1.Soft and hard tissue changes of early treatment of Angle's class II division 1 by Dynamax appliance

Objective:To evaluate the soft and hard tissue changes of class II division 1 patients treated by Dynamax appliance. Methods:15 class II division 1 cases, whose hand-wrist radiographs were in FG-G stage, were treated by Dynamax appliance. After scanning the cephalometric tracing and transcripting into the special software of Winceph 7.0, the Cephalometrics analysis was preformed on pre-treatment and post-treatment. Results:After being treated by Dynamax appliance, the SNB, Co-GN,L1-NB,Cm-Sn-Ls was increased significantly,the ANB,Wits,U1-NA,overjet,overbite,Ns-Sn-Pos decreased significantly. Conclusion:Dynamax can effectively induce forward movement of mandible, improved esthetics on soft tissue profile and make face become normal.

Objective To investigate the correlation of oncosis and TNF-? in D-galactosamine (D-GalN) induced acute hepatic injury. Methods Acute hepatic injury model was induced by D-GalN in 24 SD rats, and 12 rats served as control. At 4, 8, 12, 16, 20, 24 h after intraperitoneal injection of D-GalN, the levels of alanine aminotransferase (ALT) and aspartate aminotransferase (AST) in serum, and the tumor necrosis factor-? (TNF-?) in liver tissue were measured. The pathological changes of liver and the oncosis of hepatocytes were observed by TEM. Objective To explore the effect and side effects of Group A Streptococcus Preparation(Sapylin)com-bined with mitomycin and lentinan combined with mitomycin in treating malignant pleural effusion through intrapleural catheter.Methods 70 malignant pleural effusion patients were selected in this study.All the patients were randomly divided into Sapylin group and lentinan group.One case with penicillin positive of the Sapylin group was adjusted to enter the lentinan group.After drainage of pleural effusion,the patients were divided into Sapylin group (34 patients) and lentinan group (36 patients).For the Sapylin group,Sapylin was prior injected and mitomycin was posterior injected.For the lentinan group,lentinan was prior injected and mitomycin was posterior injected.The side effects and clinical effect were observed and reported.Results The effective rate of Sapylin group was 85.3%,whereas 83.3%of lentinan group.There was no significant difference between the two groups(χ2 =0.051,P =0.822).Both groups had the following minor side effects:chest pain,fever,vomit and decreased white blood cells.No liver or renal injuries were reported.Conclusion The clinical outcomes of Sapylin combined with mitomycin and lentinan combined with mitomycin are both positive,in addition,the side effects are minor.
4.Clinical value of transgastric endoscopic surgery for the treatment of pancreatic abscess or pancreatic cyst and infection

ObjectiveTo investigate the clinical value of transgastrie endoscopic surgery for the treatment of pancreatic abscess or pancreatic cyst and infection.MethodsThe clinical data of 22 patients with pancreatic abscess or pancreatic cyst and infection who underwent transgastric puncture and drainage or transgastric debridement under the guidance of endoscopic ultrasonography (EUS) at the Xinqiao Hospital of Third Military Medical University from July 2008 to August 2011 were retrospectively analyzed.All patients were comfirmed with bacteria infection after liquid aspiration culture. Patients with pancreatic abscess underwent endoscopic transgastric debridement,and for patients with pancreatic cyst and infection,10 F double pigtail stent and 8.5 F nasal bile duct were placed for drainage.ResultsThe results of liquid aspiration culture confirmed that 2 patients were infected by staphylococcus aureus,3 by proteus mirabilis,4 by pseudomonas aeruginosa,4 by klebsiella and 9 by escherichia coli bacilli.The double pigtail stent and nasal bile duct were installed under EUS (16 patients) or duodenoscope (6 patients).The lesions of 9 patients with pancreatic abscess were healed after endoscopic transgastric debridement with an average period of (6.5 + 1.8 )weeks,and the lesions of 13 patients with pancreatic cyst and infection were healed after transgastric puncture and drainage under the guidance of EUS with an average period of ( 8.3 ± 2.1 ) weeks.All patients were followed up for 2 years,and no recurrence of pancreatic abscess or pancreatic cyst was observed.ConclusionThe effect of transgastric endoscopic surgery for the treatment of pancreatic abscess or pancreatic cyst and infection is satisfactory.
5.Association of oxygen uptake efficiency in exercise test with exercise capacity in patients of obstructive sleep apnea hypopnea syndrome

Objective To explore the characteristics of oxygen uptake efficiency (OUES) in patients with obstructive sleep apnea hypopnea syndrome (OSAHS) and analyze the relationship between OUE and severity of disease.Methods Pulmonary function test,polysomnogram and cardiopulmonary exercise testing were performed in 35 patients with OSAHS and 25 age-matched healthy volunteers.Their successive breathing respiratory exchange parameters were collected and analyzed.And t and x2 tests were used for 2 sample comparison.Correlation analysis was performed by Pearson correlation test.Results Significant differences in peak VO2 and peak VO2 % pred existed between OSAHS and normal control groups [(18±4) vs.(28 ±6) L/min,P<0.01;(68±14) vs.(84±16) %,P<0.01].Compared with normal control group [(2.3 ±0.5) L · min-1 · lg-1 ; (36 ±4) ml/L; (36 ±4) ml/L],OUES,OUEP and OUE@AT of OSAHS group [(1.8 ± 0.4) L · min-1 · lg-1 ; (31 ± 5) ml/L; (30 ± 5) ml/L] were significantly lower (t =3.78-4.49,all P <0.01).And OUES,OUEP and OUE@AT in OSAHS patients were correlated (r =0.53-0.67,all P <0.01) positively with exercise tolerance (peak VO2% pred) while negatively with apnea hypopnea index (AHI) (r=-0.67--0.54,all P <0.01).Conclusion The oxygen uptake efficiency of patients with OSAHS is significantly reduced compared to that of normal subjects.And it is correlated negatively with severity of disease.
6.Dimethicone powder in bowel preparation before capsule endoscopy

