1.Utilization Analysis of Antidiabetic Essential Medicines in the Outpatient of Our Hospital from 2012 to 2014

OBJECTIVE:To provide reference for clinical rational use of antidiabetic essential medicines. METHODS:Retro-spective analysis was used to sort and analyze the consumption sum,DDDs and DDC of antidiabetic drugs in the outpatient of our hospital from May 1st,2012 to Apr. 30th,2014. RESULTS:The consumption sum and DDDs of insulin and oral antidiabetic drugs had upward trend. The top 3 consumption sum and DDDs of oral antidiabetic drugs were acarbose,metformin and glimepiri-de,and the highest DDDs of insulin was short-acting and premixed recombinant human insulin,all of them were in the National Essential Medicine List (2012 edition);DDDs of Glibenclamide tablets,Glipizide tablets and Chinese patent medicine were rela-tively small;DDC in long-acting insulin analogues was obviously higher than other kinds of insulin. The sales ratio of essential medicines was increased from 10.48% to 67.78% after National Essential Medicine List (2012 edition)carried out. CONCLU-SIONS:The use of antidiabetic drugs in the outpatient of our hospital is basically rational. Acarbose and metformin have the high-est DDDs,and DDDs of insulin analogues grow rapidly.
2.Comparison of two types of silicone impressions on accuracy of implant model

Objective To compare the different accuracy of implant model made by the two types of silicone impressions.Methods Fifteen impressions of standard abutment were made separately by machine-mixed and hand-mixed,then made into super-stone models and the accuracy of the different impressions was compared.Results The two types of silicone impressions showed significant difference of the margin precision,while differences on the height,diameter and the width of abutment shoulder were not found.Conclusions The two methods both have ideal accuracy,but porcelain-fused-to-metal restorations require high accuracy of the model shoulder,which is not suitable to use machine-mixed silicone impression technology,and hand-mixed method can be used in fixed restoration,removable denture and implant level impression.

Survivin expresssion in embryo spleen, embryo kidney, embryo liver and in many cancer tissues was determined by RT-PCR, while not in the health liver tissue. Construct the engineered Escherischia coli expressing human survivin and identify the expressed human survivin by Westen-blot. The combination activity of Survivin and RhSmac was determined in vitro. L929 cells transferred with Survivin can survive longer than which transferred with BSA.
4."Experimental Study of Six Testacean TCM""Endometriosis""Role on Goiter Model Rats"

[Objective] To observe the effects of Oysters, Abalone shel , Concha arcae, Mother-of-pearl, Concha cypraeae, Clamshel , these six testacean TCM on the goiter rats. [Method] 72 healthy SD rats were randomly divided into nine groups:normal group, model group, positive group and six testacean TCM groups. Rats were given daily propylthiouracil (0.15 g·kg-1), continuous modeling for 10d, weighing thyroid wet weight,assaying T3, T4 and TSH contents in serum by ELISA, observing the morphological changes of thyroid under light microscope. [Result] Rats in the model group compared with normal group in-creased the relative weight of thyroid, reduced the content of T3,T4, and there was a significant difference( P<0.01), thyroid fol icular epithelial cellhyperplasia was obvious;compared with the model group, Oysters and Clamshel group made the content of T3,T4 in rat rebound, and there was a significant difference (P<0.05);Oyster and Clamshel group’s thyroid fol icular cellhyperplasia were not obvious, while the hyperplasia had different levels for the other 4 kinds of testacean TCM. [Conclusion] Oysters and Clamshel were better to improve the thyroid function of the goiter model rats.
5.Gene Coloning,Expression and Enzymatic Assay of Human sPLA2-IIA

Objective: To clone the cDNA of human sPLA2-IIA,construct the engineered Escherischia coli expressing human sPLA2-IIA and identify the expressed human sPLA2-IIA. Methods: Total RNAs were purified from human fetal spleen. The cDNA of human sPLA2-IIA was cloned by RT-PCR and inserted into plasmid pET32a(+) between NcoI and EcoRI sites for expressing the recombinant human sPLA2-IIA in Escherischia coli BL21(DE3). The recombinants were screened by SDS-PAGE. The engineered Escherischia coli expressing trxA-human sPLA2-IIA fusion protein was established. The expressed human sPLA2-IIA exists in the form of inclusion body and accounts for about 25% of the total proteins of Escherischia coli BL21(DE3). Conclusion: the engineered E. coli methods are suitable for preparing plenty of human sPLA2-IIA which has laid base for the large-scale expression,purification and basic studies of human sPLA2-IIA.
6.Improvement of Quality Standard for Pingyou Granules

To establish the quality standard for Pingyou granules. Methods: Paeoniae radix, Carthamus tinctorius, Coicis semen and Nutgrass galingale rhizome were qualitatively identified by TLC. The content of peoniflorin was determined by HPLC. The separation was performed on a Lichrospher-C18 (250 mm × 4. 6 mm, 5 μm) column with the mobile phase of methanol-acetic acid (24∶76). The detection wavelength was 232nm. Results:Paeoniae radix, Carthamus tinctorius, Coicis semen and Nutgrass galingale rhizome could be identified by TLC without any interference from the negative control. The linear range of peoniflorin was 6. 560-104. 920 μg·ml-1(r=0. 999 9) with the average recovery of 98. 77%(RSD=2. 73%,n=9). Conclusion:The qualitative identifi-cation is specific and reproducible, and the quantitative method is simple, accurate and reliable, which can be used in the quality con-trol of Pingyou granules.
7.Clinical management and perinatal outcome of twin reversed arterial perfusion sequence in different stage

