1.Effects of continuous infusion of moderate dose tranexamic acid and ulinastatin on fibrinolysis during orthotopic liver transplantation

Objective To investigate the effects of continuous infusion of moderate dose tranexamic acid and ulinastatin on fibrinolysis during orthotopic liver transplantation (OLT).Methods Thirty ASA Ⅲ or Ⅳ patients aged 34-63 yr with a body mass index of 17-37 kg/m2 and end-stage liver disease score 6-34,undergoing OLT,were randomly assigned to one of 3 groups ( n =10 each):group control (group C) ; group tranexamic acid (group T) and group ulinastatin (group U).The patients received continuous infusion of normal saline at 10 ml/h in group C or ulinastatin at 100 000 U/h in group U immediately after tracheal intubation until 120 min after portal vein was declamped,while in group T the patients received a loading dose of tranexamic acid 1 g followed by continuous infusion at 10 mg· kg-1 ·h- 1.Prothrombin time (PT),activated partial thromboplastin time (APTT),international normalized ratio ( INR),fibrinogen (Fg),D-dimers (D-D) and fibrin degradation product (FDP) were measured before induction of anesthesia (baseline),at 120 min after skin incision,30 min after clamping of portal vein (anhepatic phase),30 and 120 min after declamping (neohepatic phase) and at the end of operation.The amount of blood loss and transfusion were recorded.The patients were followed up after operation for hepatic artery and portal vein thrombosis in groups U and T.Results There were no significant differences in PT,APTT,Fg,INR and amount of blood loss and transfusion among the 3 groups.The plasma D-D concentration and percentage of patients with plasma FDP > 20μg/ml were significantly lower in group T than in group C.There were no significant differences in plasma D-D concentration and percentage of patients with plasma FDP > 20 μg/ml between groups U and C.No hepatic artery and portal vein thrombosis was detected after operation in groups T and U.Conclusion Continuous infusion of moderate dose tranexamic acid can inhibit fibrinolysis during OLT,but can not reduce the amount of blood loss.Continuous infusion of ulinastatin ( 100 000 U/h) has no significant effect on fibrinolysis during OLT.
2.Influence of negative emotion on patients with peptic deer

Objective To investigate the influence of negative emotion on patients with peptic ulcer and their cognition about self-management knowledge. Methods Zung self-rating depression scale (SDS), self-rating anxiety scale(S/IS) and self-designed survey questionnaire of peptic deer patients' gen-end knowledge were used to examine 105 peptic ulcer patients in outpatient and inpatient in the digestive department and the data were compared with those of 106 healthy people. Results The peptic ulcer pa-tients' total score of SDS and SAS, and the occurrence rate of positive symptom were obviously higher than those of healthy people. In the group of ulcer patients, we compared the data of SDS one by one, the emo-tion of patients was different when their profession was different. The difference between patients with and without anxiety in keeping emotion steady and appropriate diet was significant. 45.8%~78.1% of patients with negative emotions didn't understand the cycle of treatment, regular consultation with a doctor or com-plication. Conclusions Nurses should evaluate the emotional state of patients, establish nursing plan based on individual patients, give different nursing and education to different patient. Thus patients' general self-management knowledge and compliance can be improved, their spirit health level can be alleviated. It can also reduce incidence of the disease and prevent relapse.
3.Insulin resistance and polycystic ovary syndrome

Insulin resistance likely plays a central pathogenic role in polycystic ovary syndrome and may explain the pleiotropie presentation and involvement of multiple organ systems.Hyperinsulinaemia leads to increased production of androgens resulting in anovulatory infertility.Women,especially those obese women with polycystic ovary syndrome,have an increased risk of developing type 2 diabetes and cardiovascular disease in their later life.Treatment of insulin resistance through lifestyle therapy or with a diabetes drug has become mainstream therapy to reduce the risk of diabetes,endometrial abnormalities,and cardiovascular disease events in women with polycystic ovary syndrome.

