1.A STUDY ON THE VALUE OF LOCAL HYPOTHERMIA THERAPY IN THE TREATMENT OF NEUROSURGICAL SEVERE PATIENTS

Objective To study the effects of hypothermia therapy on inflammation level and safety of patients with severe nervous injury.Methods A total of 82 patients with severe nerve injury were divided into study group and control group by random digits table, 41 patients in each group.The patients in these two groups had no statistical dis-parities in inflammatory agent and intracranial pressure (p>0.05).The patients in the control group were treated by routine therapy and those in the study group were treated by cephalic hypothermia therapy additionally.The two groups were compared in terms of the level of TNFα, IL-6 and IL-8 in the cerebrospinal fluid, prognosis and safety.Re-sults Before treatment there was no significant difference in inflammation level between the two groups (p>0.05). After treatment, the TNFα, IL-6 and IL-8 levels in the study group were significantly lower than those in control group (p<0.01).Before treatment there was no significant difference in ICP between two groups (p>0.05).How-ever, after treatment there was a significant difference in ICP between the two groups in all time points (p<0.01). The GOS score in the study group was significantly lower than that in the control group.There was no statistical dis-parities in complications between the two groups (p>0.0).Conclusion The hypothermia therapy has a great clini-cal efficacy and safety on severe never injury patients, which makes it worth clinical application.
2.CT Analyse of Solitary Inflammatory Lesion of the Lung

Objective To study CT findings of solitary inflammatory lung lesion,and improve CT diagnostic accuracy.Methods In 21 cases with solitary inflammatory lung lesion,four cases were treated with surgery,four cases were punctured with CT guided biopsy,and other cases were treated with anti-inflammatory agents under being followed up with CT.All were scanned with 2~5 mm thin slice.Results ①The lesions showed square,triangle and irregularity (13 cases).②Part of margin blurred(12 cases).③The density at the edge of the mass was lower than at the central area(9 cases).④After anti-inflammatory therapy,most lesions gradually reduced in size(11 cases).Conclusion Anlysizing CT findings helps to diagnose solitary inflammatory lung lesion,and improves diagnostic accuracy through puncturing with CT guided biopsy necessitiously.To the basic hospital,to scan with 2~5 mm thin slice is important especially.
3.Right cheek swelling and pain for 2 weeks---Cushing′ syndrome complicated with facial infection

[Summary] A 61-year-old female patient was diagnosed as ACTH independent Cushing′ syndrome, right adrenal adenoma, right facial infection. The situation was well controlled with antibacterial agents, abscess incision drainage and ketoconazole therapy. The patient received partial adrenolectomy of her right adrenal gland 16 months later. This case indicated that anti-adrenal agents could be a reasonable choice when the Cushing′syndrome patients were acompanied with severe infection.
4.Isolation,culture and multi-directional differentiation of rat bone marrow mesenchymal stem cells by using the whole bone marrow adherence method

BACKGROUND:There is few mesenchymal stem cells(MSCs) in the bone marrow,about one BMSC in 1?104-1?105 monocytes.Following in vitro isolation,purification and amplification,it is satisfactory for in vivo requirement.OBJECTIVE:To observe biological characteristics and potentiality of multi-directional differentiation of BMSCs in vitro.DESIGN,TIME AND SETTING:The cytological in vitro study was performed at the Institute of Urinary Surgery,Union Hospital Affiliated to Fujian Medical University from October 2007 to December 2008.MATERIALS:A total of 30 clean male Sprague Dawley rats of inbreeding line were supplied by Silaike,Shanghai,China.METHODS:BMSCs from rats were isolated and cultured in vitro by using the whole bone marrow adherence method.When 80%-90% confluency,BMSCs were digested by trypsin.At the third passage,BMSCs were induced to differentiate into osteoblasts and adipocytes.MAIN OUTCOME MEASURES:Cell morphology was observed under the inverted phase contrast microscope.BMSCs surface markers were detected using flow cytometry.Osteogenic ability was examined by alizarin red staining.Adipogenic ability was measured by Oil red O staining.RESULTS:The primary cells and the passage cells were mostly fusiform in shape,to be similar to fibroblasts.Cell still kept a high potential of growth and amplification following over 10 subcultures.At the third passage,BMSCs were positive for CD44,CD90,CD106,but negative for CD34,CD45,CD11b.Following 21 days of osteogenic induction,cell alizarin red staining showed that alizarin red was positive in osteoblasts.Following 2 weeks of adipogenic induction,oil red O staining showed that red lipid droplet existed in adipocytes.CONCLUSION:Whole bone marrow adherence method can isolate,purify and amplify BMSCs in vitro.The obtained cells have general biological characteristics of MSCs,and also have potentiality of multi-directional differentiation.
5.Corrective effect of spinal three-dimensional orthopaedic internal fixation system on scoliosis of patients with Marfan syndrome

