1.Expression and significance of platelet-derived growth factor-B in the placentas from pregnant women with pre-eclampsia or eclampsia

ObjectiveTo investigate the relationship between platelet-derived growth factor (PDGF)-B and pre-eclampsia or eclampsia by comparing the expression of PDGF-B mRNA and PDGF-BB protein in the placentas from pregnant women with normal pregnancy,pre-eclampsia or eclampsia.Methods The expression of PDGF-B mRNA and PDGF-BB protein were respectively determined with RT-PCR and immunohistochemistry combined with the analysis of computer microphotography in the placentas from 20 mild pre-eclampsia (mild pre-eclampsia group),20 severe pre-eclampsia (severe pre-eclampsia group),3 eclampsia (eclampsia group) and 20 normal pregnancy (control group).ResultsThe expression of PDGF-B mRNA and PDGF-BB protein in the placentas of mild pre-eclampsia group,severe pre-eclampsia group and eclampsia group ( 1.56 ± 0.30 and 1.32 ± 0.34,1.91 ± 0.28 and 2.00 ± 0.41,2.57 ± 0.06 and 2.73 ± 0.15)were higher than those of control group ( 1.38 ± 0.25 and 1.01 ± 0.26) with significant differences (P< 0.01 ).The expression of PDGF-B mRNA and PDGF-BB protein in the placentas of severe pre-eclampsia group and eclampsia group were higher than those of mild pre-eclampsia group (P < 0.01 ).The expression of PDGF-B mRNA and PDGF-BB protein were positively correlated with the degree of the disease (r =0.691,0.797,P <0.01 ).ConclusionsPregnant women with pre-eclampsia and eclampsia have higher expression of pDGF-B in the placentas.PDGF-B may play an important role in the development and progression of pre-eclampsia and eclampsia by promoting atherogenesis in the placental vessels.

It was pointed out by many researches that keeping the concentration of Ca2+ in cells could increase the survival rate of hypothermic preserved kidneys and the survival rate of transplants. In this study, changes of the concentration of calcium were detected within catoplasm, mitochondria, endoplasmic reticulum and nucleus in the isolated hypothermic storage cat kidney, Ca2+ be marked with calcium cytochemical probe (K2H2Sb2O7) and detected by X-ray microanalysis of microsections. After 24, 48 and 72 hours preservation, the p/b (peak/back) of calcium within cytoplasm and mitochondria increased significantly. There were no obvious changes within endoplasmic reticulum and nucleus. It demonstrates that the Ca2+ were released from calcium pool (except the endoplasmic reticulum, nucleus, mitochondria etc.) to cytoplasm during preservation; and mitochondria can uptake calcium from cytoplasm to some extent, while the calcium concentration of cytoplasm is higher than normal.

4.Effect of vitamin D supplementation on insulin resistance and blood glucose in people who are obese or people with abnormal glucose metabolism:A meta-analysis

Objective To systematically review the evidence for the effect of vitamin D supplementation on insulin resistance and blood glucose in people who are obese or people with abnormal glucose metabolism. Methods We searched databases including Pubmed, Elsevier, Web of Science, and WANFANG Database etc. for randomized controlled trials comparing vitamin D or analogues with placebo. We extracted data on insulin resistance and blood glucose, including homeostasis model assessment of insulin resistance( HOMA-IR), fasting blood glucose, HbA1C (% ). A1l data were analyzed using Review Manager 5. 0. Results Nine studies involving 867 participants were included. The results of meta-analysis showed that: ( 1 ) For people who are obese and with abnormal glucose metabolism, meta-analysis showed a small improvement in HOMA-IR(SMD -0. 34,95% CI -0. 61 to -0. 06, P<0. 05) and a small effect on fasting glucose (SMD -0. 41 mmol/ L, 95% CI -0. 68 to -0. 15, P<0. 05),while such effects were not seen in people who are obese but with normal blood glucose. (2) No serious adverse events were associated with the administration of vitamin D. Conclusion vitamin D supplementation may be benefit for improving insulin resistance and fasting blood glucose in people who are obese and with abnormal glucose metabolism, but has no effect on obese people with normal blood glucose.
5.Analysis on self-evaluation of Chinese physicians′ competency and its influencing factors

