk of breast cancer. The levels of serum lipids vary among patients with different lymph node status and tumor stages. CA153 level is positively correlated with TG level to some extent.
6.HCV infection status and risk factors in remunerated blood donors in Jiangsu province

Objective To investigate the infection status of HCV in remunerated blood donors and risk factors in Jiangsu province.Methods A Cross-sectional study was conducted among people aged >50 years.Questionnaires were used to collect the information about their demographic characteristics and risk behaviors,and venous blood samples were collected from them to detect HCV anti-body,HCV-RNA and other biochemical indicators.EpiData and Stata were used for data entry and statistical analysis.Results The overall HCV sero-prevalence rates were 22.55% and 61.05% among remunerated blood donors.Data firom multiple stepwise regression analysis showed that alanine aminotransferase (ALT) (adjusted OR=1.38,95%CI:1.18-1.62) and aspartate aminotransferase (AST) (adjusted OR=l.30,95%CI:1.10-1.54) were associated with the outcomes of HCV infection,and fasting plasma glucose (adjusted OR=1.17,95%CI:1.01-1.35) were associated with HCV RNA viral loads.Conclusion The prevalence of HCV infection in remunerated blood donors was high,clinical ALT,AST and fasting plasma glucose levels were associated with the risk for HCV infection and HCV RNA viral load.
7.Related factors for severe liver fibrosis in chronic hepatitis C patients with remunerated blood donation history in Jurong of Jiangsu province

Objective The incidence of liver fibrosis in patients with chronic hepatitis C is high.Without effective treatment,it would lead to liver cirrhosis.This study is to identify the related factors for the incidence of liver fibrosis in patients with chronic hepatitis C in order to make early intervention treatment and reduce the case fatality rate.Methods This cross-sectional survey was conducted in adults aged ≥50 years with local residence for more than 5 years in Jurong of Jiangsu province from March to May in 2015,the patients infected with hepatitis C virus through remunerated blood donation were screened and included in the analysis.Descriptive statistical analysis was done to compare the differences in the incidence of liver fibrosis among the patients with different age,sex and education level or co-infected with hepatitis B virus or not.The risk factors for severe liver fibrosis were identified with univariate and multivariate logistic regression analysis.Liver fibrosis was diagnosed by using FIB-4 index method.Results A total of 719 patients with chronic hepatitis C were surveyed.Severe liver fibrosis developed in 285 of the 719 patients,in whom 21.84% was males.Multivariate logistic regression analysis showed that the patients with higher education level (OR=0.65,95%CI:0.47-0.90) and with access of antiviral therapy (OR=0.33,95% CI:0.22-0.49) had lower risk for severe liver fibrosis,the patients with high fasting blood glucose level (OR=1.80,95%CI:1.19-2.77) and abnormal white blood cell count (OR=2.77,95%CI:1.95-3.90) had higher risk for severe liver fibrosis.Conclusions The incidence of severe liver fibrosis in patients with hepatitis C was affected by many factors.Higher education level and antiviral therapy were the protective factors,but high fasting blood glucose level and abnormal white blood cell count were the risk factors.
8.Clinical efficacy of application of hepatitis B surface antigen-positive donor liver in liver transplantation

Objective To evaluate the clinical efficacy of application of hepatitis B surface antigen (HBsAg)-positive donor liver in adult liver transplantation. Methods Clinical efficacy of 28 recipients with liver diseases induced by virus B hepatitis (hepatitis B) undergoing liver transplantation using HBsAg-positive donor liver from July 2012 to October 2015 was retrospectively analyzed. Clinical prognosis and postoperative complications of the recipients were summarized. The changing features of serum levels of HBsAg and hepatitis B virus (HBV) DNA was investigated. Results After liver transplantation, 28 recipients were orally administered with entecavir to prevent the recurrence of hepatitis B. During perioperative period, 2 recipients died from sepsis and acute heart failure. During postoperative follow-up, 2 cases died from the recurrence of hepatocellular carcinoma (liver cancer). The remaining 24 patients were followed up for 12-26 months. Throughout the follow-up, 24 recipients were positive for serum HBsAg. After treatment, the titre of HBV DNA was significantly declined to <1×102 copies/mL at postoperative 12 months. No graft dysfunction induced by hepatitis B recurrence occurred in 24 recipients alive. Conclusions As a marginal donor liver, HBsAg-positive liver graft is safe for liver transplantation in the recipients with hepatitis B-related liver diseases. Postoperatively, anti-HBV treatment should be strengthened and intimate follow-up should be delivered.
9.Study on pathogenesis and laboratory diagnosis of a family with von Willebrand disease caused by c.1117C>T/c.7288-9T>G compound heterozygous mutation

Objective To explore the diagnosis of clinically suspicious von Willebrand disease(vWD)in a family and its pathogene-sis.Methods The pedigree information and the biological specimen were collected from the clinically suspected VWD patient and her family members(4 persons in total)in Peking University First Hospital.The levels of platelet count(PLT),activated partial thrombo-plastin time(APTT),vWF antigen(vWF:Ag),vWF activity(vWF:Ac)and FⅧ activity(FⅧ:C)were detected,and vWF risto-cetin cofactor(vWF:RCo)assay,ristocetin-induced platelet aggregation assay(RIPA)and vWF collagen binding(vWF:CB)assay were performed for phenotype diagnosis.The peripheral blood genomic DNAs were extracted from the proband and her family members to perform whole-exome sequencing for identifying the mutation of vWF gene,The mutation site was analyzed by using bioinformation tools to explore the pathogenesis of the proband.Results The APTT of proband(m 1)was slightly prolonged and her vWF:Ag,vWF:Ac,vWF:RCo and vWF:CB were significantly decreased.There was no obvious aggregation in RIPA assay(1.0 mg/mL and 1.25 mg/mL).In her father(Ⅱ3),APTT,FⅧ:C,vWF:Ag,vWF:Ac and vWF:CB were normal,but vWF:RCo was slightly decreased.In her mother(Ⅱ4),APTT,FⅧ:C,vWF:Ag,vWF:RCo and vWF:CB were all normal,but vWF:Ac significantly decreased.In her brother(Ⅲ2),APTT and FⅧ:C were normal,but vWF:Ag,vWF:Ac,vWF:RCo and vWF:CB were reduced to varying degrees.In all the family members(father,mother and brpther),no apparent aggregation in RIPA(1.0 mg/mL)was shown.Genetic analysis showed that the proband(Ⅲ1)carried a compound heterozygous mutation of vWF gene c.7288-9T>G and c.1117C>T,her father(Ⅱ3)carried vWF gene c.7288-9T>G heterozygous mutation,and vWF gene c.1117C>T heterozygous mutation was presented in both mother(Ⅱ4)and brother(Ⅲ2).Conclusion According to the results of laboratory tests,the proband was diagnosed as type 2A vWD.The hetero-zygous mutation in vWF gene c.1117C>T and c.7288-9T>G may be the molecular mechanism leading to type 2A vWD in the proband.

Aortic dissection (AD) is a life-threatening vascular disease due to the tearing of aortic intimal layer, leading to the formation of pseudocavity. Once the acute progression of dissection happens, serious complications such as rupture and stroke may occur. The current imaging examinations for AD are invasive and may cause adverse effects related to contrast medium, which cannot be used for large-scale screening of AD. The latest studies have found that metabolic processes and metabolites of lipids,saccarides and proteins are involved in the pathogenesis and development of AD. In this article, we review the research progress in the caracteristics of AD related metabolism,summarize changes of specific metabolites in AD,and explore the clinical implication of studies on AD related metaboliome..
