Objective To evaluate the inhibitory effects on Candida albicans of vagina cells transferred with antimicrobial peptide LL-37 and human defensin 5 (HD5) recombinant plasmids and observe secretion of IL-8.Methods ( 1 ) The epithelial cells from female vagina were culture primarily.The pcDNA3.1 ( + )/HD5-EGFP and pcDNA3.1 ( + )/LL-37-EGFP eukaryotic recombinant plasmids were separately or jointly transferred into the fourth generation of vaginal epithelial cells.Two test groups were defined:one test group was no Candida albicans group including four subgroups which were untransferred group,HD5 group transferred with pcDNA3.1 ( + )/HD5-EGFP,LL-37 group transferred with pcDNA3.1 (+)/LL-37-EGFP,combining transferring group transferred with pcDNA3.1 ( + )/HD5-EGFP and pcDNA3.1 ( + )/LL-37-EGFP; the other test group was with Candida albicans group which the Candida albicans were coincubated with the four subgroups described above.(2) For examination of cytokines and chemokines,at 6,12,24 and 48 hours,the supernatant of every group was collected.ELISA was applied to detect the levels of LL-37,HD5 and IL-8.At each time point,the growth inhibition of Candida albicans was calculated by glucose consumption testing.Results ( 1 ) The max level of LL-37,HD5 and IL-8 reached max level after being transferred for 24 hours,then showed decreasing trend.The secretion of LL-37,HD5 and IL-8 was significant higher in combining transferring group in Candida albicans group than other groups,and the secretion level of LL-37,HD5 and IL-8 was (100.16 ±0.81 ) ng/ml,(58.50 ±2.08) μg/ml and ( 101.03 ± 1.59) pg/ml (P ＜0.01 ).(2) In different time point,the absorbance of each subgroup without Candida albicans declined slowly,and there were no statistically significant difference (P ＞0.05 ),as while as in LL-37 subgroup and HD5 subgroup with Candida albicans.In group with Candida albicans,the absorbance of combining transferring subgroup were 3.210 ± 0.010,3.150 ± 0.030,3.099 ± 0.030 and 2.970 ±0.040 at 6,12,24 and 48 hours,respectively,which was significantly higher than those in the other cells (P ＜ 0.01 ),and the declined trend was the slowest.Conclusions The antifungal ability of vaginal epithelial cell became stronger after being transferred with LL-37 and HD5 recombinant plasmids.LL-37 and HD5 could also possess immunomodulatory activity and induce chemokine IL-8 production.

Objective:To analyze the utilization of Chinese patent medicines in our hospital,and evaluate the ra- tional use of drugs in clinic.Method:The data of annual consumption and total consumption of Chinese patent medicines in our hospital during 2001-2005 were calculated,and DDDs and daily average cost of Chinese patent medicines were calcu- lated with the pharmacoeconomic method.Result:The annual consumption of Chinese patent medicines showed an increas- ing trend in our hospital during 2001-2005.The consumption of agents for injection increased clearly in Chinese patent medicines.Especially,the rank ratio of sum and DDDs always were less than one.Conclusion:In our hospital,the use of Chinese patent medicines is rational on the whole,and Chinese patent medicines play an important part in the clinical use for their cheap and effective reasons.DDDS should be stressed and management of Chinese traditional medicines for injec- tion should be intensified.

Objective To evaluate the efficacy and safety of continuous suture by QuillTM SRS self-retaining suture for closing choledoch incision. Methods From January 2015 to October 2015, 66 patients with gall bladder calculi combined with common bile duct stones who received laparoscopic gallbladder excision, common bile duct explo-ration, laparoscopy choledochotomy, suture choledoch immediately were randomly divided into two groups, experi-mental group using QuillTM SRS self-retaining suture for closing choledoch incision and control group using tradi-tional absorbable suture for closing choledoch incision. Then observe the operation time, the risk of bile leakage oc-curred in the operation and the risk of bile leakage after operation. Results All patients successfully completed in laparoscopic surgery without laparotomy. Operation time, the risk of bile leakage occurred during operation in exper-imental group has statistically significant difference compared with control group ( < 0.05). The operation time and incidence of intraoperative bile leakage of the experimental group was less and lower than control group, while the risk of bile leakage after operation show no statistically difference ( >0.05). Conclusion Compared with interrupted suture by normal sutures, continuous suture by QuillTM SRS self-retaining suture for closing choledoch incision has the characteristics of shorter operation time and lower incidence of intraoperative and postoperative bile leakage. It is worthy of promoting.

Objective To design a slide support nursing bed for the patient after cross leg flap transplant operation.Methods The bed was designed based on the principle of four-link-type mechanism slide,which was composed of a bed body,slide support,sliding rails,locking bolt,connector bar for sliding rails,extension arm,leg support and etc.Accurate localization of leg support was executed by zero setting,readjustment and etc.Results The bed could be adjusted up and down,back and forth or at different angles so as the compression on the cross leg flap was avoided to enhance the comfort of the patient after cross leg flap transplant operation.Conclusion The bed gains advantages in convenience,safety,effect and etc,and is worthy promoting clinically for the patient after cross leg flap transplant operation.

