1.Preservation of parotid masseter fascia and sternocleidomastoid muscle flap in parotid gland surgery

Objective To evaluate the clinical effect of preservation of parotid masseter fascia and sternocleidomastoid muscle flap in preventing facial concave deformity and Frey’ s syndrome. Methods 110 patients with parotid gland benign tumor were selected and ran-domly divided into treatment group(56 cases) and control group(54 cases). The treatment group was filled with sternocleidomastoid muscle flap at once and remained parotid masseter fascia in operation. The control group got parotidectomy only. All the cases were followed-up. Re-sults All the cases were successful during a follow-up period of 3~24 months. The treatment group was better than the control group at pre-venting facial concave deformity and Frey’s syndrome (P<0. 01). Conclusion Preservation of parotid masseter fascia and sternocleido-mastoid muscle flap can be the priority selection in improving facial concave deformity and preventing Frey’ s syndrome.
2.The prevention and care of the early stage serious complication caused by hypertensive cerebral hemorrhage

Objective To research into the prevention and care of the early stage serious complication caused by hypertensive cerebral hemorrhage,aiming at reducing the death rate and disability rate caused by the illness,and raising the survivors living quality.MethodsBy analysing the serious complication of 90 hyertensive cerebral hemorrhage patients confirmed by CT and the information of prevention and care.Results85 patients had high pressure in the cranial cavity;8 patients had brain hernia;18 patients had heart failure;2 patients had myocardial infarction;8 patients had brain and heart syndrome;3 patients had lung oedema;14 patients had lungs infected;22 patients,kidneys didn,t funtion fully.20 patients bleed in the alimentary canal;38 patients had high blood sugar.51 patients survived,and 39 patients died.The death rate caused by the illness was 43.3%.ConclusionThe incidence of the hypertensive cerebral hemorrhage is high.Has much complication the death rate and disability rate caused by the illness are high.So we should pay attention to the prevention and care of early stage serious the complication of hypertensive cerebral hemorrhage,Control the blood pressure effectively to reduce the serious complication,Death rate and disability rate and improve the prognoses.
3.Clinical Observation and Influence Factor Analysis on Treatment of the Aged COPD with Acute Respiratory Failure by Using Harmless Bi-level Positive Airway Pressures

Objective:To evaluate the clinical efficacy and analysis influence factors on treatment of the aged COPD with acute respiratory failure by using harmless bi-level positive airway pressure. OBJECTIVE: To compare safety and efficacy between anterior cervical decompression and fusion procedures as well as compound titanium rete implantation for the treatment of cervical spondylotic myelopathy using Meta analysis. DATA SOURCE: The relative articles were retrieved from Chinese Biomedical Literature Database (CBM) from January 1998 to December 2008, CNKI from January 1994 to December 2008, and VIP from January 1989 to December 2008. DATA SELECTION: A clinically randomized controlled study was used to evaluate safety and efficacy between anterior cervical decompression and fusion procedures as well as compound titanium rete implantation for the treatment of cervical spondylotic myelopathy. Control or vertebroplasty was excluded. MAIN OUTCOME MEASURES: JOA score, complication, and bone fusion rate. RESULTS: Six clinically control studies were included in the final analysis containing 443 patients. Among the patients, 190 cases were treated with anterior cervical decompression and fusion procedures alone, and 253 with anterior cervical decompression and fusion procedures as well as compound titanium rete implantation. 5.Clinical Analysis of Antiglaucomatous Operations on 266 Eyes

The near therapeutic effect of antiglaucomatous operations of various kinds on 266 eyes of 194 patients with various types of glaucoma were evaluated. 82 eyss of 61 patients out of those were followed up from 10 to 193 months(an average of 69 months) and thus the remote therapeutic effect was observed in these cases. In general the near operative results of this series was similar to those of the concurrent reports by other authors, but the remote effect was not satisfactory. Invariably, the therapeutic effect deteriorated as the time went by. The possible factors which led to the deterioration of the therapeutic effect were discussed by the author.Altogether 19 kinds of operative methods were used and four of them, Scheie's operation, subscleral sclerenclesis, interlamellar Scheie's operation and trabe-culectomy were the main methods used. It was found that trabeculectomy resulted in high percentage of success in controlling the intraocular pressure and brought about rather few complications. It was suggested that trabeculectomy be the operation of choice for the treatment, of glaucoma.

