Objective: To investigate the influence of Xuebijing injection (血必净注射液) on hemodynamics of dogs with endotoxic shock and its mechanism. Methods: Endotoxic shock was induced by intravenous infusion of lipopolysaccharide of E.coli. O55:B5 to dogs, and the 24 dogs were randomly divided into control group and Xuebijing injection group. Xuebijing injection was administered in the Xuebijing injection group. The hemodynamic parameters and plasma lactate levels were monitored. Results: After the administration of endotoxin, the mean arterial pressure (MAP) decreased significantly, while the mean pulmonary artery pressure (MPAP) and cardiac index (CI) increased markedly in both groups (all P0.05). Objective To investigate the intervention of Chinese herb Shengjiang San(SJS)for the imbal-ances of serum Th1/Th2 cells and related regulatory factors in sepsis patients. Methods Fifty-five sepsis patients were randomly divided into two groups:conventional treatment group of 27 cases and SJS group of 28 cases. Nine cases of healthy volunteers were enrolled as control group in the study. Cases of conventional group were treated with western medicine only and cases of SJS group were treated with both western medicine and Chinese herb SJS (100 mL twice one day). The therapy course of both groups was 3 days. Score of Chinese medical syndromes ,se-rum level of leukocyte count,C reactive protein(CRP)and procalcitonin(PCT),serum T-bet,GATA-3,Th1&Th2 cells in the proportion of whole CD4+Th cells and Th1/Th2 ratio were compared respectively in each group be-fore and after treatment. Results There were significant differences between SJS group and conventional group in score of Chinese medical syndromes,serum level of leukocyte count,T-bet,GATA-3,Th1&Th2 cells in the pro-portion of whole CD4+Th cells and Th1/Th2 ratio(P<0.05 or P<0.01). There were no significant differences be-tween the two groups in serum level of CRP,PCT and GATA3. Compared with control group,there were signifi-cant differences in all indicators except GATA3 of the two groups(P<0.01). Conclusion Chinese herb Shengji-ang San has an effective benefits to Chinese medical syndromes,inflammatory reaction,the imbalance of serum Th1/T2 and related regulatory factors(T-bet&GATA-3)in sepsis patients.
4.Different fixation methods for artificial femoral head replacement: A biomechanical comparison of joint stability

BACKGROUND: Artificial humeral head replacement is an effective method for the treatment of complex proximal humeral fractures, which has received good results in relieving pain. However, the final functional recovery is unpredictable. OBJECTIVE: To compare biomeshanical stability between anatomical and overlapping reconstruction of the greater tuberosity in cadaveric humeral head replacement models.METHODS: Eight pairs of fresh-frozen shoulder cadavers (16 shoulder joints) were match-paired into two groups. Standardized humeral head replacement procedure was performed in all specimens, and anatomical and overlapping reconstruction of thegreater tuberosity was adopted in each group respectively. For overlapping group, the greater tuberosity was reattached to the proximal humeral shaft in an overlapping style, which was achieved by an additional 5 mm bone osteotomized from the medial cortex of the humeral diaphysis. Custom mounting apparatus and fixation jigs were designed for designated shoulder motion.RESULTS AND CONCLUSION: When the shoulder was external rotated to neutral position, the mean displacement of greater tuberosity in the anatomical reconstruction group was smaller than that of the overlapping reconstruction group (P < 0.05). When the gleno-humeral joint was elevated to 30~ and 60~ forward flexion (accounting for 45° and 90° shoulder forward flexion), there was no significant difference of greater tuberosity displacement between the anatomical group and overlapping group. The findings demonstrated that, although overlapping reconstruction can increase the bone healing area between the greater tuberosity and the humeral diaphysis, there may be some loss in mechanical stability as the trade-off. Even though we strictly follow the standardized postoperative rehabilitation protocol after humeral head replacement, prominent displacement between the greater tuberosity relative to the humeral diaphysis was detected. Accordingly, postponing of the postoperative rehabilitation program after humeral head replacement for a decent period may improve tuberosity healing.
5.Proteomic screening of the proteins associated with early lung squamous carcinoma

