1.Clinical application of INRatio coagulometer for fast measuring international normalized ratio

ObjectiveTo compare the correlation and agreement of international normalized ratio (INR) measured by INRatio coagulometer and standard laboratory venous blood measurement in order to judge the clinical application value of measuring INR by INRatio coagulometer.MethodsOne hundred who received warfarin were chosen randomly.Venous blood and finger capillary blood were taken simultaneously for measurement of INR by laboratory coagulation analyzer SYSMEX CA-7000 and INRatio coagulometer respectively.The correlation and agreement of INR results tested by the two methods were analyzed.Results When INR ≤5,there was a good correlation between INRatio coagulometer and laboratory coagulation analyzer SYSMEX CA-7000 (r =0.898,P =0.000).The mean bias of the two methods was 0.36 and INR measured by INRatio coagulometer were higher than those tested by laboratory coagulation analyzer SYSMEX CA-7000.By Bland-Altman,it showed a good agreement for 91.2% (62/68) of the difference INR located 95% the limited of agreement when the mean INR 1.60 - 3.50,and a bad agreement for only 50.0%(1/2) of the difference INR located 95% the limited of agreement when the mean INR > 3.50.ConclusionsIn the therapeutic range,INR measured by INRatio coagulometer shows a good correlation and agreement with INR measured by the established laboratory method.INRatio coagulometer can be used to monitor INR in the patients received oral anticoagulant therapy.
2.Effects of ectopic pregnancy treatment on ovarian response

A total of 183 patients of ectopic pregnancy due to tubal factors were divided into the methotrexate (MTX),conservative surgery and salpingectomy groups.The dose of gonadotropin,counts of harvested oocytes and high-quality embryos and pregnancy rate were compared among three groups to analyze the differences of in vitro fertilization and embryo transfer (IVF-ET).And the above parameters showed no significant differences (P >0.05 ).The clinical pregnancy rate of the conservative group was higher than the other two groups.And the difference was statistically significant compared with salpingectomy ( P < 0.05 ).It suggested that the treatments of ectopic pregnancy had some effects on the outcome of IVF-ET.The pregnancy rate was slightly higher in the conservative surgery group.
3.Misdiagnosic Analysis of Breast Cancer on Mammogram (A Report of 15 Cases)

Objective To improve the radiological diagnosis of breast cancer .Methods The authors collected 15 cases of breast carcinoma confirmed by operation and pathology. All these cases were misdiagnosed by mammography.Mammographic manifestations and data of clinic were analyzed retrospectively. Results 4 cases changed like small fibroadenoma, 3 cases showed microcalcification, 3 cases showed localized mammary gland architecture distrotion,2 cases showed multiple small patchy shadows in the hyperplasic mammary gland, 2 case showed small focus of increased density, 1 case no abnormal sign, 4 cases among of them can not be palpated mass .Conclusion The combination between palpation and radiology is valuable for diagnosis of breast carcinoma and decreasing misdiagnosis.
4.Determination of the Concentrations of Ceftriaxone in Blood and Bile of Patients During Perioperative Period

OBJECTIVE:To study the anti-infective effect of ceftriaxone in perioperative period METHODS:28 cases of cholelithiasis complicating infections who had to undergo laparoscopic cholecystectomy were enrolled in this study Ceftriaxone,1g,im,was injected 1h~1 5h before anesthesia,and blood and bile were sampled before,during and after operation The concentration of ceftriaxone was determined by UV-spectrophotometry RESULTS:The concentrations of ceftriaxone in blood before,during and after operation were(37 59?16 13)?g/ml,(49 68?10 51)?g/ml and (27 09?12 38)?g/ml respectively The drug concentration in blood peaked 2h after drug administration,at this time,the drug concentration of bile was 4 times that of blood (201 77?20 16)?g/ml CONCLUSION:The drug concentration in blood was 49 times as high as MIC,which indicates that ceftriaxone has preventive and therapeutic effects on possible infections during perioperative period
5.Depression and its coping strategies in infertile women undergoing in vitro fertilization and embryo transfer

