1.Experience of 56 cases with modified herniorrhaphy for adult oblique inguinal hernia

Objective To investigate the modified herniorrhaphy method for adult oblique inguinal hernia. Methods From June 2001 to June 2005,the clinical data of 56 paitients with oblique inguinal her-nias were analyzed retrospectively. Results Rapid recovery,with few complication as well as low cost were achieved.Follow-up time was in 6 months to 5 years.Postoperative recurrence was not found. Conclusion With modified herniorrhaphy,the normal anatomical structure of internal ring were restored and the trans-verse fascia defect are strengthened at the same time. Normal anatomical structure of inguinal canal are main-tained. This procedure possesses a higher cure rate and postoperative recurrent rate may be reduced obviously.
2.Study on endothelial dysfunction and platelet activation in elderly hypertensive patients complicated with cerebral infarction

Objective The aim of this paper was to investigate endothelial dysfunction,platelet activation, and inflammation in elderly hypertensive patients and those complicated with cerebral infarction. Methods Twenty-eight elderly hypertensive patients complicated with cerebral infarction(within 72 hours after onset of neurological symptoms),thirty-one Stage Ⅰ~Ⅱelderly hypertensive patients and twenty-eight elderly healthy individuals were selected as subjects.Their plasma concentrations of von Willebrand factor(vWF), alpha granule membrane protein-140(GMP-140), and serum concentrations of C-reaction protein(CRP) were examined. Results Elderly hypertensive patients complicated with cerebral infarction had significantly higher plasma vWF, GMP-140, and serum CRP than elderly hypertensive patients and elderly healthy individuals. Plasma vWF and GMP-140 were higher in elderly hypertensive patients than elderly healthy individuals, whereas serum CRP was increased slightly and there was no significant difference between elderly hypertensive patients and elderly healthy individuals. Conclusion This study suggests that elderly hypertensive patients complicated with cerebral infarction have obvious endothelial dysfunction, platelet activation, and inflammatory change. Stage Ⅰ~Ⅱ elderly hypertensive patients have obvious endothelial dysfunction and platelet activation as well.
4.Experimental study of true precocious puberty model in female rats

The precocious puberty in female rats were induced by subcutaneous injection of N-methyl-DL-aspartate (NMA). The female rats with induced precocious puberty showed the same sexual feature as the rats that had normal puberty. NMA plays its role at the level of hypothalamus. Subcutaneous injection of NMA is an effective means to induce true precocious puberty in female rats.
6.Ventilator-associated Pneumonia due to Acute Lung lnjury and Acute Respiratory Distress Syndrome after Severe Injury:High Risk Factors and Clinical Characteristics

OBJECTIVE To explore the incidence rate,pathogen distribution and characteristics of drug resistance of ventilator-associated pneumonia(VAP) patients due to acute lung injury and acute respiratory distress syndrome(ALI/ARDS) after severe injury,and analyze impact factors on VAP.METHODS All 183 cases with ALI/ARDS after severe injury from Jan 2004 to Mar 2008,were admiitted and given mechanic ventilation over 48 hours in our ICU and EICU.RESULTS VAP was found in 98 cases,accounted for 53.56%.Among them,42 were early-onset VAP and the other 56 were late-onset VAP.Altogether 276 pathogens were isolated,and most of them were Pseudomonas aeruginosa,Staphylococci aureus,Klebsiella pneumoniae,Acinetobacter baumannii and Escherichia coli.In 41 cases only one pathogen was isolated,the other 57 cases were with two or above pathogens.The revealed important risk factors were coma,prolonged mechanical ventilation,tracheotomy,antiacid treatment,use of antimicrobial agents and low albumin.CONCLUSIONS The incidence rate of VAP due to ALI/ARDS is high,and most of the isolated pathogens are drug resistant.Mixed infections are found commonly in these cases.
7.Study on mechanism of forming red-skin and dark-green-skin of Panax quinquefolius

