1.The prevention and care of the early stage serious complication caused by hypertensive cerebral hemorrhage

Objective To research into the prevention and care of the early stage serious complication caused by hypertensive cerebral hemorrhage,aiming at reducing the death rate and disability rate caused by the illness,and raising the survivors living quality.MethodsBy analysing the serious complication of 90 hyertensive cerebral hemorrhage patients confirmed by CT and the information of prevention and care.Results85 patients had high pressure in the cranial cavity;8 patients had brain hernia;18 patients had heart failure;2 patients had myocardial infarction;8 patients had brain and heart syndrome;3 patients had lung oedema;14 patients had lungs infected;22 patients,kidneys didn,t funtion fully.20 patients bleed in the alimentary canal;38 patients had high blood sugar.51 patients survived,and 39 patients died.The death rate caused by the illness was 43.3%.ConclusionThe incidence of the hypertensive cerebral hemorrhage is high.Has much complication the death rate and disability rate caused by the illness are high.So we should pay attention to the prevention and care of early stage serious the complication of hypertensive cerebral hemorrhage,Control the blood pressure effectively to reduce the serious complication,Death rate and disability rate and improve the prognoses.
2.Assessment of the reliability and validity of the effect evaluation index system for health education of coronary heart disease

Objective To assess the reliability and validity of the effect evaluation index system for health education of coronary heart disease.Methods A total of 306 patients with coronary heart disease in four third-grade and A-level hospitals extacted by convenient sampling method were surveyed.Project analysis,exploratory factor analysis,correlation analysis and so on were applied to analyze and select items.Then the Cronbach's alpha coefficient,test-retest reliability,and content validity of the scale were tested.Then the original system was modified and the eventual system was formed.Results The scale was consisted of 8 first-dimensions and 35 indexes.The Cronbach's α of the total system was 0.832,and the α of subscales were from 0.629 to 0.882.The test-retest reliability of the total system was 0.966,and the testretest reliability of subscales were from 0.848 to 0.942.The S-CVI/Ave of system was 0.99,and the S-CVI/UA of indexes was 0.91.The Pearson correlation coefficient of the indexes with their dimensions were from 0.574 to 0.916,which were higher than the coefficient of the indexes with other dimensions.Factor analysis supported the eight-factor structure of the system,which could explain 65.61% of the total variance and the factor loading of each index were from 0.430 to 0.938.Conclusions The system is reliable and valid,and can provide a unified reference standard for effect evaluation of health education of coronary heart disease.
3.Construction of the effect evaluation index system for health education of coronary heart disease

Objective To establish a scientific,standard and comprehensive effect evaluation index system for health education of coronary heart disease.Methods Based on KABP (knowledge,attitude,belief,practice,KABP) model of health education,the effect evaluation index system for health education of coronary heart disease was formulated by documentary analysis,qualitative research,questionnaire survey,Delphi method,and analytic hierarchy process.Results The authoritative coefficient,determination coefficient and familiarity coefficient was 0.89,0.91,0.86,respectively.The coordination coefficients of the first-dimension and the second-dimension were 0.295 and 0.302,respectively.The index system consisted of 6 first-dimensions,45 second-dimensions.The weight coefficient of health knowledge,health belief,health behavior,physical signs,quality of life,and satisfaction degree of health education was 0.153 8,0.153 8,0.366 1,0.086 3,0.153 8,0.086 3,respectively.Conclusions The effect evaluation index system for health education of coronary heart disease is reliable and valid,which can provide a quantitative basis for effect evaluation of health education.
4.Effects of Multidisciplinary Rehabilitation on Early Heterotopic Ossification of Patients with Traumatic Elbow Joint Iinjury

Objective To observe the effects of multidisciplinary rehabilitation on early heterotopic ossification of patients with traumatic elbow joint injury.Methods 80 work injury patients of early heterotopic ossification after traumatic elbow joint injury were randomly divided into the experiment group(42 cases) and control group(38 cases).The experiment group received multidisciplinary rehabilitation,including physical therapy,occupational therapy,physical factor therapy,traditional Chinese medicine,ward rehabilitation and vocational training.The control group was treated with pain killer and diminish inflammation medicine and median frequency electrical therapy.Range of motion(ROM) of joint and visual analogue scale(VAS) were separately assessed before and after 3 months treatment.Results After 3 months treatment,ROM and VAS of the experiment group were significantly changed compared with admission(P<0.01).The effectiveness of the experiment group was superior to that of the control group(P<0.05).After discharge,39 cases returned to primary work,3 cases changed work in the experiment group,and after 1 year follow-up,no case needed to receive operation.Conclusion In the early stage of heterotopic ossification after traumatic elbow joint injury,multidisciplinary rehabilitation can significantly relax clinical signs,improve functions and make the work injury patients return to work.
5.Redution of false positivities of IgM antibodies against hepatitis E virus by a truncated immunodominant polypepfide of HEY open reading frame 2