Objective To know prevalence of depression, its coping strategies and risk factors among infertile women with in vitro fertilization and embryo transfer (IVF-ET) or intracytoplasmic sperm injection. Methods Infertile women with IVF-ET were interviewed with center for epidemiologic studies short depression scale (CES-D10) and an abbreviated version of the COPE inventory (brief COPE), and their basal sex hormone values and estradiol ( E2 ) on the day of human chorionic gonadotropin (HCG) administration were monitored, as well as the number of oocytes yielded was counted. Results Prevalence of depression was 20.6% in infertile women, higher in those with more than eight years of marriage, infertility for more than six years, and receiving treatment for at least three years. Risk of depression among the women of infertility with monthly family income less than 3000 yuan was 14 times as those with more than 3000 yuan and risk of depression among them due to the factors of their husbands was 2/5 as those not due to men's. Depressive symptoms in the women increased with increasing of their basal follicle stimulating hormone (FSH) level, counted egg number and scores of denial items. Occurrence of their depressive symptoms decreased with higher basal E2 levels, and scores of substance use and humor. Conclusions Some infertile women with IVF-ET do have depression and preventive intervention should be aimed at related factors for depression to improve their psychological health.

Neuroglobin(Ngb)is one of the members of oxygen-carrying globin family.It mainly exists in neurons in a monomeric form,which is closely correlated the oxygen supply to brain.Brain hypoxia/ischemia can induce the high expression of Ngb,and as an endogenous neuroprotective factor,it protects ischemic neurons from ischemia/hypoxia injury.This article reviews the distribution,structure and function of Ngb,and its potential protective effects and mechanisms in cerebral ischemia/hypoxia cerebral injury.
7.Knowledge requested by patients with coronary artery disease after stent implantation

Objective To survey the knowledge requested by patients with coronary artery disease (CAD) after stent implantation.Methods A survey on CAD and stent-related knowledge requested by CAD patients with stent implantation was conducted with a self-designed questionnaire from September 2013 to November 2013.Total 362 patients (235 males and 127 females) aged (67 ± 12) years participated in the survey in Yuetan Community Health Service Center and Fuxing Hospital in Beijing.Open questions and heuristic questions were used together during the survey.Results The knowledge requested by patients were:diet management(255,70.4%),side effects of medication(241,66.6%),management of physical activity (221,61.0%),prevention of CAD(181,50.0%),withdrawal of medication(176,48.6%),the life of stents (168,46.4%),post-operation check-up items (162,44.8%),the causes of CAD (156,43.1%)and drug mechanisms(134,37.0%).Patients with stent implantation less than one year requested more knowledge about the cause of CAD (x2 =37.4,P < 0.001),life-time of stent (x2 =39.9,P <0.001),prevention of CAD (x2 =13.4,P =0.001),post-operation check-up items (x2 =21.2,P =0.001) and requested less knowledge about withdrawal of medication (x2 =9.56,P =0.008),diet management (x2 =6.49,P =0.04) than those with more than one year.Patients aged < 70 years requested more knowledge about the cause of CAD (x2 =31.6,P < 0.001),prevention of CAD (x2 =9.99,P =0.007),life-time of stent (x2 =79.1,P < 0.001),side effects of medication (x2 =7.84,P =0.02) than patients aged > 70 years.Patients with education higher than junior high school requested more knowledge about the cause of CAD (x2 =26.6,P < 0.001),prevention of CAD (x2 =14.4,P =0.001) and life-time of stents (x2 =7.82,P =0.02) than those with education lower than junior high school.Patients with education higher than junior college requested more knowledge about the cause of CAD (x2 =26.6,P < 0.001) and post-operation check-up items (x2 =10.7,P =0.005) than those with education lower than junior college.Conclusion Some factors affect the knowledge requested by patients with coronary artery disease (CAD) after stent implantation,which should be considered in health education for CAD patients.
8.Effect of sodium tanshinone II (A) sulfonate on Ang II -induced atrial fibroblast collagen synthesis and TGF-beta1 activation.

OBJECTIVETo observe the effect of sodium tanshinone II (A) sulfonate (STS) on Ang II -induced atrial fibroblast collagen synthesis and TGF-beta1 activation.
METHODAtrial fibroblasts of neonatal rats were cultured to determine the content of collagen protein. The original synthesis rate determined by the [3H]-proline incorporation method was taken as the index for myocardial fibrosis. The content of active TGF-beta1 and total TGF-beta1 in cell culture supernatants were tested and cultured by ELISA. The expression of thrombospondin-1 (TSP-1) was assessed by using Western blot.
RESULTAng II could significantly increase the content of atrial fibroblast collagen and the collagen synthesis rate, the TSP-1 expression and the concentration of active TGF-beta1, without any obvious change in total TGF-beta1. After the STS treatment, all of the indexes, apart from total TGF-beta1, were obviously down-regulated.
CONCLUSIONSTS could decrease the secretion of Ang II -induced atrial fibroblast collagen and the synthesis rate. Its mechanism is related to the inhibition of TSP-1/TGF-beta1 pathway.

