1.Role of short-term follow-up MRI in the detection of postoperative breast residual cancer

Objective To evaluate the role of short-term follow-up MRI in the detection of postoperative breast residual cancer. Methods A retrospective analysis was performed on 13 patients who were diagnosed as nonmalignant breast lesions by preoperative clinical and ultrasound and mammography examinations and intraoperative frozen pathology.However,these patients were finally confirmed as malignant breast lesions by paraffin pathology and received corresponding second operations.Routine MRI,DCE-MRI and EPI-DWI scan were performed on the 13 patients within one month after the first operation and these MRI features and patholo-gy were comparatively analyzed.Results All the cases showed local mammary architecture distortion both in routine MRI and DCE-MRI.The enhancement characteristics of the 13 cases were as follows:3 cases of stippled enhancement,4 cases of small nodular en-hancement,1 dendritic enhancement,1 network enhancement,1 ring-like enhancement of cystic wall and 3 cases of no abnormal en-hancement.The lesions of 7 cases showed type Ⅰ curve (progressive enhancement pattern)and 6 cases showed type Ⅱ curve (plat-eau pattern).The lesions of 6 cases showed decreased ADC value.In summary,there were 6 cases of tumor residue diagnosed by both MRI and the second pathology,while only 1 case of residual neuroendocrine carcinoma was misdiagnosed by MRI but confirmed by the second pathology.Conclusion Short-term follow-up MRI could be of value in determining postoperative breast residual tumor, and could be helpful for surgeons to make accurate operation plan.
2.The Value of MR Diffusion-Weighted Imaging in Diagnosing the Benign and Malignant Breast Lesions

Objective To evaluate the value of diffusion-weighted imaging in distinguishing the benign and malignant breast lesions.Methods fifty-six female patients with pathologically proven breast lesions underwent MR diffusion-weighted imaging(MR-DWI)using 2 b values.The mean apparent diffusion coefficient(ADC)values and the relative ADC(rADC)values of lesions were measured and analyzed statistically by SPSS 13.0.Results(1)The mean ADC values of the malignant lesions and the benign lesions were(1.02?0.18)?10-3mm2/s and(1.61?0.32)?10-3mm2/s with b=500 s/mm2,(0.99?0.16)?10-3mm2/s and(1.59?0.33)?10-3mm2/s with b =1000 s/mm2,respectively.The mean ADC values of breast malignant lesions were lower significantly than that of the benign,and had statistical significance(P =0.001
3.Mammographic and CT Diagnosis of Lipoma in the Breast

Objective To evaluate the value of mammography and CT in diagnosing lipoma of the breast. Methods X-ray mammographic and CT manifestations of lipoma in the breast confirmed by operation and pathology mainly in 8 cases were analyzed retrospectively. Results Lipomas were in the right breast in 6 cases and in the left breast in 2 cases.Lipomas were in shallow layers of breast in 3 cases,interglandular body in 2 cases and the posterior breast in 3 cases.All masses were radiolucency with well-defined edge.The calcification in lipoma could be observed in 1 case.The diameter of lesions ranged from 2 cm to 4.5 cm.Lipoma of the breast showed rounded or ege-shaped in 7 cases,lobulated in 1 case.Conclusion Lipoma of the breast can be identified by X-ray mammography and CT.

Objective To explore the instruction value of the color Doppler ultrasonography for the diagnosis and treatment of the pediatric intussusceptions .Methods Retrospective study the color Doppler ultrasonographic performance of 114 patients with diagnosis of pediatric intussusception in the affiliated hospital of Zunyi medical college ,from October 2009 to June 2012 ,reviewed the case ,groups were compared according to the ways of therapeutic methods .Results All the 114 cases of children of the intus-susception are subject to air enema reset or surgery cured confirmed .Age younger than 12 months intussusception children ,air ene-ma reset were more likely to success ;the diameter of concentric circles levy,the length of sleeve for and the thickness of bowel wall were smaller ,the more suitable for outsiders air enema reset .The blood supply of scabbard of bowel wall and the mesenteric was rich ,the air enema reset had a higher success rate ,blood supply was poor and the success rate was lower .The success rate of air enema reset was influenced by the part of intussusception .Conclusion Combined use of conventional color ultrasonic and high fre-quency probe checking ,can raise the diagnosis rate of pediatric intussusception ,the primary lesion confirmed and the detection rate in the position of the disease ,provide guidance for clinical choice reduction treatment .
5.Transvaginal color Doppler ultrasound of patients with unexplained hypomenorrhe

