OBJECTIVE:To optimize the extraction technology of Bushen huoxue capsules. METHODS:Central composite de-sign-response surface method was used to optimize the extraction technology with the amount of water and boiling time as main fac-tors using normalized value of the contents of icariin and salvianolic acid B,the yield of dry extract as index. Validation test was al-so conducted. RESULTS:The optimal extraction technology was as follows as 15-fold water, reflux extracting for 75 min,extract-ing for 2 times. The deviation of measured value and predicted value was 0.000 3% in validation test. CONCLUSIONS:The cen-tral composite design-response surface method can be used to optimize the extraction technology of Bushen huoxue capsules with good prediction, and the technology is simple and stable.

Objective To investigate the curative effect of noninvasive mechanical ventilation (NMV) and its influence on plasma brain natriuretic peptide (BNP) and endothelin-1 (ET-1) in senile patients with cot pulmonale complicated with respiratory failure. Methods Eighty senile patients with cot pulmonale complicated with respiratory failure were randomly divided into treatment group (40 cases) and control group (40 cases). The treatment group was treated by NMV combined with routine therapy, and the control group was treated by only routine therapy including respiratory stimulant, lowering pulmonary artery pressure, cardiotonic agents, diuretics and nasal catheter oxygen inhalation. Blood pressure, heart rate (HR), respiratory rate (RR), blood gas analysis (pH, PaO2, PaCO2 ,SaO2) and clinical symptoms and signs were observed before and 72 hours after treatment. The plasma levels of BNP and ET-1 were also measured before and after treatment. Results Compared with the control group, the clinical symptoms and signs were significantly improved, SaO2 and PaO2 were significantly elevated, and PaCO2, HR and RR were significantly reduced in NMV group (t=2.23 ~ 3.92, P< 0.05 or P< 0.01). The plasma levels of BNP and ET-1 were significantly decreased after treatment in both groups, especially in the treatment group (t=2. 93,3. 56, P<0.01). The plasma levels of BNP and ET-1 had significantly negative correlation with PaO2 (r=-0.69,-0.61 ,P<0. 01) ,and positive correlation with PaCO2 (r= 0.51,0.42, P<0. 05). Conclusions NMV can improve hypoxemia and CO2 retention in senile patients with cot pulmonale complicated with respiratory failure, and may have some influences on neuroendocrine functions.

Objective To explore the effect of Miller pyramid demonstration in the teaching of shoulder physical examination for visiting orthopaedic surgeons and aim at finding a more effective teach-ing method in this area. Method 96 visiting surgeons in our department were chosen and divided even into two groups randomly from 2011 to 2013. Traditional teaching method was applied in group A, while Miller pyramid demonstration which combined theory with practice was used in group B. Basic knowledge, manipulative skill and clinical diagnosis were assessed in group A and group B respectively. Finally, the results were statistical analyzed using t-test with SPSS 19.0 software. Results In the basic professional knowledge test, the average score of Group B was better than Group A [(92.8 ±5.0) vs. (83.0 ±8.5)]; in terms of operational skills assessment, the average score of Group B was better than Group A [(94.0 ±4.1) vs. (79.3± 3.9)]; in the clinical diagnosis of the application, the average score of Group B was better than Group A [(90.6±2.3) vs. (85.1±1.9)]. There were significant differences between the two groups (P<0.05). Conclusion Miller pyramid demonstration was more effective than traditional teaching method in the teaching of shoulder physical examination for visiting orthopaedic surgeons.

Objective To evaluate the role of contrast-enhanced ultrasound (CEUS) on the the detection of hepatic metastases in comparison with conventional ultrasonography (US) and contrastenhanced computed tomography (CECT).MethodsNinety-seven patients with hepatic metastases underwent US,CEUS and CECT images.Their detection of hepatic metastases were compared. Results Hepatic metastases showed five enhancement patterns with CEUS,including bolus hyper-enhancement,peripheralrimenhancement,inhomogeneousenhancementwithnecrosis,hypo-enhancement,isoenhancement with liver parenchyma, all hepatic metastases showed dark defects in portal and delayed phase. The mean number of metastases at CEUS was greater than that of US (2.6±1.9 vs 1.6 ±1.2,P<0.05).The detection of hepatic metastases was 53.4% and 87.1% respectively(P<0.05).CEUS and CECT have no statistically significant difference in the detection of hepatic metastases (P>0.05).ConclusionsCEUS can improve detection of hepatic metastases.

Objective To analysis and summarise the feature of conventional ultrasound(US) and contrast-enhanced ultrasound(CEUS) of anterior mediastinal tumor. Methods From April 2011 to March 2015,24 patients, diagnosed as anterior mediastinal tumor by chest CT and could be detected by conventional US,were enrolled in this study.Among them, there were 11 lymphomas,5 thymic carcinomas and 8thymomas.The US and CEUS,micro flow imaging(MFI) and time intensity curve(TIC) parameters were evaluated respectively. Results Compare with lymphoma and thymoma in US,there were significant difference between the tumor shape and internal echo respectively (P <0.05,P <0.05).Compare with lymphoma and thymoma in CEUS,there were significant difference between the display rate of microvascular and feature of enhancement respectively (P < 0.05,P <0.05).The result of TIC in different tumors were analyzed respectively. The rise time of lymphoma was early than thymoma, but the half time of wash out of lymphoma was later than thymoma, there was significant difference between the two diseases respectively(P <0.05,P =0.01).Further more, the arrive time of invasive thymoma was later than noninvasive thymoma, there was significant difference between the two diseases (P < 0.05).Conclusions CEUS could further assess the characteristic of microvascular perfusion in anterior mediastinal tumors, based on the evaluation of conventional US.It could have a potential clinical value and a development capacity for differentiation diagnosis.

ObjectiveTo explore the association between angiotensin converting enzyme (ACE) gene insertion/deletion(I/D) polymorphism and cerebral infarction among Chinese people. MethodsThe ACE gene polymorphism was determined by PCR-RFLP in 242 patients with cerebral infarction and 283 controls. Multiple logistic regression was performed to explore the risk factors for cerebral infarction. ResultsAfter adjusting age, gender, alcohol drinking, smoking, education, history of diabetes mellitus and the primary hypertension, there was no significant association between ACE I/D polymorphism and cerebral infarction, either was hypertension and diabetes mellitus. The primary hypertension significanlly increased risk of cerebral infarction (OR=7.28,P =0.000). Both ACE ID/DD genotype and the primary hypertension showed a significant gene-environment interaction(r=1.62,OR=7.29), something as super multiplicative type 2 interaction. ConclusionAlthough ACE gene polymorphism is not risk factors of cerebral infarction, but ID/DD genotype had shown significant gene-environment interaction with primary hypertension in occurrence of cerebral infarcion.