9.Cardiopulmonary resuscitation registry and video records analysis of cardiopulmonary resuscitation performance in emergency department

Objective To effectually record cardiopulmonary resuscitation (CPR) procedure, analyze and compare the CPR performance of all medical and nursing staffs to find out the existed information for the improvement of the quality of CPR and its survival rate. Methods The medical data were collected according to the Utstein Criteria and CPR event was automatically recorded by a digital video-recording system, by which hands-off times within 10 minutes of CPR and times for installation of chest compression machine, establishment of endotracheal intubation and establishment of the venous channel were analyzed. Multiple regression analysis was conducted to analyze the factors affecting CPR effect. Results ① During the period from December 2009 to December 2015, a total of 376 patients with cardiac arrest (CA) was registered, including 248 males and 128 females, with a median age of 68 (53, 78) years. Estimated median time interval from CA to CPR initiation was 5.0 (0.1, 20.0) minutes and there were 189 cases less than or equal to 5 minutes. The acute myocardial infarction (AMI) with 145 cases (38.6%) was the main etiology of CA. Initial shockable rhythm was found in 16 patients (7.0%) out of 230 out-of-hospital cardiac arrest (OHCA) patients, of whom 13 underwent ventricular fibrillation (5.7%), 3 underwent ventricular tachycardia (1.3%). Initial shockable rhythm was found in 47 patients (32.2%) out of 146 in-hospital cardiac arrest (IHCA) patients, of whom 40 underwent ventricular fibrillation (27.4%), and 7 underwent ventricular tachycardia (4.8%). CPR by a mechanical device (Thumper) was performed in 219 patients (58.2%). In 376 patients, 186 patients had return of spontaneous circulation (ROSC, 49.5%), a successful CPR (ROSC ≥ 24 hours) was found in 110 patients (29.3%), 99 patients was hospitalized alive (26.3%) and 40 patients were discharged alive (10.6%). In 146 cases of IHCA, 89 patients had ROSC (61.0%), a successful CPR was found in 63 patients (43.2%), 56 patients were hospitalized alive (38.4%), and 29 patients were discharged alive (19.9%). In 230 patients of OHCA, 89 patients had ROSC (38.7%), 65 patients received pre-hospital CPR (28.3%), a successful CPR was found in 47 patients (20.4%), 43 patients were hospitalized alive (18.7%), and 11 patients were discharged alive (4.8%). There were 37 patients had a successful CPR (69.8%), and 25 patients were discharged alive (47.2%) in 53 patients with ventricular fibrillation. ② There were 77 patients with valid video information for analysis of CPR performance, with 48 patients of OHCA, and 29 patients of IHCA. Delay median time from the patients presence in the resuscitation room to be placed in rescue bed was 22 (0, 33) seconds. Hands-off median times during 10 minutes of CPR was 41 (18, 90) seconds. Thumper installment median times was 43 (31, 69) seconds. Median time for endotracheal intubation was 59 (35, 109) seconds. Median time of venous catheter placement was 112 (70, 165) seconds. It was shown by multivariate regression analysis that there was a significant correlation between estimated time interval from CA to CPR performed, hands-off time and success rate of CPR (t1 = -3.452, t2 = -2.729), rate of discharge alive (t1 = -2.328, t2 = -2.736, all P < 0.05). In 48 OHCA patients, success rate of CPR was significantly correlated with estimated time interval from collapse to CPR performed (t = -2.409, P = 0.021). In 29 IHCA patients, success rate of CPR and rate of discharge alive was significantly correlated with hands-off times (t1 = -3.412, t2 = -2.536, both P < 0.05). Conclusions Survival to hospital discharge following CA is significantly correlated with the time interval from collapse to CPR performed and hands-off times in CPR. Installment and usage of Thumper should be postponed in order to reduce hands-off times during CPR in IHCA.
10.Expression of neuroglobin after focal cerebral ischemia and its neuroprotective effect

Objective:To study the expression of neuroglobin (Ngb) in rat cerebral ischemia model and the neuroprotective effect of Ngb after ischemia and hypoxia. Methods: Totally 113 Sprague-Dawley rats were randomly divided into sham-operated group, middle cerebral artery occlusion (MCAO) group, hemorrhagic infarction (HI) group and hemin treatment group.