BACKGROUND: Previous researches suggested that correction of scoliosis of patients with Marfan syndrome (MFS) has a high incidence of complication and loss of corrective rate; however, there were seldom reports on using spinal three-dimensional orthopaedic internal fixation system to treat scoliosis of MFS patients.OBJECTIVE: To observe the corrective effect of CD and TSRH, the third generation of spinal three-dimensional orthopaedic internal fixation system, on scoliosis of MFS patients.DESIGN: Retrospective analysis.SETTING: Second Department of Orthopaedics, First People's of Hefei.PARTICIPANTS: From September 1997 to July 2003, 10 MFS patients with scoliosis were selected and corrected at posterior site with CD and TSRH system; meanwhile, confluence operation of auto-bone transplantation was undergone.Eight cases had bilateral scoliosis, 2 curvature of thoracic spine towards right side, 3 posterior curvature of thoracic spine, and 2 posterior curvature of thoracic waist.METHODS: X-ray film was imaged at standing position before operation, 1 week after operation and during follow-up;meanwhile, X-ray film was also imaged at left and right curved positions and sling position to evaluate ductility of scoliosis before operation. Angle of scoliosis was measured with Cobb technique and stable and neutral mallets were recorded. Among 10 cases, 2 were internally fixed with CD system and other 8 with TSRH system. Material of bone transplantation was derived from auto-posterior superior iliac spine and remained for 3 months after operation.MAIN OUTCOME MEASURES: Corrective rate and complication of scoliosis after operation.RESULTS: The follow up lasted for 12-60 months. Corrective rate of 8 cases with bilateral scoliosis was 48% and lost rate was 20%; corrective rate of 2 cases with lumbar curvature was 56% and lost rate was 16%; corrective rate of 2 cases with single thoracic curvature was 61% and lost rate was 8%. Body balance of all patients was improved. No one had nerve complication; 2 had junctional posterior curvature of lumbar vertebra; 1 had delayed union of incisions.CONCLUSION: It is satisfactory for spinal three-dimensional orthopaedic intemal fixation system to treat scoliosis of MFS patients. Statuses of heart and lung of patients are evaluated before operation; in addition, during operation, a great effect is obtained through prolonging confluence segments, avoiding over correction, decreasing soft tissue injury and paying much attention on confluence of transplanted bone.
6.Different Test Positions on Vestibular Evoked Myogenic Potentials in Youth

Objective To study different test positions on vestibular evoked myogenic potentials (VEMPs) in youth ,and to find a suitable position and provide a guidance for clinical practice .Methods Thirty normal young vol-unteers were tested by vestibular evoked myogenic potentials ,using three different positions :supine with the head held straight up(SHU),supine with the head held up and turned away from the test ear(SHT),sitting with the head turned away from the test(SIT) ,the derivation rate ,latency and amplitude were analyzed .Results The deri-vation rate of SHU ,SHT and SIT were 100% ,100% and 63 .3% ,respectively .The derivation rate ,p13 ,n23 la-tency and p13 n23 inter-latency between SHU and SIT ,and between SHT and SIT had statistical differences (P<0 .05) .No statistical significant differences were found in derivation rate ,p13 ,n23 latency and p13n23 inter-latency between SHU and SHT (P>0 .05) .The amplitude was significantly different among the three positions (P<0 .05) . No statistical significant difference were found in derivation rate ,p13 ,n23 latency ,p13 n23 inter -latency and am-plitude between men and women of the three positions (P> 0 .05) .Conclusion The derivation rate of SHT was 100% with maximum amplitude .SHT is the most recommended position for clinical test in youth .The derivation rate of SHU was 100% ,and no statistical significant difference were found in p13 ,n23 latency and p13n23 inter-la-tency between SHU and SHT (P>0 .05) .SHU can be used in clinical test .SIT is not recommended for using in clinical test .Gender does not affect VEM Ps test .
7.Growth and expression of rat bone marrow mesenchymal stem cells modified with nerve growth factor in the bladder