Objective:To understand the current situation of physicians′ competency in China and statistically analyze its influencing factors, hence providing referential evidences for promoting their competency.Methods:The evaluation scale for Chinese physicians′ competency was developed by referring to sophisticated overseas evaluation frameworks. Based on the electronic platform of the medical alliance, stratified sampling was conducted at 39 hospitals in 13 provinces, from which 2 156 physicians were surveyed, and statistical analysis was made on the current situation and influential factor of their competency in 6 dimensions(medical knowledge, diagnosis and treatment, scientific research and teaching, interpersonal communication, spirit of professionalism, population health)Results:The score range in multiple dimensions of physicians′ competency was between 2.85 and 3.89(5 in total), among which spirit of professionalism dimension scores the highest, and teaching and research dimension scores the lowest. Logistic regression analysis shows that their competency level in 6 dimensions is correlated to a range of factors, including gender, degree, title, supervisor qualifications, standardized training program of specialist physicians, and affiliation or not to teaching hospitals( P<0.05). Conclusions:Their overall competency is expected to be elevated. To name a few, they are lack of teaching and research abilities, and need to conduct clinical-oriented scientific research; they are expected to enrich knowledge structure, and need to embrace a wider concept of health; they need to improve their non-technical service abilities, and should further emphasize their humanistic quality; physicians′ practice competency depends upon their extent of medical education, and ensuring the quality of standardized training becomes the key measure.
6.Analysis on the frequency of urinary iodine in a population-based intervention study

Objective Through a two-year follow up program,this study was to analyze the urinary iodine frequency of a cohort in the intervention trial,concerning different doses of salt iodization,so as to explore the selection of appropriate concentration of salt iodization.Methods A multistage cluster sampling method was used to select three townships in two countries for community intervention with different doses [ ( 15 ± 5 ) mg/kg,(25 ± 5 ) mg/kg,( 35 ± 5 ) mg/kg ] of salt iodization.Results After intervention,the median of urinary iodine was reduced among the population.The urinary iodine frequencies of (15 ± 5) mg/kg and (25 ± 5) mg/kg among groups of children were mainly concentrated in 100-200 μg/L and 200-300 μg/L paragraphs in A county.While the 300 μg/Lparagraph had an overall decline in B county,the 100 μtg/L and 200 μg/L paragraph ratio increased but the trend seemed to be slow.The 100-300 μg/L paragraph of the four treatment groups took a larger proportion and kept smooth in a more ideal state.However,the control group still maintained at above 250 μg/L level.Conclusion The iodine supplementation should be gradually implemented in Chongqing.The doses of salt iodization should be reduced from the current (35 ± 15)mg/kg to (25 ± 5) mg/kg in the economically developed areas.At the same time,we need to continuously follow the changes of the condition.

Depression is a serious mental illness with a high incidence. At present, we do not fully understand the specific pathological mechanisms of depression, and the efficacy of drug treatments is very limited. Recent studies have shown that epigenetic changes that occur in specific brain regions may be a key mechanism by which environmental factors to interact with individuals to influence the risk of depression. Therefore, drugs that target epigenetic regulation may become a new direction for the development of antidepressants. Histone deacetylase inhibitors (HDACi) are a class of compounds that inhibit histone deacetylase activity, which has been reported to be associated with depression; this article addresses the use of HDACi in preclinical studies, and their potential therapeutic role and limitations of use in depression.
8.Effect of Kangxin Capsule on the expression of nerve growth factors in parietal lobe of cortex and hippocampus CA1 area of vascular dementia model rats.

OBJECTIVETo observe the effect of Kangxin Capsule (KXC) on the expression of nerve growth factor (NGF) as well as the morphology and amount of nerve synapse in the cortical parietal lobe and hippocampus CA, area of vascular dementia ( VD) model rats.
METHODSThe model rats of VD made by photochemical reaction technique were randomly divided into five groups: the model group (MG), the high-dose, middle-dose and low-dose KXC groups (HDG, MDG and LDG), and the Western medicine hydergin control group (WMG). They were treated respectively with distilled water, high, middle and low dosage of KXC suspended liquid, and hydergin for a month. Besides, a blank group consisting of normal (non-model) rats was set up for control (CG). The ultrastructure of nerve synapse in the cortical parietal lobe and hippocampus CA1 area of the rats were observed and its density estimated. The condition of NGF positive neurons in the above-mentioned two regions were also observed by immunohistochemical stain.
RESULTSAll the KXC or hydergin treated groups demonstrated a normal amount of nerve synapse with integral structure in the cortical parietal lobe and hippocampus CA, area, which approached that in the CG and was superior to that in the MG. Also, the NGF positive neuron in all the treated groups was much more than that in MG with significant difference ( P<0.01), approaching to that in the CG.
CONCLUSIONKXC could elevate the expression of NGF in the cortical parietal lobe and hippocampus CA, area, preserve the number and morphology of synapse, thus to protect the function of nerve system from ischemic injury.