Objective To discuss the methods and outcomes of twin reversed arterial perfusion sequence in different stage. Methods From August 2012 to December 2013, 11 cases were diagnosed with twin reversed arterial perfusion sequence (TRAP) by 3d color Doppler ultrasound in the Maternal ang Fetal Medicine Center ,Affiliated Shengjing Hospital, China Medical University, including 3 cases in stageⅠa, 7 in stageⅡa and 1 case in stageⅡb. We performed expected therapy to 3 cases in stageⅠa and 4 cases in stageⅡa [1 case (case 9) in stageⅡa developed to stageⅡb and was terminated by emergency cesarean section], radio frequency ablation (RFA) selective reduction to 3 cases in stage Ⅱa, emergency cesarean section to 1 case in stageⅡb. All cases except case 1,4 took cesarean section to terminate pregnancy and would be followed up on the aspect of infant′s growth. Results (1) 3 cases in stageⅠa were monitored by sequential ultrasound examination, all donors grew normally, in which 2 acardias arised spontaneous cessation of flow and were implemented of cesarean section after the normal period of gestation; 1 case suffered premature rupture of membrane and experienced the natural childbirth at 34 gestation weeks plus 5 days. The survived average labor weight was 2 923 g. (2) In the 7 stage Ⅱa cases, 3 cases were implemented of RFA selective reduction, 2 cases survived and the average labor gestation was 35 weeks plus 1 day and average labor weight was 2 050 g;in 3 expected therapy cases, 1 case suffered premature rupture of membrane at 34 gestation weeks plus 5 days and experienced cesarean section; 1 case was observed for 3 weeks and the donor was stillborn; 1 case progressed to stage Ⅱb and experienced cesarean section at 34 gestation weeks plus 5 days;and 1 case was lost of follow up. (3) For 2 cases of stageⅡb cases, one was progressed from stage Ⅱa during expected therapy; the other one had been already in stage Ⅱb since visiting and experimented an emergency cesarean section, but the neonate died of heart failure two days after labor. (4) 7 survived neonates were followed up, all had a normal condition compared to kids of the same age, except one suffering a congenital heart disease, patent ductus arteriosus and atrial septal defect and whose weight (6 kg at 7 months) was lighter. Conclusions We should make the diagnosis of TRAP as early as soon and choose appropriate therapy according to the stage. According to our research, cases in stage Ⅰa can undergo an expected therapy under a sequential ultrasound examination;cases in stageⅡa can undergo a RFA selective reduction to improve the prognosis of the donor, which can make a good perinatal outcome.
8.Diagnostic experience on familial Gitelman syndrome

Objective To report two cases of familial Gitelman syndrome and literature review regarding the updates of relevant genes,classification,treatment,and prognosis.Methods The clinical data of two sisters with Gitelman syndrome were retrospectively analyzed.Results Their blood pressures were within normal range.Hypokalaemic alkalosis,hypomagnesemia,and hypocalciuria were corrected almost completely after three days of intravenous magnesium and potassium infusion,spirolactone and indometacin.However,the maintenance of normal potassium was unsuccessful over one year.Conclusion Hypokalaemic alkalosis,hypomagnesemia,and hypocalciuria were normalized in Gitelman syndrome.There was some debate in regard to using PGE2 synthetase inhibitors.Tolerance of long-term medication will be the big challenge for curative effect.
9.Proposal on ROC-theory based quantitative assessment of radiologists'diagnostic competence

Perfofinance of the radio-diagnostic system can be evaluated quantitatively by the analysis method of the ROC curve.Areas under the ROC curves represent the diagnostic perfogrmance efficiency.A comparison of the perforrmance variance of the diagnostic system incurred by different diagnosticians,will find the diagnostic competence of individual diagnosticians.This can help hospital management in deploying personnel in the department of radiology reasonably.
10.Investigation of cerebral iron deposition in aged patients with ischemic cerebrovascular disease using susceptibility-weighted imaging

Objective To investigate iron deposition level in patients with ischemic cerebrovascular disease and its correlation with cerebral small vessel disease (CSVD) imaging markers.Methods A total of 74 patients with first-ever transient ischemic attack (TIA) (median age:69 years old;30 males,44 females) and 77 patients with positive ischemic stroke history (median age:72 years old;43 males,34 females) were studied retrospectively.On phase image of susceptibility weighted imaging (SWI),regions of interest (ROIs) were manually drawn at the bilateral head of the caudate nucleus (CN),lenticular nucleus (LN),thalamus (TH),frontal white matter (FWM),and occipital white matter (OWM).The correlation between iron deposition level and the clinical and imaging variables was also investigated.Results Iron deposition level at LN was significantly higher in patients with previous stroke history (P < 0.05).It was linearly correlated with the presence and number of cerebral microbleed (CMB) but not with white matter hyperintensity (WMH) and lacunar infarct.Conclusions Ischemia may cause iron deposition elevation at basal ganglia,and LN may be more susceptible to ischemia-induced alteration.CMB may be an independent risk factor for higher iron deposition.
