1.Investigation and analysis of brucellosis in human in Shapotou District, Zhongwei City of Ningxia in 2008

Objective To understand brucellosis infective status of Shapotou District, Zhongwei City of Ningxia. Methods The methods of stratified random sampling of epidemiological survey were used among high risk groups in 2008, who underwent erythromycin red plate cohesion test(RBDT) and test-tube agglutination test (SAT), in accordance with the diagnostic criteria for confirming cases of brucellosis(GB 15988-1995). Results Totally an occupational group of 2480 brucellosis in 10 townships were investigated, and 604 sera were tested. Tweenty eight cases were infected, the infective rate was 4.64%(28/604), 15 cases were diagnosed as brucellosis patients, prevalence rate was 2.48% (15/604); cattle attendants had the highest infection rate of 6.45% (26/403). Conclusions Brucellosis epidemic in Shapotou District, Zhongwei City of Ningxia, and it is place related to vocation.
2.Clinical characteristics and management of gestational trophoblastic disease in women aged 50 years or more

Objective To investigate the clinical characteristics, management and prognosis of gestational trophoblastic disease in women aged 50 years or more. Methods Thirty-eight cases of gestational trophoblastic disease in women aged 50 years or more, who were treated in Peking Union Medical College Hospital between 1992 and 2002, were reviewed retrospectively. Results The median age was 52 years (range from 50 to 58 years ). The lesions included 5 hydatidiform moles (13%), 19 invasive moles (50%), 12 choriocarcinomas (32%) and 2 placenta site trophoblastic tumors (5%). All of 38 cases presented with abnormal vaginal bleeding. Twenty-three cases of hydatidiform moles were diagnosed at their first visit to the hospital, and 15 of them received prophylactic chemotherapy, of whom 10 progressed to invasive mole, 3 developed lung metastasis. All of the other 8 cases without prophylactic chemotherapy progressed to malignant changes with metastasis of lung. The use of prophylactic chemotherapy reduced the incidence of subsequent metastasis. All of 38 cases received chemotherapy. Thirty-two cases underwent hysterectomy, complete remission was achieved in 91% of patients; complete remission was achieved in 2 of 6 patients without hysterectomy. Conclusions The diagnosis of pregnancy and pregnancy-related disease should be considered in the elderly women presenting with abnormal vaginal bleeding. Once gestational trophoblastic disease in women aged 50 years or more is diagnosed, chemotherapy should be given as soon as possible. Hysterectomy is frequently required to improve the prognosis of gestational trophoblastic disease in the elderly women.
3.Update on the pathogenesis of diabetic nephropathy

Diabetic nephropathy is a complication of diabete and its incidence is increasing obviously these years. The pathogenic mechanisms of DN are complex, with multiple factors involved such as the glycometabolism disorders , hemodynamic changes, aldose reductase pathway activation, alterations in cytokines, oxidative stress, protein kinase C activation, lipid metabolism disorders and genetic susceptibility etc, which are not completely clear yet. Thus, the research progress of DN pathogenesis is to be reviewed.
4.Influencing factors on microencapsulation of volatile oil microcapsules

AIM: To find optimium conditions which would be beneficial to the microencapsulation of the citrus oils microcapsules. METHODS: The influenceing factors,such as the kind and concentration of coating material、plasticizer and antiadherent,ratio of volatile oil to coating material were investigated using appearance,encapsulation efficiency and volatile oil loading as evaluation parameters. RESULTS: The microcapsules prepared by optimium conditions were smooth and possessed encapsulation efficiency of 84.50%,volatile oil loading of 28.32%. CONCLUSION: The optimum conditions are chosen reasonably to help increase the stability of volatile oils microcapsules.
5.Quality evaluation method of mandarin oil microcapsules

AIM: To establish a method of evaluating totally the quality of mandarin oils microcapsules. METHODS: The quality evaluation method of the mandarin oils microcapsules was investigated according to the appearance shape,in-vitro release,compressibility index and microcapsule dissociative film's physical characters research. RESULTS: The method to investigate compressibility index,hydroscopicity and the capability of penetrating oxygen and resisting tension was established. CONCLUSION: These methods can evaluate the quality of essential oils microcapsules rather exactly.
6.Influence factors on preparation of citrus volatile oil microcapsules by spray drying technique

Objective To investigate the influence factors on preparation of citrus volatile oil microcapsules by spray drying technique.Methods The citrus volatile oil microcapsules were prepared by spray drying technique with arabic gum as coating material.The temperature of aeration,injection speed,spray rate of flow,and exhausting volume were observed by single factor and the technical parameters were optimized by uniform design.Results The optimum condition including ratio of drug to coating material(0.9),plasticizer to coationg material(0.15),the temperature of aeration(145 ℃),injection speed(18 mL/min);and spray rate of flow(40 L/h).The microcapsules prepared by optimum conditions were smooth and possessed average encapsulation efficency in 84.50%,average drug loading in 28.32%.Conclusion The optimum conditions are chosen reasonably,which can increase the stability of citrus volatile oil microcapsules significantly.
7.A clinical and pathological analysis of 54 cases of erythema annulare centrifugum

