1.Visual quality after epipolis laser in situ keratomileusis surgery in mild and moderate myopia

AlM: To comprehensively evaluate of visual quality after epipolis laser in situ keratomileusis ( EPl-LASlK) in mild to moderate myopia.METHODS: Sixty cases ( 120 eyes ) undergone EPl-LASlK with equivalent diopter ( SE ) were divided into two groups:mild myopia group (60 eyes) and moderate myopia group ( 60 eyes ) . Objective visual acuity, aberration, contrast sensitivity and glare sensitivity were measured before and 1wk; 1, 6mo after operation, and made comparative analysis. RESULTS: Postoperative uncorrected visual acuity ( UCVA ) of two groups were better than that of preoperative(P<0. 05). Postoperative 6mo, there were significant differences on total high RMS between two groups ( P < 0. 05 ). Before operation and 6mo after operation, higher - order aberrations had statistically significant difference between two groups (P<0. 05). After 1wk and 1mo, there were statistical significances at the five frequencies between two groups (P<0. 05). After 6mo, the mild myopia group returned to preoperative levels, however, moderate myopia group in the high frequency region ( 18. 0c/d ) had significant difference with before surgery (P<0. 05). 1wk after surgery, there were statistically significant differences on the 3. 0, 6. 0, 12.0, 18. 0c/d frequency in both groups (all P<0. 05), 1mo after surgery, there were significant difference at 12.0, 18.0c/d frequencies (all P<0.05), after 6mo, both groups returned to preoperative levels, showed no significant difference.CONCLUSlON:Visual quality is poorer in mild myopia patients than that in moderate myopia at early stage after EPl-LASlK. Better visual quality is observed at mid-late stage.
2.Efficacy of Teicoplanin Treatment on Complicated Gram-positive Urinary Tract Infection

OBJECTIVE To evaluate the efficacy and safety of teicoplanin treatment on complicated Gram-positive urinary tract infection.METHODS The patients′ clinical manifestation,laboratory results,bacterial culture changes after intravascular administration of teicoplanin were observed.Each patient was administered teicoplanin(200 mg once a day for 10-14 days,but 400 mg in the first day).RESULTS The effective rate and the bacterial eradication rate were 90.0% and 86.7%,respectively.All patients experienced relief from clinical infective symptoms and systemic reactions.There were no abnormal laboratory findings and adverse reactions.CONCLUSIONS Teicoplanin is an effective and safe antibiotic for the treatment of patients with complicated urinary tract infection.
3.Surgical risk factors for patients with large hepatocellular carcinoma undergoing hepatectomy

Objective To evaluate risk factors of hepatectomy for patients with large hepatocellular carcinoma (HCC). Methods Clinical data of 310 large HCC cases receiving hepatctomy were analyzed retrospectively. Results Hepatitis B infection rate was 60.7% in this group with cirrhosis rate of 66.8%. Tumor size averaged at (9.4?3.8) cm in diameter. Child A grade was found in 51.0% of cases, Child B in 36.8% and Child C in 12.3%. Pringle′s procedure, semi-liver blood occlusion and modified Heaney procedure were used in 31.6%, 11.0% and 2.3% of cases respectively, with occlusion time of ( 17?8) min, (25?9) min and (20?10) min, respectively. Left lateral lobectomy, left hemihepatectomy, right hemihepatectomy and segmentectomy were performed in 17.1%, 11.6%, 9.0%, and 62.3% cases, respectively. Blood loss, blood transfusion and operation duration were (820?1 151) ml, (966?945) ml and (182?74) min, respectively. The overall morbidity and liver failure were 22.3%, and 5.8%, respectively, with an operative mortality of 2.6%. 4.Portal hemodynamic features and its changes following devascularization procedure in cirrhotics.

