1.THE THERAPEUTIC EFFECT OF BATROXOBIN(DF-521) ON OTITIS MEDIA WITH EFFUSION

To investigate the role of fibrinolysis in adhesion formation in the middle ear,40 patients(45 ears) with otitis media with exudation (OME) were randomly divided into 3 groups:Dexamethasone(G1),Batroxobin1BU/ml(G2),and Batroxobin 2BU/ml(G3).The results were assessed by tympanotomy and irigation to compare the efficacy of treatment of batroxobin and dexamethasone.31 patients (33 ears) were analyzed for statistical significance.The efficacy of the treatment was evaluated by qualitative analysis of symptom and the Hearing Threshold of Air conduction of pure tone audiometry (AHT) at frequencies 0 25~2kHz,and by quantitative analysis of AHT.Cure rates of G1,G2,G3 were 41 7%(5/12),54 5%(6/11),70%(7/10) respectively;G3 was higher than G1 and G2( P 0 05). 2.Reconstruction local defect after subtotal or extended resection of temporal bone

Objective To explore the best way for reconstruction of local defect after subtotal or extended resection of the temporal bone. Method The reconstruction of 33 patients who had undergone subtotal or extended resection of temporal bone between July 1993 to June 2003 in our hospital were retrospectively analysed. Results Thirty-three patients, aged 14-69 years old (average 45.0) were enrolled. Among them, 26 were suffering from malignancy (12 with recurrences, undergoing several operations). Subtotal resection of temporal bone was performed in 22 cases and extended resection in 11. Total survival rate of reconstructive tissues was 81.8%(27/33), in which pectoralis major island musculocutaneous flap accounted for 77.8%(7/9), temporalis musculocutaneous flap survived 100% (9/9), pedicled temporalis musculocutaneous flap associated with other local musculocutaneous flap survived 100%(4/4), 60.0% (3/5) of upper sternocleidomastoid musculocutaneous flap survived, and 66.7% (4/6) of fat transplantation survived. Nature and extent of the diseases, preoperative radiotherapy, relapse and postoperative leakage of cerebrospinal fluid were important influential factors for postoperative healing. Conclusions Reconstructive measures depended on the nature and extent of diseases, necessary size of tissue, local circulation, and influential factors of healing. The optimum way for the repair of a small defect after resection of non-malignant tumor was fat transplantation with dermis, and pedicled temporalis musculocutaneous flap for malignant tumors. Pectoralis major island musculocutaneous flap was employed in large defect and that with lack of blood supply. Skin grafting met several failures in these patients. Theoretically, free scapular skin flap is the optimum way for defects after subtotal or extended resection of temporal bone.
3.Control infection of expanded skin flap by stretching during auricle reconstruction

Objective To explore the best way to deal with infection of expanded skin flap during auricle reconstruction. Methods 12 cases (12 ears) of expanded skin flap infection was analyzed and treated with the method of stretching the expanded skin flap during auricle reconstruction during Jan. 2003 to Dec. 2005. Results In 12 cases (12 ears) the infection was controlled well, auricles were reconstructed smoothly and no obvious difference was detected between the infected and normal skin flaps after the reconstruction of auricles. Conclusion Stretching the expanded skin flap is a good method to deal with its infection during auricle reconstruction.
4.Selection of surgical plan for the congenital microtia with deformities of external auditory meatus (EAM) and middle ear

Objective To investigate the best way for the operation of congenital microtia with deformities of external auditory meatus (EAM) and middle ear. Method To analyze retrospectively 305 cases (332 ears) of auricle reconstruction operated in the Plastic Department of Auricle, Plastic Surgery Hospital, PUMC, CAMS during June to October 2005, and 49 cases (58 ears) of reconstructions of EAM and middle ear operated in the Otolaryngology Department, General hospital of PLA from December 1988 to July 2002. Results The patients/families rested content with the reconstructed auricles operated with the 3-stage way. The hearing was improved obviously after reconstruction of the EAM and ossicle for the patients with normal stapes footplate. In those patients, for whom EAM was reconstructed first, the scar developed around the orifice which affected the skin flap for auricle reconstruction later, while in those patients, for whom the auricle was reconstructed first, wrong location of the EAM orifice happened easily. Conclusions For the patients with congenital deformities of EAM and middle ear and with the requirement of recovering the appearance and hearing function, the EAM orifice should be located first according to the results of temporal CT, then the auricle reconstruction was conducted with 3-stage method. A simultaneous reconstruction of EAM and ossicle at the third stage may ensure the improvement of both outward appearance and hearing function.

