1.Effect of Haikun Shenxi Capsule on disorder of lipid metabolism in patients with chronic kidney disease

AIM:To investigate the effect of Haikun Shenxi Capsule(fucoidan)on disorder of lipid metabolism on the patients with chronic kidney disease(CKD).METHODS:98 patients with CKD 3、4 stage were divided into 2 groups at random:control group were given routine therapy and therapy group were given routine therapy plus Haikun Shenxi Capsule.The changes of the levels of TCH,TG,HDL-Ch,LDL-Ch,VLDL-Ch,ApoA-1,ApoB in 2 groups were observed.RESULTS:The level of TCH,TG decreased significantly(P 0.05).CONCLUSION:Haikun Shenxi Capsule has a positive effect on disorder of lipid metabolism on the patients with CKD.
2.Clinicopathological analysis of malignant triton tumor of the posterior mediastinum

Objective To investigate the pathologic appearances,immunohistochemical features,and genetic changes of malignant triton tumor(MTT). Methods One case of MTT was studied pathologically and immunohistochemically,and the related literatures were reviewed. Results A huge mass,demonstrated in the thorax by X ray and CT scan was seen in the posterior mediastinum in the surgery.Histologically,the tumor was composed of spindle cells with significant atypia.Some of the tumor cells had dense eosinophilic cytoplasm.Immunohistochemical staining revealed positive for myoglobin,desmin and S-100 in most of the tumor cells.The pathological diagnosis was MTT of the posterior mediastinum. Conclusion Cases of MTT in the mediastinum are very rare,with less specific clinical and imaging manifestations.The diagnosis is mainly made on the basis of pathological examination and immunohistochemical staining.
3. Liver microvessel density in evaluating therapeutic effect of sorafenib combined with argon-helium cryoablation in treatment of advanced hepatocellular carcinoma

Objective To investigate the therapeutic effect of argon-helium cryoablation combined with sorafenib on hepatitis B-related advanced hepatocellular carcinoma (HCC) and to evaluate the role of microvessel density (MVD) in prognostic evaluation. Methods A total of 102 patients with advanced HCC were randomly divided into two groups, with 50 receiving sorafenib plus cryoablation(group S&-C) and 52 receiving only cryoablation (group C). The endpoint of the treatment was tumor progression or untolerable adverse reaction. Tumor tissues were obtained before treatment. MVD was evaluated by immunohistochemical analysis using CD34 antibody. The therapeutic effects were evaluated according to RECIST criterion every 4-6 weeks. The adverse events were observed in both groups; the therapeutic effect, overall survival time (OS), and time to progression(TTP) were compared between two groups. Results In group ScV-C, complete response (CR) was achieved in 2 patients(4%), partial response (PR) in 9 (18%), and stable disease(SD) in 22(44%), with the disease control rate(DCR) being 66%; in group C. no CR, PR in 4(7. 6%), and SD in 19(36. 5%), with the DCR being 44. 2% (P<0. 05). The overall survival (OS) and the time to progression (TTP) were significantly longer in group S&-C than in group C (12. 5 months vs 8. 6 months, 9. 6 months vs 5. 3 months; P<0.01). The mean MVD in group CR&.PR (111/0.74 mm2) was significantly lower than that in group PD( 339/0. 74 mm2, P<0.01). Patients with lower MVD receiving sorafenib plus cryoablation had longer OS and TTP than those only receiving cryoablation. The OS and TTP in patients with higher MVD had no significant difference between different groups. Conclusion Sorafenib plus cryoablation is a safe and effective therapy for patients with advanced HCC; it can improve the OS and TTP of patients. Patients with higher MVD have a lower response to therapy and poor prognosis.
4.Autologous iliac crest graft combined with autologous bone marrow stem cell transplantation for bone nonunion after fracture surgery

BACKGROUND:Bone nonunion is a common complication in the orthopedic treatment, and its morbidity reached 5%-10%, which results in the long-term functional disturbance of the limbs, and even disability. Autogenous iliac crest graft has been commonly used to treat bone nonunion, but some limitations stil exist.
OBJECTIVE:To investigate the treatment outcomes of autogenous iliac crest graft combined with autologous bone marrow stem cel transplantation for bone nonunion after fracture surgery.
METHODS:Clinical and fol ow-up data from 69 patients with bone nonunion were analyzed retrospectively. Al patients were al otted to combination (n=37) and iliac (n=32) groups, fol owed by treated with autologous iliac crest graft combined with autologous bone marrow stem cel transplantation or autologous crest graft, respectively. Afterwards, the hospitalization time, fracture healing time, bone mineral density and Fereadez-Esteve cal us scores were detected and compared between groups.
RESULTS AND CONCLUSION:The hospitalization time did not differ significantly between groups (P>0.05). The fracture healing time in the combination group was significantly shortened compared with the iliac group (P<0.05). The bone mineral density and Fereadez-Esteve callus scores in the combination group were significantly higher than those in the iliac group at 3 and 6 months after surgery (P<0.05). The excellent and good rate of the affected limb function in the combination group was significantly higher than that in the iliac group (P<0.05). These results suggest that autogenous iliac crest graft combined with autologous bone marrow stem cell transplantation for bone nonunion can accelerate fracture healing, promote porosis and improve the functional recovery of affected limbs.
5.Formulation and preparation method of long-acting interferon ?-2b loaded injectable microspheres

