1.Application of adipose-derived mesenchymal cells in bone/cartilage/intervertebral disc/tendon tissues repair

Autologous cells of bone, cartilage, intervertebral disc and tendon, are hard to be cultured and proliferate in vitro and thus have difficulty to be introduced in the repair of damaged tissue. Stem cells have the ability to self-renew and differentiate into many tissue types. Recent progress in stem cell research has led to an enthusiastic effort to utilize stem cells for orthopaedic tissue regeneration. Due to the abundance and easiness of harvest, adipose-derived mesenchymal celI(ASC) is an attractive, readily available aduh stem cell that has become increasingly popular for use in stem cell and tissue engineering applications. This review focuses on the use of ASC in orthopaedic tissue repair. Recent results from in vivo defect repair utilizing ASC suggested the great potential of ACS in clinical orthopaedic tissue regeneration.
2.Anti-integrin β1 monoclonal antibody P5D2 inhibits invasion of human cervical squamous carcinoma HCE1 multicellular spheroids into umbilical vein endo-thelium cells

Objective: To probe into the relationship between integrin β1 expression in human cervical carcinoma HCE1 multicellular spheroids (HCE1/MCS) and their invasion into human umbilical vein endothelium cells (HUVEC), so as to assess the inhibitory effect of anti-integrin β1 monoclonal antibody P5D2 on the invasion of HCE1/MCS into HUVEC. Methods: A model of HUVEC monolayer invaded by HCE1/MCS was established (in brief, the HCE1 invading model). Morphology changes of the HCE1 invading model were observed under inverted microscope and analyzed by Motic Med System after P5D2 treatment. The expressions of β1 integrin on HCE1 monolayer cells, HCE1/MCS, HVUEC monolayer cells, and P5D2-treated HCE1 invading models were measured by immunocytochemistry SABC assay. Results: A HCE1 invading model was successfully established. The HCE1/MCS proliferated rapidly after culture, and on the 7th day the invading area of HCE1/MCS was 40.42 folds larger than the original HCE1/MCS. Invading areas of P5D2-treated HCE1/MCS were significantly smaller than that of the control group after 1, 4, 7 day (P<0.05, or P<0.01), with the invading area after 7 days reduced by (84.68±0.08) % compared with the control group. Integrin β1 expression in HCE1/MCS was significantly higher than that in HCE1 monolayer cells (P<0.01), and the expression was negative in HUVEC. Integrin β1 expression in P5D2-treated HCE1/MCS was significantly lower than that the in untreated HCE1/MCS (P<0.01). Conclusion: Upregulated expression of integrin β1 in HCE1/MCS and HCE1 invading model may be associated with their enhanced adhesion and invasion abilities. Anti-integrin β1 monoclonal antibody P5D2 can partially block the invasion of HCE1/MCS into HUVEC.
3.The effects of gingival retraction in the restoration of subgingival non-carious cervical lesions

120 teeth with wedge-shaped defects at subgingival depth of 0 ~2 mm were selected and divided into 3 groups(n =40).Gingival retraction code and EXPASYL gingival retraction paste were used for gingival retraction in the 2 retraction groups,and none retraction was used in the control group.After restoration of the defects,all cases were followed up for 1 year and 2 years.The results were evaluated by modified USPHS criteria.No significant difference was detected for 1 year and 2 year successful rates between the extraction groups(P >0.05).The successful rate in extraction groups was higher than that in the control group(P <0.05).
4.Involvement of Caspase-dependent Mitochondrial Pathway in CORM-2 Induced Apoptosis in Neuronal Cells and Mechanism of Intervention Effect of Xingnaojing Injection

