1.The clinical and social factors associated with functional disturbance of acute hemiparetic stroke patients

Objective To investigate the prognostic factors of life and functional outcome of the first hemiparetic stroke patients. Methods One hundred and eighteen stroke subjects were registered prospectively. The Barthel index (BI) , Rankin scale (RS) , Mortricity index(MI) , Mini-mental state examination (MMSE) , Montgomery-Asberg depression scale (MADRS) and a scale of general state and risk factors were used to evaluate at the 48th hour, the 15th day and the 90th day after stroke. Results The patients' performance, as demonstrated by their scores with all the evaluation instruments, changed significantly at all the time points of evaluation after stroke (P <0.05). There was no significant difference between the performance at the 48th hour and the 15th day after stroke ( P > 0.05 ). But at the 90th day after stroke, the activity of daily living performance and the depression status recovered significantly (P < 0.01 ). Logistic regression analysis showed that, such factors as pneumonia, urinary incontinence within 48th hour and deep sensation disturbance might adversely influence patients' activity of daily living performance at the 90th day after stroke; the muscle strength of upper extremities at the 48th hour, and MMSE scores at the 15th day after stroke acted as the protective factors. Conclusions The stroke patients improved significantly with regard to their clinical and functional manifestations when evaluated 90 days after stroke onset. The main factors influencing the activity of daily living performance 90 days after stroke onset included deep sensation disturbance,pneumonia, urinary incontinence and muscles strength of upper extremities at 48th hour, and MMSE scores at the 15th days after onset.

OBJECTIVETo explore the feasibility to evaluate objectively auditory temporal resolution using the iso-modulation depth temporal modulation transfer function (TMTF) derived from the amplitude of evoked response to sinusoidally modulated signals.
METHODSChronic electrodes were implanted in inferior colliculus and auditory cortex of guinea pigs. We recorded the evoked response to sinusoidally modulated tones with modulation frequency being varied from 20 to 400 Hz and modulation depth fixed at 100%. The response amplitude in uV was converted into relative amplitude using the fast Fourier transform (FFT) function provided by Biosig software, and then the iso-modulation depth TMTF was plotted with the relative amplitude changed with modulation frequency. Then we recorded the evoked response to the sinusoidally modulated tones with modulation depth being varied from 100% to 10% and derived the iso-amplitude TMTF comparable to conventional modulation depth threshold TMTF. The derived iso-amplitude TMTF was compared to iso-modulation depth TMTF to determine the validity of iso-modulation depth TMTF.
RESULTSThe iso-modulation-depth TMTF and iso-amplitude TMTF in inferior colliculus and auditory cortex of guinea pigs represented respectively bandpass and lowpass characteristic. The cut-off frequency calculated from the two TMTF methods didn't differ significantly and the cut-off frequency derived from auditory cortex iso-modulation depth TMTF was consistent with behavioral results.
CONCLUSIONSThe TMTF plotted with the response amplitude to sinusoidally modulated tones with the modulation depth fixed at 100% and the modulation frequency was a valid method to evaluate objectively auditory temporal resolution.

Objective To explore the effect of low constant direct current (LCDC) in the medullary cavity on repairing of rabbit radial defect as to evaluate its clinical value. Methods DBX was put into the segmental osteoperiosteum defect of rabbit bilateral radius. The rabbits were divided into 2 groups: group A (subject group) with the defects stimulated by LCDC and group B (control group) with the defects left untreated. In week 2,5 and 9, 4 animals from each group were put into death, and the radius was taken for study using photograph, electron microscope, histological & morphologic technique, and the contents of the Calcium (Ca) and ALP were determined. Objective To determine the random spot albuminuria to creatinine ratio (ACR) of normal pregnant women , to track the pregnancy outcome , and to discuss the predictive value of ACR in women with hy-pertensive disorders complicating pregnancy (HDCP). Methods Except for 87 pregnant women suffering from HDCP, 2 038 pregnant women were enrolled in this study. ACR, routine examinations of blood and urine, blood biochemical, 24-hr urinary protein were determined. Results ACR, but not 24-hr urinary protein level,was sig-nificantly higher in women with HDCP. There was positive correlation between the ACR and 24-hr urinary protein quantitation. Age, gestational weeks, ACR, red blood cells, fasting plasma glucose, serum creatinine, total pro-tein were the independent risk factors for HDCP. The sensitivity , specificity and optimal cut off value of ACR for predicting HDCP were 0.78, 0.63, 1.46 mg/mmol. Conclusions There was positive correlation between ACR and 24-hr urinary protein quantitation , and ACR provided a more sensitive pathway for early predictionof HDCP.

