Under the guidance of strengthening medical undergraduates' practical ability and their competence in future jobs, Dalian Medical University has established a clinical medical talents practical ability training mode. This model has implementedThree Turnsclinical skills training pattern to strengthen the three stage (before, during and after practice) clinical skills training and concentrated on humanistic quality education through overall evaluations and set up clinical skill examination system to evaluate teach-ing effectiveness comprehensively and truly , which has effectively improved the quality of education by perfecting safeguard mechanisms and guaranteeing teaching quality.
2.Closed treatment for type III humeral supracondylar fractures and prevention of ischemic contracture of forearm in children.

OBJECTIVETo evaluate the curative effects of manipulative reduction for children's type III humeral supracondylar fracture and the preventions of ischemic contracture of forearm in the early period.
METHODSFrom September 2008 to September 2011, 38 patients with humeral supracondylar fractures were treated with manipulative reduction and plaster stabilization, including 20 males and 18 females with an average age of 7.5 years (ranged, 2 to 13 years); the average time from injury to visit was 1.8 days(ranged,0.5 h to 6 d). There were 21 cases in straighten-ulnar deviation type and 17 cases in straighten-radial deviation type, 1 case in flexion type,all of them without vascular nerve injury. It was important to process swelling correctly in early stage of fracture. To decide fixed position according to the original displacement, and make a regular X-ray review, if found another displacement to correct it in 1-2 weeks after injury in time. Dismantle the plaster on the basis of bone healing and guide the functional exercise of elbow joint. According to Dodgt standard to evaluate clinical effects.
RESULTSAll patients were followed up from 3 months to 1 year with an average of 7 months. All fractures healed. According to Dodgt standard, 14 patients got an excellent results, 19 good, 4 fair and 1 poor. The excellent and good rate was 86.84%.
CONCLUSIONIt can obtain satisfactory clinical effects to treat humeral supracondylar fracture in children with closed manipulative reduction and plaster stabilization, while without vascular nerve injury. Early correct processing swelling and paying attention to gypsous angle can effectively prevent the ischemic contracture of forearm.

4.Damage of nasal mucociliary movement after intensity-modulated radiation therapy of nasopharyngeal carcinoma.

BACKGROUND AND OBJECTIVERadiation usually results in paranasal sinusitis in patients with nasopharyngeal carcinoma (NPC), which influences patients' quality of life. This study aimed to determine the relationships between dose distribution in the nasal cavity and nasal mucous injury in patients with NPC treated by intensity-modulated radiation therapy (IMRT), and to find the tolerable radiation dose for the nasal mucous.
METHODSSixty-six patients with NPC treated by IMRT between October 2006 and November 2008 were enrolled. The irradiation dose in the nasal cavity was determined by the computer with the IMRT work platform. Mucociliary transport rate (MTR) was detected by modified saccharine test before IMRT, at the end of IMRT, and at 3, 6, and 12 months after IMRT.
RESULTSThe data were available for 129 nasal cavities. The cavities receiving a mean dose below or equal to 37 Gy showed substantial preservation of nasal mucous after IMRT. The MRT decreased to (62.82 ± 38.59)%, (56.78 ± 37.79)%, (64.05 ± 39.37)%, and (71.13 ± 39.55)% of pre-IMRT value at 4 time points after IMRT, with significant differences among the data (P < 0.05). In contrast, when the cavities received a mean dose higher than 37 Gy, no significant differences in MTR among the time points were observed. At 3 months after IMRT, the MTR was the lowest (38.27% of pre-RT value).
CONCLUSIONSA mean radiation dose of ≤ 37 Gy for the nasal cavity is an optimal dose to protect the nasal cavity function.