Objective:To prepare injectable interferon ?-2b(IFN-?-2b) loaded microsphere and develop a long-acting dosage form.Methods: IFN-?-2b loaded microspheres were prepared with poly(lactic-co-glycolic acid)(PLGA) as carrier material by double emulsion(w/o/w) method and solid in oil in oil(s/o/o) method separately.Physical and chemical characteristics of microspheres(mean diameter,morphology and drug entrapment efficiency) were evaluated;the in vitro release behavior and influencing factors of the microspheres were determined by micro-BCA(bicinchoninic acid) method;and IFN-?-2b stability during encapsulation and in vitro release was evaluated by sodium dodecyl sulfate polyacrylamide gel electropheresis.Results: The 2 types of microspheres produced had good shape and dispersive quality and a drug entrapment efficiency of more than 80%.IFN-?-2b bulk ultrafitration can significantly influence the mean diameter and in vitro release behavior of microspheres prepared by w/o/w method.The accumulated release(within 1 month) of the microspheres prepared by both methods was significantly improved when using PLGA with lower inherent viscosity.SDS-PAGE test showed aggregation of IFN-?-2b with s/o/o method,while there was no difference between the electrophoretic behavior of bulk IFN-?-2b and IFN-?-2b in microspheres prepared by w/o/w method.Conclusion: IFN-?-2b can be encapsulated into injectable microspheres to yield a one-month continuous release by both w/o/w method and s/o/o method.
6.The growth inhibition effect of 131I-fulvestrant on the human breast cancer cells

Objective To elucidate the inhibitory effect of 131I-fulvestrant on the growth of human breast cancer cells and the effect on the important organs.Methods MTT assay was used to clarify the difference in killing effects of the 131I-fulvestranton on MCF-7 cells and MDA-MB-231 cells.Breast cancer MCF-7 cell xenografts in nude mice was establishied,and two different administration methods of the 131I-fulvestrant in the MCF-7 cell to nude mice were given respectively.Organs and tumours of nude mice were observed.Results MTT assay demonstrated that 131I-fulvestrant had similar cytotoxicity against MCF-7 cells and MDA-MB-231 cells,and the former was slightly stronger.Transient contact experiments showed that 131I-fulvestrant could play a tumor suppressor effect on MCF-7 cells continually,but MDA-MB-231 cells wasn't.After the injection of 131I-fulvestrant via caudal vein,the radioactivity concentration on tumor site accounted for (4.33 ± 0.28)% of the total injection,and the volume of the tumor reduced before gradually increasing again.Radioactivity in the blood accounted for (20.76 ± 2.54)% of the total injection.Qrgans like liver and kidney also showed radioaction distribution.Its distribution was accorded with the distribution of estrogen receptor.Local injection of 131I-fulvestrant got powerful killing effect on the tumor,and the distribution of the radioaction was mainly confined within the tumor.Conclusion 131I-fulvestrant has a good inhibitory effect on MCF-7 breast cancer cells,which is a superposition of radiotherapy and endocrine therapy,and it is controllable on the general condition and important organs of nude mice.
7.Expression of thyroid transcription factor-1 in hepatocellular carcinoma

Objective To investigate the value of thyroid transcription factor-1(TTF-1) in the diagnosis and biological behavior assessment of hepatocellular carcinoma (HCC). Methods Thirty liver specimens obtained from benign lesions were analysed, among which 25 were hepatic cirrhosis and inflammatory diseases, and the other 5 were adenomas. And there were 176 specimens of liver tumors, among which 142 were HCC (well differentiated, n=12; moderately differentiated, n=57; poorly differentiated, n=73), 17 were intrahepatic cholangiocellular carcinoma (ICC) and the other 17 were liver metastatic carcinoma (MC). The expression of TTF-1 was examined immunohistochemically in the above tissues, and the difference in expression of TTF-1 among different tissues was examined by Fisher's exact test, Kruskal-Wallis test and Spearman rank correlation analysis. Results TTF-1 was significantly expressed in the cytoplasms of all the hepatocytes besides tumors and liver benign lesions. Objective To assess the technical feasibility, patency and outcomes of subintimal angioplasty (SIA) for treatment of the lower extremity arteriosclerosis occlusions. Methods During the period from December 2003 to August 2008, 122 lower extremities with arteriosclerosis occlusions (median length, 10.25 cm;range, 4.5 to 28 cm) were treated on an intention-to-cure basis with SIA. Twenty-three lower extremities had disabling claudication and ninety-nine had limb-threatening ischemia. Patient history, demographics, procedural details, complications, and follow-up information were collected and analyzed. Patency, limb salvage, sustained improvement in claudication was determined by Kaplan-Meier analysis. Results The technical success rate of SIA was 83%. Following successful SIA, the mean ankle-brachial index increased from 0.19±0.11 to 0.67±0.29 (P<0.01). Primary patency at 12 and 24 months was 54%±5% and 45%±4% respectively, the clinical effective rates (rates of limb salvage and improvement in claudication) at 12 and 24 months were 82%±5% and 79%±4% respectively. No serious complications occurred. Conclusion In a selected group of patients, SIA is feasible with a high technical success rate and the clinical effective rates are satisfactory, there were no severe complications. SIA is a good treatment alternative in patients suffering from chronic ischemia with arteriosclerosis occlusions in the lower extremity.
9.Protective effects of imipramine on alveolar epithelial barrier function against acute lung injury in mice

