1.Discrepancy of Independent Risk Factors of Smoking Relapse in Different Age Patients after Percutaneous Coronary Intervention

Objective To study the independent risk factors of relapse of smoking among current smokers of different ages who underwent percuta?neous coronary intervention(PCI). Methods Totally 197 current smokers who received PCI from January 2007 to December 2009 in the Fourth Affiliated Hospital of China Medical University were enrolled. Information about relapse of smoking was collected through the medical records and the telephone follow?up. Discrepancy of risk factors of smoking resumption was compared in different age groups. Results Totally 112 patients re?sumed smoking after PCI. There were 87 patients in the non?elderly group and 25 in the elderly group. According to the correlation analysis,more than one stents(P=0.025),severe nicotine dependence(P=0.001)and long time for daily passive smoking(P=0.000)were related with relapse in the non?elderly group,and there was a negative correlation between more than one stents and relapse. But in the elderly group,relapse was associ?ated with history of hypertension(P=0.034). According to the logistic regression analysis in different age groups,independent predictors of smoking relapse in the non?elderly group were severe nicotine dependence(OR:3.914,P=0.003)and long time for daily passive smoking(OR:1.352,P=0.000). And in the elderly group,independent risk factors of smoking relapse were male(OR:17.325,P=0.027),history of hypertension(OR:13.185,P=0.005),long time for daily passive smoking(OR:1.273,P=0.045)Conclusion Independent risk factors of smoking relapse were different in the elderly group and the non?elderly group. In the non?elderly group,these factors were severe nicotine dependence and long time for dai?ly passive smoking. And in elderly group,these factors were male,history of hypertension,long time for daily passive smoking.
2.Relationship between neutrophil to lymphocyte ratio and cardiovascular risk factors among community population

Objective:To study the relationship between neutrophil to lymphocyte ratio (N/L) and traditional cardio‐vascular risk factors among community 35~64‐year‐old residents .Methods :A total of 1884 residents (548 males and 1336 females) from urban Shenyang city received baseline condition questionnaire on cardiovascular diseases and re‐lated diseases from Apr 2011 to Feb 2012. According to presence of cardiovascular risk factors or not ,subjects were divided into healthy control group (n=675) and risk factor group (n=1209);according to number of risk factors , risk factor group was further divided into one risk factor group (n=491) ,two risk factors group (n=263) and ≥3 risk factors group (n=455) .Morning blood sample and urine sample were retained to measure blood and urine rou‐tine ,blood glucose and blood lipid profile etc in all subjects .N/L was compared and analyzed among all groups .Re‐sults:Among patients with only one of following risk factors [hypertension ,diabetes mellitus (DM) ,dyslipidemia and obesity] ,N/L levels of patients with hypertension or DM were significantly higher than that of healthy control group [1.55(1.15 ,1.95) ,1.60(1.21 ,2.07) vs .1.45(1.09 ,1.91)] , P<0.05 both ,and there were no significant difference between any other one risk factor group and healthy control group , P>0.05 all .Among risk factor sub‐groups ,N/L level of ≥3 risk factors group was significantly higher than that of two risk factors group [1.57(1.16 , 2.04) vs .1.41(1.07 ,1.89) ,P<0.05] ,and there was no significant difference between any other two groups (P>0.05) .Conclusion:N/L significantly related to hypertension or DM ,and N/L level of ≥3 risk factors group was sig‐nificantly higher than that of two risk factors group ,N/L is helpful to assess risk of cardiovascular diseases .
3.Effects of tooth germ microenvironment in vitro on the differentiation of dental pulp stem cell and ectoblast mesenchyme stem cell.

OBJECTIVETo observe the differential ability of dental pulp stem cell (DPSC) and ectoblast mesenchyme stem cell (EMSC) that were cultured with tooth germ cell (TGC) as the tooth germ microenvironment.
METHODSThe TGC of 4-day old rat was used as the tooth germ microenvironment. The BrdU marked and determined DPSC and EMSC were cultured with the TGC respectively. The expression of cell surface antigen dentin sialoprotein (DSP) and alkaline phosphatase (ALP) activity were determined with double marker immunofluorescence. The differential ability of DPSC and EMSC were determined by the immunohistochemistry and image analysis in the tooth germ microenvironment.
RESULTSThe transformation efficiency of DSP positive cell in the EMSC co-culture group was higher than that in the DPSC co-culture group (P < 0.05). The transformation efficiency in the co-culture groups was higher than that in the non co-culture group after 7 days by the image analysis of immunohistochemistry (P < 0.05). The ALP activity in the co-culture groups increased after 3 and 7 days. The ALP activity in the EMSC co-culture group was higher than that in the DPSC co-culture group.
CONCLUSIONDPSC and EMSC cultured with TGC as the tooth germ microenvironment can be induced to differentiate into odontoblast. The ability of EMSC is higher than that of DPSC.

