1.Beneficial effects of position on patent foramen ovale-induced right-to-left shunt by contrast transcranial Doppler ultrasonography

Objective To investigate the influence on right-to-left shunt (RLS) of patent foramen ovale with contrast transcranial Dopple (cTCD) in different positions.Methods 276 consecutive patients included ischemic cerebrovascular disease (ICVD),migraine,episodic vertigo and syncope.They underwent cTCD at rest and after Valsalva maneuver in 4 positions:upright sitting,supine,left lateral decubitus and right lateral decubitus,in random order.RLS was graded 0 (no microbubbles detected),1 (1-10microbubbles),2 (> 10 microbubbles but no curtain),and 3 (curtain,shower of microbubbles).Meanwhile,each patients was examined by contrast transthoracic echocardiography(cTTE),contrast transesophageal echocardiography (cTEE) or right heart catheterization.Results RLS detection rate was 48.2% (133/276)through cTCD.They were also confirmed patent foramen ovale by 82 cases in cTTE,28 cases in cTEE,and 23 patients in right heart catheterization.At rest,RLS detection rate and microbubbles of 4 positions had no significant difference (P >0.05).After the Valsalva maneuver,RLS detection rate and microbubbles of each position were significantly higher than at rest (P <0.001),and the RLS detection rate and microbubbles in upright sitting were higher than that at rest(P <0.05).Conclusions RLS is best detected in the upright sitting position with Valsalva maneuver with cTCD.Upright sitting detection helps to improve the detection rate and microbubbles of RLS.
2.Association of MYH9 SNPs with end-stage renal disease in Chinese Han popula-tion

Objective:To explore the associations of MYH9 gene polymorphisms with ESRD in Han population in the fragment between exon 23 and 24.Methods:A hospital-based case control study was carried out including 180 patients and 118 controls in this study.Single nucleotide polymorphisms of MYH9 gene were determined using PCR sequencing,and the haplotypes were calculated using phase software(version 2.0),and transcription factor binding sites were predicted using AliBaba2.Univariate analysis was conducted for exploring the associations between polymorphisms and ESRD.Results: Five newly discovered and three previously reported SNP loci [Rs4821480(MYH9-92),Rs2032487(MYH9-273) and Rs4821481(MYH9-787)]were homozygote genotyped by bidirectional se-quencing.Among newly discovered polymorphisms,two were found at the 489 locus(G→A)and the 616 locus(A→C) in the 901 bp fragment which located in the intron 23 of MYH9 gene.A G489A transversion was very likely a risk mutation contribute to the occurrence of ESRD(P=0.013).No association was observed between ESRD and three previous reported sites [Rs4821480(MYH9-92),Rs2032487(MYH9-273)and Rs4821481(MYH9-787)].The most common haplotype was TCTCGGAT,which was less frequent in the cases than that in the controls.Moreover, TCTCGGCT and TCTCAGAT haplotypes were more in the cases than that in the controls.The number of transcription binding sites increased from 82 ( wild ) to 85 ( mutation ) in the 23th intron of MYH9 gene.Conclusion:Polymorphisms of MYH9 at intron 23 may influence the prevalence of ESRD in Chinese Han population and TCTCGGAT haplotype may be one protective haplotype.TCTCGGCT and TCTCAGAT may be risk haplotypes attributed to ESRD.The polymorphism of MYH9 at the 23th intron may company with the amount alteration of transcription factor binding sites.
3.Effect of Yiqi Huayu Jiedu Prescription on the Growth of HepG2 Nude Mice Transplantation Tumor and the Expression of Related Factors of Vascular Mimicry

