Objective To study the optimum parameters of the supercritical CO2 fluid extraction of roasted Rhizoma Atractylodis Macrocephalae and roasted Fructus aurantii immaturus,and chemical constituents of extractive matters. Methods The experiment was performed with orthogonal design. Four factors were extractive pressure,temperature,extractive time and the flow rate of CO2. GC-MS was applied for analyzing. Results The optimum condition were obtained:the extractive pressure was 33 MPa,the temperature of extraction was 40 ℃,the extractive time was 60 minutes and the flow rate of CO2 was 27 L/h. The main chemical constituents was gamma-Elemene. Conclusion The method applied to obtain the extractive matters from roasted Rhizoma Atractylodis Macrocephalae and roasted Fructus aurantii immaturus was quickly and efficiently with satisfactory results. It provides foundation for exploration.

OBJECTIVE: To study the expression of Endophilin II and VEGF-C in human Laryngeal cancer and its relationship with the clinical treatment effect. METHOD: The expression and distribution of Endophilin II and VEGF-C protein were checked in 47 patients with laryngeal carcinoma in our hospital in 2006-09-2009-10 and 23 samples of normal tissues around the cancer by immunohistochemical staining SP method. After an appropriate surgical treatment, patients accepted a 5-year followup to evaluate their recurrence situation and 5-year survival rate. RESULT: Immunohistochemistry showed that Endophilin II protein was strongly positive expressed in normal tissues around the cancer (positive rate 82.61%). And the expression level was dependent on the lymph metastasis, T stage and lymph invasion (P < 0.05). Patients with positive Endophilin II expression had a higher survival rate and better recurrence situation. Expression of VEGF-C was significantly higher in laryngeal carcinoma than in normal tissue around the cancer (positive rate 51.06% and 13.04%). the expression level was dependent on the lymph metastasis and lymph invasion (P < 0.05), and negatively correlated with recurrence and 5-year survival rate. CONCLUSION Endophilin II and VEGF-C may involve in laryngeal cancer, and the expression of Endophilin II and VEGF-C has a positive correlation with pathological features and lymph metastasis, and both are related with recurrence and 5-year survival rate. So these two proteins are likely to be a potential target for diagnosis and treatment of laryngeal cancer.

There are a wide variety of spiders on the earth and most them can secrete venom,which contains a variety of chemi-cal compositions that have multiple influences on organism be-sides toxic effects.The pharmacological efficiency includes cardi-ovascular and cerebrovascular activities,analgesic activities,an-tibacterial and anticancer activities etc.Ion channels are one of the important targets of spider toxins.They act on different ion channels,such as potassium channel,calcium channel and differ-ent subtypes of sodium channels.Therefore the spider toxins pres-ent different pharmacological activities and potential medicinal value.The venoms of spiders are less well studied than those from other venomous taxa such as conotoxin,scorpions and snakes etc. However,in recent years,spider toxins are turning to a new hot subject in related research areas.This review summarizes the lat-est progress in biological activities of spider toxins as well as its application in medical practice and development.

Objective To investigate the change of serum homocysteine (Hcy) and plasma fibrinogen (Fg) levels in patients with ischemic cerebrovascular diseases (ICVD) and the correlation between Hcy and Fg.Methods Blood samples were collected from 74 patients with ICVD and 40 controls. The concentrations of Hcy, folate and Fg were determinated by means of fluorescence polarization immunoassay, electrochemiluminescence, and immunoturbidimetry, respectively. In the meantime, plasma lipid was detected by routine method.Results Both Hcy and Fg levels in the patients with ICVD increased significantly as compared with the controls (all P0.05). Multivariate logistic regression analysis indicated that hypertension, high cholesterol, high Fg and high Hcy concentrations played significiant roles in ICVD.Conclusion Both hyperhomocysteinemia and hyperfibrinogenemia are independent risk factors of ICVD.

