The development of medical institutes needs not only marvelous hardware construction,but also software strength,such as the hospital culture,brand establishment,etc.Only by the persistent and coordinated development of hardware strength and software strength,can hospital's comprehensive strength and core competitiveness be effectively improved.To enhance hospital software strength,it is necessary to cultivate hospital culture with unique features,emphasize brand establishment,devise long-term development blueprint of medical institutes and activate hospital's potential for further development.

Objective To study the application value of T lymphocyte subsets and inflammatory factors in evaluating severity andprognosisofaplasticanemia.Methods 40patientswithaplasticanemiainthehospitalwereenrolledinthestudyasAAgroup, which was further divided into acute aplastic anemia(AAA) subgroup and chronic aplastic anemia(CAA) subgroup .In addition to that ,40 healthy people who received physical examination in the hospital during the same period were enrolled in the study as con‐trol group .Then blood routine tests were performed ,T lymphocyte subsets content in peripheral blood and serum inflammatory fac‐tor content were detected and the results were compared .Results Th1 ,Th17 contents and Th1/Th2 ,Th17/Treg ratio in AA group were higher than those in control group(P<0 .05) ,while Th2 ,Treg contents were lower(P<0 .05);Th1 ,Th17 contents and Th1/Th2 ,Th17/Treg ratio in AAA group were higher than those in CAA group(P<0 .05) ,while Th2 ,Treg cells contents were lower . Serum IL‐2 ,IFN‐γ ,IL‐17 contents in AA group were higher than those in control group(P<0 .05) ,while IL‐4 ,IL‐6 ,IL‐10 contents were lower(P<0 .05);serum IL‐2 ,IFN‐γ,IL‐17 contents in AAA group were higher than those of CAA group ,while IL‐4 ,IL‐6 , IL‐10 contents were lower .Hb ,PTL contents were negatively correlated with Th1 ,Th17 contents and serum IL‐2 ,IFN‐γ ,IL‐17 contents ,while positively correlated with Th2 ,Treg cells contents and serum IL‐4 ,IL‐6 ,IL‐10 contents .Conclusion Peripheral blood T lymphocyte subsets and serum inflammatory factors contents in aplastic anemia are abnormal and are correlated with the severity and prognosis related indicators .

To identify Taenia cestodes from 6 regions of Yunnan province by PCR and sequencing of mtCOXⅠfragment. the genomic DNA of Taenia cestodes was extracted from proglottid collected in 6 region of Yunnan province, and mtCOXⅠ gene fragments were amplified by PCR, and then sequenced. The genomic distance and phylogenetic tree were constructed in comparison with other known mtCOXⅠgene sequences of T.solium , T.saginata and T. asiatica in GenBank using DNA MAN software. Through distance matrix,it was found that the homologie of NJ4, NJ1 and DQ2 was 99.8%, DL4 and NJ3 homologie was 99.5%, NJ2 and DQ3 homologie was 98.8%; the homologie of DL3 and BZ3 was 98.3%, while the homologie was 96.0% with BZ2; The phylogenetic tree demonstrated that 10 Taenia cestodes including NJ1-4, DL2-3and DQ1-3 occupied one brance with BZ3. BN1, CX1, LC1 and BZ2 occupied one brance, then two brance occupied and occupied with other one which was occupied by DL1 and BZ1. Taenia cestodes from Nujiang and Diqing were T. asiatica. Taenia cestodes from XiShangbanna,Lincang and Chuxiong were T.saginata.Taenia cestodes from Dali were T.solium or T.asiatica.Because same species have no difference from different regions. mtCOXⅠfragment sequencing is valid for tapeworms identification.

AIM: To evaluate the cytotoxicity of a novel anticholinergic drug penehyclidine hydrochloride (PHC) and its four optical isomers R-1, R-2, S-1, and S-2. METHODS: Two in vitro assays, MTT assay and neutral red uptake assay, were used to evaluate the cytotoxicity following PHC and its isomers exposure to HepG2 cells at different concentrations. RESULTS: PHC and its isomers induced decreases of viability of HepG2 cells in a concentration-dependent manner. Comparison of the cytotoxicity of the five anticholinergic agents with 50% inhibitory concentration (IC50) values indicated that the order of potency was PHC>R-2>R-1>S-2>S-1 for MTT assay, and R-2>PHC≈R-1>S-2>S-1 for neutral red uptake assay. CONCLUSION: With respect to the cytotoxicity of the four isomers on HepG2 cells, the R configuration was more potent than the S configuration, and R-2 was the most potent isomer whereas S-1 was the least potent isomer among the four optical isomers.

Objective To explore the clinical efficacy of Shuxuening combined with epalrestat in the treatment of diabetic nephropathy. Methods Eighty patients from March 2014 to June 2015 in Metabolic Disease Department of the Second Hospital of Tianjin were randomly divided into observation group and control group with 40 patients in each group.The control group received epalrestat, and the observation group received Shuxuening injection on the baisis of control group. The related indicators were compared between two groups.Results After treatment, the 24 h-urine microalbumin, plasma low density lipoprotein and serum creatinine in observation group [(239.31 ±106.54)mg/L,(2.45 ±0.55)mmol/L,(95.54 ± 22.13)mol/L]were lower than those in control group [(349.90 ±148.40),(3.41 ±0.52),(108.76 ±34.30)](P<0.05).The β2-microglobulin (β2-MG) and plasma high-sensitive C-reactive protein (hs-CRP) in observation group [(0.39 ±0.06),(6.31 ±1.58)mg/L]were lower than those in control group [(0.49 ±0.12),(7.89 ±1.35)](P<0.05).The total efficacy in observation group was higher than that in control group (70.0%vs.45.0%,P<0.05).Conclusion Shuxuening injection joint epalrestat has the exact efficacy in treatment of patients with diabetic nephropathy.