Objective To confirm that rat bone marrow mesenchymal stem cells (MSC) transfected with nerve growth factor (NGF) gene in the bladder tissue of diabetic rats bladder tissues can survive and stably express NGF. Methods A diabetic rat model was constructed. The BrdU-labelled MSC transfected with NGF gene were transplanted into the diabetic rats bladder tissues. BrdUlabelled immunohistochemistry was used to observe the growth of MSC transfected with NGF gene in the diabetic rats bladder tissues. The expression of NGF mRNA and protein were checked by RT-PCR and ELISA. Results A diabetic rat model was successfully built by a single intraperitoneal injectionof STZ. The blood glucose was still high after 8 weeks. NGF gene modified MSC could be detected in the bladder of diabetic rats by BrdU-labelled immunohistochemistry. The concentration of NGF in the control group, disease group and treatment group were ( 114 ± 3), ( 70 ± 2), ( 110 ± 2) pg/ml by ELISA and mRNA quantity by RT-PCR were 0. 183±0. 004, 0. 032±0. 139, 0. 130±0. 165, respectively. Compared with the control group, the expression of NGF gene was decreased (P<0. 05) in the incidence group. The expression of NGF gene was increased (P<0. 05) in the treatment group compared with the disease group. Conclusions The NGF gene-modified MSC could survive in diabetic rats bladder tissues. The NGF gene in MSC could stably express in diabetic rats bladder tissues.
8.Analysis of ten-year mortality in the male senile population with elevated fasting plasma glucose

Objective To observe the all-cause and cardiocerebrovascular disease(CCVD)-related mortalities in the senile male population and the relationship of them with fasting plasma glucose (FPG) level.Methods A survey was performed among 1 572 male subjects aged 60-90.All the subjects were groups were calculated by Kaplan-Meier method and the log-rank test was used to compare the survival rates of the 4 groups.Logistic regression model was used to analyze the correlation factors of all-cause mortality and CCVD-related regression model analysis showed that age was related with the all-cause mortality.Among all these factors, age, body mass index, FPG and the history of CCVD had the relationships with the mortality of CCVD.Conclusion In the older male population, CCVD are considered as one of the main causes of death.With the increased levels of FPG, the all-cause mortality and CCVD mortality of senile male population are increased.
9.Effect of noise on antioxidant capacity of brain tissue in guinea pigs.

OBJECTIVETo investigate the effect of noise on the antioxidant capacity in different regions of brain tissue in guinea pigs.
METHODSThirty male white red-eye guinea pigs were equally and randomly divided into five groups: 1-, 3-, 7-, and 14-day groups after noise exposure and control group. The guinea pigs of the experimental groups were exposed to steady white noise with a sound pressure level at 100 dB for 8 h per day and for 2 consecutive days. The auditory brainstem response (ABR) of guinea pigs, as well as the glutathione (GSH) level, methane dicarboxylic aldehyde (MDA) level, and superoxide dismutase (SOD) activity in the cerebrum, cerebellum, and hippocampus, was determined prior to and 1, 3, 7, and 14 days after noise exposure.
RESULTSAfter noise exposure, the shifts in ABR threshold of the experimental groups were significantly higher than that of the control group (P<0.05). Compared with those in the control group, the SOD activity and GSH level both significantly decreased in the cerebrum tissue of each experimental group after noise exposure (P<0.05) and MDA content significantly increased in the 1-day group (P<0.05). As for cerebellum tissue, the SOD activity and GSH level in the 7-day group were significantly lower than those in the control group (P<0.05), but there was no difference in MDA level between each experimental group and the control group (P>0.05). In comparison with those in the control group, the GSH and MDA levels in the 1-day group after noise exposure were significantly higher, and the GSH and MDA levels in the 3-day group and the MDA level in the 7-day group after noise exposure were significantly lower (all P<0.05).
CONCLUSIONNoise exposure can lead to hearing loss and affect the antioxidant capacity of brain tissue, which indicates that the improvement in antioxidant levels may alleviate noise-induced damage.

10.In vitro pharmacodynamics study of an anti-HIV Chinese herbal formulation.

AIDS caused by HIV-1, is a major threat to human being. An anti-HIV formulation from Chinese herbs, so called "Qu Du Zeng Ning", have been recently developed. In this work, the pharmacodynamics of the formulation in vitro was studied. The results showed that Qu Du Zeng Ning inhibit the replication of HIV-1 efficiently in all cell-based assay, with IC50 at 105.2, 70.7, 77.4 microg mL(-1), separately. A significant synergy between the formulation and zidovudine (AZT) was observed, and it also showed a potent activity against HIV-1 drug-resistant mutant.
