1.Effect of α-keto acid combined with low protein diet on kidney function, calcium-phosphorus metabolism and adenosine deaminase activity in malnourished patients with long term peritoneal dialysis

Objective To investigate the effect of α-keto acid combined with low protein diet on kidney function, calcium-phosphorus metabolism and adenosine deaminase activity in malnourished patients with long term peritoneal dialysis.Methods Forty-six cases malnourished patients with long term peritoneal dialysis were selected from May 2013 to August 2014 in the hospital and divided into two groups.The control group (n=23) were treated by conventional therapy, acceptable daily intake of protein 1.2 g/(kg? d).The experiment group (n=23) were treated by α-keto acid combined with low protein diet, acceptable daily intake of protein 0.8 g/(kg? d).Two groups were treated for three months.Serum calcium, phosphorus, nutritional status assessment, adenosine deaminase activity and renal function were observed and compared after treatment.ResuIts After twelve months ’ treatment, compared with control group, serum creatinine, blood urea nitrogen, serum uric acid of experiment group were lower ( P<0.05); serum calcium level was higher (P<0.05), serum phosphorus level was lower (P<0.05);ADA activity and white blood cell count were lower (P<0.05). The incidence of malnutrition rate had no significant difference between two groups(χ2 =3.136,P>0.05).ConcIusion α-keto acid combined with low protein diet could effectively increase serum calcium level, reduce serum phosphorus level, ADA activity and WBC count, and improve nutritional status in long-term peritoneal dialysis patients, which has important significance.
2.A novel chromene with anti-tumor activities from fungus Phomopsis sp.

A new chromene (1) and six known compounds identified as 6-hydroxymellein (2), 6-hydroxy-5-methylmellein (3) nectriapyrone (4), chermesinone A(5), chermesinone B(6), and pomopxanthone A(7), were isolated in our investigation of the cytotoxic constituents from the fermented rice substrate of endophytic fungus Phomopsis sp. HCCB03519. The structures of these com pounds were elucidated through spectroscopic data analysis. All compounds exhibited inhibitory activities against cancer cell lines. Compound 7 showed stronger inhibition against cancer cells than the positive control 5-Fu.

3.Treatment of Unstable Angina Pectoris with Fleabane Injection

Objective To observe clinical effects of fleabane injection treating patients with unstable angina pectoris (UAP). Methods One hundred and sixty cases with UAP were randomly recruited into two groups: a control group (including 80 patients) received Asprin and Nitrate medication, and a treatment group(including 80 patients) received fleabane injection (20 mg/d, ivdrip) on the basis of the control group. Both groups were treated for two weeks. Results Compared with the control group, the treatment group showed significantly better clinical effects and electrocardiograph. Conclusion Fleabane injection has obvious clinical effects for treating patients with UAP.
4.Fatty Replacement of Pancreas:CT Appearances and Clinical Significance (Report of 3 Cases)

Objective To investigate the CT imaging features of pancreatic fatty replacement and its clinical significance. Methods Three patients with pancreatic fatty replacement detected by CT were retrospectively analyzed. CT examination included plain scan and contrast-enhanced scanning at the arterial and portal venous phases. The shape, size, density, pancreatic lobulation and interlobular spaces, course of the pancreatic duct were carefully observed. The clinic and laboratory data were also analyzed to determine the clinical significance of pancreatic fatty replacement. Results ①Imaging features: Two patients had complete fatty replacement involving the entire pancreas, another one had most fatty replacement sparing the posterior aspect of head and tail. Two patients had regular configuration of pancreas. The size of pancreas was slightly enlarged in 2 patients. Lobular atrophy and widening of interlobular spaces were present in all 3 patients. The pancreatic duct was normal in 3 patients. ②Clinic findings: Chronic diarrhea was present in 3 patients. Two patiens had diabetes (one had chronic cholangitis with choledochal lithiasis), another one had small stone in the common bile duct. Serum lipase was low in 3 patients, of which one had low serum amylase. Conclusion Pancreatic fatty replacement demonstrates certain characteristic CT appearances, and is usually associated with disturbances of both the endocrine and exocrine functions of the pancreas.

6.Exploration of residents' choices for first contact care in Beijing communities and influencing factors

A random sample survey was conducted in 17 community medical institutes in 4 districts in Beijing.Among an effective sample of 385 residents.47.5% (n =183)of community residents chose community medical institutes for first contact care while 52.5% (n =202) opted for class other hospitals.Residents with lower education levels tended to choose community medical institutes (P =0.01).And those with chronic diseases preferred to choose communities (P=0.00).
7.Osteogenesis study of compound antitumor coral hydroxyapatite in vivo

