OBJECTIVE:To optimize the extraction of effective ingredients and purification for crude glycyrrhizic acid and liquiritin from Radix et Rhizoma Glycyrrhizae with different methods. METHODS:The contents and extraction percentages of glycyrrhizic acid and liquiritin were used as indexes to optimize the extraction solvent. Then the extraction process was optimized with orthogonal experiment taking the concentration of ammonium ethanol,the amount of solvent and the extracting times as indexes. Crude glycyrrhizic acid was purified with the method of recrystallization and crude liquiritin was purified with organic solvent method.RESULTS:The optimal extracting solvent was ammonium ethanol. The optimum extraction conditions were as follows:extracting the solvent 4 times(2 h/time) by adding 5 times volume of 60% ethanol(containing 0.3% ammonia water). The purity of glycyrrhizic acid was over 85% with total extraction rate at 43%,and content of liquiritin was above 15% with its total extraction rate at 67%. CONCLUSION:The method is simple and feasible and from which glycyrrhizic acid and liquiritin was been obtained,Radix et Rhizoma Glycyrrhizae can broaden the scope of the application.

Objective To study the expression of retinoblastoma(Rb) protein in patients with breast cancer and its significance.Methods The expressions of Rb protein were detected in 52 patients with breast cancer,20 patients with breast fibroadenoma or breast cystic hyperplasia(control group) by S-P immunohistochemical method.(Results The negative) rate of Rb expression in breast cancer(76.91%) was higher than that in control group(15.00%)(P0.05).Of all 46 follow-up patients with breast cancer,the peritoneal recurrence and metastatic rate was 10.53% in 38 cases of the negative expression of Rb protein,and while the rate was 0 in 8 cases of the positive expression of Rb protein,there was significant difference(P

OBJECTIVE: To optimize the preparation technology of phloroglucinol phospholipids complex and study its physicochemical properties.METHODS: The preparation technology for phloroglucinol phospholipids complex was optimized taking the molar ratio of phloroglucinol to phospholipids(A),reaction temperature(B) and reaction time(C) as factors and complex ratio of phloroglucinol and phospholipid as index.The dissolubility and oil/water partition coefficient of the complex in different solvents were determined and compared with crude drug,and the new complex's physicochemical properties were validated by UV and IR(infrared spectra).RESULTS: The optimized preparation conditions for phloroglucinol phospholipids complex were obtained as follows: A was 1∶2;B was 60 ℃ and C was 2 h.The complex ratio remained at above 98%.Compared with phloroglucinol crude drug,the phloroglucinol phospholipids complex in water or chloroform showed increase in both dissolubility and oil/water partition coefficient.UV and IR confirmed that no new complex had been formed from phloroglucinol and phospholipids.CONCLUSION: The optimized technology greatly enhanced the dissolubility and lipotropy of the phloroglucinol phospholipids complex.

Objective To evaluate the distribution of Schmorl's nodes (SN) in patients with low back pain or radiculopathy,and to analyze the correlation between SN and degeneration of lumbar intervertebral discs.Methods In 2012,1024 patients with low back pain or sciatica were examined by magnetic resonance (MR) scan and plain film.There were 448 males and 576 females,with an average age of (54.3 ± 12.7) years (range,22-90).The features distribution of SN in lumbar endplate on age,sex,segment and the type and grade of intervertebral disc degeneration were analyzed retrospectively.Results Among 5120 lumbar intervertebral segments of the 1024 patients,295 (28.8％) cases and 532 (5.2％) endplates were involved with SN,302 located in the cranial and 230 in the caudal endplate.According to percent prevalence per lumbar segment,L1,2 was the most common level (29.1％),followed by L3,4 (23.7％) and L2,3 (21.1％).The incidence of SN was positively correlated with elder age,but not with sex,body weight,height,or body mass index.SN occurred more often in bulging,extrusion,Modic changes,spondylolisthesis and osteophyte comparing with normal disc or protrusion or high intensity zone.The distributions of the grade of intervertebral disc degeneration were significantly different between groups with and without adjacent SN.Intervertebral discs with adjacent SN were more degenerated than those without adjacent SN.The number,size and volume of SN were associated with the degrade of intervertebral discs degeneration.SN was divided into two types,i.e.,acute edematous SN and non-acuteSN,according to the signal type on T1-and T2-weighted MR images,and the degeneration of the corresponding intervertebral discs was more severe in the latter than the former group.Conclusion SN occurred more often in the upper lumbar spine and cranial endplate in patients with low back pain or radiculopathy.SN were correlated with elder age and the degeneration of lumbar intervertebral discs.The number,size,volume and signal type on MR images of SN impacted on its correlation with lumbar intervertebral disc degeneration.

OBJECTIVE To investigate the distribution and the drug resistance of pathogens in cerebral stroke patients with hospital infction and to provide a foundation for reasonabie application of antibacterials.METHODS Pathogenic bacteria were isolated from inpatients of burn department from 2003 to 2008,instruments assay or K-B disk diffusion method was taken to carry out the sensitive test.The drug resistance rate of the pathogenic bacteria was analyzed.RESULTS Totally 222 strains were isolated,among which 122 strains were Gram-negative bacteria,58 strains were Gram-positive ones,and 42 strains were fungi.The percentage of these three groups was 54.95%,26.13% and 18.92%,respectively.The main strains of Gram-negative bacteria were Pseudomonas aeruginosa,Escherichia coli,and Enterobacter cloacae,and that of the Gram-positive bacteria were Staphylococcus aureus,Enterococcus and S.epidermidis.The major strain of fungi was Candida albicans.The pathogenic bacteria tested showed high drug resistance.The detection rate of the meticillin-resistant S.aureus(MRSA)was 73.68%.The detection rate of the ESBLs from the K.pneumoniae and the E.coli was 60.00% and 33.33 %.CONCLUSIONS The pathogenic bacteria are multidrug-resistant.Inspecting pathogens and strengthening susceptibility tests are very important in reducing drug abuse,decreasing the resistance rate and raising the cure rate in hospital.

