1.A Comparative Study of Mini-invasive Percutaneous Nephrolithotomy and Ureteroscopic Lithotripsy Combined with Occlusion Devices for the Treatment of Upper Incarcerated Ureteral Stones

Objective To compare the efficacy of ureteroscopic lithotripsy ( URL) combined with occlusion devices and mini-invasive percutaneous nephrolithotomy ( MPCNL) in the treatment of upper incarcerated ureteral stones. Methods A total of 89 patients with unilateral upper ureteral calculi were enrolled from April 2011 to July 2013, 42 of which were treated with MPCNL and 47 of which were treated by rigid ureteroscopic lithotripsy combined with occlusion devices. Resuts There were no significant differences in the success rate of primary lithotripsy (97.6% vs.95.7%, χ2 =0.000, P=1.000).No marked differences were detected in the stone clearance rate (97.5% vs.95.6%, χ2 =0.000, P=1.000), complication rate (7.1% vs.8.5%, χ2 =0.000, P=1.000) and operation time [(80.2 ±11.9) min vs.(65.1 ±10.9) min, t =6.248, P=0.000] between the two groups.The hospitalization time in the URL group was (3.5 ±1.5) d, which was obviously shorter than that in the MPCNL group , in which it was (7.3 ±1.5) d(t=11.931, P=0.000). Conclusion URL combined with occlusion devices can obtain satisfactory results as well as MPCNL in the treatment of upper ureteral calculi by proper case selection and careful operation performance .
2.Influence of Shenmai injection on immune function in the treatment of locally advanced non-small cell lung cancer patients with concurrent chemoradiotherapy

Objective To observe the effect of Shenmai injection combined concurrent chemoradiotherapy on immune function in patients with locally advanced non-small cell lung cancer.Methods A total of 80 patients with locally advanced non-small cell lung cancer were randomly divided into treatment group and control group.In treatment group,40 patients received simultaneous three-dimensional conformal radiotherapy combined with TP(docetaxel+cisplatin)chemotherapy,while given Shenmai injection in the process of concurrent chemoradiotherapy.In control group,40 patients only received chemoradiotherapy.The changes of T cell subsets were observed between the two groups.Results It showed no significant differences in percentages of CD+3,CD+4,CD+16CD+56and CD+4/CD+8 ratio between two groups pre-treatment(all P>0.05).There were no significant adverse reactions occurred after received Shenmai injection in treatment group.Conclusion It play an important role in improving immune function of patients with locally advanced non-small cell lung cancer who received Shenmai injection combined concurrent chemoradiotherapy.
3.Peri-operative nursing of minimally invasive surgery on osteoradionecrosis of skull base after radiotherapy for nasopharyngeal carcinoma

Objective This paper summarize peri-operative nursing of minimally invasive surgery on osteoradionecrosis of skull base after radiotherapy for nasopharyngeal carcinoma.Methods 47 cases of osteoradionecrosis of skull base after radiotherapy for nasopharyngeal carcinoma were operated via nasal endoscope from March 2006 to March 2013.Focused peri-operative psychological support and adequate medical care were given to patients.Strengthened nasal nasopharyngeal cavity nursing,the observation and nursing of complications were carried out after operation and health guidance pre-discharge was given.Results The operations of 47 cases were successful and no major complication of intra-operation and post-operation occurred.Headache and foul odor relieved after operation.The follow-up continued for 3 to 84 months,46 cases were survived,1 case died of nasopharyngeal massive bleeding.Conclusions Effective peri-operative nursing of minimally invasive surgery on osteoradionecrosis of skull base after radiotherapy could ensure patients' cooperation and the smooth implementation of surgery and improve the quality of life.
4.Nursing care in reservation of the catheters in eleven nasopharyngeal carcinoma patients with symptomatic thrombosis after implantation of double-lumen PICCs

Objective To summarize the key points of nursing care in reservation of catheters in 11 cases of nasopharyngeal carcinoma patients who received concurrent chemo-radiotherapy with symptomatic thrombosis after ultrasound-guided implantation of double-lumen PICCs(PowerPICC).Methods From January,2014 to December,2015,totally 11 cases with symptomatic venous thromboembolism were identified among 109 cases of nasopharyngeal carcinoma patients receiving concurrent radiotherapy and chemotherapy,and observation and nursing care were provided at the early stage of thrombosis and during thrombosis.Results All double-lumen PICCs were reserved,and no recurrence or aggravation of thrombosis was recorded to the end of the treatment.The duration of carrying PICCs was 67~89(77.45±6.65) days.Conclusion With careful treatment and nursing,PICC catheter-related complications can be reduced and the duration of carrying catheters can be prolonged,which leads to accomplishment of the treatment plan for patients.
5.Determination of specific IgG4 for diagnosis and therapeutic evaluation of cerebral cysticercosis.

OBJECTIVETo probe the significance of specific IgG4 in sera of patients with cerebral cysticercosis for diagnosis and therapeutic evaluation.
METHODSSpecific IgG4 in sera of patients with cerebral cysticercosis was assessed using colloidal gold-labeled mouse-anti-human IgG4 McAb as probe. The results were compared with the CT image manifestation.
RESULTSThe specific IgG4 positive rate in sera of patients with cerebral cysticercosis was 97.8%, whereas sera from patients with other kinds of parasitosis or central nerve system disease and the control group were all negative, except for a weak cross-reaction of sera from patients with hepatic echinococoosis. The determination of specific IgG4 in sera of patients with cerebral cysticercosis during different times of treatment showed that along with an increase in treatment time and improvement of clinical symptoms, specific IgG4 level gradually decreased. The positive rate and intensity of specific IgG4 in sera from patients with cerebral cysticercosis were consistent with the number of cysticercus parasites in the brain and pathologic changes, such as survival, disintegration, death and calcification. Survival of cysticercus in the brain was objectively evaluated using this technique.
CONCLUSIONSThe determination of specific IgG4 in sera is a practical method for diagnosis and therapeutic evaluation of cerebral cysticercosis.

6.Deep femoral artery third perforating flap for repair tissue defected of arrounding Pilon fracture in I stage

Objective:To investigate the clinical effect of free deep femoral artery third perforating flap repaired soft tissue loss after Pilon fracture surgery in I stage.Methods:Fifteen patients were treated from April, 2013 to January, 2020. Miller AO classification: 8 cases 43-C1, 4 cases 43-C2 and 3 cases 43-C3. All cases were accompanied with severe soft tissue contusion and skin necrosis. After fracture reduction, soft tissue defects, internal fixation exposure and tendon exposure around the wound. Free deep femoral artery third perforating flap (3.5 cm ×15.5 cm to 5.5 cm×12.5 cm) for the repair of soft tissue defects around ankle in the I stage, the blood vessels of the flap were end-to-side anastomosed with vessels of the posterior tibial or anterior tibial. Regular follow-up after surgery.Results:One case of venous crisis occurred, other 14 cases survived, were followed-up from 5 to 18 months, the ankle joint function was good, did not affect the foot shoes, with excellent color and texture, the flap restored protective sensation, and leaving only linear scar, no muscle adhesion.Conclusion:Free deep femoral artery third perforating flap repaired soft tissue loss of surgical incision after fracture operated than significantly reduce the postoperative fracture infection and protect the blood supply around the fracture. It is an effective method of repair.