ObjectiveTo investigate portal hemodynamic features and its changes following devascularization in cirrhotics. MethodsHemodynamics of portal trunk (PT), right anterior branch (RAB) and splenic vein (SV), including maximum cross-sectional velocity(CS-Vmax), flow volume and congestion index(CI), were assessed in 69 cirrhotics and 46 normal volunteers by using a recently-developed color Doppler velocity profile(CDVP). Of 28 patients undergone devascularization procedure, portal hemodynamics were studied and compared before and after operation. ResultsCSVmax of PT and RAB was significantly lower in cirrhotic group than normal group;PT and SV flow volume and the ratio of SV to PT flow volume(SV/PT)were significantly greater in cirrhotic group compared with those of normal group;CI of PT, RAB and SV was significantly higher in cirrhotic group than normal group. Postoperative PT flow volume was significantly reduced( P <0.01),and the reduction was closely related to preoperative SV flow volume( r = 0.65, P <0.001). CS-Vmax and flow volume in RAB were decreased significantly following operation( P <0.01), and the reduction of RAB flow volume was highly related to preoperative RAB flow volume( r =0.74, P <0.001). After operation, free portal pressure(FPP)was declined by (6 ± 5)cmH2O[ (0.59 ± 0.49)kPa] ( P < 0.001 ). There were no significant changes in CS-Vmax, CI in PT and CI in RAB following operation. ConclusionIn cirrhotics with portal hypertension, portal venous system coexists the elevated vascular resistance and hyperdynamics, but with different redorainance at different portion. SV hyperdynamics is the main source of increased portal blood flow. Devascularization procedure could markedly relieve portal hyperdynamics by elimination of SV inflow, which is one of the main mechanisms in obmining therapeutic goal. But the operation has no favorable effects on the increased portal resistance,and portal perfusion to the liver would be further declined after relieving portal hyperdynamics, which is unfavorable to maintenance of liver function.
6.18F-FDG PET/CT Manifestation and Staging of Nasal Extranodal NK/T-cell Lymphoma

Purpose To investigate the 18F-FDG PET/CT manifestation ofextranodal NK/T-cell lymphoma (ENKTCL) for providing important reference for the early diagnosis,accurate staging and guiding treatment of the disease.Materials and Methods The imagings and clinical data of twenty-six patients with nasal ENKTCL were analyzed retrospectively.The diagnosis was confirmed by pathological biopsy and immunohistochemistry.The distribution and metabolic signs of the lesions were summarized,and the influence of PET/CT on nasal ENKTCL staging was analyzed.Results Totally 102 positive lesions were detected in 26 patients.The SUVmax of all lesions was 12.2±5.4 (3.2-28.5).The primary lesions were mainly located in the nasal cavity (16/26,61.5%).Facial soft tissue and paranasal sinus tissue involvement were most common adjacent invasions.The distribution of the remaining affected tissues and organs in the whole body showed no obvious regularity.Lymph node involvement was most common in cervical lymph nodes.The lesion detection rates of PET/CT and conventional imaging (plain scan/enhanced CT,plain scar/ enhanced MRI,ultrasound,etc.) in lymph node and extranodal organ involvement were 96.7% and 65.5%,respectively,and the accuracy of staging were 92.3% and 73.1%,respectively.Clinical stages were changed in 15 patients (57.7%)because of PET/CT examination,among which,the staging was improved in twelve cases for finding more lesions,the staging of the suspicious lesions were reduced and the nature was defined in three cases because of their lack of metabolism on the PET/CT,and the treatment plan was affected in nine cases for staging adjustment.Conclusion 18F-FDG PET/CT can accurately show the distribution of nasal ENKTCL lesions.Compared with the conventional imaging methods,more lymph nodes and extranodal organ involvement can be detected by 18F-FDG PET/CT,which makes the staging more accurate and guides the clinical treatment more effectively.
7.Autophagy induced by aminophylline in peripheral blood T lymphocytes

Objective:To observe autophagy induced by aminophylline in human peripheral blood T lymphocytes.Methods:Peripheral blood T lymphocytes from health adults were separated with Percoll(1.073 g/ml) and harvested by using nylon column.The cultured cells were divided into control group,aminophylline group(10-8~10-3mol/L,or 0.001 8~180 ?g/ml)and dexamethasone(DXM) group,and the rates of autophagy and apoptosis were analyzed by flow cytometry.Results:①The purities of T lymphocyte were 81.3%~94.5%。②In control group for 72 h cultivation,the autophagic rate of T lymphocytes was significantly lower than those of 10-5~10-3mol/L aminophylline group(P0.05);In 10-5mol/L aminophylline group,the autophagic rate of different T lymphocyte density after cultivation(0,24,48,72 h) had a tendency of increase along with time lasting and decrease of cell density,but the difference did not show significance.③There was significant statistic difference in incidence rate of apoptotic T lymphocytes between control and DXM group in 72 h cultivation(P=0.000);But there was no significant statistic difference in the rate of autophagic(P=0.481).Conclusion:10-3~10-5mol/L aminophylline can induce autophagy in peripheral blood T lymphocytes,10-3~10-8mol/L aminophylline could induce both autophagy and apoptosis,and there is no significant correlation between autophagy and apoptosis;DXM can induce apoptosis in peripheral blood T lymphocytes,which indicates different programmed cell death of T lymphocytes could be induced by different medicines.
8.Effect of acupuncture along affected meridian on the MME gene expression of migraine patients without aura of gan-yang hyperactivity syndrome.

