1.The relationship study between insulin resistance and the risk factors of coronary heart disease as well as the severity of coronary lesions

Objective To investigate the mechanism of insulin resistance (IR) with coronary heart disease (CHD) by the relationship study between IR and the risk factors of CHD as well as the severity of coronary lesions. Methods One hundred and twenty three patients with at least one vessel ≥50% luminal narrowing were analyzed from our consecutive angiographic data base, excluding patients concomitant with hepatic, renal, pancreatic diseases, thyroid disorders and diabetes mellitus (DM) with insulin therapy. Oral glucose tolerance test (OGTT), blood lipid, plasma insulin were performed in all patients. Insulin resistance was assessed by homeostasis model assessment (HOMA) to investigate the relationship study between insulin resistance and the risk factors of CHD,severity of coronary lesions as well as the clustering of risk factors (hypertension, overweight, impaired glucose tolerance/DM, high serum triglycerides, low high density lipoprotein cholesterol). Results The level of IR was correlated with the levels of body mass index (BMI), fasting glucose, postprandial glucose, fasting insulin, postprandial insulin, hypertension, serum triglycerides, and high density lipoprotein cholesterol. Meanwhile the level of IR was associated with clustering of risk factors and the extent and the severity of coronary lesions. Conclusion Insulin resistance may be the basis for disease. In the primary and secondary prevention of CHD, we should not only treat syndromes but also reduce the level of IR.
2.Observation on serum IFN-γ,IL-4 ,TGF-β1 ,IL-9 and IL-17 levels from youth patients with first episode depression

Objective To explore the significance of serum cytokine interferon-gamma (IFN-γ) ,interleukin-4(IL ) ,transforming growth factor-beta 1 (TGF-β1) ,interleukin-9 (IL-9) ,interleukin-17 (IL-17) in detection of youth first-episode depression .Methods Ninety cases of youth first-episode depression as the experimental group were equally divided into 3 groups according to different disease courses(<6 months ,6 months to 2 years ,>2 years)and other 30 normal persons served as the control group .The levels of serum cytokines were measured by ELISA .The level of each cytokine and Hamilton Depression Scale (HAMD) score were per-formed the Spearman correlation analysis .Results The IL-9 level in the 6 months to 2 years group and > 2 years group was high-er than that in the control group (P< 0 .05);the IFN-γlevel was negatively correlated with psychic anxiety (P<0 .05);the IL-9 level was positively correlated with the somatic anxiety and systemic symptoms (P<0 .05);the IL-17 level was positively correla-ted with the depressive mood and suicide (P<0 .05);the IFN-γlevel was positively correlated with difficulty falling asleep ,early a-wakening ,work and interest in the 6 months to 2 years group ,while positively correlated with somatic anxiety (P<0 .05);the IFN-γ level was positively correlated with depressive mood and block in the >2 years group(P<0 .05);the IL-4 level was positively correlated with the depressive mood (P<0 .05) .Conclusion Different cytokines play different roles in youth first-episode depres-sion ,cytokines may involve in the occurrence and development of depression .
3.A novel feature vector selection method for the CBCT image elastic registration.

The image guided radio therapy (IGRT) Imaging System based on cone-beam computer tomography (CBCT) can reach the goal of improving the accuracy of the radiotherapy. However, because the clinical registration between CBCT images and Planning CT images is carried out manually, it inevitably reduces radiation positioning accuracy to some extent. To tackle the problem, we proposed a new feature vector selection method for the CBCT image elastic registration in the framework of hierarchical attribute matching mechanism for elastic registration (HAMMER) algorithm. We analyzed the characteristics of HAMMER algorithm and used Canny operator which has a better edge detection and positioning performance to replace the noise-sensitive gradient amplitude. Therefore, we used a new attribute vector, which consisted of the intensity, Laplacian of the Gaussian and Canny operator, to ex tract the image feature points in CBCT and planning CT images. We also presented an adaptive feature-point selection method and the choice criteria of attribute vector weights. Experimental results showed that the new feature vector effectively avoided the noise interference resulted from scattering lines in CBCT images to improve registration accuracy, and it also decreased the required feature point numbers and reduced the computation redundancy, so that it provided a new approach for the clinical elastic registration of CBCT and Planning CT rapidly and accurately.