Objective: To evaluate effects of atrioventricular nodal ablation, pacemaker and pharmaco logic agents on ventricular rate control. Methods:Three patients with chronic sustained atrial fibrillation ( AF) were treated by atrioventricular nodal ablation. Fast-slow junctional cardiac rhythm in this course meant the ablation was successful. Unless the ablation was able to get Ⅱ? Ⅱ type or Ⅲ? AVB, R-R intervals ≥3 s, ventricular rate≤40 bpm, it would be canceled. Otherwise implantation of a permanent pacemaker was performed. Results:One patients having complete atrioventricular block and a ventricular rate of 38 -40 bpm had implantation of a permanent pacemaker. Two patients having no atrioventricular block, prolongation of R-R intervals and ventricular rate of 90 - 120 bpm were given antiarrhythmic drugs. Conclusion: Patients with chronic sustained AF can be treaded with atrioventricular nodal ablation, pacemaker and pharmacologic agents effectively.

Objective To understand the situation of imported schistosomiasis in non?endemic areas along the Yangtze Riv?er in Changshu City,so as to provide the evidence for formulating and adjusting control measures. Methods The data of Onco?melania hupensis snails and schistosomiasis patients in Changshu City from 2006 to 2015 were collected and analyzed. Re?sults Totally 1 650 residents of Changshu City were tested by serum tests from 2006 to 2015,and 35 cases were positive,with a positive rate of 2.12%. No positive cases were found in etiological tests. No O. hupensis snails were found. Conclusion No im?ported schistosomiasis cases are found in the areas along the Yangtze River in Changshu City,but the infection source is still possible to be imported,and the surveillance work should be strengthened.

To construct a recombinant adenovirus containing CDglyES fusion gene, which can directly inhibit human ovarian cancer cell and indirectly inhibit vascular endothelial cell growth. Methods:We constructed prAdCDglyES using a homolo-gous recombination method in bacteria. The prAdCDglyES was transfected to 293 packaging cells using liposome, in which rAdCDgly-ES was packaged and amplified. MTT was used to observe the proliferation inhibition effect of rAdCDglyES on human ovarian cancer cells and the growth inhibition effect of expressing products of rAdCDglyES on ECV-304. Results:The titer of rAdCDglyES was 1 × 1013.3 TCID50/L, whereas the inhibition rate on human ovarian cancer cell SKOV-3 was (83.1±6.3)%. This result is significantly different from the control rAd-LacZ, which had an inhibition rate of (24.1 ± 13.2)% (P<0.01). The concentrated culture supernatant from cells transfected with rAdCDglyES can inhibit ECV-304 cell proliferation at a rate of (78.7 ± 1.6)%. This rate is significantly different com-pared with that of the control with the same concentration of culture supernatant from cells transfected with rAd-CD, with an effect on ECV-304 cell shown by an inhibition rate of (23.9 ± 9.7)%(P<0.01). Conclusion:The results showed that the recombinant adenovirus rAdCDglyES could inhibit human ovarian cancer cells directly and indirectly.

BACKGROUNDCytochrome P450 2E1 (CYP2E1) has an important role in the metabolic activation of precarcinogens such as N-nitrosoamines and other low relative molecular mass, organic compounds. This study examined whether CYP2E1 RsaI and DraI polymorphism are associated with susceptibility to esophageal squamous cell carcinoma and the correlation between the genotypes and expression levels of CYP2E1 mRNA.

METHODSSeventy-seven patients with newly diagnosed, untreated esophageal squamous cell carcinoma and 79 healthy controls matched in age, gender and residence were recruited for the control study. An RsaI polymorphism in the 5'-flanking region and a DraI polymorphism in the sixth intron of the CYP2E1 gene, which could possibly affect its transcription, were determined in this study by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) and mRNA level of CYP2E1 was measured by quantitative real-time reverse transcription PCR.