Objective To observe the influencing of Yiqi Huayu Jiedu Prescription on the growth of HepG2 nude mice transplantation tumor and the expression of related factors of vascular mimicry. Methods Models of transplanted tumors, which were made by HepG2 cells in nude mice, were randomly divided into 7 groups, Yiqi Huayu Jiedu Prescription group, Astragali Radix group, Curcumae Rhizoma group, Paridis Rhizoma group, Gecko group, cis-platinum group, and model group. Except for the model group, the rest groups were given relevant medicine for intervention. 21 days later. HIF-1α, MMP-2, MMP-9, and E-cad were detected by immunohistochemistry, and Twist1 and Bcl-2 were detected by fluorescence quantitative PCR. Results Compared with the model group, tumor volume in the rest groups decreased (P<0.05), and the effect in the Yiqi Huayu Jiedu Prescription group was more obvious than the Astragali Radix group, Paridis Rhizoma group and Gecko group (P<0.05);The expression of vasculogenic mimicry structure was rare in each group, and the model group and cis-platinum group were the most obvious;Except for the Astragali Radix group, the expressions of HIF-1α, MMP-2, and MMP-9 showed statistical significance compared with model group (P<0.05);The expression of E-cad in the Yiqi Huayu Jiedu Prescription group and Astragali Radix group showed statistical significance (P<0.05);The expression of Bcl-2 in the Yiqi Huayu Jiedu prescription group, Paridis Rhizoma group, and Gecko group decreased significantly compared with the model group (P<0.05);The expression of Bcl-2 in the Yiqi Huayu Jiedu prescription group was much better than the other groups (P<0.05);The expression of Twist1 showed statistical significance in the Yiqi Huayu Jiedu Prescription group, Curcumae Rhizoma group, Paridis Rhizoma group, and cis-platinum group (P<0.05). Conclusion Yiqi Huayu Jiedu Prescription can reduce expression of HIF-1α, Twist1, Bcl-2, MMP-2, and MMP-9, and increase expression of E-cad, thereby inhibiting the formation of vascular mimicry.
4.Application of a Q Method Study to Understanding Nurses' Perspective of Adopting Evidence-Based Nursing.

PURPOSE: This study applied the Q method to identify and describe the various types of nurse perceptions that are crucially associated with their engagement in evidence-based nursing (EBN). METHODS: The study participants were nurses at a medical center and a regional teaching hospital. A series of Q sorts was performed by nurses to subjectively rank the Q statements. Q statements were constructed based on the literature related to EBN adoption by nurses and face-to-face interviews. RESULTS: A total of 60 participants were invited to rank 44 Q statements related to EBN. Factor analysis was conducted on the rankings of the Q statements. The following are the five prominent shared perspectives: (1) emphasized the obstacles to evidence searching and reading ability; (2) emphasized the organizational promotive strategies; (3) emphasized the available supportive resources; (4) emphasized the significance of EBN; and (5) emphasized the evidence-searching ability and external incentives. The five identified groups of perspectives can enhance hospital administrators to acknowledge the barriers and incentives associated with EBN practices. CONCLUSION: The exploration of clustering nurses' perceptions may facilitate the development of customized strategies to enable more appropriate training.

5.Investigation on quality of life and its influencing factors in senior women with urinary incontinence

Objective:To investigate the quality of life of senior women with urinary incontinence and to explore its influencing factors.Methods:A convenient sampling method was used to select 152 senior women with urinary incontinence in Fuzhou Hot-Spring Street Community, Dongjie Street Community and Old-age Care Institutions from June 2017 to September 2017. The quality of life of elderly women with urinary incontinence were observed and the influencing factors were analyzed via International Consultation on Incontinence Questionnaire Short Form (ICI-Q-SF) and Incontinence Quality of Life Scale (I-QOL).Results:The quality of life of senior women with urinary incontinence was (98.7±12.2) points. The scores of the three dimensions including restrictive behavior, social activity limitation and psychological impact were 34.5±4.4 and 41.4±5.4 and 20.8 ± 3.5, respectively. There were 4 factors that influencing the quality of life of patients, namely, the severity of urinary incontinence, the type of urinary incontinence, diabetes and the frequency of leaking urine ( P<0.01). Conclusions:Urinary incontinence has a certain negative impact on the quality of life of patients, and its influencing factors are complex. Individualized care measures should be taken based on the influencing factors to improve the quality of life of patients.
6.Effects of nurse-led stress inoculation training on fear of self-injecting and self-testing in elderly type 2 diabetic patients