AIM: To investigate the cytological basis and differentiating conditions of human bone marrow mesenchymal stem cells (hMSCs) differentiated into cells of the endothelial lineage in vitro. METHODS: hMSCs were isolated by density gradient centrifugation and fractionated on a 1 073 g/L Percoll. The combination of VEGF165 and various matrix proteins including fibronectin (FN) and type I collagen (Col) was used to induce hMSCs in vitro. Cells were characterized by immunohistochemistry, cytochemistry, FACS and ultrastructure to identify and detect the differentiated population and markers. RESULTS: hMSCs was positive for KDR. PAS reaction was positive and ultrastructure of hMSCs showed glycogen- pool in ectoplasm. Glycogen reducing or disappear suggested that stem cells have occurred differentiation. Induction of hMSCs resulted in the increase of KDR, β1 integrin and CD34. CONCLUSION: hMSCs were induced to a transit population (TP( )) that differentiated toward the endothelial progenitor cells ( EPC), but not a really EPC. hMSCs pedigree diagram of differentiation was hMSCs→TP→EPC→endothelial cells (Ecs).

Objective To observe the curative effect and safety of operation combined with recombinant human erythropoietin on patients with severe intracerebral hemorrhage. Methods Seventy-six surgery patients with severe intracerebral hemorrhage were divided into rHu-EPO group (40 cases) and control group (36 cases) by random digits table method. The rHu-EPO group was injected subcutaneously with rHu-EPO, and the control group was treated with placebo. Neurologic impairment (National Institute of Health Stroke Scale, NIHSS score) and activities of daily living (Barthel index) were evaluated 1 month and 3 months after treatment respectively. Moreover, blood pressure, hemoglobin, and adverse reaction were also observed. Results The scores of NIHSS and Barthel index in two groups before treatment had no significant differences (P>0.05). One month and 3 months after treatment, the scores of NIHSS and Barthel index in rHu-EPO group were significantly better: (12.27±5.26) scores vs. (15.36±4.34) scores and (8.17±2.40) scores vs. (13.90±2.54) scores, (54.36±21.87) scores vs. (43.47±20.29) scores and (69.71±23.08) scores vs. (52.56±21.32) scores, there were statistical differences(P<0.05). There were no statistical differences in systemic blood pressure, diastolic blood pressure and hemoglobin between 2 groups (P>0.05). There were no apparente adverse reactions such as fever, erythra, itching and deep venous thrombosis in rHu-EPO group. Conclusions Operation combined with recombinant human erythropoietin has nerve protective effect, and might be an effective and safe therapy target in severe intracerebral hemorrhage.

Objective To investigate the CT and MRI features and morphology classification of intraductal papillary mucinous neoplasm of the bile duct (IPMN-B).Methods A total of 18 patients with IPMN-B proved by pathology were retrospectively analyzed.Out of 18 patients,16 patients underwent enhanced and non-enhanced CT,13 underwent contrast enhanced MR,and 11 out of 13 underwent both CT and MRI.IPMN-B was classified into 4 types:typical IPMN-B,cystic-forming IPMN-B,non-tumor IPMN-B and invasive IPMN-B,according to imaging findings and gross pathological findings.Results Typical IPMN-B (9 cases):tumors were distributed along the bile ducts,both upstream and downstream bile ducts were obviously dilated.Cystic-forming IPMN-B (5 cases):single or multiple tumors were found in aneurysmal dilatation of bile ducts.Non-tumor 1PMN-B (2 cases):no mass was found in the widely dilated bile ducts with smooth bile duct wall.Invasive IPMN-B (2 cases):tumors protruded into the dilated bile ducts causing jagged wall of bile duct,with accompanied abnormal density or signal intensity outside the bile ducts.Bile duct dilatations were shown in all 18 cases,and tumors were shown in 16 cases.In 2 cases no mass was displayed in widely dilated bile ducts.CT density of the tumor was lower than that of liver parenchyma,and higher than that of the bile and intraductal mucin.Signal intensity of the tumor was higher than that of stones,and lower than that of bile and intraductal mucin at MR T2WI.All tumors showed high intensity on DWI.Tumors showed mild to moderate enhancement after injection of contrast agent,CT density or signal intensity of the tumors were lower than that of the liver parenchyma during all three phases of contrast-enhanced CT or MRI.Conclusion IPMN-B has some specific CT and MR imaging features,which are helpful for the diagnosis and classification of IPMN-B.

Objective To establish WeChat official account-based platform for evidence dissemination,and to evaluate the effects of the platform.Methods The WeChat official account-based platform for evidence dissemination was established,and big data analysis and sampling survey were adopted to analyze information and its communication effects published from December,2014 to September,2016.Results Totally 22 369 followers used the platform and conducted 404 232 hits on its pages in total.The most frequent searches were Evidence and Knowledge of evidence-based nursing.The WeChat Communication Index was 433.07.The overall evaluation score was 4.34± 0.67.Conclusion WeChat official account-based platform for evidence dissemination can promote the accessibility of evidence and receives high evaluation score from followers.