Objective To investigate the clinical, histopathological and immunopathological charac teristics of erythema annulare centrifugum (EAC). Methods A retrospective study was performed on 54 cases of EAC collected from 2001 to 2005. Information was gathered about patients' sex, age, disease course, distribution and morphology of eruptions, symptoms, complications. Also, the findings of histopathology and direct immunofluorescence examination in some patients were evaluated. Remits EAC most commonly occurred on the lower limb, and was usually complicated by various diseases among which mycosis predominated. Histological examination revealed compact lymphocyte infiltration in dermal vessels in 32 of these 54 patients. Direct immunofluorescence showed the deposition of IgG, lgM, or C3 on the walls of small vessels in 6 of 12 tissue samples tested. Conclusions EAC is a multifactorial disease, and it seems that the infiltration of lymphocytes and deposition of circulatory immune complex on small blood vessels in dermis may play important roles in its pathogenesis.
8.Investigation in knowledge rate of 120 first aid and satisfaction rate with emergency services among Nanchong citizens

Objective To investigate in knowledge rate of 120 first aid and satisfaction rate with emergency services among Nanchong citizens. Methods 360 patients who received prehospital first aid from three hospitals and their accompanies were selected from January to June 2010. The investigation content included knowledge rate of 120 first aid and satisfaction rate with emergency services. Results Results showed that about half of those surveyed still did not know the emergency phone"120" and its function, lacked self-help and self-protection knowledge, some respondents felt quite helpless upon daily life of electric shock, gas poisoning, coma and traumatic accidents. The attitude of the majority of emergency personnel was good or better, but still very stiff or irresponsible attitude existed in those few. This undermined the "120" and the image of credibility and emergency personnel. Conclusions There is an urgent need for universal coverage of on-site first aid knowledge and enhance awareness of first aid. Full-time staff in emergency department must pay close attention to the knowledge of first aid training, mastering the technique of bleeding control and bandaging, fixing, handling and CPR, as well as the use of instruments and equipment equipped in vehicles. Only in this way can we continuously improve the treatment of critically ill patients and increase survival rate.
9.Analysis of the applicationeffect of micro classroom in the experimental teaching of diagnostics

Objective Analysis of the teaching effect of the micro-classroom in the experimental teaching of diagnostics.Methods In the 50 classes of undergraduate clinical specialty of 2014 level of guangdong medical mniversity,2 classes were selected as the control group (n=67) by random number table method,2 classes were selected as the experience group (n=65).Theoretical knowledge and clinical skills were calculated.Recognition of two groups of students on two kinds of teaching methods.The data of two groups were compared using t-test and chi-square test.Results The results of the theoretical knowledge and clinical skills of the experimental group were significantly higher than the control group (P< 0.05).There was a significant difference between the two groups (P< 0.05),which was found to be able to deepen the understanding of theoretical knowledge,to improve the ability of clinical skills operation,to cultivate clinical thinking and to mobilize the enthusiasm of independent learning.Conclusions Micro classroom teaching can significantly improve the quality of experimental training of diagnoses.
10.Effect of Blood Glucose Levels on Lipid-lowering Therapy in Patients with Myocardial Infraction

Objective To investigate the effect of blood glucose levels on lipid-lowering therapy in patients with myocardial infraction (MI).Methods The data of 174 patients with acute or old MI and finished a two-year follow-up were analyzed retrospectively. The patients were divided into four groups: diabetes mellitus (DM) with well blood glucose control (DM+W group), diabetes mellitus with bad blood glucose control (DM+B group), non-diabetes mellitus with well blood glucose control (NDM+W group) and non-diabetes mellitus with bad blood glucose control (NDM+B group). Blood lipids levels, dose and side effects of statins were statistically analyzed.Results All the 174 patients had a mean follow-up period of 28±7 months, and 51 patients (25.9%) had diabetes mellitus. In DM+B group (n=13, 25.5%) and NDM+B group (n=24, 19.5%), there were no statistically difference between enrollment and 2-year follow-up in the mean levels of blood total cholesterol (TC), triglycerides (TG), high density lipoprotein cholesterol (HDL-c) and low density lipoprotein cholesterol (LDL-c), but the mean level of LDL-c had a trend of elevation. In DM+W group, the mean level of LDL-c was 2.57 mmol/L at 2-year follow-up, which did not achieve the recommended LDL-c goal (2.06 mmol/L). There were no differences in statin therapy between each group. After 2-year follow-up, there were only 93 patients (53.4%) received normal dose of statin, 60 patients (34.5%) took low dose of statin and 16 patients (9.2%) had no statin therapy. There were no differences in hepatic enzyme abnormality between each group.Conclusion The bad control of blood glucose in patients with MI effect the efficacy of statins in lipid-lowering therapy. The doses of statin are insufficient especial in patients with MI and DM.