This study was aimed to investigate the mechanism of CO-releasing molecules (CORM-2) induced apoptosis and the intervention effect of Xingnaojing (XNJ) Injection in neuronal cells of rats. Optical microscope was applied to observe morphologic changes of neuronal cells. MTT assay was performed to assess the survival rates of CORM-2 on neuronal cells. Apoptosis was examined by flow cytometric analysis and the expression of relative proteins was measured by western blotting analysis. At the same time the morphologic changes, survival rates and expression of relative proteins of neuronal cells were also checked after XNJ treatment. The results showed that CORM-2 can influence survival rates in a time- and concentration-dependent manner. Survival rates decreased gradually after the cultures subjected to 24 h with 100 μmol·L-1, 200 μmol·L-1, 400 μmol·L-1 and 800 μmol·L-1 of CORM-2. It can induce neuronal apoptosis and activate Caspase-3, Caspase-9 and Cytochrome C in a concentration-dependent manner. The neuronal cells were treated with 200 μmol·L-1 of CORM-2 and then incubated with 10 mL·L-1 and 20 mL·L-1 XNJ injection for 20 h. It turned out early neuronal apoptosis decreased and the expression of Caspase-3, Caspase-9 and Cytochrome C also decreased. To sum up, CORM-2 may induce neuronal apoptosis through Caspase-dependent mitochondrial pathway, which can be intervened by XNJ Injection through inhibiting Caspase-dependent mitochondrial pathway.
5.Holmium laser percutaneous nephrolithotomy for treating upper ureteral callui: Report of 132 cases

Objective To explore the safety and efficiency of holmium laser percutaneous nephrolithotomy in the treatment of upper ureteral calcui.Methods A retrospective analysis was made on 132 cases of upper ureteral calcui treated by holmium laser percutaneous nephrolithotomy.After retrograde placement of the ureteral catheter,a track from the skin surface into the middle renal calyx was established under the guidance of a C-arm fluoroscopy.Then a ureteroscope was passed to introduce a holmium laser lithotriptor.Stones were broken up and a nephrostomy tube was placed in the incision to carry fluid from the kidney.Results Holmium laser percutaneous nephrolithotomy was completed on one session in all the 132 cases(155 sides),including 116 cases of single-tract nephrolithotomy and 16 cases of double-tract nephrolithotomy.The stone-free rate was 95.5%(148/155).The duration of operation was 30~320 min(mean,108 min).The intraoperative blood loss was 50~200 ml(mean,100 ml).No blood transfusion was required and no postoperative hemorrhage happened.No analgesics were needed after operation.The nephrostomy tube was left for 3~8 d(4.6 d).The length of postoperative hospital stay was 5~11 d(mean,7.4 d).Follow-up observations in the 132 cases for 1~6 months(mean,3 months) showed complete relief of symptoms.B-ultrasonography showed a mean of renal pelvis separation of 15 mm,which had decreased by 9~28 mm postoperatively.Conclusions Holmium laser percutaneous nephrolithotomy is safe and effective in the treatment of upper ureteral calcui.
6.Laparoscopic treatment of ureteropelvic junction obstruction

Objective To evaluate the clinical efficiency of laparoscopic operation for the treatment of ureteropelvic junction obstruction(UPJO).Methods A total of 19 patients with UPJO underwent laparoscopic dismembered pyeloplasty.The operation was performed under general anesthesia.The patients were maintained in a lateral decubitus on unaffected side.After the establishment of pneumoperitoneum,3~4 trocars were introduced at proper sites.During the operation,the lateral peritoneum was opened and the ureteropelvic junction obstruction was dissociated and dissected.The end-to-end anastomosis of the renal pelvis and ureter was conducted and a double-J stent was placed in the ureter.Results All the operations were performed successfully.The operation time was 110~240 min(mean,150 min),the blood loss was 50~100 ml(mean,80 ml),and the postoperative hospitalization time,6~10 d(mean,7.8 d).No severe complications occurred.Follow-up assessment with intravenous urography for 3~15 months(mean,6 months) in 14 patients showed no ureteral stricture.Conclusions Laparoscopic dismembered pyeloplasty is an effective and safe procedure in the treatment of UPJO.It can replace the open surgery.
7.Evaluation value of blood lactic acid dynamic monitoring and APACHE Ⅱ score in prognosis of sepsis