BACKGROUND:The incorporating of zeolites or porous fillers into polymer membranes can improve the pervaporation separation properties of membranes.But the effect mechanism of zeolites or porous fillers on membrane properties needs to be further studied.OBJECTIVE:The study was designed to prepare ZSM-5 zeolite incorporated chitosan membrane by solution blending method to investigate the effecIs of zeolite on membrane pervaporation properties.DESIGN:A controlled observation.SETTING:Dalian Institute of Chemical Physics,Chinese Academy of Sciences.MATERIALS:This study was performed at the Laboratory of New Type Membrane Separation Technology,Dalian Institute of Chemical Physics,Chinese Academy of Sciences in May 2006.Chitosan,with deacetylation degree of 75%-85%,was obtained from Sigma-Aldrich Chemical Company,USA;Dimethyl carbonate(DMC),with purity of 99%,was purchased from Fluka Chemical Company,USA:ZSM-5 zeolite was kindly supplied by Dalian Institute of Chemical Physics,Chinese Academy of Sciences,China;Methanol,with purity>99.5%,were purchased from Shenyang Lianbang Chemical Company,China.METHODS:ZSM-5 zeolite incorporated chitosan membranes were prepared by solution blending method.MAIN OUTCOME MEASURES:①Swelling and sorption of ZSM-5 zeolite-incorporated chitosan membranes in the DMC/methanol mixture.②Effects of zeolite content in the membrane and operating temperature on pervaporation properties.RESULTS:Scanning electron mieroscope demonstrated that zeolite was uniformly distributed in the membrane matrix and the membranes were free from possible defects.The separation selectivity of dimethyl carbonate(DMC)/methanol mixtures was dominated by solubility selectivity rathcr than diffusivity selectivity.Swelling degree increased and the permeation flux of the membranes increased significantly with the zeolite content increasing.From the temperature-dependent permcation values,the Arrhenius activation parameten were estimated.CONCLUSION:The pervaporation results indicated that the membranes incorporated with the ZSM-5 zeolite exhibited better separation properties for DMC/methanol mixtures comparing with homogeneous chitosan membranes.
7.Electrical stimulation of cerebellum′s fastigial nucleus protects neurogenic autonomic function in rats after ischemic stroke

Objective To observe the effects of electrical stimulation of the cerebellum′s fastigial nucleus on neurogenic autonomic functional disturbances in rats after ischemic stroke. Methods Right middle cerebral artery occlusions (MCAOs) were performed on rats and their heart rate variability (HRV) was analysed. The MCAO rats were randomly divided into a group whose cerebellar fastigial nuclei (FNS) were stimulated and a control group. The power spectrum components and chaos of their heart rate variability were analysed. The MCAOs were performed after two cerebellar fastigial nuclei had been destroyed by pretreatment with ibotenic acid (IBO), to investigate the effect of stimulating the cerebellum′s fastigial nucleus on heart rate variability. 8.Optimization of breviscapin sustainded release tablets by mixture uniform design

AIM: To optimize the excipient formulation of lactose,HPMC_(SH4000) and Carbopol 71 G in breviscapin sustained release tablets by mixture uniform design. METHODS: Different formulations,single factor f_2 and multifactors of cumulative release as index,were evaluated respectively. RESULTS: The optical formulations obtained by the two methods were identical,and the tablets with optical formulation had ideal sustained release. CONCLUSION: Mixture uniform design is simple,direct and uniform.It can be used in optimization of formulation with invariable amount.
9.Protective role of heme oxygnase in diabetes and diabetic complications

Oxidative stress is a major contributor to the pathogenesis of diabetes and diabetic complications.Heme oxygenase(HO),an important part of the human defense system,has been recognized to have significant physiological function against oxidative stress.HO can be stimulated by oxidative stress and decompose into carbon monoxide,biliverdin and iron,all with important anti-inflammatory,anti-oxidative stress and anti-apoptosis effects.Overexpression of HO induced by hyperglycemia can protect cells(including islet cells,endothelial cells and monocytes) and tissues from oxidative damage.Increased HO metabolic products not only maintain the physiological function of islet cells, delay diabetes progression,but also regulate vascular,nerve and muscle function,so as to prevent diabetic complications.
10.Norepinephrine induces brain tissue VEGF protein expression in severe burn rats

AIM: To investigate the effect of norepinephrine (NE) on brain tissue VEGF protein expression in 24 h after severe burns. METHODS: (1) 40% total body surface area (TBSA) Ⅲ? burn model was made in Wistar males rats. Brain water (%) was examined at 24 h after burn. (2) NE levels in the rat brain tissue were determined by high performence liquid chromatography. (3) VEGF protein levels in the rat brain tissue were determined by Western blotting at 24 h of postburn. RESULTS: (1) Brain edema exhibited at the burns and burn with norepinephrine groups. (2) A increase in NE levels in the rat brain tissue was observed at the burns. NE levels increased significantly in burn with norepinephrine groups. (3) VEGF protein expression in the rat brain tissue was gradually increased with increasing in norepinephrine stimulating dose. VEGF protein expression in the burn rats brain tissue was significantly increased in burn with high dose of norepinephrine stimulation. CONCLUSION: Norepinephrine induced VEGF protein expression in rat brain tissue at 24 h after severe burn, suggesting that increases in norepinephrine level postburn induce brain edema.
