1.Wound healing effects of silk fibroin-bone morphogenetic protein-2 scaffolds on inflammatory pulp in rats

Objective:To identify the healing effect of electrospun silk fibroin-BMP-2 as a biologic pulp capping agent to inflammatory pulp in rat caused by lipopolysaccharide ( LPS) .Methods:A total of 30 healthy adult male Wistar rats were randomly divided into five groups:(1) normal control group without operation;(2) blank control group without capping agents;(3) calcium hydroxide capping group;(4) electrospun silk fibroin capping group;(5) electrospun silk fibroin-BMP-2 capping group .Bilateral up-per first molars of each rat in group 2-5 were drilled to expose the pulp to LPS which was used to estab-lish a model of inflammatory pulp .The exposed pulp was capped with different capping agents or without capping agents.Then the hole was sealed.The animals were sacrificed on days 3, 7, and 14 post-opera-tion and histological analysis was carried out , including HE stain and CD 14 immunohistochemical stain . Results:On day 7 and 14 , the lowest inflammatory reaction score in HE stain among pulp capping groups was that of silk fibroin-BMP-2 group .The next were calcium hydroxide group and silk fibroin group .That of blank control group was the highest .The ranking of reparative dentine scores of those groups was just reversed.The D values of immunohistochemical stain of CD 14 were not significantly different in groups applied pulp capping agents but significantly lower than blank control group on days 3 and 7.However, the D value of silk fibroin-BMP-2 group ( 0 .145 ±0 .011 ) was significantly lower than blank control group (0.287 ±0.019), calcium hydroxide group (0.170 ±0.017) and silk fibroin group (0.175 ± 0 .018 ) on day 14 .Conclusion:Electrospun silk fibroin compounded with BMP-2 promoted wound hea-ling of exposed pulp and had better potential to stimulate formation of reparative dentine to establish a suitable environment for pulp recovery .
2.Expression and its significance of Her-2/neu protein and androgen receptor in human prostate cancer

Objective To observe the expression of Her-2/neu protein and androgen receptor (AR) in human prostate cancer and to evaluate their significances in the progression of prostate cancer. Methods The Her-2/neu protein and AR immunohistochemical stain were carried out in human prostate tissue microarray that consisted of prostate cancer (107 cases) and benign prostate tissue (42 cases). The prostate cancer cases were divided into 4 groups: group one (Gleason score 6),group two (Gleasonscore 7), group three (Gleasonscore 8) and group four (Gleasonscore 9) according to the Gleason score. The immunostains immunohistochemical stain were interpreted in two aspects of the staining intensity and the percentage of positive cells. The significance and relationships between the expression of Her-2/neu protein and AR in prostate cancer and benign prostate tissue (BPT) and the grouping of different Gleason scores of prostate cancer were then evaluated. Results The positive expression rate of Her-2/neu protein was significantly higher in prostate cancer tissue than in BPT [43.9%(47/107) vs. 14.3%(6/42), x2=11.562, P=0.009], and the positive expression intensity of Her-2/neu immunoreactivity was also higher (x2= 11.764, P=0.008). There were significant differences in positive expression intensity of Her-2/neu immunoreactivity among the different Gleason scores groups (x2 = 20. 512, P = 0. 015), and the expression intensity was significantly positively correlated with Gleason scores ( r= 0. 269, P = 0. 005). There was significant difference in AR immunoreactivity between in prostate cancer (67 %, 72/107) and in BPT (50 %, 21/42, x2 =3. 843, P=0. 050). Among prostate cancer cases, the positive expression intensity of AR was not significantly different among groups 1 through 4 (x2 = 4. 318, P = 0. 229), and was not significantly correlated with Gleason scores ( r = - 0. 065, P = 0. 505 ). Moreover, the positive expression intensity of Her-2/neu protein was not significantly correlated with that of AR (r = -0. 115, P=0. 237). Conclusions Overexpression of Her-2/neu protein in human prostate cancer tissue suggests that Her-2/neu may have some role in prostate tumorigenesis. Her-2/neu protein expression is positively correlated with Gleason score in prostate cancer, which suggests that Her-2/neu may be a potential prognostic predictor of prostate cancer.
3.Interaction on rifampicin and levofloxacin in retreatment in smear positive pulmonary tuberculosis

Objective To investigate the interaction between rifampin(RFP)and levofloxacin(LVFX)in retreatment of smear positive pulmonary tuberculosis(TB). Methods One hundred retreated smear positive TB patients admitted to our hospital between January 2013 and December 2015 were randomly divided into two groups:the control group in 50 patients with only RFP treatment,and the treatment group in 50 patients with both RFP and LVFX treatments. The RFP plasma concentration was tested 0.5 h,1 h,and 2 h after intake of medicine. Sputum smear test was performed at the end of February,May,August and September to investigate the interaction between RFP and LVFX in retreatment of smear positive TB. Results The plasma concentration of RFP did not show significant difference(P > 0.05)between the control group and the treatment group at 0.5 h,1 h,and 2 h after intake of medicine. As the treatment time went on,the level of smear positive gradually decreased. The sputum smear positive rate after 2,5,8,and 9 months of treatment showed significant difference between the control group and the treatment group. After 8 months of treatment,the ratio of stabilized patients and patients under treatment also showed significant difference(P<0.05). Conclusion Rifampicin and levofloxacin have no antagonism effect in retreatment of smear positive TB,and RFP and LVFX can be used together to treat the repeated smear positive TB.
4.Investigation of enterotoxin gene in clinical isolates of Staphylococcus aureus.

