1.The clinical forensic identification of deep vein thrombosis

Deep vein thrombosis is the common complication of trafifc injury , it is one of the dififculties of forensic identiifcation to identify disability grade and the relationship between injury and disease. When post-thrombotic syndrome happens, circulatory system and leg will be dysfunction, caused by the destruction of valve and venous wall and by the blockage of thrombus. The quality of life and working ability will be seriously affected. The impact of different types of deep vein thrombosis on human body was analysised after looking up relevant literatures, then it was discussed that the identiifcation of following questions including: the relationship between injury and disease, how to identify disability grade and it’s best time.
2.Value of color Doppler flow imaging and Doppler power imaging in diagnosis of choroidal detachment

Objective To investigate value of color Doppler flow imaging(CDFI) and Doppler power imaging(CDPI) in the diagnosis of choroidal detachment(CD). Methods Color Doppler ultrasound was performed in 30 cases with choroidal detachment. At the base of eyeball shape, lens, vitreous and retina examined by ultrasound, intraocular band-shaped echo was examined by CDFI and CDPI, to understand its blood flow. The observation was compared with that of surgical results. Results Of 30 cases with choroidal detachment, the diagnostic positive correspondonce of ultrasound was 80.0%, while ultrasound combined with CDFI and CDPI, the correspondence increased up to 96.7%. The sonographic features of CDFI and CDPI in choroidal detachment patients were abundant blood flow in band-shape or short stick-like. In 3 cases out of 4 cases with retinal detachment, blood flow was not abundant,their blood flow showed a sparked pattern. In addition, 14 cases combined with vitreous hemorrhage, 8 cases had vitreous opacity and 4 cases vitreous proliferation. Ultrasound diagnosis corresponded with that of clinical surgery in 27 choroidal detachment cases. Conclusions Ultrasound combined with CDFI and CDPI provide valuable references for diagnosis and treatment of choroidal detachment.
4.Effects of splenectomy on peripheral blood Th17 cells and immune function in patients with portal hypertension

Objective To investigate the effects of splenectomy on peripheral immune cells including Th17 cells in patients with portal hy-pertension and to analyze the influence of splenectomy on their immune function.Methods Twenty-five portal hypertension patients trea-ted with splenectomy in No.180 Hospital of PLA from June 2012 to June 2014 were selected as observation group,and 25 healthy controls who underwent physical examination in the same period as control group.Flow cytometry was used to determine the percentages of CD3 , CD4,and CD8 T lymphocytes,CD4/CD8 ratio,and percentage of Th17 cells in the peripheral blood of portal hypertension patients (1 day before operation and at 7 days,1 month,and 3 months after operation)and healthy controls.ELISA was used to evaluate the changes in ser-um interleukin-6 (IL-6),interleukin-17 (IL-17),and interleukin-23 (IL-23).Comparison between groups was made by one-way ANOVA and LSD-t test.Results Before splenectomy,the portal hypertension patients had significantly lower percentages of CD3, CD4,and CD8 T lymphocytes and CD4/CD8 ratio (P<0.01 for all)and significantly higher percentage of Th17 cells and levels of their as-sociated inflammatory cytokines (IL-6,IL-17,and IL-23)(P<0.01 for all),as compared with the control group.At 7 days,1 month,and 3 months after operation,the percentages of CD3,CD4,and CD8 T cells and CD4/CD8 ratio in portal hypertension patients first decreased and then increased,and these values were significantly lower at 7 days after operation than before operation (P<0.01 for all);at 3 months after operation,the patients showed no significant differences in the percentages of CD3,CD4,and CD8 T cells and CD4/CD8 ra-tio compared with the control group (P>0.05 for all).At 7 days,1 month,and 3 months after operation,the percentage of Th17 cells and levels of their associated inflammatory cytokines (IL-6,IL-17,and IL-23)in portal hypertension patients decreased gradually (P<0.05 for all).Conclusion Th17 cells and their associated inflammatory cytokines (IL-6,IL-17,and IL-23)are reduced in portal hy-pertension patients treated with splenectomy,so their immune function can be improved.
6.Primary non-Hodgkin′s lymphoma of the pancreas: report of six cases

ObjectiveTo evaluate the diagnosis and treatment of primary non-Hodgkin′s lymphoma (PNHL) of the pancreas. MethodThe therapeutic result of 6 cases admitted from January 1980 to March 2000 was reviewed retrospectively. ResultsThe lesion was located in the head of the pancreas in 2 cases and the body and/or tail in 4, respectively. Clinical manifestations included epigastric discomfort, loss of weight, abdominal mass and jaundice. Two cases underwent Whipple′s procedure and distal pancreatectomy respectively, the other 4 underwent laparotomy and biopsy. B cell non-Hodgkin′s lymphoma of 5 cases and T cell non-Hodgkin′s lymphoma of one were verified by pathology. One patient lost follow-up, the other 5 patients survived 58, 49, 22, 13 and 4 months respectively. Conclusion The clinical manifestation and radiological features of PNHL of the pancreas are not specific. Comprehensive therapy including surgery is beneficial to the long-term survival of patients with PNHL of the pancreas.
7.Mela-analysis of the outcomes of liver transplantation from donation after cardiac death donors