Objective To investigate the evaluation value of blood lactic acid dynamic monitoring and APACHE Ⅱ score in trhe prognosis of sepsis.Methods The blood lactic acid level,lactic acid clearance rate,shock and MODS occurrence rate and mortality in 76 patients with sepsis in ICU of our hospital from January to October 2015 were statistically analyzed.Various indexes and prognosis were performed the multi-variable Logistic regression analysis.Results The APACHEⅡ scores,shock and MODS occurrence rate and mortality in the lactic acid elevation group were higher than those in the lactic acid normal group,the difference was statistically significant(P<0.05).In grouping according to the lactic acid peak value,the observation indexes had no statistical difference among 3 groups(P>0.05);in grouping according to 6 h lactic acid clearance rate,except APACHEⅡscore,the other observation indexes in the high clearance rate group were significantly lower than those in the low clearance rate group,the difference was statistically significant (P<0.05).The lactic acid level at admission,lactic acid peak value,6 h lactic acid clearance rate and APACHEⅡscore in the survival group were significantly better than those in the death group,the difference was statistically significant(P<0.05).In 3 groups according to APACHEⅡ score,the lactic acid peak value,shock,MODS occurrence rate and mortality rate had statistically significant differences among 3 groups (P<0.05).The lactic acid level at admission,6 h lactic acid clearance rate and APACHEⅡscore were the independent risk factors in the patients with sepsis.Conclusion The lactic acid level at admission,6 h lactic acid clearance rate and APACHEⅡ score were the independent risk factors in the patients with sepsis.
8.Surgical treatment of the spine at the cervicothoracic junction through the trans-upper-sternal approach

[Objective]To explore the operative method through the trans-upper-sternal approach in the treatment of the cervicothoracic spinal lesions and evaluate its clinical effects.[Method]From August 1999 to February 2006,11 cervicothoracic patients,8 males and 3 females,age ranged from 17 to 77 years with a mean of 41.5 years underwent the trans-uppersternal approach surgical treatment.There were four traumatic lesions,six tumors and one tuberculosis.The lesions were located at T3,C7~T1 and T1、2 in 1 case respectively,C7 and C6~T1 in 2 cases respectively,T1 in 4 cases.The combined cervicothoracic incision and upper sternotomy were performed for the exposure of the vertebral bodies,then tumor or vertebral body was resected,the spinal cord was decompressed,the spinal column was reconstructed and fixed.Neurologic status was assessed using the Frankel classification.[Result]The duration of follow-up ranged from 10 to 56 months with an average of 31 months.One patient developed chyle leakage of 50 ml one day after surgery and the leakage stopped 2 days after continuous drainage.One patient had transient vocal cord paresis which recovered in 3 months.All patients had their neurological improvement at different level.Nonunion or instrument-related complications were not observed,and the vertebral column had good stability.[Conclusion]The trans-upper-sternal approach gives an excellent exposure of the cervicothoracic junction.It is a technically simple,safe and effective method for anterior decompression,maintenance of anatomic alignment,fusion with bone graft and internal fixation with less complications and trauma.Attention should be paid to avoid injury of the recurrent laryngeal nerve and the thoracic duct.
9.Oncocytic papilloma in nasal cavity and paranasal sinuses: 3 of cases.

Oncocytic papilloma (OP) of nasal cavity and paranasal sinuses are uncommon. We report 3 cases of oncocytic papilloma arising in the nasal cavity and paranasal sinuses, and review the relevant literatures. Unilateral sinonasal lesions are the main clinical manifestation. Pathological feature is multiple layers of epithelial cells. The cytoplasm contains abundant eosinophilic granular and microcapsules filling with mucus. The neutrophils aggregate in the microcapsules which form microabscess. The tendence of recurrence and malignant transformation are the biological character. The best treatment choice is surgical excision completely, and long-term postoperative follow-up is necessary to prevent relapse.

10.Application of cotton rolls as protective sleeves of endodontic files

The effects of cotton roll and the traditional gauze wrapper in the protection of endodontic files were observed.Cotton roll showed firm protection and feasible operation advantages,it was more effective than gauze wrapper in the protection of endodontic files(P<0.01).
