Objective To investigate the proliferative response of peripheral blood mononuclear cells(PBMC) and its depressant effect on small cell lung cancer(SCLC)cells(H446) in patients with paraneoplastic neurological syndrome(PNS).Methods PBMC of 7 patients with PNS and 6 patients with SCLC were cultured with interleukin(IL)-2 in vitro,then cultured separately or mixed with H446 respectively.The proliferation index(PI,the ratio of cellular score which had proliferated and that had not proliferated) of H446,PBMC,CD4+,CD8+ T cell and the ratio of CD4+,CD8+,CD4+/CD8+ were analyzed with flow cytometry and compared with normal control group.Results Compared with cultured separately,the PI of H446,PBMC,CD4+,CD8+T cell in PBMC of PNS and SCLC groups cultured with H446 were not significantly different.Stimulated by IL-2,the ratio of CD4+ T cell in PNS patients [(76.54 ? 3.96)%]was higher than that in normal control group[(51.75 ? 17.3)%](P

Objective To observe the effects of serum containing the medicine of Tujian Mixture on phospho-Akt and phospho-PTEN of glomerular mesangial cell (MC) cultured in high concentrations of glucose. Method The glomerular mesangial cells from SD rat were divided into six groups:the normal control group, the group of MC cultured in high concentrations of glucose, and four other groups cultured in high concentrations of glucose + different concentrations of rat’s serum containing Tujian Mixture. Objective To analyze the changes of energy metabolism-related proteins(enzymes) and transformation of energy metabolism in transplanted heart.Methods The hearts transplanted from Lewis to Wister rats as allografts and from Lewis to Lewis rats as isografts were used to estab- lish Ono model.The changes in proteins were observed by using proteomics technique and compared between allografts and isografts in the left ventricular myocardial tissues 2 and 8 weeks after heart transplantation.Peptide mass fingerprint map was obtained by using matrix-assisted laser desorption/ ionization time of flight mass spectrometry(MALDI-TOF-MS)and the matched proteins achieved by using Matrix Science software.Results Eighty proteins(enzymes)of total 65 differential proteins that regulated energy metabolism were identified in allografted hearts,which correlated to the Krebs cycle and electron transport system involving glycometabolism,fatty acid and branched-chain amino acids oxidation.Conclusion The energy metabolism in allografts heart was transformed:mitochondria function was decreased in Krebs cycle but enhanced in electron transport system and outer mitoehon- drial membrane permeability to ATP,accelerated in glycolysis and fatty acid oxidation,however, branched-chain amino acids oxidation were suppressed.
5.The clinical feature and prognosis predictors associated with outcome for convulsive status epilepticus children

Objective To investigate the clinical manifestation of convulsive status epilepticus (CSE) associated with the outcome of children contracted this illness in order to make an effort to prevent and treat those children. Method The patients with CSE admitted from October 1998 to October 2008 were prospectively and consecutively enrolled. All the patients met the diagnostic criteria set by International Anti-epilepsy Union (ILAE) in 1993. The prognosis predictors of outcome were analyzed with the Rank sum test, Chi-square analysis and Logistic regression analysis. Results A total of 169 patients with CSE were enrolled. Of them, 92 patients were from rural areas (54.4% ). The primary cause was central nervous system infection, and the minor ones were a reduction of dose or withdrawal of the antiepileptics, and cerebral hemorrhage. The duration of CSE before treatment was longer in rural patients than in urban residents ( P < 0.01) . The duration of CSE before treatment and the total persistence time of CSE was significantly associated with the mortality 14.2% . The source of patients, the duration before treatment, and respiratory depression were independent prognostic predictor variables of CSE (P < 0.05) .Conclusions The rural patients account for the major ratio of convulsive status epilepticus. Central nervous system infection is the primary cause. The duration before treatment and total persistence time of CSE are highly related to the mortality and morbidity.
6.Effects of endoplasmic reticulum stress on hepatocellular injury in young rats fed with high-fat diet

Objective To study the effects of endoplasmic reticulum stress on liver damage in young rats fed with high-fat diet.Methods We divided 48 male weaned young rats randomly into high-fat diet group and control group,which were separately fed with high-fat diet and normal diet.After feeding 8,12 and 1 6 weeks,the body weight and visceral fat of the rats were measured.The serum liver function was measured.The morphology of livers was observed by HE and transmission electron microscopy. The mRNA expressions of ATF6 and GRP78 in hepatocytes were measured with RT-PCR.Results ① The body weight and visceral fat weight of rats in high-fat diet group increased compared with those in control group (P <0.05).② Alanine aminotransferase and aspartate aminotransferase in high-fat diet group increased slightly over time (P >0.05);alanine aminotransferase at week 1 6 was increased significantly compared with that in controls (P < 0.05 ).③ Liver cells in high-fat diet group had steatosis at week 8 and the steatosis became more serious between week 12 and week 1 6.④ In high-fat diet group at week 8 there were a large number of lipid droplets in the cytoplasm,and the cell structure was close to that of normal cells;rough endoplasmic reticulum was nearly normal and the ribosome was visible.At week 12 and week 1 6,besides a large number of lipid droplets,we could also see that some substances with line-like structure deposited in rough endoplasmic reticulum pool.⑤ The expressions of ATF6 and GRP78 mRNA in hepatocytes in high-fat group at weeks 8, 12 and 1 6 were significantly increased compared with those in control group (P <0.05). Conclusion High-fat diet in infants can cause visceral fat accumulation,fatty degeneration of hepatocytes and liver injury.ATF6-mediated endoplasmic reticulum may be closely related to the liver injury which results from highfat diet.
7.Effects of health education and stove improvement in preventing coal-burning-borne endemic fluorosis in Pingba County of Guizhou Province