Objective:To screen the early biomarkers associated with lung carcinoma by using proteomic method. Methods:The proteins of early lung squamous cell carcinoma tissues and adjacent normal tissues were separated by two-dimensional gel electrophoresis (2-DE) and differentially expressed proteins were screened by mass spectrometry (MS). The differential expressions of part proteins were verified by immunohistochemical method. Results:The proteomic profiles of lung squamous cell carcinoma and the adjacent normal tissues were established and the average matched protein spots in the two group were 626 and 602, respectively. Ten protein spots with higher expression in lung squamous cell carcinoma tissues were identified as Annexin 1(Anx-A1),heat shock proteins (HSP27), etc. The proteins were mainly related with cell cycle and signal transduction. The results of immunohistochemistry showed that the positive expression rates of Anx-A1 and HSP27 in lung squamous cell carcinoma tissues were significantly increased (P<0.05). Conclusion:Proteomic technology was an effective method for preliminary identification of the proteins associated with early lung carcinoma. The identified proteins laid a solid foundation for further screening the molecular markers for early diagnosis and treatment of lung cancer.
6.Identification of Major Metabolites of Salbutamol in Swine Urine and Plasma Using Ultra-High Performance Liquid Chromatography-Electrospray-Time of Flight-Mass Spectrometry

Theexcretionofurinaryandplasmametabolitesofsalbutamolwasstudiedusingultrahigh performance liquid chromatography electrospray time-of-flight mass spectrometry, after a single intragastric gavaged dose administration with salbutamol. The software of Agilent MassHunter Metabolite ID was employed to find and identify the chemical structure of metabolites of salbutamol. Five metabolites from salbutamol were identified. Metabolites identified in swine urine included glucuronide conjugate salbutamol, N-oxide-salbutamol, hydroxyl-salbutamol, methoxyl-salbutamol and dehydrated hydroxyl-salbutamol. Whereas, only the parent drug, glucuronide conjugate salbutamol and dehydrated hydroxyl-salbutamol were observed in swine plasma.
7.Clinical diagnosis of 5 cases of pituitary stalk interruption syndrome

Objective To raise the level of clinical diagnosis for the patients with pituitary stalk interruption syndrome (PSIS). Methods Five patients (4 males) with PSIS were retrospectively analyzed with respects to the clinical features, endocrine status and image characteristics. Magnetic resonance imaging (MRI) scan was performed at hypothalamic-pituitary region. Results The clinical manifestations of all patients consisted of growth retardation and delayed puberty without polyuria. Deficiency of multiple anterior pituitary hormones was revealed in all patients by the evaluation of endocrine status. The features of MRI included a lack of visible pituitary stalk, absence of posterior lobe hypersignal in the sella turcica and a hyperintense spot in the region of the thalamus opticus. Conclusion The clinical characteristics of patients with PSIS are growth retardation and delayed puberty. The evaluation of anterior pituitary function is necessary. The detection of an anatomical abnormality around hypothalamic-pituitary region by MRI is important diagnostic evidence.
9.A case report of polyglandular syndrome induced by programmed death-1 inhibitor and literature review

We reported a case of polyglandular syndrome induced by programmed death-1(PD-1) inhibitors. The patient was a 51-years-old male with non-small cell lung cancer, treated with PD-1 inhibitor nivolumab/pembrolizumab because of postoperative subcarinal lymph node metastasis indicated by PET-CT. During 14 cycles of PD-1 inhibitor treatment, the patient successively developed primary hypothyroidism, and type 1 diabetes mellitus(T1DM). More than five months after the withdrawal of pembrolizumab, the patient experienced recurrentce. Laboratory examinations showed mild hyponatremia and hypopituitarism including ACTH and growth hormone(GH)/insulin-like growth factor-1(IGF-1) insufficiency. This is the first report of a patient diagnosed as polyglandular syndrome caused by PD-1 inhibitor. In particularly, the hypothyroidism and T1DM did not improve after drug withdrawal, while hypopituitarism was further aggravated. This case reminds us that we should pay more attention to the changes of endocrine function during and after the treatment of PD-1 inhibitor, so that we can make the correct diagnosis and take proper medical measures timely, to avoide missed diagnosis, and improper treatment.
10.Outcome of postoperative hypopituitarism and hormone replacement situation in 215 patients with pituitary adenoma

Objective To investigate the outcome of postoperative hypopituitarism and hormone replacement in patients with pituitary adenoma,and to analyze the potential factors related to postoperative hypopituitarism.Methods A total of 215 postoperative patients with pituitary adenoma were analyzed.Pituitary functions( including gonadal,thyroid,and adrenal axes ) were asessed by strict criteria.Data of surgery history and hormone replacement situation were collected for statistical analysis.Results The prevalence of hypopituitarism was 54.0%,including 36.7% hypogonadism,32.6% hypothyroidism,and 28.4% hypoadrenalism.Replacements of gonadal steroid,glucocorticoid,and thyroxine were carried out in 25.6%,84.3%,and 80.6% of the cases,respectively.Univariate analysis showed that male sex and large tumor were related to hypopituitarism. Conclusion After pituitary adenomectomy,approximately half of the patients present anterior pituitary dysfunction,while quite a number of them have not been treated appropriately.
