1.Preparation and Pharmaceutical Quality Evaluation of Nattokinase Immunoliposomes and Its Lyophilized Product

OBJECTIVE:To prepare nattokinase immunoliposomes and its lyophilized product and to evaluate its pharmaceutical quality. METHODS: Nattokinse liposomes were prepared by film dispersion method, then the immunoliposomes were prepared by carbodiimide method coupled anti-human fibrin D-dimer monoclone antibody SZ-63 and lyophilized and evaluated about its pharmaceutical quality. RESULTS: Lyophilized immunoliposomes were multilamellar vesicles with its mean diameter, Zeta potentials at 251.6 nm, —34.6 mV, respectively. The lyophilized product of nattokinase immunoliposomes exhibited good appearance and satisfactory redispersibility, with its loss on drying at 14.30% and critical relative humidity at 45.66%. CONCLUSION: Nattokinase immunoliposomes and its lyophilized product have been prepared successfully.
2.Meticulous nursing of indwelling urinary catheter between the children with hypospadias during ;perioperative

Objective To summarize the meticulous nursing experience of indwelling urinary catheter between the children with hypospadias during perioperative. Methods 317 cases of indwelling urethral only in the silicone Foley tube, and without other urinary diversion with hypospadias were retrospectively analyzed, summarized the fine points of nursing, including preoperative children with comprehensive risk assessment, standardized preoperative skin preparation and postoperative protective constraint, effective properly fixed catheter keep the urine drainage unobstructed, close observation of urine drainage characteristics, maintain urine collection system tightness, maintain perineal skin clean and improved extubation nursing method, and do a good job after extubation nursing. Results 266 cases were cured with a cure rate of 83.9%(266/317);the incidence of urine leakage in 42 cases, the incidence rate was 13.2%(42/317); 7 cases of urethral stenosis, the incidence rate was 2.2%(7/317); 2 cases of urethral closing, the incidence was 0.6% (2/317), none of the patients had accidental extubation. Conclusions The meticulous nursing has a better effect in hypospadias patients during the perioperative period and it can reduce the incidence of postoperative complications;shorten the time of hospitalization and alleviate the suffering of children;improve the quality of nursing service, which is worthy of clinical application.
3.Analysis of effectiveness and challenges in preventing the re-establishment of malaria transmission in China

Objective To analyze the characteristics of the malaria epidemic and the indicators for "1-3-7" from 2021 to 2023 in China, understand the effectiveness and challenges in preventing re-establishment malaria in China, propose response strategies, and provide references for consolidating the achievements of malaria elimination. Methods The individual malaria case data and focus data from 2021 to 2023 in the "Information System for Infectious Disease Surveillance" and the "Information System for Parasitic Diseases Prevention and Control" were collected, and epidemiological characteristics and indicators for “1-3-7” were analyzed. Results From 2021 to 2023, a total of 4 132 malaria cases were reported in 31 provinces and Xinjiang Production and Construction Corps, with mainly falciparum malaria (59.2%, 2 445/4 132). The national malaria epidemic reached a historical low of 799 cases in 2021, before rebounding significantly to 2 488 cases in 2023. The top five provinces for malaria cases were Yunnan, Guangdong, Henan, Sichuan, and Shandong, accounting for 49.4% of the total (2 043/4 132) cases. Except for two long incubation cases infected with P. malariae and one non-mosquito-transmitted case, the remaining cases were imported from abroad, mainly from African countries (81.7%, 3 374/4 129), with P. vivax malaria mainly coming from Myanmar (63.7%, 638/1 001). Malaria cases mainly occur in middle-aged men and migrant overseas workers. 142 severe cases and 21 deaths of malaria were reported. 81.8% (3 378/4 132) of malaria cases sought medical attention within 3 days of symptom onset, and the initial diagnosis institutions were mainly county-level, municipal, and provincial hospitals(77.2%), with an accuracy malaria diagnosis rate of above 80.0%. The completion rates of the malaria indicators for "1-3-7" were all above 90.0%. Conclusions Since the malaria elimination in China, there has been no re-establishment of malaria, and the surveillance response capability has been maintained at a high level. However, the epidemic of imported malaria continued to rise, severely endangering public health in China, especially in areas such as the China-Myanmar border where the risk of re-establishment was high. Currently, it is necessary to further enhance the awareness of the key populations about timely medical consultation for malaria, as well as healthcare workers' vigilance, diagnostic capabilities, and awareness of timely referrals. Efforts should be maintained to investigate and manage epidemics, strengthen prevention and control in key areas such as the China-Myanmar border in Yunnan, and continuously consolidate elimination achievements.
6.Efficency of Intravenous Azithromycin for Bacterial Infection s in 59 Cases

