Objective To evaluate the diagnostic value of multi-slice spiral CT(MSCT)perfusion imaging in chronic obstructive pulmonary disease(COPD).Methods Twenty COPD patients and20 volunteers underwent 8-row detector spiral CT(MSCT)perfusion imaging using cine scan mode with5 mm slice thickness.0.5 s rotation time and a total scan time of 45 s with 5 s intervals.60 ml contrast agent(300 mg I/ml)were administered at a rate of 4 ml/s from the forearm superficial vein. The imaging data were transferred to a workstation.A time-density curve and pseudo-color map were generated automatically with GE CT perfusion 3 software, the blood flow(BF),blood volume(BV),mean transit time(MTr)and permeability surface(PS)were measured. Results Time-density curve was flatter and the peak of the curve was obviously lower in COPD patients than the volunteers. The BF.BV.PS in COPD volunteers was(10.58 ±4.85)s and(4.50 ±1.71)s respectively. The BF,BV and PS in COPD patients Was lower than the volunteers, the MTY was higher(P<0.01).Conclusion MSCT perfusion imaging is helpful for the diagnosis of COPD.

Objective To analyze the phenotypes of lymphocytes in cerebrospinal fluid derived from the patients with neurosyphilis. Methods Samples of cerebrospinal fluid from 12 patients with neurosyphilis and 20 patients with latent syphilis were collected and analyzed by flow cytometry for CD4 and CD25 expression. Results There was a significant increase in the number of white blood cells in the cerebrospinal fluid of patients with neurosyphilis. FACS analysis showed that most leukocytes were lymphocytes predominated with CD4 + T cells in neurosyphilis patients which were almost 4 times more than that in latent syphilis. However, there was a significant decrease in the proportion of CD4+ CD25high regulatory T cells (Tr) in neurosyphilis patients compared with that in latent syphilis patients. Conclusion A dramatic increase in CD4+ T cell frequency suggested its pathogenic role in neurosyphilis, whereas a decrease in CD25high Tr frequency indicated that CD4 + CD25high Tr cells might play an important role in immune homeostasis of central nervous system.

Objective To assess the clinical presentations and treatment of neurosyphilis with mania as the first manifestation. Methods A retrospective study was performed. Clinical data on neurosyphilis patients with mania as the first manifestation collected from July 2009 to June 2010 were analyzed. Results Twenty cases of neurosyphilis were included in this study, which were all misdiagnosed as schizophrenia, anxiety, cerebral infarction, etc. All the patients had manic symptoms at onset, such as irritability, bad temper, impulsive behavior, disturbance in thinking, and so on. Some patients also suffered from a marked decrease in memory, calculation and cognitive ability. Rapid plasma reagin (RPR) test, Treponema pallidum hemagglutination (TPHA)test and cerebrospinal fluid (CSF) venereal disease research laboratory (VDRL) test were positive in all the patients. Varying degrees of symptomatic improvement was achieved after anti-syphilis and anti-psychotic treatment. CSF was retested in 13 patients 3 months after the end of treatment, and CSF VDRL titer decreased in 10 patients, remained unchanged in 2 patients, and turned negative in 1 patient. Conclusions Neurosyphilis lacks distinctive clinical characteristics. For patients with poor response to conventional antipsychotic therapy, neurosyphilis should be considered, and serology and cerebrospinal fluid tests for syphilis are warranted.

Objective To observe the intervention effects of Xintongfang on the expression of P-selectin (PS), P-selectin glycoprotein ligand-1 (PSGL-1), platelet-leukocyte aggregates (PLA) and platelet-monocyte aggregates (PMA) in patients of coronary heart disease with carotid artery plaque. Methods Sixty patients were randomly divided into Xintongfang group and the control group, with 30 cases in each group. Xintongfang group was given Xintongfang, the control group was given aspirin and atorvastatin calcium for two months. The expression of PS, PSGL-1, PLA and PMA were tested by flow cytometry before and after treatment. Results The expression of PS, PSGL-1, PLA and PMA in two groups were reduced (P<0.01). Xintongfang group had more obvious effects on the expression of PLA and PMA than the control group (P<0.05). Conclusion Xintongfang can reduce the degree of inflammatory in patients of coronary heart disease with carotid artery plaque by inhibiting platelet-leukocyte interaction.

OBJECTIVE:
To delineate cytogenetic and molecular abnormalities of a fetus carrying a de novo 46,X,der(X),t(X;Y)(p22.3;p11.2).
METHODS:
G-banded karyotyping and next-generation sequencing (NGS) were used to analyze the fetus, his father and sister. Single nucleotide polymorphism-based arrays (SNP-array), multiple PCR and fluorescence in situ hybridization (FISH) were utilized to verify the result.
RESULTS:
G-banded karyotyping at 320 bands showed that the fetus had a normal karyotype, while NGS has identified a 3.58 Mb microdeletion at Xp22.33 and a Y chromosomal segment of about 10 Mb at Yp11.32p11.2. With the sequencing results, high-resolution karyotyping at 550-750 bands level has determined the fetus to be 46,X,der(X)t(X;Y)(p22.3;p11.2). The result was confirmed by PCR amplification of the SRY gene, FISH and SNP-array assays. The karyotypes of his father and sister were both normal. His sister also showed no amplification of the SRY gene, and her NGS results were normal too, suggesting that the karyotype of the fetus was de novo.
CONCLUSION
Combined karyotyping, NGS, SNP-array, PCR and FISH assay can facilitate diagnosis of XX disorder of sex development.

Objective To investigate the possible brain diffusion changes in different sites of the brain, particularly in areas associated with vision function,of patients with proliferative and nonproliferative retinopathy by using diffusion-weighted imaging (DWI).Methods Forty-five type 2 diabetic patients, admitted to our hospital from March 2012 to March 2013,were chosen in our study;according to the fundus examination, they were divided into group of proliferative retinopathy (n=15) group of non-proliferative retinopathy (n=15) and group of no retinopathy (n=15); another 15 healthy controls were chosen.T2WI and DWI with 3.0T magnetic resonance imaging (MRI) were performed; the apparent diffusion coefficient (ADC) values in the orbitofrontal cortex, cingulated gyrus, thalamus, dorsomedial and dorsolateral frontal cortex, and corona radiate were calculated; and then,Spearman's correlation analysis was performed between ADC values and both course of disease and glycosylated hemoglobin level. Results The ADC values of orbitofrontal cortex, cingulated gyrus and visual cortex in groups of proliferative and non-proliferative retinopathy were significantly increased as compared with group of no retinopathy and control group (F=95.268,P<0.05); the ADC values of orbitofrontal cortex, cingulated gyrus and visual cortex in group of proliferative retinopathy were significantly increased as compared with those in group of non-proliferative retinopathy (P=0.004,0.001 and 0.009,respectively);no significant difference of them was noted between group of no retinopathy and control group (P>0.05).The mean ADC values of orbitofiontal cortex, cingulated gyrus and visual cortex was positively correlated to the duration of disease (r=0.567 and P=0.001; r=0.491 and P=0.01; r=0.428 and P=0.003) and glycosylated hemoglobin level (r=0.336 and P=0.014; r=0.296 and P=0.049; r=0.370 and P=0.012).Conclusion Increased ADC values of visual center (orbitofrontal cortex, cingulated gyrus and visual cortex) conform the association between diabetic retinopathy and brain functioning impairment; DWI may serve to assess subclinical neurogical involvement in diabetic retinopathy, even when brain structural changes are absent.
