1.Epidemic situation of emerging influenza virus A/H1N1 in 2009 pandemic and the prevention/control measures

Since the first reported case of swine influenza A in Mexico,a total of 15 510 cases have been confirmed in 53 countries by May 29,2009.On April 29,2009,the World Health Organization(WHO)raised its pandemic alert from grade 4 to grade 5.The virus is described as a new subtype of A/H1N1 and is not detected in pigs or humans previously.The virus is sensitive to oseltamivir and zanamivir,but resistant to amantadine and rimantadine.The genetics of the virus are so novel that humans are unlikely to have much immunity to it.The virus can transmit from human to human;therefore it is necessary to enforce quarantine measure for close contactors because the virus transmits during latency.Precaution methods like covering noses and mouths with a tissue when coughing or sneezing can reduce the transmission opportunity.Hands should be washed frequently with soap,especially after coughing or sneezing.Public places with ventilation conditions,personal health behavior and health condition are critical for the prevention and control of this epidemic.
2.Identification of zebrafish shield organizer-specific genes

Objective During zebrafish gastrulation, dramatic movements rearrange cells into three germ layers and contribute to the formation of the shield organizer, which acts as a dorsal signal center to pattern the body axis.Global identification of shield organizer-specific genes in early gastrulas will be valuable for studying the regulatory cascades during organizer formation and body axis establishment.Methods Tg( gsc:GFP) transgenic embryos express GFP in the shield organizer, which is controlled by a 1.8 kb gsc promoter.Flow cytometry technology and RNA deep sequencing analysis were applied to isolate GFP positive cells from the Tg( gsc:GFP) transgenic embryos and systematically uncover the genes highly expressed in the dorsal organizer.Subsequently, the expression of shield organizer-specific genes was further con-firmed by quantitative real-time PCR and whole mount in situ hybridization method during zebrafish embryonic develop-ment.Results GFP-positive cells exceeding 96%purity were isolated from shield-stage Tg(gsc:GFP) transgenic embryos and 657 organizer highly expressed genes were identified through RNA deep sequencing analysis.The results of in situ hy-bridization experiments revealed that a number of genes including KIAA1324, ripply1, twist2, isthmin1, nme4, zgc174153 and rrbp1b were expressed in shield organizer during zebrafish gastrulation.Conclusions The identification of these shield organizer-specific genes in the current study provides useful clues to explore the zebrafish developmental functions in further studies.
3.Effects of group psychotherapy on damage of theory of mind in patients with early-onset schizophrenia

Objective:To explore the effect of group psychotherapy on damage of theory of mind (TOM) in patients with early-onset schizophrenia.Methods:Totally 60 patients with early-onset schizophrenia were included and divided into the intervention group (n = 30) and the health education group (n = 30).The patients in the intervention group were offered 10 twice a week 2-hour sessions of group psychotherapy,and those in the control were offered group health education twice a week.All participants completed baseline measures and post-intervention measures with the Eye Emotion Recognition and Theory of mind Picture-sequencing Task (TOM-PST).Results:Totally 23 patients in the intervention group and 29 patients in the health education group finished the post-intervention measures.After 5-week intervention,subscores of sad and fear and total score of emotion recognition,total score of TOM-PST,understanding first order error belief,second order error belief,third order error belief,sense of reality,detecting fraud compared to baseline had statistically significant difference in intervention group.In health education group,subscores of joy emotion recognition,understanding first order error belief,third order error belief,total score of TOM-PST compared to baseline had statstically significant difference.The patients with group psychotherapy got higher scores of total score of TOM-PST and understanding second order error belief and third order error belief than the health education group after 5-week intervention (P < 0.05).Conclusion:This study suggests the group psychotherapy could partially improve theory of mind with early-onset schizophrenia,and promote the recovery of social cognition.
4.Identification of a zebrafish sat1 .a mutant

Objective In our previous study, we had generated various zebrafish mutant lines with tissue?specific GFP expression by Tol2 transposon?mediated insertional mutagenesis. Among these mutants,the Tol2:20141221t line ex?presses GFP in nervous system, while the position within zebrafish genome where transposon inserted has not yet been iden?tified. The aim of this study was to identify and analyze this genetically modified mutant. Methods The transgenic inser?tion loci in the genome of Tol2:20141221t line was identified byTAIL?PCR and the spatial and temporal expression profile of the affected gene was examined by in situ hybridization. Homozygous mutant of Tol2:20141221t was generated for explo?ring related developmental defects. Results Tol2 transposon was inserted into the 8th intron region of sat1.a gene, and in?duced premature transcription termination. The maternal and zygotic mutants of Tol2:20141221t was generated, while with?out apparent developmental defects. Conclusions We have generated and identified the zebrafishsat1.a mutant mediated by Tol2 transposon. This gene insertion mutant exhibits no obvious developmental abnormalities, but may serve as a power?ful tool to study the development of nervous system.
5.Quality evaluation of Bolbostemmatis Rhizoma by UPLC fingerprint combined with QAMS.