5.Practice and exploration of training innovative talents through tutorial system in the whole process of "5+3" integrated undergraduate education for clinical medicine specialty

Dalian Medical University, aiming at cultivating innovative talents, has formulated and implemented a "5+3" innovative talents training reform plan. In the whole process of medical undergraduate education, tutorial system is used as the carrier to develop a phased innovative ability training system, which covers basic scientific research ability training such as courses, lectures, experimental design, papers and so on, and strengthens the cultivation of undergraduate scientific research ability. Through statistical analysis of the results of the first "5+3" students' periodic training, it is found that the proportion of "5+3" students publishing Chinese periodicals and SCI, and hosting national innovation projects and provincial innovation projects is significantly higher than that of ordinary 5-year students ( P<0.05). Although there are some problems and deficiencies in the implementation process, the basic scientific research training with tutorial system as the core has a significant effect on improving students' scientific research thinking and innovation ability, and is feasible for training medical innovative talents.
6.Relationship between blood uric acid level and plasma atherogenic index in patients with type 2 diabetes

Objective:To explore the correlation between serum uric acid level and atherogenic index of plama (AIP) in patients with type 2 diabetes mellitus (T2DM).Methods:A retrospective analysis of 485 T2DM patients hospitalized in the First Hospital of Qin Huangdao was performed in August 2019 to August 2021. They were divided into atherogenic phenotype group (the case group, AIP≥0.06, n=326) and non atherogenic phenotype group (the control group, AIP<0.06, n=159) with AIP=0.06 as the cut-off point. The age, sex, body mass index, uric acid, triglyceride, total cholesterol, high density lipoprotein-cholesterol, low density lipoprotein-cholesterol, glycosylated hemoglobin, smoking history and coronary heart disease history were compared between the two groups. The data processing using sample t-test and chi-square test. Pearson correlation analysis and multivariate logistic regression analysis were performed. Results:Body mass index (27.7±3.6) kg/m 2, systolic pressure (138.4±17.5) mmHg, uric acid (351.6±93.0) μmol/L, triglyceride (3.0±3.4) mmol/L and the proportion of men (61.6%(201/326)), coronary heart disease history (24.3%(79/326)) and smoking history (33.5%(109/326)) in case group were higher than those in control group (25.8±3.5) kg/m 2, (132.2±17.7) mmHg, (291.6±73.8) μmol/L, (1.0±0.3) mmol/L, (51.6%(82/159)), (15.7%(25/159)), (19.5%(31/159)) and the level of high density lipoprotein-cholesterol (1.1±0.3) mmol/L was lower than that in control group (1.3±0.3) mmol/L,with all statistically significant differences ( t=5.43, P<0.001; t=3.64, P<0.001; t=7.70, P=0.001; t=10.40, P<0.001; χ 2=4.47, P=0.034;χ 2=4.60, P=0.032;χ 2=10.11, P=0.001; t=5.18, P<0.001). The prevalence of hyperuricemia in case group (21.5%(70/326)) was 4.3 times higher than that in control group (5.0%(8/159)). AIP was positively correlated with body mass index ( r=0.300, P<0.001), uric acid ( r=0.343, P<0.001), systolic pressure ( r=0.117, P=0.010), diastolic pressure (r=0.119, P=0.009), triglyceride ( r=0.724, P<0.001), total cholesterol ( r=0.226, P<0.001), while that was negatively correlated with high density lipoprotein-cholesterol ( r=-0.185, P<0.001). Logistic regression analysis showed that after excluding the interference of other factors, uric acid was still related to AIP ( OR=3.727, 95% CI=1.702-8.158, P=0.001), and the risk of AIP increase increased with the increase of uric acid level. Conclusion:The level of serum uric acid in T2DM patients is related to AIP, and high uric acid is an independent risk factor for AIP in T2DM patients.
7.Hotspots and frontiers of human resource allocation research in public hospitals:a CiteSpace-based analysis of domestic and international studies

