1.Preservation of parotid masseter fascia and sternocleidomastoid muscle flap in parotid gland surgery

Objective To evaluate the clinical effect of preservation of parotid masseter fascia and sternocleidomastoid muscle flap in preventing facial concave deformity and Frey’ s syndrome. Methods 110 patients with parotid gland benign tumor were selected and ran-domly divided into treatment group(56 cases) and control group(54 cases). The treatment group was filled with sternocleidomastoid muscle flap at once and remained parotid masseter fascia in operation. The control group got parotidectomy only. All the cases were followed-up. Re-sults All the cases were successful during a follow-up period of 3~24 months. The treatment group was better than the control group at pre-venting facial concave deformity and Frey’s syndrome (P<0. 01). Conclusion Preservation of parotid masseter fascia and sternocleido-mastoid muscle flap can be the priority selection in improving facial concave deformity and preventing Frey’ s syndrome.
2.Effects of brucine on metabolism in multiple myeloma

ObjectiveTo explore the effects of brucine on multiple myeloma (MM) and to compare the effects between brucine and bontezomib on MM.MethodsMTT method was used to determine the median inhibitory concentratiom (IC50) of brucine and bortezomib on the MM cell line U266.The supernatant of cultured U266 cell line was added to the culture system for inducing the differentiation of osteoblast cell line MC3T3-E1.After aseptic assay,RT-PCR was used to determine the RNA levels of alkaline phosphatase (ALP),osteocalcine (OC),osteoprotegerin (OPG) and recepter activator of NF-κB ligand (RANKL).Results IC50 of bortezomib on U266 cell line for 48 h was 22.4 nmol/L,and that of strychnine was 0.16 nmol/L.The mRNA levels of ALP,OC and OPG in osteoblast co-intervened by brucine combined with the supernatant of MM cells were higher than those in supernatant of U266 cells,while the level of RANKL mRNA was lower (P <0.05).The degree of increasing or reducing was greater than the level of control group intervened only by bortezomib (P <0.05).ConclusionThe therapeutic effects of brucine on MM might be carried out through the regulation of osteoclast by osteoblast,and the experiment confirmed that the therapeutic effect of brucine on MM was superior to that of bortezomib.
3.Diffuse Panbronchiolitis and Its Clinical Analysis

OBJECTIVE To highlight the characteristics of diffuse panbronchiolitis(DPB) in community hospital.METHODS One patient with DPB confirmed by clinic was described and relevant literatures were reviewed.RESULTS DPB was a chronic lower respiratory tract disease common in Japanese,rare in China,characterized by infiltration of inflammatory cells around bronchioles.Symptoms included cough,expectoration,and dyspnea after exercises.Pseudomonas aeruginosa was isolated from sputum in some cases.Some patients had the history and X-ray manifestation of sinusitis.The reason for a high misdiagnosis rate was due to unknown to its clinical findings and its diagnosis standard among doctors in community hospital.Chest high resolution CT(HRCT) could be attributed to the diagnosis of diffuse panbronchiolitis.The early treatment with macrolides was satisfied.CONCLUSIONS The patients with the history of chronic cough,expectoration and sinusitis,should be diagnosed.Long term,low dose macrolide therapy may improve the prognosis through an anti-inflammatory effect.
4.Drug Resistance and Pathogens in Critically Ill Patients with Pulmonary Infection

OBJECTIVE To investigate the pathogens distribution and their antibiotic resistance in critically ill patients with pulmonary infection.METHODS Totally 171 isolates from sputa were studied and the drug sensitive test was analyzed in 103 critically ill patients.RESULTS These results showed that 29 G+ bacteria strains occupied 17.0%,there were 15 Staphylococcus aureus strains,11 S.epidermidis strains and 3 Enterococcus strains,respectively.In 107 G-bacteria strains occupied 62.6% were composed Stenotrophomonas maltophilia,Acinetobacter baumannii,Burkholderia cepacia and Klebsiella pneumoniae,and 35 strains were fungi,accounted for 20.5%,the main fungi were Candida albicans and C.tropicalis.The result of drug sensitive test showed that there were high rates of multiple drug resistance in most bacteria strains,but there were different drug resistance rates to different antibiotics.CONCLUSIONS G-bacilli play an important role in the pulmonary infections of these critically ill patients,the opportunistic pathogens,such as S.maltophilia are the main G-pathogens,and there are high percentages of drug resistance.The other bacteria such as S.aureus,K.pneumoniae,etc,have a multidrug resistance.Inspecting pathogens and studying the trend of their drug resistance are very important in reducing drug abuse and decreasing the rate of infection in hospital.
5.Prevention of radiative Rhinitis in Patients with Nasopharyngeal carcinoma by psychological Intervention combined with cod liver oil ointment

Objective To investigate the preventive effect of psychological intervention combined with cod liver oil ointment on radiation nasal inflammation in nasopharyngeal carcinoma patients. Methods 90 patients with nasopharyngeal carcinoma treated in our hospital from January 2015 to December 2016 were randomly divided into study group and control group. The control group was treated with nasopharyngeal irrigation. The study group was given psychological intervention combined with cod liver oil ointment. Results The treatment effect of the study group was significantly better than that of the control group, and the incidence of radiation rhinitis was significantly lower than that of the control group. There was no significant difference between the two groups before the endoscopic score, the CT score of the sinus and the cilia transmission rate Improved, the study group was significantly better than the control group(P<0.05); before treatment, the two groups of patients with no significant differences in clinical symptoms, after treatment have been improved, the study group was significantly better than the control group(P<0.05). Conclusion The application of psychological intervention combined with cod liver oil ointment in radiotherapy of nasopharyngeal carcinoma is beneficial to prevent radiation rhinitis and improve the function of sinus function.
6.Immunal rejection not occurred after intracerebral transplantation of heterogenous neural stem cells in rats