Objective To explore the clinical values of the serum homocysteine (Hcy) and platelet aggregation rate in type 2 dia‐betes patient with cerebral infarction .Methods Totally 65 cases of type 2 diabetes patients with cerebral infarction ,55 simple type 2 diabetes patients and 50 cases who took health examination were enrolled in the study .The serum homocysteine and platelet ag‐gregation rate were detected ,the blood pressure ,weight and height of the patients were measured .The multi‐factor Logistic regres‐sion analysis was used to detecting the risk factors in type 2 diabetes patients with cerebral infarction .Results The serum homo‐cysteine level and platelet aggregation rate were significantly higher in diabetes patients with cerebral infarction than those simple diabetes patients or healthy group (P< 0 .05) .The levels of serum homocysteine and platelet aggregation rate were positively corre‐lated with the severity of diabetes patients with cerebral infarction .Conclusion Serum homocysteine and platelet aggregation rate are much higher in type 2 diabetes patients with cerebral infarction ,which indicated that we can predict the process of the cerebro‐vascular disease through detecting the serum homocysteine and platelet aggregation rate .
8.Advances in genetic study of primary open angle glaucoma

Primary open angle glaucoma (POAG) is a complex and heterogeneous neurodegenerative disease caused by genetic and environmental factors,and it is also one of the leading causes of irreversible blindness worldwide.Recent molecular genetic studies revealed that numbers of susceptible gene variants are associated with POAG,and the researching technology includes genome wide association study and whole exon sequence.Studies of POAG families discovered 16 loci linked to the disease.To date,three genes were reported to be the causative genes of POAG,they are these studies MYOC,OPTN and WDR36.Other causative or presumably causative genes are thought to contribute to POAG,such as NTF4 and TBK1.Genetic factor for the path ogenesis POAG is being widely concerned,and provides a solid foundation for genetic research and gene therapy of this disease.In this paper,we reviewed a comprehensive discussion of the genetics and research strategies of POAG.
9.Coronary arterial remodeling and angiotensin Ⅱ type 2 receptor expression in ischemia-reperfusion injury rat after valsartan pretreatment

Objective To observe the expression of angiotensin Ⅱ type 2 receptor (AT2R) and coronary arterial remodeling in ischemia-reperfusion(I-R) injury rat after the renin -angiotensin blocker valsartan pretreatment .Methods 123 rats were random-ly divided into 3 groups :sham -operation group(sham) ,control group and valsartan group(ARB) .The control group and the sham group received the gavage of normal saline and the ARB group received valsartan gavage 10 mg/(kg · d) for 4 weeks before surger-y .The I-R injury rat model was established by thoracotomy for ligating the left anterior descending (LAD) coronary artery and re-moving the ligation after 30 min .The sham group was performed the thoracotomy without ligation .At the 4 timepoints of postoper-ative 3 ,7 ,14 ,28 d ,the left ventricular diastolic and systolic pressures were measured ,then the rats were killed for collecting the rat heart sample .The section was stained by sirius red .The collagen deposition in coronary arterial remodeling and coronary arterial pe-ripheral area ,and the dynamic change of location and expression of AT2R in the coronary artery were observed by the immunohisto-chemical streptavidinbiotin peroxidase complex (SABC) .Results The immunohistochemical analysis revealed that AT 2R was local-ized in the adventitia of coronary arteries with the radial distribution ,showing the higher density especially in large coronary arterial peripheral area ,the partial expression existed in the coronary arterai intima .The expression of AT2R was transient ,which on 7 d af-ter I-R operation in the control group reached the peak value ,while the expression peak value of AT2R in the valsartan group was higher and earlier than that in the control group .The heart diastolic function on 3 d after I-R operation was impaired ;the left ven-tricular end diastolic presure (LVEDP) in the valsartan group was significantly lower than that in the control group .The collagen deposition of coronary peripheral mesenchyma on postoperative 28 d in the valsartan group was significantly lower than that in the control group;the coronary peripheral collagen deposition in the control group was gradually increased with the time progression , which on postoperative 14 ,28 d was significantly higher than that in the sham group .Conclusion Valsartan could inhibit the coronary arterial remodeling for protecting the heart function possible by the transient high AT 2R expression after myocardial I-R injury .
10.Lentivirus-based RNAi and its application in the study of T lymphocytes

The technique of lentivirus-based RNAi ( RNA interference) is a combination of the effective infec-tion and integrating characteristics of lentivirus vector and the specificity of RNAi to silence a homologous gene expression. It caused the post-transcriptional gene silencing ( PTGS) by a specific target sequence of mRNA degradation.As a kind of commonly used target cells in gene therapy, T lymphocytes play an important role in graft-versus-host disease ( GVHD) and other diseases.It is of great significance for increasing the infection efficiency of the target gene to T lymphocytes on this kind of disease for gene therapy.In this paper, we summarize the application of lentivirus-based RNAi in T lymphocytes.