4.Progress of leukemia with NUTM1 gene rearrangement

Molecular typing of leukemia is the basis of risk assessment and treatment options. NUTM1 gene (15q14) rearrangement is a novel molecular type of acute B lymphoblastic leukemia (B-ALL), which is mainly found in children (≥1 year old) and infants (< 1 year old). The number of patients is slightly more in children than infants. However, in infantile ALL, NUTM1 rearrangement is the second most common molecular abnormality. These children respond well to conventional chemotherapy regimens and with a good prognosis. The number of leukemia patients with NUTM1 gene rearrangement is still small, and there is no relevant study or case report in China. NUT protein encoded by NUTM1 gene is a chromatin regulator, which is related to histone acetylation regulation and chromatin remodeling. This article aims to introduce the clinicopathological features, detection methods, possible tumorigenic mechanisms and therapeutic prospects of leukemia with NUTM1 gene rearrangement, to increase the understanding of this type of leukemia and provide reference for the precise molecular subtyping and treatment.

6.Genotype-phenotype analysis of five families with congenital ectopia lentis accompanied with cardiovascular abnormalities

Objective To analyze the genotype-phenotype correlation in 5 families with congenital ectopia lentis (CEL) accompanied with cardiovascular abnormal manifestation.Methods Detailed clinical data of 15 family members in 5 families were collected from August 2017 to March 2018 in Zhongshan Ophthalmic Center,including examination of the condition of lens before and after mydriasis by slit-lamp,evaluation of the cardiovascular system using transthoracic echocardiography,and evaluation of the degree of involvement of the subjects' skeletal system using X-ray images.Genomic DNAs were extracted from whole blood sample of the 5 probands and 10 relatives,and screened for FBN1 mutation by targeted exome sequencing.The possible genotype-phenotype correlation was analyzed by reviewing previous literatures into these mutation sites.The study followed the principles of the Helsinki Declaration and written informed consent was obtained from each subject prior to any examination.Results All of the five probands were diagnosed as CEL accompanied with cardiovascular abnormal manifestation.FBN1 gene mutations were identified in all of the five probands,including four missense mutations (c.2741G>T,c.2585G>T,c.1633C>T,c.4260C>G) and one splicing mutation (c.2114-1G>C).It was predicted that all of the 5 mutations would alter the protein structure.Conclusions FBN1 gene has a high degree of clinical heterogeneity,and the early detection of ocular phenotypes combined with genetic screening is of great significance in the diagnosis of cardiovascular abnormalities.
7.The characteristics of trunk muscle activity in patients with Parkinson's disease

Objective To explore the characteristics of trunk muscle activity in persons with Parkinson's disease (PD) in search of better treatments for abnormal posture.Methods Ninety persons with PD of different severity and 8 normal controls were studied using surface electromyography (sEMG).A sixteen-lead sEMG instrument was used to collect sEMG amplitudes from the bilateral thoracic erector spinae,lumbar erector spinae,external oblique and rectus abdominis muscles when the subjects were lying,standing and walking.Results Compared with the control group,a significant increase was observed in the sEMG amplitude of the lumbar erector spinal muscle [(18.56±14.57) μV] and rectus abdominis muscles [(24.00±18.80)μV] of the PD group when lying.Significant relative increases in (P<0.05),of the external oblique muscles were observed [(27.87± 11.49)μV] when standing (P<0.05),and in the thoracic erector spinae muscle [(58.74±29.69)μV] and external oblique muscles [(59.01± 25.20) μV] increased when walking (P<0.05).Compared with the control group,the sEMG activity of the external oblique muscles was significantly elevated in PD patients at Hoehn and Yahr stage 1 to 1.5 when walking.One or more of the trunk muscle groups showed significantly greater activity in the PD patients in Hoehn and Yahr stage 2 or 3 in all three positions.Conclusions The sEMG activities of the extensor and flexor muscles increase simultaneously in PD patients.More trunk muscles are involved in PD patients with higher H-Y grades.These findings provide a neurophysiological basis for the customizaton of rehabilitation therapy for patients with Parkinson's disease and for the precise selection of muscles for botulinum toxin injection.
8.Correlation between preoperative serum γ-gultamyl transferase level and postoperative survival in patients with renal cell carcinoma