RESULTSNo significant association of RsaI or DraI polymorphism of CYP2E1 with susceptibility of esophageal squamous cell carcinoma were demonstrated (OR = 1.67, 95% CI: 0.89 - 3.15, P = 0.11; OR = 1.11, 95% CI: 0.59 - 2.09, P = 0.74, respectively). With SHEsis software, no linkage disequilibrium was detected between RsaI and DraI polymorphism (D' = 0.528, r(2) = 0.27). When combined RsaI polymorphism with DraI polymorphism, the association between that carrying c2 allele and DD genotype and the risk for esophageal squamous cell carcinoma were found (OR = 5.77, 95% CI: 1.65 - 20.22). Compared with the normal controls, the mRNA levels with RsaI polymorphism, DraI polymorphism, or any combined genotypes in cases showed no statistical difference.

CONCLUSIONSThis study suggests that carrying c2 allele and DD genotype conferreded an elevated risk for esophageal squamous cell carcinoma. There was no significant statistical relationship between the genotypes c1/c2, D/C, or the combined allele and mRNA expression.

ObjectiveTo compare the clinical effects of the outside and under temporal muscle titanium cranioplasty in mass frontal-temporal skull defect.MethodsClinical data of forty-two frontal-temporal skull defect pa tients who had cranioplasty with digital forming titanium nets were analyzed retrospectively.Two groups were divided according to the surgical method.Twenty-two cases underwent outside temporal and twenty cases under went temporal muscle titanium mesh.Compared with two groups of surgery condition( operation time,blood loss,titanium nail dos age) and postoperative complications( wound infection,subcutaneous effusion,epilepsy,intracranial hematoma,chew limited,facial paralysis) and the satisfaction rate of appearance discharged after one month.ResultsThe operation time of outside temporal muscle group was obviously less than under temporal muscle group( t =2.42,P ＜ 0.05 ),but the under temporal muscle group patients were more satisfied with the postoperative appearance ( x2 =36.31,P ＜0.05 ).There was no obvious difference of the postoperative complication between the two groups ( x2 =1.80,P ＞0.05 ).ConclusionBoth the outside and under temporal muscle method had its advantages and disadvantages.Operation methods selection should comprehensively and individually according to the specific condition of patients,surgi cal doctor's clinical experience.

Objective To observe the risks of hypoxemia after uvulopalatopharyngoplasty (UPPP) in patients with obstructive sleep apnea syndrome (OSAS).Methods Forty-six ASA Ⅱ or Ⅲ male patients with OSAS,aged 30-50 yr,with body mass index 27-33 kg/m2,Mallampati Ⅰ-Ⅳ,underwent UPPP under general anesthesia with propofol and remifentanil.O2 was inhaled for 24 h via a nasal catheter starting from the end of surgery.SpO2 was monitored within 24 h after surgery.Oxygen desaturation index (ODI,hourly average number of desaturation episodes in which the decrease in SpO2 ≥4% and duration ≥ 10 s) and the cumulative time percentage with SpO2 ＜ 90% (CT90) from oximetry were recorded.Results Compared with the baseline value before surgery,ODI and CT90 were significantly decreased at 2 and 2-4 h after extubation and on 1 st night after surgery (11:00 pm-6:00 am) (P ＜ 0.05).ODI and CT90 were significantly lower on 1st night after surgery than at 2 and 2-4 h after extubation (P ＜ 0.05).The rate of ODI abnormalities was 100%,48% and 50% before surgery and at 2 and 2-4 h after extubation,respectively.Compared with the baseline value before surgery,the rate of ODI abnormalities was significantly decreased at 2 and 2-4 h after extubation,while increased on 1 st night after surgery (P ＜ 0.05).There was no significant difference in the rate of ODI abnormalities between that on 1 st night after surgery and that before surgery (P ＞ 0.05).Conclusion Although UPPP can significantly improve airway obstruction in patients with OSAS,hypoxemic episodes still occur after surgery,suggesting that UPPP should not be treated as an ambulatory surgery.