Objective To investigate the protective effects of imipramine on alveolar epithelial barrier functiou in mice against LPS-induced acute lung injury (ALI),and explore the possible mechanisms.Methods Total of 32 SPF male Balb/c mice were randomly (random number) divided into four groups:control group,Imipramine group,LPS group,LPS + Imipramine group.To establish an animal model of ALI,mice were administered intraperitoneally with LPS in 20 mg/kg.Mice were treated with imipramine in 25 mg/kg 30 min prior to LPS administration.FITC-FD4 was administered in mice via the tail vein with FITC-FD4 10 min before mice sacrificed under anesthesia at 12 hours after LPS administration,then bronchoalveolar lavage fluid (BALF) and lung tissue were obtained.HE staining was used to observe histopathological changes,and pathology scores;lung tissue wet-to-dry weight ratio and BALF/serum FD4 ratio were used to assess pulmonary edema and alveolar epithelial permeability.Real-time PCR,western blot and immunochemistry were employed to detect the mRNA expressions and protein levels of Occludin,Claudin-4 and ZO-1.Data were analyzed with SPSS 16.0 software,one way analysis of variance (ANOVA) was used to compare multiple sets of variables,the intergroup comparisons were analyzed by the least-significant-difference (LSD) tests with P < 0.05 for the statistically significant difference.Results Compared with LPS group,LPS + lmipramine group had a statistically significant decrease in pathological score [(9.22 ± 0.21) vs.(11.23 ± O.55),P < O.05);the wet-to-dry weight ratio in LPS + Imipramine group was less than that in LPS group and the difference was statistically significant (P < 0.05);compared with LPS group,the ratio of BALF/serum FD4 in LPS + Imipramine was less and the difference was statistically significant (P < 0.05);compared with LPS group,the mRNA expressions and protein levels of Occludin,Claudin-4 and ZO-1 in LPS + Imipramine group were significantly increased (mRNA:Occludin:P < 0.05;Claudin-4:P < 0.05;ZO-1:P < 0.05 . western blot:Occludin:P < 0.05;Claudin-4:P < 0.05;ZO-1:P < 0.05).Immunochemistry showed that Occludin and Claudin-4 were present mainly in alveolar epithelial cell membrane,Z0-1 was found mainly in cytoplasm of alveolar epithelial cell.In control group and Imipramine group,tight junction proteins were obviously expressed.Compared with control group,protein levels in LPS group were significantly decreased (Occludin:P < 0.05;Claudin-4:P < 0.05;ZO-1:t =6.59,P < 0.05);compared with LPS group,the tight junction proteins in LPS + Imipramine group were significantly increased (Occludin:P < 0.05;Claudin-4:P < 0.05;ZO-1:P < 0.05).Conclusion The protective effects of imipramine on alveolar epithelial barrier function by up-regulating tight junction proteins expression in murine LPS-induced ALI.
10.Analysis on quality control of blood in blood banks in a five-year program in Hunan province.

OBJECTIVETo investigate the blood quality of blood banks.
METHODSHBsAg, Anti-HCV, Anti-HIV were detected by enzyme linked immunoadsorbent assay (ELISA). Treponema pallidum was detected by Trust or rapid plasma regain card test (RPR) before August 2001 and ELISA was used afterwards. Alanine aminotransferase (ALT) was detected by Reitman-Frankel before December 1999 and by continuous monitoring assay afterwards. Results were compared between (1) sampling and spot check, (2) different determent methods, (3) two kinds of reagents with different antigen coating.
RESULTSSeventy-two (8.28 per thousand ) of the 8,699 plasma samples were found unqualified with levels of anti-HCV, ALT, HBsAg, Treponema pallidum and anti-HIV 24 (2.76 per thousand ), 19 (2.18 per thousand ), 16 (1.84 per thousand ), 7 (0.80 per thousand ), 6 (0.69 per thousand ), respectively. There were significant differences between different determent methods and different reagents, but only ALT showed significant difference in the sampling and spot check.
CONCLUSIONThe unqualified samples were associated with testing methods, quality of reagents as well as ability and responsibility of the staff.