Objective To exclude false positivities in detection of IgM antibodies against hepatitis E vires of genotype 4 (HEV-4) using a truncated immunodominant polypeptide of HEV open reading frames (ORF2). Methods The recombinant ORF2 immunodominant polypeptide corresponding to amino acids (AA) 459-607 and a truncated polypeptide corresponding to AA 472-607 were separately applied to coat ELISA plates. Anti-HEV IgM from 35 serum samples with HEV RNA positive, 69 serum samples from healthy individuals and 117 clinically suspicious HEV RNA positive serum samples was detected by an indirect ELISA and was confirmed by western blot in protein level and RT-PCR detecting in RNA level. Results Western blot analysis showed that the sera from HEV patients reacted with the dimmer of peptide 459-607, but they didn't react with the monomer and peptide 472-607. The ELISA showed that all 35 serum HEV RNA positive samples reacted with peptide 459-607 but not with peptide 472-607 and none of the 69 serum samples from healthy individuals reacted with either polypeptide. Among 117 chnically suspicious HEV RNA serum samples, 5 samples reacted simultaneously with both polypeptides. But the difference between 450 nm absorbance (A450) value was less than 0. 5. Western blot analysis demonstrated that all the 5 serum samples were anti-HEV IgM- negative. The 5 serum samples was detected negative by RT-PCR, indicating that the false pesitivities were caused by non-specific absorption. Conclusions ORF2 peptide 459-607 may be used to detect anti-HEV lgM efficiently. The false positivities caused by non-specific absorption can be largely excluded according to the difference between 45Ohm absorbance (A450) value when serum reacts with both polypeptides.
6.Characterization of the homodimer and antigenicity of ORF2 polypeptides of genotype 4 hepatitis Evirus

Objective To characterize the dimerization and the antigenicity of the ORF2 polypep-tide of hepatitis E virus (HEV, genotype 4). Methods HEV ORF2 gene was cloned from the serum of a patient with hepatitis E. The genotype was determined by sequencing. Three ORF2 polypeptides differing in size and other polypeptides with point mutations were produced in E. coli. The recombinant polypeptides were purified and analyzed by SDS-PAGE and Western blot. Results The ORF2 polypeptide containing 459-607 amino acid formed homedimer even in 8 mol/L urea. The truncated polypeptides containing amino acid 472-607 or 459-594 formed monomer only. The mutations at amino acid 562 or 595 disrupted the ho-modimer, whereas the mutations at amino acid 476 or 580 did not. Anti-HEV from hepatitis E patients only reacted with the homodimer form of the polypeptide 459-607 and did not react with monomer or tnmcated pol-ypeptides. Conclusion The amino acid 459-607 of HEV ORF2 is essential for dimerization of the ORF2 polypeptide. Residues at amino acid 562 and 595 are critical for the dimerization. The antigenicity of the polypeptide 459-607 mainly depends on its homodimer form.
7.The role of spatio-temporal image correlation with tomographic ultrasound imaging in the prenatal diagnosis of conotruncal defects

Objective To investigate the application value of spatio-temporal image correlation (STIC) combined with tomographic ultrasound imaging(TUI) in the prenatal diagnosis of conotruncal defects(CTD).Methods Two-dimensional(2D) fetal echocardiography to screen and TUI-STIC volumes from 1508 cases of fetuses of high risk with congenital heart disease.Postnatal work-up and pathological results were available for all fetuses with CTD.Results Thirty nine cases with CTD were found by TUI-STIC while thirty five cases were found by 2D echocardiography,but TUI-STIC had new findings and corrected the diagnosis in 9 cases as compared with 2D echocardiography.The sensitivity,specificity,positivity predictive value,negative predictive value and accuracy of TUI-STIC in evaluating CTD were 97.5 %,100%,100 %,99.9 % and 99 %.The Kappa value of consistency test between 2DE and TUI-SIC was 0.244(P < 0.01),McNemar test showed that the difference was statistically significant (P < 0.01).Conclusions TUI-STIC allows a complete sequential analysis of fetal conotruncal defects and supplying additional information over 2D fetal echocardiography,it could improve the prenatal diagnosis rate.TUI-STIC is helpful in diagnosis of prenatal conotruncal defects.