4.Expression of peripheral blood CD54,CD106 and CD62p in patients with multi-infarct dementia

Objective:To investigate the expression of peripheral blood CD54,CD106,CD62p in patients with multi-infarct dementia.Methods:The levels of serum CD54,CD106,CD62P of 82 patients with multi-infarct dementia were measured with ELISA,and were compared with 23 normal controls.Results:The levels of serum CD54,CD106,CD62p in patients with multi-infarct dementia(CD54:469?76.33 ng/ml,CD106:1 103.3?98.96 ng/ml,CD62p:18.22?8.90 ng/ml) were higher than those in normal controls(CD54:196?45.91 ng/ml,CD106:601.0?76.30 ng/ml,CD62p:6.70?3.30 ng/ml). There were significant difference between the two groups.The levels of serum CD54,CD106,CD62p were positively correlated with the degree of dmentia.Conclusion:CD54,CD106,CD62p are closely related to the development of MID and play an important role in pathlogical procedure of cerebral damage after MID.The levels of CD54,CD106,CD62p in patients with MID implies the degree of their neurological function deficit scores and might be an important indicator to observe the changes of disease.
5.Application of vascularized anterolateral thigh perforator flap harvested with free-style approach in oral and maxillofacial reconstruction

Objective To assess the harvest method and application value of free-style anterolateral thigh perforator flap (ALTPF) in oral-maxillofacial reconstruction.Methods Fifty-three patients who suffered from oral and maxillofacial tumor underwent simultaneous reconstruction using free-style ALTPFs after radical resection from August,2013 to May,2014.Perforators of the ALTPF region were marked through hand-held Doppler probe preoperatively.Flaps were designed centered on perforators according to the defect size of the accepting site.Incisions were first made at the inner border of the designed flap.Perforators were exposed above the fascia lata femoris and then dissected retrogradely through the vastus lateralis muscle to harvest a vascular pedicle with desired caliber and length.Flap thinning was applied under microscope in some cases to compromise the need of the accepting site.Results All 53 flaps survived after transplantation while skin exfoliation occurred in 5 cases due to flap thinning.Four cases sustained partial necrosis and was cured by trimming and dressing changes.Five to 14 months' post-operative followup showed satisfactory accepting-site morphology with good speech function and swallowing recovery.All donor sites were closed primarily without skin-grafting,leaving no donor-site complications including incision disruption,scar hyperplasia and muscle strength degeneration of the lower limb.Conclusion Multiple perforators have been accu rately located preoperatively in free-style harvest approach of ALTPFs,thus optimal effects can be reached with decreased donor-site morbidity and improved aesthetic outcome to the uttermost,which accords with the refined,personalized and minimal invasive development concept of modem reconstructive surgery.
6.Regulatory effect of yishou tiaozhi tablet on lipids in patients with primary hyperlipidemia.

OBJECTIVETo study the regulatory effect of Yishou Tiaozhi Tablet (YSTZT) on blood lipids in patients with primary hyperlipidemia.
METHODSOne hundred and sixty-eight patients with primary hyperlipidemia were randomly divided into two groups. The treated group was treated with YSTZT 4 tablets 3 times a day, and the control group was treated with Zocor by oral taking 1 tablet before sleep every evening. The therapeutic course for both groups was 60 days. The effect in regulating blood lipids in patients was observed.
RESULTSThe levels of total cholesterol (TC), triglyceride (TG), low density lipoprotein cholesterol (LDL-C), arteriosclerosis index (AI), apo-lipoprotein B (apoB), TC/HDL-C lowered and apoA/apoB ratio increased obviously in the treated group. As compared the effects between the two groups, the lowering of TC, AI and apoB in the two groups were similar (P > 0.05), effect of YSTZT in lowering TG was superior but in lowering of LDL-C was inferior to those of Zocor respectively (all P < 0.01). The effect of both remedies in elevating HDL-C was not satisfactory. The total effective rate of YSTZT was 91.9%, its individual effects were similar to those of SJZ respectively except in lowering TG and raising HDL-C. The adverse reaction occurred in YSTZT treatment course was mild without any influence on the medication.
CONCLUSIONYSTZT had definite regulatory effect on the blood lipids and lipoproteins in patients with hyperlipidemia.

7.Anti-dementia effect of Tongluo Xingnao effervescent tablet based on urinary metabonomics.