Objective To evaluate the effect of the dimethicone powder in bowel preparation before capsule endoscopy (CE) and to observe its possible adverse effects. Methods A total of 60 patients receiv-ing CE were prospectively randomized into 2 groups according to bowel preparation method. In dimethicone powder group, patients were arranged to take dimethicone powder 30 rain before the examination on basis of macrogol electrolytes powder and in control group, patients had macrogol electrolytes powder only. Images of small intestine were equally divided into segments A, B and C according to intestinal transit time, and re-viewed by 2 experienced physicians independently. Intraluminal gas bubbles were graded and any possible adverse effects were monitored. Results Interobserver agreement was excellent (P < 0.05). In segments A and C, images from dimethicone powder group were less interfered by gas bubble than those from control group (P < 0. 05), but in segment B there was no difference between 2 groups (P > O. 05). No adverse effects were observed. Conclusion The dimethicone powder administration before capsule endoscopy im-proves the visualization of the intestinal mucosa.
7.Feasibility of endoscopic resection-closure for non-intraluminal gastric stromal tumors originating from the muscularis propria layer

Objective To evaluate the feasibility of endoscopic resection and closure for non-intraluminal gastric stromal tumors originating from the muscularis propria layer.Methods Included in the study were 46 patients with gastric submucosal lesions originating from the muscularis propria layer, detected by gastroscopy and endoscopic ultrasonography.The lesions were removed by endoscopic resection and closure, which were further diagnosed as stromal tumor by means of pathologic and immunohistochemical examinations.The patients were followed up with endoscopy for evaluation of therapeutic effect and complications.Results All lesions were successfully removed, with serosa layer remained in 2 cases and full layer resection in other 44, which were all closed by endoscopic clips.Combination managements of acid suppression,gastrointestinal decompression and intravenous antibiotics were applied in all patients.Pathology reports confirmed complete resection of all lesions, with 0.5 to 3.7 cm in diameter.Normal diet was restored in 44 patients 48 ~ 72 h after the procedure.Pneumoperitoneum and focal peritonitis occurred in 2 cases, one of which underwent rupture and was clamped again.The 2 patients recovered after 10-12 days of conservative treatments.Follow-up endoscopy revealed white ulcerous scar in all cases.Conclusion Endoscopic resection and closure therapy is a safe, economic and less invasive treatment for non-intraluminal gastric stromal tumors originating from the muscularis propria layer.

Neurofibromatosis type Ⅰ(NF1)is an autosomal dominant genetic disease triggered by mutations of nf1gene, nf1 gene and its encoded protein product neurofibromatoprotein play important roles in tumor supressive activity. Plexiform neurofibroma was the main manifestation among some patients For plexiform neurofibroma, surgical treatment did not have satisfactory effect. meanwhile, traditional radiotherapy and chemotherapy are ineffective. All of those above serve as challenges for clinical treatment and have been received much more attention from study of multimoics and targeting therapy In this Review, the clinical features of NF1-associated plexiform neurofibromasand, the progress regarding investigation of drug targets and clinical trials for the drug of plexiform neurofibroma will be presented.

Objective The aim of this study was to investigate the possibility of type 2 vesicular monoamine transporter molecular probe,18 F-FP-(+)-DTBZ, in the monitoring of total islet β cell mass in animal models. Methods Two groups of Wistar rats were included in this study. In the type 1 diabetes group ( n = 6 ) , the streptozotocin ( STZ) was intraperitoneally injected at a dose of 65 mg/kg, and the control group ( n= 6 ) was likewisely injected with an equal volume of saline, Micro- positron emission tomography ( PET )/ computed tomography ( CT) imaging was performed at these rats post injection of18 F-FP-(+)-DTBZ at 0. 5, 1, 4, 6, and 12 months after STZ or saline injection, bodyweight and glucose level were also measured. Results The average standardized uptake values ( SUV) in the pancreas in the type 1 diabetes rats were decreased significantly than that of the control group at 0.5, 1, and, 4 months ( P<0.05) , and there was no significant difference at 6th and 12th months ( P>0.05) post injection of STZ and saline. Fasting blood glucose positively correlated with pancreatic SUV in the two groups at 0.5, 1, and 4 months (P<0.05) post injection of STZ and saline. Conclusion 18F-FP-(+)-DTBZ PET imaging is a promising method for dynamic monitoringβcell mass in type 1 diabetic rats.
10.Operation technology and clinical application of 18F-AV45 PET imaging in Alzheimer's disease

18F-florbetapir (AV45) positron emission tomography (PET) imaging, as a non-invasive method for early diagnosis of Alzheimer's disease, has been gradually recognized in China. Standard examination procedures, standard examination techniques, and standard image interpretation and reports will help us to obtain high-quality images and accurate quantitative analysis data. Therefore, in reference to the domestic and foreign guidelines, the standardization examinations and clinical researches of 18F-AV45 PET in China are introduced based on the the current status of our country and clinical practical experience to provide accurate diagnosis services to the patients.