BACKGROUND: Compound antitumor coral hydroxyapatite (CCHA) has a good delayed-release and anti-tumor effect. However, whether the high-dose drug contained in the CCHA influences normal induction, conduction and growth of bone tissues at the implant site is unclear.OBJECTIVE: To establish an osteogenesis model of CCHA and to investigate the osteogensis effect and rule of self-made CCHA in vivo. METHODS: Implants of CCHA (20%CDDP-CHA w/w) and CHA(control, 0% CDDP w/w) were implanted into the metaphyseal holes of rabbit femur. X-rays and decalcified histological section of rabbit femoral bone with hematoxylin and eosin staining were used regularly to investigate the degradation of CCHA and CHA, and how bone tissues grow at the implant site. RESULTS AND CONCLUSION: After implantation, CHA crystals were faster than CCHA in connecting with surrounding bone tissues and forming bone bridges. The borderlines of implanted CHA became obscure in 4 weeks. Loose connective tissues were found in pores of the CHA and osteoblasts were growing on the surface. Bone tissues of the surrounding gradually grew into the CHA, finally repaired the bone defects. At the beginning of implantation, CCHA mainly inhibited the growth of surrounding tissues until 6-12 weeks later, normal bone tissues gradually grew into pores of CCHAs, and healed bone defects at 26 weeks. CCHA can inhibit the osteogenesis effects at early stage; however, it can achieve bone healing with surrounding bone defect ultimately.
8.Application of argon beam coagulator under electronic bronchoscope in the management of pediatric glottic and subglottic diseases.

OBJECTIVE:
To summarize clinical experience of application of argon knife under electronic bronchoscope in the management of pediatric glottic and subglottic diseases.
METHOD:
Reports in the literature were reviewed. Outcome and prognosis of 4 cases with pediatric glottic and subglottic diseases managed with argon beam coagulator technique in Shanghai children's Medical Center since 2011 were retrospectively analyzed.
RESULT:
Granulation tissue hyperplasia in 3 cases with subglottic lesions did not recur after surgery. Scarring or stenosis were not found in the airway. Intubation of the trachea and tube of tracheotomy was pulled out successfully in all cases, restoring normal breathing. No lesion recurrence was found in 1 case with squamous cell moderate atypical hyperplasia one month after the surgery.
CONCLUSION
Electronic bronchoscope has advantages of clear image, fine and soft lens body, optional bending, angle conversion, accurate positioning. And the argon knife with small head diameter can pass the pediatric supraglottic. It can be manipulated precisely with short operation time and no damage to surrounding normal tissues. Its worth in popularization and application of management for with glottic and subglottie disease in pediatric patients.

9.Study on Relationship between Red Blood Cell Distribution Width Changes and Prognosis of Acute ;Myocardial Infarction

Objective To explore the value of? red blood cell distribution width (RDW) changes in acute myocardial infarction (AMI) prognosis by analysing relationship between red blood cell distribution width, cardiac troponin I (cTnI) and high-sensitivity C-reactive protein. Methods 140 hospitalized patients with AMI were selected from May 2014 to October 2014 and devided into 2 groups. 70 patients without heart failure were AMI1 group and the other 70 patients with heart failure were AMI2 group. 70 cases of healthy persons were selected as control group. RDW was compared between three groups and the relationship between the level of RDW, cTnI and hs-CRP was analyzed. Evaluating value of RDW in predicting heart failure in AMI patients by using univariate logistic regression analysis and ROC curve analysis. Results In the comparison of AMI1 group, AMI2 group and control group, either of RDW, cTnI and hs-CRP showed a trend of increasing (P < 0.01). In the AMI patients , RDW level and cTnI level were positive correlation , and there was no linear correlation between RDW level and hs-CRP level. RDW≥14.55% was the independent risk factors in predicting heart failure in patients with AMI. Conclusion RDW≥14.55% can be an available independent predictor of the prognosis of AMI.
10.Surgical strategy for gastric cancer patients complicating portal hypertension

Objective To evaluate the outcome of surgical approaches in patients of gastric cancer with portal hypertension.Methods The clinical data of 80 patients with portal hypertension undergoing curative surgery for gastric cancer or simultaneous surgery for portal hypertension were retrospectively analyzed.Results The radical gastrectomy alone had no tremendous impact on postoperative liver function.But simultaneous surgery for portal hypertension affected patients' liver function dramatically (P =0.018).For those who underwent surgery for portal hypertension simultaneously,the incidence of complications in Child B patients was much higher than that in Child A patients (P =0.018).However,the incidence of complications did not differ between Child A and B patients who underwent radical gastrectomy alone.In addition,patients undergoing simultaneous surgery for portal hypertension had more severe complications than those who underwent radical gastrectomy only (P =0.042).Age > 50 (P =0.012),tumor stage (P =0.015),and simultaneous surgery for portal hypertension (P =0.007) were the independent risk factors for postoperative liver dysfunction.The survival time of patients undergoing simultaneous surgery for portal hypertension was significantly shorter than that of patients undergoing radical gastrectomy only (in Child A patients,P =0.009,in Child B patients,P =0.000).Conclusions Individualized surgical approaches for the treatment of gastric cancer with portal hypertension should be decided by preoperative liver function.Simultaneous management of portal hypertension was not recommended.