Objective:To investigate the correlation between preoperative serum γ-glutamyl transferase (GGT) level and the postoperative survival of patients with renal cell carcinoma.Methods:The clinical data of 235 patients with renal cell carcinoma who underwent nephrectomy from December 2005 to December 2011 in the Affiliated Hospital of China University of Mining and Technology were retrospectively analyzed. According to the preoperative serum GGT level at 1.5 times the upper limit of normal value 90 U/L (the upper limit of normal value of samples in this study was 60 U/L), patients were divided into 2 groups: GGT ≤ 90 U/L group (218 cases) and GGT>90 U/L group (17 cases). The correlation between GGT and clinicopathological characteristics as well as postoperative survival of patient with renal cell carcinoma was also analyzed.Results:There were statistically significant differences in gender, age, the level of the aspartate aminotransferase (AST), the level of alanine aminotransferase (ALT), the neutrpphil-to-lymphocyte ratio (NLR), lymph node metastasis, distant metastasis, neoplasm staging between GGT ≤ 90 U/L group and >90 U/L group (all P < 0.05). Kaplan-Meier survival analysis showed that the median overall survival time was 84 months and 54 months, respectively in GGT ≤ 90 U/L group and GGT > 90 U/L group, and the difference was statistically significant of both groups ( χ2 = 4.334, P = 0.037). Univariate Cox proportional risk regression model showed that pathologic type, pathology T staging, preoperative GGT level, lactate dehydrogenase (LDH) level, ALT level were influencing factors of postoperative overall survival in patient with renal cell carcinoma (all P < 0.05). The multivariate Cox regression model analysis showed that the pathological type ( HR = 2.323, 95% CI 1.228-4.396, P = 0.010), GGT level ( HR = 2.406, 95% CI 1.077-5.376, P = 0.032), LDH level ( HR = 2.320, 95% CI 1.080-4.981, P = 0.031) were independent influencing factors of postoperative overall survival in patient with renal cell carcinoma. Conclusions:Preoperative elevated serum GGT level is associated with poor prognosis of patients with renal cell carcinoma, and the monitoring of it may help to evaluate the prognosis of patients and provide guidelines for individual treatment method.
9.Analysis of resistance situation and resistance genes of clinical isolates of carbapenem-resistant Klebsiella pneumoniae and Escherichia coli

Objective To analyze the current status of anti-bacterial activity of carbapenem-resistant Klebsiella pneumoniae and Escherichia coli clinically isolated from hospitalized patients,detect their related resistance genes,and provide reference for the clinical treatment of carbapenem resistant Enterobacteria(CRE)infections and the rational use of antibiotics.Methods A total of 400 non-repetitive isolates of Klebsiella pneumoniae and Escherichia coli isolated from clinical specimens of Punan Branch of Renji Hospital,Shanghai Jiao Tong University School of Medicine from January 2022 to December were collected.The minimum inhibitory concentrations of these strains against commonly used antibiotics were determined by the broth microdilution method.The carbapenemase and related resistance genes of CRE were detected by drug resistance phenotype testing and PCR.Results Among the 400 strains,51 strains were identified as CRE,accounting for 12.75%.Among these,49 strains produced carbapenemases,with 41 strains(80.39%)being CR Klebsiella pneumoniae and 10 strains(19.61%)being CR Escherichia coli.Among the CRE strains,34 strains(66.67%)carried blaKPC,13 strains(25.49%)carried blaNDM,and 2 strains(3.92%)carried blaOXA-48.Conclusion Compared with other commonly used antibiotics,colistin and tigecycline exhibited good in vitro antibacterial activity against carbapenemase-producing Klebsiella pneumoniae and Escherichia coli.In addition,there was good concordance between drug resistance phenotype testing and genotyping.Clinical microbiology laboratories could continuously monitor the drug resistance phenotype and genotype of CRE and develop appropriate treatment plans based on actual conditions.
10.Research progress on the involvement of parents in nursing in Neonatal intensive care unit

The involvement of parents in nursing is a new nursing model that promotes the recovery of children and increases parental satisfaction. It has the advantages of optimizing medical resources and promoting doctor-patient harmony. Now it is been widely introduced in the nursing of neonatal intensive care units. However, there are huge differences in the implementation of the involvement of parents in nursing, and there is also a lack of evaluation tools for such involvement. This article reviews the theoretical basis and significance, implementation path and effect evaluation of the involvement of parents in nursing at home and abroad, in order to provide a reference for the construction of local involvement programs of parents in nursing in neonatal intensive care units.