Objective To conduct a meta-analysis to evaluate the outcomes of donation after cardiac death (DCD) compared with donation after brain death (DBD) liver transplantatior.Methods The MELDINE (1950-2011),EMBASE,and Cochrane Library databases were searched.All original single institution studies reporting outcomes of comparing donation after DCD and DBD liver transplantation were considered.A meta-analysis of complication incidence and patients/grafts survival after liver transplantation was conducted.Odds ratios (OR) and 95 % confidence intervals (CI) based on random effects models were calculated.Results Thirteen studies,all retrospective cohort studies,involving 5867 DCD and 619 DBD recipients,were included.DCD recipients had a 2.5 times increased odds of biliary complications (95 % CI =2.0~3.12),an 11.24 times increased odds of ischemic cholangiopathy (IC) (95 % CI =5.58 ~ 22.64 ),and a 2.12 times increased odds of primary nonfunction (PNF).DCD recipients also experienced lower odds of 1-year patient survival (OR =0.78,95 % CI=0.59~1.02),83.8 %,87.2 %,separately,and 1-year graft survival (OR=0.55,95% CI=0.45~0.68),72.2 % and 82.4 %,separately.Three-year patient survival was present in 81.5 % of DCD vs 78.9 % of DBD,which has no significant difference.The 3-year graft survival was lower inDCD than that in DBD (OR =0.73,95 % CI =0.56~0.94),69.5 % and 73.6%,separately.Conclusion DCD liver transplantation is associated with higher risks of biliary complications.But regarding the comparable general outcomes with DBD transplantation,DCD could be a source of liver.
8.Clinical observation of lower extremity deep venous hrombosis (DVT) postoperative fracture treated by Maixuekang capsule and low molecular heparin calcium (Subilin).

OBJECTIVETo observe the clinical curative effects of lower extremity deep venous thrombosis (DVT) postoperative fracture treated by maixuekang capsule and low molecular heparin calcium (Subilin).
METHODFrom Feb 2008 to Apr 2010, 214 cases of lower extremity DVT postoperative fracture were randomly divided into experimental group and control group. The 107 cases of them were treated by maixuekang capsule based on the comprehensive treatment.
RESULTThe 89 cases were cured and improved in the observation group, but 18 cases were ineffectiveness. The 66 cases were cured and improved in the control group, but 41 cases were ineffectiveness. Maixuekang capsule group had 83. 18% (89/107) total effective rate and 61.68% (66/107) total effective rate in the control group. There was significant difference between the two groups (P < 0.05). After the treatment, the perimeter between the suffered limb and the healthy one was significantly reduced, the blood rheology examination had been improved significantly. There was also significant difference between the two groups (P < 0.05).
CONCLUSIONMaixuekang capsule for lower extremity deep vein thrombasis has good effect It has both thrombolytic and anticoagulant effects for lower extremity DVT postoperative fracture treated by maixuekang capsule and low molecular heparin calcium (Subilin). It's a recommended treatment.

9.Value of ultrasound on diagnosis of posterior dislocation of the lens

ObjectiveTo investigate the value of ultrasound on the diagnosis of posterior dislocation of the lens. MethodsThe eyes of 38 patients with different kinds of posterior dislocation of the lens were examined by color Doppler ultrasound to understand location, forms, and echogram of the lens and relation with their environmental tissues. ResultsOf 38 cases, 13 cases with complete posterior dislocation of the lens, 3 cases with posterior dislocation of intraocular lens, and 22 cases with incomplete posterior dislocation of the lens were proved. Also, 4 cases with vitreous hemorrhage, 13 cases with vitreous opacity and fibrous membrane, 15 cases with posterior detachment of the vitreous, 3 cases with retinal detachment were found. In addition, 33 cases underwent vitrectomy combining lensectomy or removal of the lens, among them, diagnostic ratio of ultrasound was 100%. ConclusionsHigh practical value is shown in posterior dislocation of the lens by ultrasound as one of the important methods on the diagnosis. At the same time, it provides valuable references for choosing surgical methods of posterior dislocation of the lens.
10.A Clinical Comparative Study Between Ticagrelor and Clopidogrel for Treating the Patients With Acute ST-segment Elevation Myocardial Infarction

Objective: To explore the clinical efficacy of a new drug for acute coronary syndrome (ACS), ticagrelor in treating the patients with acute ST-segment elevation myocardial infarction (STEMI), and to compare the effect between ticagrelor and clopidogrel in order to obtain the prospective information of ticagrelor in clinical practice.
Methods: A total of 307 STEMI patients were divided into 2 groups, Treatment group, the patients received ticagrelor with aspirin, n=157 and Control group, the patients received clopidogrel with aspirin, n=150. With a period of treatment, the major adverse cardiac events (MACE), rate of thrombosis re-formation and routine blood test result were compared between 2 groups.
Results: Treatment group presented the better LVDD, LVEF, platelet level and ST-segment recovery than those in Control group, P<0.05 or P<0.01. Treatment group showed slightly higher rate of side effect such as dififcult breathing and bleeding, while those reaction did not affect the treatment or special management for the patients.
Conclusion: The clinical effect of ticagrelor is similar to clopidogre for improving the platelet level and cardiac function in STEMI patients, while the clinical advantage reported in abroad should be further studied.