Objective:To investigate the effect of nurse-led stress inoculation training on fear of self-injecting and self-testing and self-management behaviors in elderly type 2 diabetic patients and provide reference for diabetes nursing care.Methods:A total of 110 elderly type 2 diabetic patients of Department of Endocrinology of Hainan People′s Hospital from January 2018 to January 2020 were divided into experimental group and control group according to odd and even numbers, with 55 patients in each group. The control group received routine nursing care, while the experimental group implemented nurse-led stress inoculation training for 4 weeks. The intervention effect was assessed by Diabetes Fear of Injecting and Self-testing Ouestionnaire (D-FISQ) and Diabetes self-management behaviors among older (DSMB-O), respectively.Results:In the study, one patient in the experimental group fell off, and finally included 54 cases in the experimental group and 55 cases in the control group. After intervention, the fear of self-injecting scores, fear of self-testing scores, and total D-FISQ scores were 13.15 ± 3.02, 15.67 ± 3.59 and 28.81 ± 5.08 in the experimental group, significantly lower than those in the control group (15.25 ± 3.18, 17.56 ± 3.92 and 32.82 ± 4.89), the difference was statistically significant ( t=3.55, 2.63, 4.19, P<0.05). Active exercises, current medication, blood glucose monitoring, dealing with problem, active response, reducing risks scores and total DSMB-O scores were 2.39 ± 0.49, 2.39 ± 0.49, 2.20 ± 0.81, 4.41 ± 0.92, 4.70 ± 1.13, 5.06 ± 0.79 and 25.28 ± 2.57 in the experimental group, significantly higher than those in the control group (3.95 ± 0.85, 2.11 ± 0.85, 1.51 ± 0.50, 3.95 ± 0.78, 4.13 ± 1.43, 4.38 ± 1.16 and 22.09 ± 2.24), the difference was statistically significant ( t values were 2.10-6.90, P<0.05). Conclusions:Nurse-led stress inoculation training can effectively alleviate fear of self-injecting and self-testing and promote self-management behaviors of elderly patients with type 2 diabetes.
7.The effect of aerobic combined stretching exercise training on restless legs syndrome, quality of life in hemodialysis patients

Objective:To investigate the effects of aerobic combined stretching exercise during dialysis on the severity of restless legs syndrome(RLS), quality of life and sleep quality in hemodialysis (HD) patients.Methods:A total of 63 HD patients with RLS who were admitted to the Hemodialysis Center, Renmin Hospital of Wuhan University from March 2018 to March 2019 were selected as research objects. According to the random number table method, they were divided into the observation group (32 cases) and the control group (31 cases). Patients in the control group were given conventional nursing methods while patients in the observation group were given the aerobic combined stretching exercise during dialysis. The International Restless Legs Syndrome Rating Scale (IRLS) Kidney Disease Quality of Life, (KDQOL-36 TM), Pittsburgh Sleep Quality Index(PSQI) and blood chemistry were used to evaluate the effect of exercise on HD patients with RLS. Results:After intervention, the observation group ′s IRLS scores was 12.19±4.48, lower than the control group (17.87±3.91), the difference was statistically significant ( t value was 3.021, P<0.05). The scores of physical health, mental health, kidney burden, symptoms and discomfort, effects of kidney disease in KDQOL-36 TM of the observation group were 44.56±6.64, 46.12±4.95, 19.92±15.91, 87.59±5.65, 64.55±13.20, higher than the control group (37.43±4.81, 41.81±4.87,12.10±14.87, 78.83±7.96, 55.45±15.52), the differences were statistically significant ( t values were -5.027--2.015, P<0.05). The PSQI total score was 8.94±2.54, lower than that of the control group (13.13±1.31), the difference was statistically significant ( t value was 8.271, P<0.05). The blood phosphorus was (1.80±0.48) mmol/L in the observation group, lower than (2.04±0.34) mmol/L in the control group, and hemoglobin, blood calcium [(105.31±13.58) g/L, (2.26±0.20) mg/L] were higher than those in the control group[(99.52±8.21) g/L, (2.05±0.29) mg/L], the differences were statistically significant ( t values were 2.275, -2.042, -3.325, P<0.05). Conclusions:The aerobic combined stretching exercise during dialysis relieves the symptom of RLS, improves the sleeping quality and quality of life.
8.Nursing-related Patient Safety Events in Hospitals

To explore the method of identifying nursing-related patient safety events,types,contributing factors and evaluate consequences of these events in hospitals of China,incident report program was established and implemented in 15 patient units in two teaching hospitals of China to get the relevant information.Among 2935 hospitalized patients,141 nursing-related patient safety events were reported by nurses.Theses events were categorized into 15 types.Various factors contributed to the events and the consequence varied from no harm to patient death.Most of the events were preventable.It is concluded that incident reporting can provide more information about patient safety,and establishment of a program of voluntary incident reporting in hospitals of China is not only urgent but also feasible.