For rapid screening, we constructed two levels pools (primary and secondary pools) of the bacterial artificial chromosome (BAC) library of Chinese fine wool merino sheep. The primary pools were based on the individual 384-well microtiter plate and were prepared with a three-dimensional pooling scheme. Three dimension (plate, row and column) pools were made for each. The secondary pools were based on the entire BAC library. We developed a PCR based strategy to identify positive BACs from sheep BAC library. First, we analyzed secondary pools DNAs, according to the result, we analyzed correlative primary pools. It was one-step screening (66 PCR reactions) that we could screen a single positive clone from 74 000 BACs by our method, or three-step screening (less than 100 PCR reactions) could screen more clones. By one-step screening (66 PCR reactions), we screened successfully a positive clone 373D13 with polymorphism marker BF94-1.

Objective:To analyze the features and clinical significance of damages to non pulmonary organs like the heart,kidney and peripheral blood in SARS patients.Methods:SPSS 11.0 and SDAS software packages were used to retrospectively analyze the damages to the heart,kidney and peripheral blood in 330 clinically confirmed SARS cases.Results:(1)The heart damage occurred within 1 week after SARS onset in mild cases; the rates of LDH,HBDH,CK and CK MB returning to normal were 74.2%,80.6%,100% and 100%,respectively.In severe cases,the heart damage usually occurred 1 week after SARS onset; the rates of LDH,HBDH,CK and CK MB returning to normal were 32.2%,29.5%,55.2% and 44.4%,respectively.Ten patients whose CK MB didnot return to normal died.(2) The changes of RBC count appeared after an average of 1 week (1 10 d) in 54.2% of the patients.It returned to normal in all the mild SARS cases after an average of 12.5(4 21) d but didnot in 18.4% of the severe cases ( P
8.Diagnosis and treatment of esophagus perforation caused by esophageal foreign bodies.

OBJECTIVE:
Summarize the treatment skills of esophagus perforation caused by esophageal foreign bodies.
METHOD:
Retrospectively analyze the seven cases of esophagus foreign body perforation with various complications in our department.
RESULT:
Six cases recovered in 3 to 18 days after operation, on average 14.2 day, while one case recovered in 49 days by conservative method.
CONCLUSION
Foreign bodies removing, fistulae repairing, abscess incising, effusion rinsing, effective antibiotics administrating and support treatment are effective to treat esophagus perforation caused by esophageal foreign bodies. It will take a relative long time to be treated only by conservative way.

OBJECTIVE:
To investigate the sources of foreign bodies in the middle ear, and to analyze experience and lessons of clinical diagnosis and therapy.
METHOD:
Five cases of foreign bodies in the middle ear with general information presenting to our department and receiving surgical treatment were reviewed retrospectively.
RESULT:
The foreign bodies of 4 cases were inserted by trauma, while 1 case by iatrogenic injury. The remain of foreign bodies result in symptoms such as otorrhea and hearing loss, and long-term foreign bodies in the middle ear might cause mental effects. All 5 patients received surgical intervention under general anesthesia to remove the foreign bodies. We found that the foreign bodies of 3 cases located in the mesotympanum, 1 located in the epitympanum and the aperture of the eustachian tube, 1 located in the squamous portion of the temporal bone and the gas room of lateral wall of epitympanum.
CONCLUSION
Foreign bodies in the middle ear are prone to be missed diagnosed and misdiagnosed. Careful specialized examination is appreciated. Patients who are suspected with foreign bodies in the middle ear should regularly take temporal bone HRCT scan. Foreign bodies could be removed by surgery when a definite diagnosis is made.

10.Clinical analysis of 182 cases esophageal foreign bodies and treatment for special cases.

OBJECTIVE:
To provide clinical reference by summarizing and analyzing characteristics, therapeutic principles and methods for special cases of esophageal foreign bodies.
METHOD:
Statistical analysis 182 cases with esophageal foreign bodies, including 41 special cases. Surgery 165 cases, 17 cases without surgery.
RESULT:
One hundred and seventy-five cases were cured, 7 cases improved. Hospitalization time 0.5-50 d (average 4.5 d).
CONCLUSION
It is important to learn about characteristics of esophageal foreign bodies, therapeutic principles and methods for special cases, and it's a key point to diagnosis early and remove the foreign bodies as soon as possible, and comprehensive treatment measures are really effective to process special cases.