OBJECTIVETo detect the enterotoxin genes of Staphylococcus aureus (SA) isolated from clinical specimens and analyze the correlation between enterotoxin genes and drug resistance of SA.
METHODSThe mecA gene and enterotoxin genes A-F of clinical SA isolates were identified by polymerase chain reaction (PCR), and the genes were sequenced to investigate the correlation of these genes to drug resistance.
RESULTSThe detection rate of enterotoxin genes was 100% in 67 methicillin- resistant SA (MRSA), showing no significant difference from the rate in 57 methicillin-sensitive SA (MSSA) (83.5%, χ(2)=0.203, P>0.05). Of the 116 strains carrying enterotoxin genes (93.5%), the detection rates of SEA, SEB, SEC, SED and SEF were 90.5%, 6.9%, 61.3%, 5.2%, 25.9% and 93.5%, respectively, and none of the strains were positive for SEE gene. In these strains, 78 (67.2%) carried 2 or more enterotoxin genes, and the main genotypes were SEA and SEC (33.6%), SEA and SEF (7.8%), and SEA and SEC and SEF (13.8%). Compared with the strains carrying a single enterotoxin gene, those with multiple enterotoxin genes showed a higher drug resistance rate, among which 75% of the SA strains carrying SEA+SEC+SEF were resistant to SXT, significantly higher than the rates of SA carrying SEA (28.6%) and SEA+SEC (38.7%) (P<0.05). The SA strains carrying SEA+SEC+SEF and SEA+SEF showed significantly higher amikacin resistance rates than SA strain carrying SEA (75.0%, 77.0%, 21.5%, respectively, P<0.05).
CONCLUSIONClinical isolates of SA carrying multiple enterotoxin genes have a higher drug resistance rate than those with a single enterotoxin gene, suggesting the the important role of enterotoxin in multidrug resistance.

5.Meta-analysis of the Effects of National Essential Medicine System on the Utilization Rate of Hormone Drugs in Primary Medical Institutions

OBJECTIVE:To evaluate the effects of the implementation of National Essential Medicine System on the utilization rate of hormone drugs in primary medical institutions in China systematically.METHODS:Retrieved from CJFD,Wanfang database and VIP,the literatures about the use of hormone drugs were collected before and after the implementation of National Essential Medicine System.Meta-analysis was performed by using Stata 13.0 software after utilization rate dam extraction and quality evaluation with Cochrane system evaluator manual 5.1.0.RESULTS:A total of 20 literatures were included.Results of Meta-analysis showed that utilization rate of hormone drugs after the implementation of National Essential Medicine System was significantly lower than before implementation,with statistical significance [RD =-0.03,95 % CI (-0.05,-0.02),P< 0.001].Results of subgroup Meta-analysis of utilization rate of hormone drugs in different areas before and after the implementation of National Essential Medicine System showed that the utilization rate of hormone drugs in eastern areas was significantly lower after the implementation of National Essential Medicine System than before [RD=-0.06,95% CI(0.09,-0.03),P=0.001] with statistical significance.There was no statistical significance in the utilization rate of hormone drugs in middle area [RD=-0.02,95 % CI(0.06,0.02),P=0.235] or western area [RD=-0.01,95% CI (-0.02,0),P=0.122] before and after the implementation of National Essential Medicine System.CONCLUSIONS:The implementation of National Essential Medicine System reduces the utilization of hormone drugs in primary medical institutions.The effect of the eastern area is more obvious than in middle and western area.
6.Exogenous lipid pneumonia: a clinical analysis of 9 cases

The clinical data of 9 patients with exogenous lipid pneumonia confirmed by pathology were retrospectively analyzed. There were 4 males and 5 females, aged 57-79 years, with a history of intestinal obstruction and oral administration of liquid paraffin. The main clinical symptoms were cough, expectoration and fever (8 cases), and combined with wheezing (5 case); and crackles and rhonchi were revealed on chest auscultation in all patients. The right lung lower lobe was involved in all 9 cases, the left lower lobe, right middle and lower lobe were also involved in 6 case. The CT scan showed that infiltration (7 case) and consolidation (6 case) were the most common imaging features, followed by the septal thickening (5 case) and pleural effusion (4 case); the low density fat-like changes within the consolidative opacities was a specific feature of lipid pneumonia (3 case). The lung biopsy was performed in the target site through bronchoscope, and pathology showed the interstitial fibrous tissue hyperplasia with chronic inflammatory cell infiltration and adipocyte-like cells. Six patients were treated with glucocorticoid and repeated bronchoalveolar lavage with good response, one cases died and two cases discharged automatically. The clinical manifestation and imaging features of exogenous lipid pneumonia are lack of specificity; transbronchial lung biopsy is an effective method for the diagnosis. The repeated bronchoalveolar lavage with saline combined with medication of glucocorticoids are effective for the treatment of exogenous lipid pneumonia.
