1.A preliminary interview on counselor's decision making of breaking confidentiality about self-inflicted injury and suicide in college situation

Objective: To investigate the ethical decision-making process of breaking confidentiality when counselors dealing with self-inflicted injury and suicide issues in college situation. Methods: A semi-structural interview was addressed to 10 counselors from 7 college counseling centers in Beijing, among whom with (10 ± 8) years of experience on average in this field. Content analysis method was used to transcription of the interviewing data. Results: Totally 8 counselors had received ethical training more or less, and attached great importance to ethical codes. There were still some conflicts between school regulations and confidentiality rules in 7 university counseling centers. Different counselors varied greatly in decision-making on breaking confidentiality when facing college students' self-inflicted injury and suicide. Faced with conflicts between college demands and confidentiality principles, counselors could take the professional standpoint and consider more of the interests of students. Conclusion: The decision-making process on self-inflicted injury and suicide confidentiality breakthrough needs to be standardized. College's attention and support to the counseling work should be strengthen and enhance ethical awareness.
2.Research on the compatibility between traditional Chinese medicine culture and Marxism in terms of ethical implications and value utility from the perspective of the "second combination"

Traditional Chinese medicine (TCM) culture is a typical representative of excellent traditional Chinese culture,and its core concept is embodied in the ethical value principles of "benevolence,harmony,refinement,and sincerity",which are highly similar to Marxism in terms of value stance,ethical connotation,social ideal,and internal development path.TCM culture embodies the core values concepts and original ways of thinking of excellent traditional Chinese culture.It emphasizes the value position of benevolent love for others,the ethical connotation of putting life first,and the social ideals of "medical care for the world" and "universal harmony",which are highly compatible with Marxism's value position of putting people first,as well as the global governance ideal of building a community with a shared future for mankind emphasized by socialism with Chinese characteristics for a new era,in terms of value orientation,moral matrix,and discourse system.Socialism with Chinese characteristics has entered a new era,and TCM culture has realized its integration with modern science and technology,as well as with global public health practices under the guidance of Marxism,contributing Chinese solutions and Chinese wisdom to the construction of a community of common health for mankind.In the process of creative transformation and innovative development of TCM culture,it is both theoretically and practically necessary and urgent to dig the deeper points of convergence and explore the complementary utility space between the two sides,so as to enable them to absorb each other in openness,update and develop in integration,and continuously promote the "second combination".
3.Assessment of left ventricular systolic function using pressure-strain loop in patients with light-chain amyloidosis

Objective:To discuss the quantitative evaluation of left ventricular systolic function in patients with light-chain amyloidosis (AL) by pressure-strain loop (PSL).Methods:Forty-six patients with clinically diagnosed as AL in the Affiliated Tumor Hospital of Zhengzhou University from January 2018 to December 2020 were selected as case group, and they were divided into 2 groups according to whether the thickness of interventricular septum and posterior wall at end-diastole was >12 mm; ①cardiac amyloidosis (CA) group (21 cases, the thickness>12mm); ②non cardiac amyloidosis (NCA) group (25 cases, the thickness≤12 mm). Twenty five healthy volunteers were selected as control group at the same time. Routine echocardiography was performed in all subjects.Two-dimensional dynamic images of the left ventricular apical two-chamber, three-chamber, and four-chamber views were collected for three consecutive cardiac cycles using two-dimensional speckle-tracking. A tracing analysis was conducted and blood pressure was entered on the off-line Echo PAC 203 software, and the left ventricular global longitudinal strain (GLS), peak strain time dispersion(PSD), global myocardial work index (GWI), global constructive work (GCW), global wasted work (GWW) and global work efficiency (GWE) were obtained. The differences of GLS, PSD and myocardial work (GWI, GCW, GWW, GWE) parameters were compared between groups, and Pearson correlation was used to analyze the correlations.Results:①Routine echocardiography: Compared with the control group and NCA group, inter-ventricular septum thickness (IVST), left ventricle posterior wall thickness (LVPWT), left ventricular mass index (LVMI), left atrial diameter (LAD), and E/e′ in CA group were increased, while left ventricular ejection fraction (LVEF) was decreased; ②Compared with the control group, GLS, GWI, and GCW in NCA group were decreased, while there were no statistically differences in GWE, PSD, and GWW between the two groups (all P>0.05); Compared with the control group and NCA group, GLS, GWI, GCW, and GWE were obviously decreased, while PSD and GWW were obviously increased in CA group.③ Correlation analysis showed that: the absolute value of GLS was positively correlated with GWI, GCW and GWE ( r=0.654, 0.695, 0.788; all P<0.001), and negatively correlated with GWW, and PSD ( r=-0.710, -0.625; all P<0.001). Besides, PSD had negative correlation with GWI, GCW and GWE ( r=-0.754, -0.653, -0.702; all P<0.001), and positive correlation with GWW ( r=0.676, P<0.001). Conclusions:PSL can quantitatively evaluate the left ventricular systolic function of AL patients, while the myocardial work parameters are conducive to evaluate the degree of cardiac involvement in the course of AL patients.