Objective To investigate the quality control and quality assurance of the SPECT/CT system.Methods The energy peak,energy resolution capacity and the intrinsic uniformity of the device's detector were regularly examined, and the quality control was performed. Results The daily hardware maintain could reduce the rate of system's trouble. The device's energy peak,energy resolution capacity were consistent during half year's observation period. The two detector's intrinsic uniformity were better after calibration than before. [ detector Ⅰ: ( 2.71 ± 0.28 ) vs (2.37±0.11)(t=2.489,P<0. 05);detector Ⅱ:(2.68 ±0.12)vs(2.38 ±0. 19)(t =6.421,P <0.01) ] .Conclusion Regular quality control and maintain could keep the function stabilization,enhance the availability rate of the SPECT/CT system.
9.Characterization of intertype specific epitopes on adenoviruses hexon.

BACKGROUNDTo characterize the intertype epitopes on human adenovirus (HAdV) hexon.
METHODSBased on computerized analysis on adenoviruses sequence of genomic alignment, antigenicity prediction and 3-D structure characteristics of hexon subunit, several peptides of hexon of adenoviruses were chosen to be synthesized or recombinant proteins of the hexon were expressed in E. coli by use of PGEX-5X. To identify the existence of intertype epitopes, the antisera raised with synthetic peptides or purified recombinant proteins were analyzed with Western blot and immunofluorescent assay.
RESULTSThe results of Western blot indicated that both peptide and recombinant antibodies showed specific reactivities with hexons of HADv-3, 4, 7 individually. Meanwhile, typical stain of immunofluorescence was found on HeLa cells infected with these HAdV by incubation with peptide as well as recombinant antibodies. Also, antibodies raised against peptide recognized the recombinant hexon protein in which a corresponding region of peptides was covered.
CONCLUSIONSMost of the predicted intertype epitopes of HAdV hexon wer e exclusively found in synthetic peptides and recombinant proteins. These intertype epitopes showed to be continuous and sequential which could be employed for development of antibodies of diagnostic use.

10.ARTERIAL SUPPLY OF THE PAPILLARY MUSCLES OF THE LEFT VENTRICLE

1.The external features and arterial supply of the papillary muscles of the leftventricle were studied in 54 human,52 dogs' and 110 rabbits' hearts.Radiopaquemedium or Chinese ink were injected into coronary arteries.After injection,arter-iography of some hearts were taken,and serial celloidin sections of the papillarymuscles of other hearts were made.The diameters of the papillary arteries and thedensity of the vessels were measured.2.The papillary muscles could be divided into three patterns,depending on theextent of their attachment to the ventricular wall and the relative length of theirfree part protruding into the ventricular cavity.The three patterns were as follows:(1)The attached type,the papillary muscle was largely adherent to the sucbjacentventricular myocardium,with only one third or less of its length protruding into theventricular cavity(34% in human hearts,100% in dogs' and 38% in rabbits').(2)The free type,the free end of the papillary muscle was one half of its length ormore(28% in human hearts,10% in rabbits').(3)The intermediate type,thelength of the free protruding part of the papillary muscle was intermediate between(1)and(2)(38% in human hearts,52% in rabbits').3.In the human and the dogs' hearts the anterolateral papillary muscle receivedbranches from the anterior descending artery and the diagonal left ventricularbranches or the left circumflex artery;while in the rabbits' it received branchesmainly from the anterior branches of the left ventricular artery.The posteromedialpapillary muscle received a variable supply from the left circumflex artery and orthe branches of the right coronary artery in the dogs' hearts and in the human hearts,while in the rabbits' hearts,it mainly received branches from the posteriorbranches of the left ventricular artery.4.The arrangement of the arterial vasculature of the papillary muscle seemedto be related to the different patterns of the papillary muscles.The free typereceived a large central artery which coursed through the entire papillary muscle toits apex(87.5%).The attached type had a segmental supply of 3~9 long penetrat-ing intramyocardial vessels(94.29%).The intermediate type had a combination ofboth types of vascular arrangement(98.5%).5.The average diameter of the central arteries was 617.50 ? in human hearts atits entry into the base of the papillary muscle,and 236.05 ? in rabbits.The dia-meters of the segmental arteries were 323.60 ? in human hearts,300.05 ? in dogs',121.78 ? in rabbits' respectively.The diameters of the capillary vessels were 5~8 ?in human hearts,3~6 ? in dogs',2.5~6 ? in rabbits' respectively.The numbersof capillaries that observed in a length of 200? were 15.45 in human hearts,16.2in dogs',19.3 in rabbits'.The specific values of the density of vessels per unitarea were 34.4% in human hearts,34.1% in dogs',56% in rabbits'.