ObjectiveTo investigate whether intracerebral transplantation of heterogenous neural stem cells(NSCs) may cause immunal rejection or not.MethodsNSCs derived from the brain of embryonic 16-day Wistar rat were cultured in vitro. Cerebral ischemic SD rat models were made by middle cerebral artery occlusion and NSCs were transplanted into the ipsilateral or contralateral caudate nucleus 4 days later. Rats were sacrificed at different time points after transplantation, the reaction and proliferation of mononuclear phagocytic system(MPS) and splenic follicles as well as the survival and migration of NSCs were checked.ResultsNumerous NSCs survived and migrated toward the lesion in vivo. Comparing with control group that did not receive NSCs transplantation, there was no significant infiltration of mononuclear phagocytes or proliferation of splenic follicles in NSCs transplanted rats.ConclusionsIntracerebral transplantation of heterogenous neural stem cells did not cause remarkable immunal rejection.
7.Effect of soluble vascular endothelial growth factor receptor-1 on atherosclerotic plaque

BACKGROUND: Angiogenesis in atherosclerotic plaque occurred on the basis of atherosclerotic lesions, and the new formed blood vessels promoted the development of angiogenesis. Soluble vascular endothelial growth factor receptor 1 (sFlt-1) gene transfection reduces neointimal formation after vascular injury in rabbits, also reduces early vascular inflammation and proliferation, and the formation of neointima lately.
OBJECTIVE: To observe the effects of highly expressed sFlt-1 on atherosclerotic plaque.
METHODS: A total of 48 male New Zealand rabbits were divided into four groups randomly: normal control group (n=8), sham operation group (n=8), bal oon injury+high lipid diet+pEGFP-N1 group (n=16) and bal oon injury+high lipid diet+pEGFP-N1-sFlt-1 group (n=16). Normal control group and sham operation group were supplied with normal diet. Bal oon injury+high lipid diet+pEGFP-N1 group and bal oon injury+high lipid diet+pEGFP-N1-sFlt-1 group were supplied with high lipid diet and injured by bal oon 2 weeks later. Bal oon injury+high lipid diet+pEGFP-N1 group was transfected with pEGFP-N1 and bal oon injury+high lipid diet+pEGFP-N1-sFlt-1 group was transfected with pEGFP-N1-sFlt-1.
RESULTS AND CONCLUSION: In the bal oon injury+high lipid diet+pEGFP-N1 group and bal oon injury+high lipid diet+pEGFP-N1-sFlt-1 group, blood lipid levels apparently increased. At 3 days after model establishment, sFlt-1 expression levels noticeably increased, and atherosclerotic plaque formed to different degrees. Plaque area, plaque perimeter, plaque maximum thickness and the number of positive-cel s within the plaque were significantly less in the bal oon injury+high lipid diet+pEGFP-N1-sFlt-1 group than in the bal oon injury+high lipid diet+pEGFP-N1 group. These results confirm that sFlt-1 gene may express effectively in iliac artery wal of rabbit, inhibited the formation of atherosclerotic plaque and delayed the development of atherosclerotic plaque.
8.Pathogenic Bacteria Isolated from Subglottic Secretion in 43 Cases with Ventilator-associated Pneumonia: Their Distribution and Drug Resistance

CONCLUSIONS The pathogenic bacteria from VAP subglottic secretion are mostly Gram-negative rods, in which the dominant bacteria are A. baumannii, P. aeruginosa, S. maltophilia, K. pneumoniae and Escherichia coli. Most of them are multi-drug resistant. The kinds of pathogenic bacterial strains in subglottic secretion and lower respiratory tract secretion are not only consistent but also similar to each other in terms of composition and drug resistance.
9.The Biological Characteristics of IL-2 Gene Modified Tumor Cells

In this experiment, we introduced human interleukin-2(lL-2)gene and Neomycin resistance (Neo~R) gene into a human lung adenocarcinoma cell line GLC, using a retroviral vector PLXSN. The positive cells obtained by selecting in G418 were compared with the wild-type cells. We observed their growth characteristics in vitro and the quantity of IL-2 in the culture supernatants. PCR result demonstrated the integration of foreign genes into tumor genome DNA after transfection and permanent existence. Under electron microscope, what can be seen was the surface villi of trans-duced-cells are less and shorter. It deserves further investigation to determine whether the change of cell surface influence metastatic ability of cells.
10.Chemical components of Rumex gmelini (Ⅳ)

Objective To study the chemical constituents from the root of Rumex gmelini.MethodsThe compounds were isolated and purified by chromatography,polyamide column chromatography,and preparation HPLC etc.Their structures were elucidated by physicochemical and spectroscopic evidences.Results Ten compounds were identified as:nepodin (Ⅰ),emodin (Ⅱ),citreorosein (Ⅲ),chrysophanol 8-O-?-(6'-acetyl) glucopyranoside (Ⅳ),chrysophanol 8-O-?-D-glucopyranoside (Ⅴ),resveratrol (Ⅵ),9,9'-dianthranone-2,2'-dimethyl-5,5'-bis (?-D-glucopyranose)-9,9',10,10'-tetrahydro-4,4'-dihydroxy-10,10'-dioxo (trivial name:rumoside A) (Ⅶ),emodin-8-O-?-D-glucopyranoside (Ⅷ),resveratrol-3-O-?-D-glucoside (Ⅸ),and rutin (Ⅹ).Conclusion Compound Ⅶ is a new compound,named rumoside A.Compounds Ⅴ,Ⅷ,and Ⅹ are separated from R.gmelini for the first time.Compounds Ⅲ and Ⅳ are the compounds which have been found in the plants of Rumex L.for the first time.