This study aimed to explore the mechanism of volatile oil from purslane in treating itch induced by eczema through establishing the itch model,stimulating the keratinocyte with capsaicine (CAP).SD rats were divided into the control group,the model group,the high dose group of volatile oil from purslane,and low dose group of volatile oil from purslane.After finishing the experiment,the morphology of keratinocytes was observed by immunofluorescence technique,while Ca2+ concentration was detected by flow cytometry,and the contents of leukotriene A4 methyl ester (LTA4),interleukin-31 (IL-31) and hydroxy trptamine H1 (HTH1) were quantified by ELISA assay.The expression of TRPV1 mRNA in keratinocytes was tested by RT-PCR,while the protein level of TRPV1 was quantified by western blot.Compared with the model group,it was found that the cell count of positive keratinocytes,the Ca2+ concentration,the levels of LTA4,IL-31 and HTH1,and the mRNA and protein expressions of TRPV1 in the high dose and low dose groups were significantly decreased (P ＜ 0.05).In conclusion,it was demonstrated that the volatile oil from purslane may relieve itch through inhibiting the activation of TRPV1 and reducing secondary inflammatory reaction.

Objective To identify the efficacy and safety of aspirin in reducing the serum high sensitivity C reactive protein(hsCRP)level and preventing the internal arteriovenous fistulas(AVF)embolism in maintenance hemodialysis patients. Methods One hundred and ten hemodialysis patients using AVF more than 3 months were randomly divided into 2 groups,the intervention group(n=55,received aspirin 100 mg/d)and the control group(n=55).Examinations were performed at baseline and 6-month after treatment.Serum hsCRP level,platelet aggregates ratio(PAR),coagulation and inflammation indicators were measured.AVF embolism and the adverse events were monitored. Results Six months later.PAR and hsCRP level of the intervention group patients aged≤60 years decreased significantly [(68.14±8.45)%vs (82.37±9.12)%;(4.79±4.81)mg/L vs(6.94±10.26)mg/L,all P＜0.05],and were significantly lower as compared to.the control group[(68.14±8.45)%vs(74.7±11.50)%,(4.79±4.81)mg/L vs(5.12±9.25)mg/L,all P＜0.05].There were 2 cases of AVF embolism occured in both groups,which showed no statistical difierence (P=0.676).The incidence of adverse effeets was higher in the intervention group than that in the control group but no statistical significance was found (20.0%vs 7.2%,P=0.052),while the mortality rate had no difference between 2 groups (3.6%vs 0,P=0.495).Conclusion Use of aspirin 100 mg/d for 6 months can significantly reduce the serum hsCRP level and PAR in hemodialysis patients under 60-year-old without serious adverse reactions.

Objective To explore the clinical characteristics and prevention management of death events caused by infections in end-stage renal disease (ESRD)patients undergoing hemodialysis. Methods Clinical data of ESRD patients undergoing hemodialysis in Nephrology Department of Zhongshan Hospital from 1998 to 2008 were retrospectively studied.Death causes,primary diseases,complications,infections,and survival time were analyzed. Results A total of 252 patients died including 162 males(64.29%)and 90 females(35.71%).Average death age was (63.48±14.77)years.In death events,emergency dialysis accounted for 59.52%,and primary glomerular disease was the major primary diseases(27.23%),then diabetic nephropathy(16.90%)and hypertensive nephrosclerosis (14.55%).34.8%death was caused by infections or promoted by infections,secondly by cerebrovascular events(23.6%).The elderly accounted for the majority of infection-associated deaths.48.15%and 38.71%patients with deaths caused or promoted by infections respectively had shorter dialysis duration(<3 months),whose percentage was much higher than those with non-infection-associated deaths (30.34%).Primary infection was still pulmonary infection(77.59%),then blood infection(10.34%)and catheter-associated infection(5.17%).58.62%infection-associated deaths had positive microbiologic test results,and gram negative bacillus accounted for 38.24%.50% of patients with positive test was complicated with fungal infection. Conclusions Infection is a main cause and a critical promotion to death in ESRD hemodialysis patients,besides it is the main cause of death in the elderly (>75 years)and hemodialysis duration within 3 months,which may result in shorter survival.Pulmonary infection and gram negative bacillus combined with fungal infection should be considered in the treatment.Prophylaxis of nosocomial infection and pulmonary infection in hemodialysis patients should be more emphasized.

This paper analyzes the recently epidemic status of schistosomiasis,the change of natural and social factors,and field survey and evaluation data of schistosomiasis in Ya’an City after Lushan Earthquake on April 20,2013,and proposes that it is necessary to strengthen the conventional schistosomiasis control measures,the control of exogenous infection sources,the con-trol of Oncomelania hupensis snails and health education for ensuring no major epidemics after the disaster. This paper also recom-mends the direction and suggestions for future schistosomiasis control in Ya’an City.