Object To study the causes for the occurrence of red skin and dark green skin of Panax quinquefolius L. during processing and its mechanism. Methods To observe the phenomena and analyze the result which simulated the processing condition of P. quinquefolius based on its components. Results The cross section of P. quinquefolius began to appear light red at 40 ℃, lasting 72 h. Whereas the time of browning was shortened with the temperature rising. The cross section of P. quinquefolius turned green while dipping in solution of Fe 3+ ion of 0 01 mol/L for 25 min. The time of turning green was shortened with the increasing concentration of Fe 2+ , Fe 3+ solution. Conclusion The results show that red skin of P. quinquefolius was caused by the Maillard reaction while drying at the excessive higher temperature. Whereas the complex reaction between phenolic substances in P. quinquefolius and metal ions during processing might result in dark green skin of P. quinquefolius. This expounds the mechanism of red skin and dark green skin turning during the P. quinquefolius processing in the theory.
8.Preparation of Platycodin-D for Reference Substance

Objective To establish the preparation techniques for preparing the reference substance of platycodin-D.Methods Raw material of platycodi radix was extracted with 70 %MeOH.The concentrated extract passed through D101 macroporous resin column,then the 40 %alcohol eluate was separated by silica gel column chromatography with chloroform-methanol gradient elution.The fractions contained platycodin-D were collected and purified by HPLC.Results HPLC analysis and normalization of peak areas showed the purity of the platycodin-D was 98.8 %.Conclusions The preparation techniques are simple and high yield,can be used for preparing the reference substance of platycodin-D.
9.The relationship between IL-1β,intercellular adhesion molecule in serum and diabetic renal microangiopathy

Objective To study the influence of interleukin(IL)-1β,intercellular adhesion molecule(ICAM)on diahetic renal microangiopathy.Methods Sixty-two patients with type 2 diabetic mellitus were divided into three groups:normal albumin urine group(20 patients),microamount albumin urine group(22 patients),and dominancy albumin urine group(20 patients)according to their urinary albumin excretion rate(UAER).IL-1β,ICAM was measured by enzyme-linkedimmuno sorbent assay(ELISA)method in the type 2 diabetic mellitus and the healthy persons.Results The concentration of IL-1β,ICAM in the three groups with type 2 diabetic mellitus was higher than that of the twenty healthy persons(P<0.05),higher in the dominancy albumin urine group than that in the microamount albumin urine and normal albumin urine groups(P<0.01).The concentration of IL-1β in the microamount albumin urine group was higher than that in the normal albumin urine group(P<0.05),the concentration of ICAM in the microamount albumin urine and normal albumin urine groups was equal(P>0.05).Higher serum IL-1β and ICAM was associated with higher UAER(P<0.05).The concentrations of IL-1βwas positively correlated with ICAM.Conclusion ICAM,IL-1β is probably correlated to UAER of diabetic renal microangiopathy,80 it has very important meaning and clinic value to measure the concentration of IL-1β,ICAM in serum earlier.
10.Clinical analysis of caspofungin treatment for invasive fungal disease in advanced age patients

Objective To evaluate the efficacy and safety of caspofungin in the treatment of invasive fungal disease (IFD) in advanced age patients. Methods A retrospective analysis of the clinical data was conducted. IFD patients who had received caspofungin treatment in elderly wards were included. Results From January 2007 to August 2009, 29 IFD patients aged 80-100 years (mean 89 ) were treated with caspofungin. Except that one patient dead on the first drug administration day, there were 28 evaluable patients, 13 were cured (46.4%), 6 were markedly improved (21.4%), 3 progressed (10.8%) and 6 dead (21.4%). The overall effective rate was 67. 8%. Of 13cured patients, 12 were Candida bacreremia, 1 was Candida albicans disease of lung. Of 6 dead patients, 2 were Candida bacteremia, 1 was Candida albicans disease of lung and 3 was suspected of lung IFD. One patient performed that alanine aminotransferase was increased, considering drug-related impairment of liver function. Conclusions Caspofungin is effective and safe in the treatment of IFD in advanced age patients.