OBJECTIVETo observe the effect of acupuncture along affected meridian on the mem- brane metallo-endopeptidase (MME) gene expression of migraine patients without aura (MO) of Gan-yang hyperactivity syndrome (GYHS).
METHODSTotally 20 MO patients of GYHS were randomly assigned to the acupoint group (acupuncture along affected meridian) and the non-acupoint group, 10 cases in each group. Needling was performed once per day for 10 consecutive days. Gene chip technology was used to obtain two sets of gene expression profiles and analyzed using Gene Ontology (GO).
RESULTSIn the acupoint group, MME gene expression decreased after needling (P = 0.0023).That gene was rich in the beta-amyloid metabolic process (P = 3.16E-05) and the peptide metabolic process (P = 0.009612). Its expression was not seen in the non-acupoint group.
CONCLUSIONThe effect of point selection along affected meridian could be achieved possibly by regulating the MME gene expression.

9.The role of multi-detector row CT in evaluation or living renal donors

Objective To assess the role of muhi-detector row CT (MDCT) in preoperative evaluation of living renal donors. Methods The data of 104 potential donors who underwent MDCT were retrospectively analyzed. All the candidates underwent 64-MDCT examination. First,unenhanced scans were performed on the kidneys. After administration of Ⅳ contrast medium,enhanced CT images of the arterial phase, venous phase, and excretory phase were obtained. The enhanced scan scope was from the top of diaphragmatic muscle to pubic symphysis. The scanning data obtained was post-processed for reconstructed images. The anatomy and variations displayed in MDCT images on kidneys, ureters, arteries and veins were recorded. The findings in surgery constituted the standard of reference for imaging findings, and the recorded results from images were compared with the findings in surgery to assess the role of MDCT in evaluation of potential donors. Results MDCT examination was successfully performed on 104 candidates. Anomalies of kidneys and ureters were found in 8 donors before surgery. The prevalence of accessory arteries and early branching in image was 27. 2 % (28/103) and 12. 6 % (13/103) respectively. There were 3 candidates with double veins and 3 with retroaortie left renal vein found in preoperative assessment. Ninety-three candidates underwent successful donor nephreetomy. The anomalies and variations of kidneys and ureters in images were all confirmed surgically. The detection rate of the accessory renal artery (ARA) was 80 %. The ARAs measuring > 1 mm in diameter and early branching were all detected by MDCT.The findings of veins found in performed sides coincided with those of MDCT images. Conclusion MDCT can accurately assess the anatomic information and variations of the donors' kidneys, and facilitate triaging donors and planning operation proposal
10.Effects of hydrogen sulfide on cerebral edema and nestin after cardiopulmonary resuscitation

Objective To explore the effects of H2S on cerebral injury after cardiopulmonary resuscitation (CPR) and its mechanism.Methods Forty-five healthy Sprague-Dawley (SD) rats were randomly (random number) divided into shame-operated group ( group A,n =5 ),resuscitation group ( group B,further divided into four subgroups as per rats sacrificed 6 h,12 h,24 h,and 72 h after resuscitation,n =5),and NaHS pretreatment group ( group C,further divided into 4 subgroups as done in group B).The ratio of water content in brain tissue was calculated.The content of H2S in cerebral cortex of rats in all groups was determined by using universal microplate reader. Immunohistochemistry method was used to count the Nestin-positive cells. Results The content of H2S in hippocampus area of brain showed dramatic changes from rising up at first and then to lowering down to the minimum and finally returning to the original level in 72 h in B group.Compare to group B,brain water content was lesser ( P <0.05 or P < 0.01 ) and the levels of Nestin in hippocampus increased in group C(P<0.05 or P <0.01).The neurological deficit score (NDS) was improved (P <0.05 or P <0.01) and pathological changes in hippocampus of rat brain detected by using hemotoxylin - eosin staining were slighter in group C in comparison with group B.Conclusions Endogenous H2S may involve in the course of formation and progress of cerebral injury after CPR and small dose of NaHS (exogenous H2S) can improve NDS by decreasing cerebral edema and up-regulating Nestin level in hippocampus of brain,playing a protection role in cerebral injury after CPR.