Talaromycosis marneffei has been increasing in recent years. Our understanding of this disease has gradually deepened through extensive basic and clinical research, but there are still many limitations. In this article, by incorporating the latest research advancements, we discuss important issues in managing Talaromycosis marneffei trends, aiming to guide effective prevention and control of the disease, improving public health, and reducing the healthcare burden.
5.Analysis of adverse events following immunization of adsorbed acellular DPT vaccine at different vaccination sites

ObjectiveTo analyze the occurrence of suspected adverse events following immunization (AEFI) after changing the priority vaccination sites of the adsorbed acellular diphtherior-pertussis-tetanus vaccine (hereinafter referred to as DPT vaccine), so as to provide scientific basis for mass vaccination. MethodsMonitoring data of AEFI for the DPT vaccine in Wujiang District from September 2020 to August 2022 were collected from China's disease prevention and control information system, and the vaccination information of DPT vaccine in all children's vaccination clinics in Wujiang District during the same period was selected. The incidence of AEFI for the DPT vaccine was analyzed and compared. ResultsThe reported incidence of AEFI was significantly lower in the buttocks than that in other sites (P<0.05). The reported incidence of AEFI was significantly higher in booster immunization than that in basic immunization (P<0.05). After inoculation at different sites, the main clinical symptoms of AEFI were local redness and swelling. There were significant differences in the incidence of local redness and swelling, local induration, pruritus and other symptoms (lethargy, abnormal crying, etc.) (P<0.05). There were significant differences in the severity of local redness and swelling in different sites (P<0.05). The degree of redness and swelling in the anterolateral thigh was lower than that in other sites (P<0.05). The local strong reaction of swelling (>5.0 cm) in the deltoid muscle of the upper arm was significantly higher than that in the buttocks (P<0.05). ConclusionThe DPT vaccine is safe in different parts of the body and is worth popularizing.

New threats posed by the emerging circulating variants of SARS-CoV-2 highlight the need to find conserved neutralizing epitopes for therapeutic antibodies and efficient vaccine design. Here, we identified a receptor-binding domain (RBD)-binding antibody, XG014, which potently neutralizes β-coronavirus lineage B (β-CoV-B), including SARS-CoV-2, its circulating variants, SARS-CoV and bat SARSr-CoV WIV1. Interestingly, antibody family members competing with XG014 binding show reduced levels of cross-reactivity and induce antibody-dependent SARS-CoV-2 spike (S) protein-mediated cell-cell fusion, suggesting a unique mode of recognition by XG014. Structural analyses reveal that XG014 recognizes a conserved epitope outside the ACE2 binding site and completely locks RBD in the non-functional "down" conformation, while its family member XG005 directly competes with ACE2 binding and position the RBD "up". Single administration of XG014 is effective in protection against and therapy of SARS-CoV-2 infection in vivo. Our findings suggest the potential to develop XG014 as pan-β-CoV-B therapeutics and the importance of the XG014 conserved antigenic epitope for designing broadly protective vaccines against β-CoV-B and newly emerging SARS-CoV-2 variants of concern.