Tongluo Xingnao effervescent tablet (TLXNET) is a patented prescription, which comes from modified Xionggui decoction and can improve cognitive function. However, its effect on the urine metabolites and anti-dementia mechanism in the dementia model rats induced by hippocampal injection with Aβ25-35 remains unclear. The experiment focused on the changes in trajectory and inter-relationship among the urinary metabolite of rats in the blank group, Aβ25-35 hippocampal injection dementia model group and the TLXNET intervention group, in order to determine theirs characteristic metabolic markers and explain the anti-dementia effect of TLX-NET base on the change of metabolic trajectory of these bio-markers. According to the experimental results, 5, 6-indolequinone, 4-hydroxyphenyl pyruvic acid (4-HPPA), cortisol and 3-thiosulfate lactic were preliminarily identified as the characteristic metabolic markers. They mainly participate in dopamine system, glucocorticoids and energy metabolic pathways. TLXNET can apparently downregulate the disturbances of metabolic trajectory of the four bio-markers. The experiment indicates that the dementia model induced by injecting Aβ25-3 into hippocampus has its characteristic endogenous metabolic markers in urine, and ELXNET can ameliorate dementia by down-regulating the disturbances of metabolic trajectory.

8.Clinical study of carbon dioxide laser tonsillectomy

Objective:To investigate the therapeutic effect of carbon dioxide laser tonsillectomy.Method:In this prospective,randomized study, One hundred and two patients were divided into laser group or control group. Patients of laser group were cured with carbon dioxide laser tonsillectom,and the control group was cured with routine method. All operations are executed by one person. Observation index included operation time, hemorrhage in operation, ache after operation, inflammatory reaction of raw surface, repair time of raw surface, rehaemorrhagia and scar.Result:Laser group had advantages of less operation time, less hemorrhage, less ache and less inflammatory reaction of raw surface. Laser group have hemorrhage in operation (7.2±2.1)ml, while control group have hemorrhage in operation (92.0±35.0)ml. Laser group have pseudomembrane early but desquamate late.Conclusion:Carbon dioxide laser tonsillectomy is effective to relieve pain, inflammatory reaction and with less time ,it's an safe , efficient and mini-trauma operation.
9.The clinical value of prenatal ultrasonography combined with maternal serology screening for chromosomal abnormality in 15 to 20+6 gestational weeks

ObjectiveTo evaluate the clinical value of prenatal ultrasonography combined with maternal serology screening for chromosomal abnormality in 15 to 20+6 gestational weeks.MethodsSix hundred and twenty-eight pregnant women (628 fetuses) in 15 to 20+6 gestational weeks were selected to undergo prenatal ultrasonography, who were in critical risk of trisomy 21 or trisomy 18 by maternal serology screening. Transabdominal ultrasonography were performed and fetal nasal bone and nuchal fold were detected. Those who had nasal bone hypoplasia, thickened nuchal fold (NF>6 mm) and other abnormal fetal soft markers underwent amniocentesis for karyotyping analysis. ResultsThere were 6 cases of nasal bone hypoplasia (0.96%, 6/628), including one case of thickened nuchal fold, two cases of echogenic bowel, two cases of choroid plexus cysts and 1 case of echogenic cardiac focus. All these 6 cases underwent amniocentesis and 2 were trisomy 21 (33.3%, 2/6). The other 4 cases had no significant chromosomal abnormality.ConclusionsPrenatal ultrasonography may improve the detection rate of chromosomal abnormality for those pregnant women who are in critical risk of chromosomal abnormality prompted by serology screening. But invasive procedures are still needed to verify the chromosomal abnormality.
10.Therapeutic effects of gypenosides on hypercholesterolemia and it protective effect on liver injury

In order to explore the therapeutic effects and preliminary mechanism of gypenosides (GP) on hypercholesterolemia, as well as the protective effect on liver injury induced by high-dose simvastatin and high cholesterol diet (HCD), the hypercholesterolemia model of golden hamster was established by high cholesterol diet. The experimental animals were divided into blank group, model group, GP low and high dose groups (60 mg/kg, 120 mg/kg), simvastatin group (10 mg/kg), and GP high dose combined with simvastatin group (120 mg/kg + 10 mg/kg).The efficacy was investigated through dynamic monitoring serum cholesterol and liver function related indexes after drug treatment of 14 and 23 days. The results showed that GP could significantly reduce the levels of serum low density lipoprotein cholesterol (LDL-C), total cholesterol (TC), triglyceride (TG), alanine aminotransferase (ALT), aspartate aminotransferase (AST), and alkaline phosphatase (ALP), increase the level of serum high density lipoprotein cholesterol (HDL-C), and reduce the secretion of PCSK9. It is suggested that GP has a good therapeutic effect on HCD diet-induced hypercholesterolemia hamsters, which may be related to its inhibition of PCSK9 secretion. In addition, GP can significantly ameliorate liver damage caused by HCD diet and high-dose simvastatin. These findings provide a scientific basis and useful reference for the combination of GP and statins to reduce toxicity and increase efficacy.