Objective To observe the relationship between morphological and hemodynamic changes of female internal genital organs in the patients with hypomenorrhea with transvaginal color Doppler ultrasound (TVCDS). Methods Thirty female patients with hypomenorrhea of unknown origin were studied, and 30 healthy women of eumenorrhea aged 20 to 40 years were selected as control group. The size and shape of uterus, ovaries, and the thickness of endometria of uterus were observed with TVCDS in follicular phase, ovulation phase, luteal phase and luteal atrophy phase, respectively. Hemodynamic parameters of ovarian artery, uterus artery and their branches were measured. At the same time, hormones of female were examined in 30 patients in follicular phase, ovulation phase and luteal atrophy phase, respectively. Results ①No difference of the size of uterus and ovaries, the thickness of endometria and the amount of egg follicles was found between two groups (P>0.05). ②Compared with that in the control group, type C of endometria increased in hypomenorrhea group (P<0.05). ③RI, PI and S/D of spiral arteries in hypomenorrhea group were higher than those in control group, but no significance in these parameters of uterine arteries, arcuate arteries and radiate arteries was found. RI of uterine arteries, arcuate arteries and radiate arteries decreased successively. ④RI, PI and S/D of ovarian arteries in hypomenorrhea group was higher than those in control group in ovulation phase (P<0.05). Conclusion Reduction in blood supply of ovary and endometria may be the causes of hypomenorrhea with unknown origin. TVCDS can be used as a conventional examination method for these patients.
6.Effects of PDCA method on improving the accuracy of the Autar Deep Venous Thrombosis Scale

Objective:To investigate the effects of PDCA method on improving the accuracy of the Autar Deep Venous Thrombosis (DVT) Scale used by nurses to assess the risk of deep venous thrombosis in surgical patients.Methods:Applied the way of PDCA, namely, raising questions, analyzing reasons, implementing measures, feeding back effects to professional explanate the difficulty in using the scale, moreover, training the nurses used level education method between August 2017 and December2018.The accuracy and consistency of the scale were compared before and after interventions.Results:A total 396 patients were evaluated in the study. There were significant differences in the consistency of evaluation among hospitalized, post-operative and discharged patients after intervention compared with before intervention ( P< 0.0167).The consistency of assessment of team members and responsible nurses increased from 68 to 120 cases, and the consistency of assessment reached 90.91%. The Kappa consistency of assessment of responsible nurses and team members increased from 0.354 before intervention to 0.879 after intervention. The effect was remarkable. Conclusions:PDCA cycle method is based on the problems found in clinical practice. It clarifies the professional terms and rules used in the scale, improves the accuracy and consistency of the Autar DVT scale used by nurses, correctly reflects the risk of thrombosis, ensures the safety of patients, and is worthy of promotion and reference.

9. An investigation of an accident of acute mixed gas poisoning

An accident of mixed acute gas poisoning accident happened in a place in GuangDong in March 2018. To investigates three poisoning workers and related clinical data were summarized., we tested the field air and analyzed the accident reasons. This event due to the staff lack of occupational protection awareness and illegal operation. The working environment must be ventilated before limited space operation, and must be sure that the limited space is safe by toxic gas monitoring. In case of occupational acute gas poisoning, rescuers should help the persons who are poisoned reasonably and meanwhile their own safety.