OBJECTIVE:To analyze the drug utilization of traditional Chinese medicine injections in our hospital so as to provide scientific basis for clinical rational use of traditional Chinese medicine injections.METHODS:The data of annual consumption and total consumption of traditional Chinese medicine injections in our hospital during 2001～2006 were calculated,and the DDDs and the daily average costs of traditional Chinese medicine injections were analyzed statistically.RESULTS: The annual consumption of traditional Chinese medicine injections showed an increasing trend in our hospital during 2001～2006,with the consumption sum of the top 10 traditional Chinese medicine injections showing a higher proportion,which had a great impact on the total consumption of traditional Chinese medicine injections.Moreover,the rank ratios were less than one for both consumption sum and DDDs of those drugs with high average daily costs.CONCLUSION: Great importance should be attached to the DDDs of traditional Chinese medicine injections and the post-marketing reevaluation for traditional Chinese medicine injections should be intensified.

Objective: To observe the impact of mind-regulating acupuncture plus donepezil on the cognitive ability, mean cerebral blood flow velocity, event-related potential P300, and activities of daily living (ADL) in the aged patients with Alzheimer disease (AD).Methods: Sixty senile AD patients were divided into a treatment group and a control group following the envelope method for random allocation, with 30 cases in each group. Based on the conventional treatment of the internal medicine, the control group received oral donepezil, and the treatment group received oral donepezil plus mind-regulating acupuncture. After 4-week treatment, the two groups were evaluated by the mini-mental state examination (MMSE), Alzheimer disease assessment scale-cognitive part (ADAS-Cog), and ADL; changes in P300 and the mean cerebral blood flow velocity were also observed.Results: Before treatment, there were no significant differences in the scores of MMSE, ADAS-Cog, or ADL between the two groups (P>0.05). The MMSE score increased after treatment in both groups and was notably higher in the treatment group than in the control group, showing intra-group and inter-group statistical significance (P<0.05). After treatment, the ADAS-Cog and ADL scores dropped in both groups and were markedly lower in the treatment group than in the control group, also showing intra-group and inter-group statistical significance (P<0.05). Compared with the same group before treatment, the latency of P300 was shortened and the amplitude was extended in both groups, all with statistical significance (P<0.05); the latency was shorter and the amplitude was larger in the treatment group than in the control group after treatment, presenting significant between-group differences (P<0.05). The mean blood flow velocity accelerated after the intervention in both groups, and the differences were statistically significant (P<0.05); the improvement in the treatment group was more notable than that in the control group (P<0.05).Conclusion: Mind-regulating acupuncture plus donepezil can regulate the latency and amplitude of P300, increase cerebral blood flow, and improve the learning and memory abilities of AD patients.

OBJECTIVETo analyze the related factors of the infection after operation of open tibiofibula fractures.

METHODSThe clinical data of 141 patients with open tibiofibula fractures underwent surgical treatment from June 2009 to December 2012 were retrospectively analyzed. All the patients were male and aged from 18 to 61 years old with an average of 39.2 years; all the fractures were unilateral. According to Gustilo typing of fracure, 5 cases were type I, 44 cases were type II, 27 cases were type III a, 56 cases were type III b and 9 cases were type III c. These clinical data included patients' age, gender, body mass index (BMI), underlying diseases, time of operation, fracture site, fracture type, fixation method, postoperative drainage, debridement condition and so on. The postoperative infection conditions were recorded, and the correlation between above factors and infections were analyzed. Finally, the significant related variables were introduced into a Logistic regression model to evaluated their risk.

RESULTSAmong the 141 patients, 22 cases developed with infection (15.6%). The significant relative factors with infection contained fixation method, debridement condition, underlying diseases and postoperative drainage (P < 0.05). The correlativities were stepped up in order, their odds ratios value was 2.451, 3.164, 3.414, 5.117, respectively.

CONCLUSIONActive treatment for underlying diseases before operation, thorough debridement for open wound, suitable fixation method for fracture and thorough draining are effective measures in preventing postoperative infection.

In chronic kidney disease (CKD), inflammatory responses during the progression of renal tissue and tissue injury related causes its progression to end-state renal disease. Among them, nuclear factor (nuclear factor, NF)-kappaB signaling pathway by regulating the corresponding nuclear expression of target gene transcription, as well as affecting the synthesis of inflammatory mediators, induction of inflammation lead to kidney damage and renal fibrosis. Some single herbs and their extracts (such as Astragali Radix, Scutellariae Radix, Ginkgo Folium) and some traditional Chinese medicine (such as Danggui Buxue decoction, Qilian decoction) can reduce the inflammatory damage induced by renal tissue NF-kappaB signaling pathway and delay the progression of CKD.
