1.Progress in diagnosis and treatment of autoimmune pancreatitis
Journal of Clinical Hepatology 2015;31(5):665-667
Autoimmune pancreatitis (AIP)is a special type of chronic pancreatitis that originates from an autoimmune -mediated mecha-nism.AIP has unique radiological, serological, and histopathological features, often accompanied by peripancreatic lesions.AIP may be easily confused with pancreatic cancer and cholangiocarcinoma. It is necessary to diagnose AIP while integrating a variety of clinical indica-tors.Steroid therapy should be performed for patients diagnosed with AIP,and surgical treatment can be selected if necessary.

Objective To investigate the application value of CT perfusion imaging (CTPI) combined with CT angiography (CTA) for determination and diagnosis of transient ischemic attack (TIA).Methods Totally 50 TIA patients from April 2014 to April 2016 in the neurology department of some hospital underwent examinations by CT scan,CTPI and CTA in time, and the values of cerebral blood volume (CBV),cerebral blood flow (CBF),mean transit time (MTT) and time to peak (TTP) were calculated at the uninjured and injured sides of the areas of interest. The relationship between cerebral blood perfusion and cerebral angiostenosis of the TIA patient was analyzed. Results The 50 patients had the values of CBF and CBV at the injured sides lower while the ones of MTI and TTP higher than those at the uninjured sides (P<0.05).The numbers of the abnormal cerebral blood perfusion patients found by CT scan,CTPI and CTA were 28 (56.00%),45 (90.00%) and 38 (76.00%) respectively, who were confirmed clinically simultaneously. There were significant differences between the TIA positive result rates by CT scan,CTPI and CTA (P<0.05).Conclusion Carotid artery hypoperfusion is one of the pathogeneses of TIA,and positive rate by CTPI is closely related to the conditions of TIA.CTPI combined with CTA contributes to the diagnosis of TIA,and can be used for its individualized treatment.

Journal of Clinical Hepatology 2017;33(2):268-271
Ampullary tumors mainly manifest as obstructive jaundice and ampullary mass in clinical practice and are difficult to be identified in early stage due to a complex structure of the anatomical site,a deep location, and hidden symptoms.Sometimes a qualitative diagnosis cannot be made.Based on the experience in the treatment of ampullary tumors for many years in our center, this article summarizes the features of ampullary tumors from the aspects of clinical manifestations, diagnosis, treatment, and prognosis, especially the issues regarding imaging evaluation of ampullary tumors, selection of surgical procedure, and prognosis. An early diagnosis is the key to the treatment of ampullary tumors, and early identification and treatment of lesions have great impacts on patients' prognosis.Accurate preoperative imaging evaluation,a professional diagnosis and treatment team, accurate preoperative and intraoperative pathological analysis, and implementation of reasonable therapeutic strategy are the key to patients' recovery.

Objective To investigate the expression and significance of Bmi-1 in pancreatic cancer. Methods The expression of Bmi-1 mRNA was detected in pancreatic cancer and matched adjacent normal tissues by using real-time PCR in 22 cases of human pancreatic carcinoma.Bmi-1 and p16 protein were determined in 61 cases of human pancreatic carcinoma with immunohistochemistry and their relationship with clinicopathologic parameters was analyzed. Results The expression of Bmi-1 mRNA was 0.314 ±0.040 in cancerous tissues, and 0.143 ±0.056 in adjacent normal tissues, and the difference was statistically significant (P <0.01 ).36 cases (59.0%) of PC were detected with positive expression of Bmi-1 protein, and 34 cases (55.7%) with no expression of p16 protein. The expression of Bmi-1 was significantly correlated with lymph node metastasis ( P = 0.020 ) and tumor differentiation ( P = 0.018 ); the expression of Bmi-1 was negatively correlated with the expression of p16 ( P = 0.033 ).Conclusions Expression of Bmi-1 was increased in pancreatic cancer, which may be involved in the malignant behavior of pancreatic cancer by inhibiting p16 expression.

BACKGROUND: Previous studies demonstrated that freeze-drying had great influence on internal heat transfer, such as pore distribution and void connectivity, however, the mechanisms involved in heat and mass transfer during drying is poorly understood.OBJECTIVE: Based on the previous studies, micro CT was served as analytical tool, to observe the movement characteristics of sublimation interface in freeze-drying process.METHODS: With aorta of pig, micro-CT scanning, image reconstruction and the technique of grey value analysis were used to observe and analyze the sublimation process in freeze-drying. RESULTS AND CONCLUSION: The slope of freeze-drying temperature curve was large at first, causing quick sublimation rate. The grey value of freeze-drying and ice crystal area was very different in second-dimensional cross-sectional reconstructed image.Sublimation interface was clear and sublimation took place simultaneously both in the outer'surface and inner surface. The movement of sublimation interface was obvious at this time. Along with the time, the slope of freeze-drying temperature curve became small, leading to slow sublimation rate. The movement of sublimation interface turned unciearly. For the vertically placed artery, when heated at the bottom, the sublimation takes place both in the outer surface and inner surfaceat the same time. The sublimation interface slowly approaches to the middle of vessel wall. With time prolonged, resistance for heat and mass transfer in freeze-drying is increased, and the sublimation rate becomes slowly.

The polyclonal antibodies against VLDL receptor were prepared and identified. Rabbits were immunized with polypeptide fragment of VLDL receptor as antigen. The collected blood serum of the immunized rabbits was analyzed and identified by using ELISA and Western Blot. The results showed that the rabbit against mouse and human VLDL receptor antibodies were obtained with high titer and could recognize the natural VLDL receptors through Western blot. The prepared polyclonal antibodies against VLDL receptor provide a new tool to study the protein of VLDL receptor.

Chinese Journal of Digestive Surgery 2009;8(4):269-271
Objective To investigate the causes, characteristics, diagnosis and treatment of severe acute pancreatitis (SAP) complicated with mental disorders. Methods The clinical data of 86 patients with SAP complicated with mental disorders who had been admitted to Union Hospital of Tongji Medical College from 2000 to 2007 were retrospectively analyzed. The causes, clinical manifestations, treatment and prognosis were analyzed. Results Of the 86 patients, 38 were caused by internal milieu disorder, 9 by pancreatic encephalopathy, 23 by infectious-toxic encephalopathy, and 16 by Wernicke encephalopathy. The manifestation of the patients with SAP complicated with mental disorders was complex, varied and nonspecific. Individualized treatment was applied to all patients after the analysis of the causes, and most patients recovered except 14 who died of hemorrhage or multiple organ dysfunction syndrome. Conclusions Internal milieu disorder should be first considered when patients have mental disorders which occurred at the early phase of SAP, then pancreatic encephalopathy should be considered. The infectious-toxic encephalopathy and Wernicke encephalopathy should beware of when the mental disorders occur in the late phase of SAP. Individualized treatment according to the causation and comprehensive treatment are effective in preventing and treating mental disorders which occurred in the course of SAP.