Objective To investigate the bcl?2 gene modification on neurological function recovery in rats with spinal cord injury in neural stem cell transplantation. Methods Cultured rat neural stem cells by Ad?EGFP as vector?mediated side B?cell lymphoma 2 gene ( bcl?2 ) gene transfection of neural stem cells were divided into 3 groups: control group, negative transfection group, bcl?2 transfection group. Use western?blot to detect the expression of bcl?2 protein in neural stem cells before and after transfection. 85 adult female SD rats, successful model 72, were randomly divided into control group, NSCs group, bcl?2?NSCs groups, 24/group, rat acute spinal cord injury model in accordance with a modified Allen’ s method. Assess the motor function by BBB rating and the swash plate test. 7 days after modeling by RT?PCR and Western blot detection of spinal cord injury around HSP27, c?fos gene expression, TUNEL assay apoptosis. Aim To study the prevalence of blood- lipid level and abnormal lipdemia in Xinjiang pasturing area minority,and select high risk population.Methods A current investigation of 1 084 Mongol and Kazakan people with the methods of random sample, meanwhile, according to criterion to collect vein blood to measure blood TC,TG,LDL- C , to calculate HDL- C with numeration.Statistical analysis with SPSS software package. To take corresponding statistical methods according to characteristics of the documents.Results (1) Hyper LDL- C,low HDL- C are the key types in this pasturing area, the former prevalence was 51.0% and 32.1% among the male and female; and the later was 50.2% and 29.8% (χ 2=39.50,45.60,P< 0.01), both near average standard in our country.(2)Abnormal lipdemia was related to age and sex,that was more higher in the male group than that in female group, and increased with the age, but the female group was more higher than male group over 60 years old.(3)The general standard tendency of HDL- C in female group was higher than that in male group, but with no statistics difference.(4)Compared with other areas,TC,LDL- C standard on the higher level,but TG,HDL- C on the lower.(5)Compared with the blood- lipid level of the same minority ten years before,TC,TG,LDL- C showed an increasing trend,however,HDL- C which had protection to human body showed a decreasing trend.Conclusion The Mongol and Kazakan people in Xinjiang pasturing areas are the hyper prevalence of the abnormal blood- lipid level.The etiology may be connected with the heredity susceptibility, on the other hand,the dietetic structure and habits can not be ignored.
7.Preliminary study of neoadjuvant plus chemotherapy combined with concurrent chemoradiotherapy for locally advanced giant cervical cancer

Objective:To preliminarily investigate the efficacy and safety of bevacizumab plus neoadjuvant chemotherapy combined with concurrent chemoradiotherapy in the treatment of locally advanced giant cervical cancer (LACC).Methods:Twenty-four patients with LACC who were treated with bevacizumab combined with neoadjuvant chemotherapy were assigned into the study group, and 30 patients receiving neoadjuvant chemotherapy in phase Ⅱ clinical trial (ChiCTR-TRC-11001832) were allocated in the control group. The survival rate was calculated by the Kaplan- Meier method, and the significance of differences between the variables was determined by the log-rank test. Results:The tumor volumes were (1.64±23.15) cm 3 and (12.83±15.08)( P=0.037), and the complete remission (CR) rates were calculated as 45.8% and 13.3%( P=0.004) in the study and control groups after neoadjuvant chemotherapy. The tumor volumes were (0±1.5) cm 3 and (1.00±10.63) cm 3( P=0.022) and the CR rates were 70% and 50%( P=0.009) in the study and control groups before afterloading treatment. The median follow-up was 24.6(9.3-101.7) months. The 1-and 2-year overall survival rates were 96%, 96% and 90%, 71%( P=0.110), the recurrence-free survival rates was 96%, 96% and 97%, 89%( P=0.512), and the distant metastasis-free survival rates were 96%, 88% and 83%, 80%( P=0.297) in the study and control groups, respectively. Adverse reactions were acceptable in both groups. Conclusion:Bevacizumab combined with neoadjuvant chemotherapy can significantly reduce the tumor volume, improve the tumor CR rate and yield tolerable adverse reactions.

9.Relative factors in home safety evaluated by multilevel statistical models

Objective To understand the situations of home establishments and the related domestic behaviors that causing injuries. Methods A total of 9760 families with regular residents in a community in Shanghai were investigated. Questionnaire was designed bascd on the Guidelines for Conducting Community Surveys on Injuries and Violence and International Classification of External Causes of Injuries. Results Inside the home settings, relative factors were found on issues as fire protection and using of electricity. In terms of household settings, 14.85% of the families had smoke alarm systems in the kitchen; 40.75% had no windows set for emergence. 50% to 70% of the residents had the idea of safe behaviors, including 35.93% of the families stored cleaning products,other chemicals or medical substances in alternative containers, such as used drinking bottle. Only 1.81% of the people being investigated thought that home was also a high risk place where injury might occur and it was placed number 9 in a multiple choice questionnaire. Data from the multiple level model analysis showed that factors as number of family members, space, education, profession etc. were closely related to the situation of home settings and their resident's behaviors. Conclusion Many injury related factors were found related to home establishments and people' s daily behaviors at home which called for interventions to be taken.

Aim To investigate the effects of dihydromyricetin on the apoptosis in AGZY-83-a tumor cells and its relevant mechanisms.Methods The survival rate of AGZY-83-a cells was assayed by MTT dye reduction.The cellular apoptosis was analyzed by TUNEL method and caspase-3 activity.The intracellular free calcium i was detected to explore the apoptotic mechanism of dihydromyricetin.Results In the MTT assay,dihydromyricetin 50?mol?L-1 significantly inhibited survival ratios of AGZY-83-a cells at dose-and time-dependent manner.It was demonstrated in TUNEL assay that dihydromyricetin could induce the apoptosis of AGZY-83-a cells in a concentration-dependent manner.The examination of Caspase-3 activity indicated that the dihydromyricetin could induce the apoptosis of AGZY-83-a cells,which was dose-dependent activation of Caspase-3 in AGZY-83-a cells.The detection of the intracellular i showed that the average FI of the i could be markedly increased to 20-fold as the basic condition.Conclusions Dihydromyricetin can induce the apoptosis in AGZY-83-a cells,which is associated with the overload of the intracellular i.