Objective To investigate the current status,evolving hotspots,and emerging trends in the field of human re-source allocation research in public hospitals,both domestically and internationally,to provide a reference for future research di-rections in China.Methods CiteSpace was used to conduct a visual analysis of the research literature on human resource alloca-tion in public hospitals based on China National Knowledge Infrastructure(CNKI)and the Web of Science(WOS).The analysis encompassed co-authorship,institutional collaboration,keyword co-occurrence and clustering,and burst detection.Results A total of 1 417 Chinese articles and 981 international articles were included.Domestic research in this field focused more on healthcare reform and management,resource allocation,hierarchical diagnosis,and treatment,and informatization and efficiency improvement.On the contrary,international research primarily centered on the employee satisfaction,healthcare system quality,work environment and medical staff.Future trends in domestic research included cost reduction,efficiency enhancement,and a greater emphasis on public welfare in public hospitals,while international research was beginning to explore the influence of polit-ical concepts in this field.Conclusion Compared to international research,domestic research needs to further improve its theo-retical and localized understanding,broaden its research scope,explore the interdisciplinary collaboration opportunities,and delve into research directions such as the application of artificial intelligence and automation technology in healthcare services,management of a diverse workforce,and innovative management techniques and applications.

9.Experience of surgical resection of 103 hilar cholangiocarcinoma.

OBJECTIVETo summarize the experience of surgical resection of 103 hilar cholangiocarcinoma.
METHODSOne hundred and three consecutive cases of hilar cholangiocarcinoma who underwent surgical resection at our hospital over the past ten years were reviewed retrospectively. The clinical data and long-term outcome were analyzed.
RESULTSOut of 103 cases, 43 patients underwent radical resection (41.7%), and 60 patients underwent palliative resection. There were 34 patients developed postoperative complications and 8 patients died in hospital. For the radical resection group, the median survival time was 29.9 months and 1-year, 3-year, 5-year survival rate was 69.6%, 42.0%, 20.9%, respectively, which was significant greater than 34.1%, 10.2%, 0 of the palliative resection group (P < 0.05). Over the past five years, 42 cases underwent pre-operative drainage of bile and the rate of combined liver resection reached 53.8%. The tumor radical resection rate has increased to 45.7%, the median survival time have reached 24.7 months (P < 0.05).
CONCLUSIONSImprovement of pre-operative management, intraoperative pathology for resection margin, and combined liver resection may help in increasing the radical resection rate. Radical resection can improve postoperative survival, and produce a satisfactory outcome for patient with hepatic hilar cholangiocarcinoma.

10.Effects of Family Cohesion and Child-Teacher Dependency on Children's Attention-Deficit/Hyperactivity Behaviors at one Year after Kindergarten Entry

[Objective]To explore the effects of family cohesion and child-teacher dependency on children's attention-deficit/hy?peractivity behaviors at one year after their kindergarten entry.[Methods]Based on a cohort of 7465 first-year preschoolers in Long?hua district of Shenzhen in 2014,the information about social-demographic characteristics,family cohesion(assessed with Family Environment Scale-Chinese Version)were collected from their primary-caregivers at kindergarten entry. The teacher rating scores of child-teacher dependency(assessed with Student-Teacher Relationship Scale)and parental rating scores of attention-deficit/hyperac?tivity behaviors(assessed with Conners'Parent Symptom Questionnaire)were examined at one year after their kindergarten entry. Lo?gistic regression were performed to analyze the relationship and interaction between family cohesion and child-teacher dependency in attention-deficit/hyperactivity behaviors,after adjusting for potential confounders.[Results]Children's attention-deficit/hyperactivity behaviors at one year after kindergarten entry were negatively associated with family cohesion (OR=0.85,95% CI=0.75~0.97,P=0.01),but were not significantly associated with child-teacher dependency(OR=1.00,95%CI=0.93~1.08,P=0.98). Furthermore, family cohesion moderated the impact of child-teacher dependency on children ' s attention-deficit/hyperactivity behaviors at one year after kindergarten entry(OR=0.96,95% CI=0.92~0.99,P=0.03). In addition,neither family cohesion(OR=1.08,95%CI=0.88~1.34,P=0.47)nor child-teacher dependency(OR=1.03,95%CI=0.93~1.15,P=0.53)could protect preschoolers from persistence of attention-deficit/hyperactivity behaviors in the first year of kindergarten.[Conclusion]The association of child-teacher dependency and onset of attention-deficit/hyperactivity behaviors is modified by the level of family cohesion in preschool children.