Objective To observe the effect of health education and installation of improved stove in control of coal-burning-borne endemic fluorosis in Pingba County of Guizhou Province.Methods Two towns(Shizi and Qibo) of Pingba County that had implemented the health education and installed the improved stoves for control of coal-burning-borne endemic fluorosis were selected as the investigation sites.Two villages in each selected town and 30 householders as well as all students of grade 4-6 were investigated.Questionnaires were made by survey of the fluorosis control knowledge,household investigations were used to know the use of household stoves,corn and peppers drying method,and the dental fluorosis of children aged 8-12 were examined by Dean methods before the intervention(2006) and after implementation(2009) of health education and improvement of stoves.Results The awareness of fluorosis control among students and households after the intervention was 97.36% (405/416) and 86.46%(415/480),respectively,and the difference was statistically significant(x2 =658.86,163.10,all P < 0.01) compared with that before the intervention [32.19%(1083/3364),43.33%(130/300)].Correct using rate of the household stoves was 95.42% (229/240) after the intervention and 45.85% (3976/8672) before the intervention,the difference was statistically significant(x2 =230.25,P < 0.01).The correct rates of drying corn and chili were 91.67% (110/120) and 97.50% (117/120) before the intervention,and 20.00% (12/60),26.67% (16/60) after the intervention,the difference was statistically significant(x2 =94.07,104.02,all P < 0.01).The incidences of dental fluorosis of children aged 8-12 were 31.21%(1305/4182) after the intervention and 87.71%(2856/3256) before the intervention,the difference was statistically significant(x2 =2371.91,P < 0.01).Conclusions The measures of health education and intervention of stove improvement in Pingba County of Guizhou Province for prevention and control of coal-burning-borne endemic fluorosis is effective.
8.Study on cost-effectiveness of primary cerebral infarction patients

Objective To study cost-effectiveness of primary cerebral infarction patients. Methods The neural function of primary cerebral infarction patients was studied by the National Institutes of Health Stroke Scale (NIHSS) with rank test and multiple regression analysis. Results After treatment in hospitals of different grades, no obvious difference was detected in the NIHSS score, but difference was found in therapy cost and therapy time; the NIHSS score and therapy time of primary cerebral infarction patients in different attack positions were obviously different, but therapy cost had no significant statistical difference. The NIHSS score of primary cerebral infarction patients with different treatments was obviously different, but therapy cost and therapy time was not. Conclusions Patients are advised to choose grade hospitals in nearby cities for treatment. Clinician are expected to choose low-cost treatment plan to relieve patients' economic burden.
9.Pulmonary ectopic lesion of acute schistosomiasis:a report of two cases

This paper reports the diagnosis and treatment of 2 cases of acute schistosomiasis with ectopic lesion in the lung. It suggests that in schistosomiasis endemic areas,if the patients with the contact history of infested water have the symptom of fe-ver,while the effects of anti-infection and the corresponding treatments are not good,the clinician should consider acute schisto-somiasis.
10.Experience of treatment in patients with penile defect after necrosis caused by shortwave diathermy

Objective To explore the strategy of repairing penile defect after necrosis caused by shortwave diathermy.Methods Between January 2004 and June 2012,18 cases with penile defect after necrosis caused by shortwave diathermy were admitted in our hospital.The age of those patients ranged from 18 to 55 years old (mean 34 years old).Among them,15 cases have been married.All patients have the history of penile shortwave diathermy therapy after surgery.After debridement,penile complete defect was found in 6 cases,incomplete defect was found in 10 cases and corpus cavernosum and corpus spongiosum partly defect was found in 2 cases.Under general anesthesia,we applied penis extent in 10 cases with incomplete defect,forearm free flap in 2 cases with penile complete defect,abdominal artery perforators flap in other 4 cases with penile complete defect.In 2 cases with corpus cavernosum and corpus spongiosum partly defect,the scrotal septal skin flap and fascia pedicle flap was used for 1-stage urethral reconstruction and costicartilage was implanted for 2-stage cavernosum reconstruction.Results 18 patients were followed up for 0.5-10.0 years (mean 2.3 years).16 cases with penile complete defect or incomplete defect had normal length after operation,ranged from 6.3 to 9.2cm.During erectile phase,the length of their penis ranged from 8.7 to 11.5 cm.No feeling abnormality or erectile dysfunction was complained.All patients exhibited the excellent morphology and felling of penis with minor scar in donor site.Early (half a month) complications included distant flap necrosis in 1 case and disruption of the wound in 1 case,which were cured after debridement and conventional treatment.After one year,we did not see the flap atrophy.One case had tumid which influenced penile beautiful and sexual intercourse.It was improved by local liposuction.One month after surgery,two patients with corpora cavernosa and corpus spongiosum partial defect had normal voiding.No feeling and erectile dysfunction were complained during the following-up.Conclusions According to the characteristics of the penile defect due to the shortwave diathermy,selecting the optimal skin flap and individual repair strategy are the keys to successful treatment.