The present study was performed to establish the UPLC fingerprints of Bolbostemmatis Rhizoma and determine the contents of three saponins by quantitative analysis of multi-components by single marker(QAMS), and provide basis for quality evaluation of Bolbostemmatis Rhizoma. The analysis was carried out on an analytical column of Waters Cortecs T3(2.1 mm×100 mm,1.6 μm)with gradient elution by acetonitrile-0.1% phosphoric acid solution, at a flow rate of 0.3 mL·min~(-1). The detection wavelength was 203 nm, the column temperature was 30 ℃ and the injection volume was 1 μL. The UPLC fingerprints of Bolbostemmatis Rhizoma were established and evaluated by similarity calculation, cluster analysis and principal component analysis. The relative calibration factors of toberoside B and toberoside C were determined with toberoside A as internal reference. The content was calculated by relative calibration factors to develop a method of QAMS. Comparing the results of QAMS with those of ESM, the accuracy and feasibility of one-eva-luation and multi-evaluation can be determined. RESULTS:: showed that the fingerprints of 19 batches of Bolbostemmatis Rhizoma have four common peaks with similarities ranging from 0.754 to 1.000. Cluster analysis and principal component analysis classified 19 batches of Bolbostemmatis Rhizoma into three categories, which was consistent with the similarity evaluation results. The relative deviation between the content of tubeicosides B and C in 19 batches of Bolbostemmatis Rhizoma determined by QAMS and ESM is less than 5.0%, indicating that there was no significant difference between the two methods. Therefore, the UPLC fingerprints combined with QAMS and similarity evaluation can be effectively used to evaluate the quality of Bolbostemmatis Rhizoma.

6.Effect of percutaneous nephrostomy and ureteral stent drainage in the treatment of infectious hydronephrosis

Objective:To compare the therapeutic effects of percutaneous nephrostomy and ureteral stent drainage in the treatment of infectious hydronephrosis.Methods:From June 2017 to June 2018, 92 patients with infectious hydronephrosis in the First Hospital of Jiaxing were selected.The patients' hospital number was entered into the computer and the patients were divided into group A(percutaneous nephrolithotomy and drainage treatment, 46 cases) and group B(transurethral ureteral stent drainage treatment, 46 cases) by lottery.The treatment effect and safety were compared between the two groups.Results:There were no statistically significant differences in age, male/female, duration of disease, site of renal disease, primary disease, and underlying disease between the two groups(all P>0.05). There was no statistically significant difference in the success rate of disposable catheter placement between group A and group B(97.73% vs.100.00%, χ 2=0.126, P>0.05). The efficacy of group A was significantly higher than that of group B(97.73% vs.81.40%, χ 2=4.617, P<0.05). There were no statistically significant differences in the incidence rates of bleeding, infection, drainage tube abscess and puncture abscess between the two groups(all P>0.05). Conclusion:Percutaneous nephrostomy in the treatment of infectious hydronephrosis is more effective than ureteral stent drainage.Both of two methods have high safety of treatment.Clinical treatment should be based on the specific circumstances of the patients to obtain a significant clinical effect.
7.Abiraterone acetate rechallenge for mCRPC patients with AR mutant

A 75-year-old patient was admitted with "progressive dysuria for more than 2 months" in January 2017. The tPSA level was 498 ng/ml and then diagnosed as metastatic prostate cancer (cT 3bN xM 1). For the resistance of abiraterone, gene mutation was detected during the endocrine therapy. After 5 months of endocrine therapy, the serum tPSA was decreased to a minimum of 12.5 ng/ml. Since July, the serum PSA level gradually rebounded, and the endocrine therapy was altered to androgen deprivation therapy (ADT). The level of tPSA was maintained at 104 ng/ml in October 2017, and ADT was discontinued. After 1 year of discontinuation, the re-examination of PSA was 3 205 ng/ml. As the first-line regimen for mCRPC, abiratone and prednisone combined with goserelin was used. After 5 months of treatment, the level of tPSA still showed progression. The drug resistance of abiraterone was considered, so the treatment was discontinued. Next-generation sequencing technology (NGS) revealed the presence of AR, FGFR3 and RIT1 mutations, while no HRR germline mutation was detected. Docetaxel combined with ADT was performed. It was changed to comprehensive treatment of goserelin + docetaxel in March 2019. During chemotherapy CT images indicated significant reduction of pelvic lymph nodes and left inguinal lymph nodes, while bone metastasis showed stable condition. In April 2020, the chemotherapy was terminated for the lower extremity edema, joint pain and other related discomfort. The level of tPSA was 289 ng/ml after the last chemotherapy. DNA sequencing testing were performed again, and the mutation of AR and AR-V7 was negative. According to the results of genetic testing, the tPSA continued to decrease to 23 ng/ml after 6 months of abiraterone rechallenge, the imagings suggested that no disease progression. After AR mutation turns negative after chemotherapy, patients with refractory CRPC can still obtain a good PSA response such as tumor control and other clinical benefits from abiraterone. Abiraterone rechallenge is probably a new attempt for AR mutant patients with refractory CRPC.
8.Clinical application of 3D printed titanium chest and rib implants in chest wall reconstruction

Objective In this study, three-dimensional printed (3DP) titanium implants were used for skeletal reconstructions after wide excision of chest wall. 3DP titanium implants were expected to provide a valid option with perfect anatomic fitting and personalized design in chest wall reconstruction. Methods There were 13 patients [mean age of 46 (24-78) years with 9 males and 4 females] who underwent adequate radical wide excision for tumors and chest wall reconstruction using 3DP titanium implants. Surgical data including patient demographic characteristics, perioperative clinical data and data from 1-year follow-up were collected and analyzed. Results Six patients of rib tumors, six patients of sternal tumors and one patient of sternal pyogenic osteomyelitis were finally selected for the study. The chest wall defect area was 221.0±206.0 cm2. All patients were able to maintain the integrity of the chest wall after surgery, and no abnormal breathing was found, achieving personalized and anatomical repair. Thirteen patients were successfully discharged from the hospital. Two patients developed pneumonia in the perioperative period. During the follow-up period in the first year after surgery, no implant related adverse reaction was observed, including implant rupture, implant shift, rejection reaction and allergies. One patient had wound ulcer after chemotherapy. Three patients had tumor recurrence, with the recurrence rate of 25.0%. Two patients died of tumor recurrence, with a mortality rate of 16.7%. Conclusion 3DP titanium implant is a safe and effective material for chest wall reconstruction.