To evaluate the efficacy and safety of Azithromycin in the treatment of the bacterial infections. Method s: 94 patients with lower respiratory tract infection were randomly d ivided into 2 groups(47 for each gruop). The treated group were given Azithrom ycin 500mg in 5% glucose injection 500mL, iv drip, bid, for 5-7 days. Another 12 patients (including 4 patients with pelvic inflammatory disease and 8 patients with lower respiratory tract infection) were treated with Azithromycin as the op en group. Results: The treated group yielded a recovery ra te 61.7%, aeffective rate 91.5% and abacterial clearance rate 95.8%, which wer e higher than the controlled group [31.9%, 70.2%, and 76.6% (P<0.01)]0,res pectively. The total response rate and the cure rate in 59 patients treated with Azithromycin were 93.2% and 62.7%, respectively. The incidence of clinical adve rse drug reactions in the treated group was 12.8%, being lower than 34.0% in the controlled group (P<0.05). Conclusion: Azithromycin is an effective agent in the treatment of the acquired lower respiratory tract infection, urogenital and urogenital tract infection with slight adverse reaction .

8.Magnesium alloy-and titanium alloy-made transoralpharyngeal atlantoaxial reduction plate systems for atlantoaxial dislocation: a three-dimensional finite element analysis

ObjectiveTo evaluate biomechanical properties of transoralpharyngeal atlantoaxial reduction plate (TARP) prepared from magnesium alloy and titanium alloy for the atlantoaxial dislocation by using three-dimensional finite element analysis and to exam the feasibility of using magnesium alloy for preparation of TARP system so as to provide a theoretical basis for clinical surgery.MethodsA patient with typical atlantoaxial fracture dislocation was involved in the study,and received thin CT scan with clinically used titanium alloy TARP system for obtaining DICOM image data.Three-dimensional finite element analysis software was imported to simulate magnesium alloy and titanium alloy TARP systems for reduction and fixation.Then,stress changes of the atlas,axis,internal fixators and C2/3 zygapophysial joints were determined with three-dimensional finite element analysis and analyzed statistically.Results ( 1 ) The finite element model of atlantoaxial dislocation reduction and fixation had lifelike outline and good geometric similarity.There were 53 586 nodes and 180 784 units.(2) During the simulation of head in neutral position,the stress concentration region was C2/3 zygapophysial joints followed by the anterior arch,posterior arch and lateral mass of atlas respectively,and C2 vertebral arch again.( 3 )Magnesium alloy and titanium alloy TARP systems showed significant difference in stress distribution (P <0.05).Conclusions(1)The atlantoaxial model established according to its structure information on CT can be used for biomechanical experiments.(2) For the treatment of atlantoaxial dislocation using the existing titanium TARP system,maintaining the integrity of anterior and posterior arch of atlas and confirming the bone fusion in lateral mass can better keep the stability of the atlantoaxis.After atlantoaxial fusion,the increased stress of the zygapophysial joints of the adjacent segments accelerates structural degeneration,which should be closely followed up.( 3 ) Magnesium alloy TARP system for fixation and reduction shows the fall in peak value of the stress concentration region,and improvement of the uniformity of stress distribution as compared with titanium alloy TARP.

10.Extracorporeal shock wave therapy for treating disorders of the temopromandibular joint

Objective To study the therapeutic effects of extracorporeal shock wave therapy (ESWT) on temporomandibular joint disorders (TMDs).Methods Sixty patients with a temporomandibular joint disorder were randomly divided into an ESWT group and a control group,each of 30.Both groups were given routine rehabilitation training,but the treatment group additionally received ESWT for 2 weeks.Fricton's index was used to evaluate temporomandibular joint function.The subjects used a visual analogue scale (VAS) to evaluate their pain levels.The SF-36 scale was applied to assess their life quality.Results After two weeks of treatment,the average assessment score and VAS score had decreased significantly in both groups.After treatment the average scores for mandibular movement,muscle palpation,joint noise and joint palpation,and the palpation,dysfunction and craniomandibular indexes were all significantly lower in the ESWT group than in the control group.Significant differences were also found in the average VAS scores and on the social functioning domain of the SF-36 between the ESWT group and the control group.Conclusion ESWT combined with rehabilitation training can significantly relieve the symptoms and pain of TMDs.
